class="elsevierStylePara elsevierViewall">Sensorineural hearing loss of 30<span class="elsevierStyleHsp" style=""></span>dB or greater over at least three contiguous audiometric frequencies occurring over 72<span class="elsevierStyleHsp" style=""></span>h is the most commonly used criteria to define sudden sensorineural hearing loss (SSNHL).<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> Its incidence varies between 5 and 30 per 100,000<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">2</span></a> but its true incidence may be greater since affected individuals with spontaneous or quick recovery rates do not necessarily consult for medical care.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a> The peak incidence of SSNHL is reported to occur between the fifth and sixth decade of life, but individuals of all ages can be affected, with equal incidence between men and women.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a> The main accompanying symptoms include tinnitus and vertigo.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">4,5</span></a> Also, there are studies that report speech discrimination deficits that appear to be worse than expected for the degree of hearing loss in cases of SSNHL.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">6,7</span></a> This aspect has been overlooked over the years.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The majority of patients with SSNHL are classified as idiopathic. Identifiable causes are found in up to 30%<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> and the differential diagnosis includes over a hundred proposed etiologies.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a> A systemic review published 10 years ago, reports that the most frequent causes identified are infectious (13%) and otologic disease (5%), followed by traumatic (4%), vascular or hematologic (3%), neoplastic (2%), and others (2%).<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> The evaluation of SSNHL is mainly based on pure tone audiometry. Also, magnetic resonance imaging (MRI) is used to evaluate the internal auditory canal and cerebellopontine angle for tumors, including vestibular schwannomas, labyrinthine hemorrhage or demyelinating processes.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The treatment of SSNHL is controversial and several authors have questioned the necessity of medication due to variable rates of spontaneous recovery being reported, and the lack of knowledge about its pathophysiology.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> However, in the most recent clinical practice guideline of the American Academy of Otolaryngology – Head and Neck Surgery (AAO-HNS), steroid therapy is considered an option as an initial therapy, and transtympanic steroids are recommended as salvage therapy.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">5</span></a> The Spanish Consensus also recommends the use of steroid therapy: PO (orally), transtympanic steroid injection (TSI), or combined.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">11</span></a> Although the evidence supporting their efficacy is still debated, systemic steroids are still the most widespread primary treatment.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> If patients do not recover their hearing, audiologic rehabilitation, hearing aids, or cochlear implants may be offered, depending on the severity of their hearing loss.</p><p id="par0020" class="elsevierStylePara elsevierViewall">While spontaneous recovery has been reported in 32–65% of cases, generally during the first two weeks of onset, various prognostic factors have been described for poor hearing recovery including age (elderly), severity of hearing loss, audiometric pattern, presence of vertigo, duration of hearing loss before consulting, and cardiovascular comorbidities.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> With regards to audiometric characteristics, there is agreement that for a profound loss, the recovery rate is poor,<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">13,14</span></a> however, for the other frequencies, there is some controversy as different authors have used different classifications. For instance, Chang et al. report that most of their cases had flat or total losses (68.9%), and that a midtone loss had complete recovery in 55.6%, however, midtone was taken to account for a U-shaped curve with >30<span class="elsevierStyleHsp" style=""></span>dB loss at 1000<span class="elsevierStyleHsp" style=""></span>Hz (and <30<span class="elsevierStyleHsp" style=""></span>dB loss at 250 and 4000<span class="elsevierStyleHsp" style=""></span>Hz in comparison with the other ear).<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">14</span></a> Byl reported that upsloping and midfrequency (“cookie-bite”) had a higher rate of recovery, and most of the cases were downsloping or flat losses (53%), followed by profound loss.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> On the other hand, Nosrati-Zarenoe et al. observed that the most common presentations involved low or high frequencies (72.3%), and that the recovery rates were best for the midfrequency cases, in this case, the calculation included 1000, 1500 and 2000<span class="elsevierStyleHsp" style=""></span>Hz.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">We performed a retrospective study of SSNHL cases seen at our health care center over the last ten years. The primary outcomes of this study were to analyze the audiometric patterns and hearing recovery rates based on affected frequencies; and our secondary outcome was to assess speech intelligibility deficits, an issue that has been neglected in the SSNHL literature.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">All cases of SSNHL seen at the Pontificia Universidad Católica de Chile healthcare center from January 2010 until June 2020 were included. Inclusion criteria were sensorineural hearing loss of 30<span class="elsevierStyleHsp" style=""></span>dB or more across at least three contiguous frequencies occurring within 72<span class="elsevierStyleHsp" style=""></span>h as evidenced by pure tone audiometry. Cases were excluded if they had incomplete charts or if the audiograms were performed at another center. There were 926 cases seen during this time period, however, 628 cases were excluded for the aforementioned criteria, and 17 were excluded because they did not meet the inclusion criteria. Hence, a total of 211 cases were reviewed. The study was approved by the local ethics committee of the Pontificia Universidad Católica de Chile, ID 200624009.</p><p id="par0035" class="elsevierStylePara elsevierViewall">All patients underwent an otoscopic examination to exclude potential conductive hearing loss causes. Acoustic immittance testing and pure tone audiometry testing was then performed using equipment calibrated to current American National Standards Institute standards. Acoustic immittance was performed with the Madsen® Zodiac® – Advanced Tympanometer, and for the last two years, with GSI TympStar Pro™.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Pure tone audiometry was performed with the GSI AudioStar Pro™ or GSI Pello™ audiometers. Ear-specific air and bone conduction thresholds were measured with appropriate masking. Air conduction thresholds were measured for 0.125, 0.25, 0.5, 1, 2, 3, 4, 5, 6, and 8<span class="elsevierStyleHsp" style=""></span>kHz. Bone conduction hearing thresholds were measured at octave intervals 0.250 – 4<span class="elsevierStyleHsp" style=""></span>kHz and 3<span class="elsevierStyleHsp" style=""></span>kHz. If a threshold was greater than 110<span class="elsevierStyleHsp" style=""></span>dB, or there was no response at the limit of the audiometer, a value of 110<span class="elsevierStyleHsp" style=""></span>dB was used for calculating the pure tone average (PTA). PTAs were calculated as follows: low-mid frequency (0.125, 0.25, 0.5, 1, 2, 3<span class="elsevierStyleHsp" style=""></span>kHz), high frequency (4, 6, 8<span class="elsevierStyleHsp" style=""></span>kHz), and speech frequency (0.5, 1, 2, 4<span class="elsevierStyleHsp" style=""></span>kHz). The degree of hearing loss was reported based on the speech PTA as follows: mild (26–40<span class="elsevierStyleHsp" style=""></span>dB), moderate (41–60<span class="elsevierStyleHsp" style=""></span>dB), severe (61–80<span class="elsevierStyleHsp" style=""></span>dB), and profound (≥81<span class="elsevierStyleHsp" style=""></span>dB).<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> This classification was used in order for readers to be able to compare our results to previously published data. Audiometric curve morphology was classified subjectively as upward-sloping, downward-sloping, flat loss, trough shaped (“cookie bite”) and reverse U-shaped curve. Because the reverse U-shaped curves could be to an actual upward-sloping SSNHL with a concomitant prior high frequency hearing loss (e.g. presbyacusis), we compared the curves with the contralateral ear. Speech discrimination scores (SDS) were measured using standardized word lists by monitored-live voice, and using disyllabic words.</p><p id="par0045" class="elsevierStylePara elsevierViewall">To evaluate the recovery rates through time, we assessed the PTAs at 14 days, and between 15 days to 3 months. Whenever audiometry results were available for both time periods, we also evaluated whether there was an improvement in hearing over time. To establish recovery rates, we used the contralateral ear as a baseline as it has been recommended by the Spanish Consensus and the AAO-HNS guideline.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,11</span></a> The PTA chosen for this calculation was the PTA mostly affected depending on the audiometric curve, and was calculated as follows<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a>: (PTA pre<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>PTA post)/(PTA pre<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>PTA contralat)<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>100%. A recovery of ≤10% was considered as “no recovery”, “minimal recovery” was considered as 11–50% recovery, “partial recovery” was considered at ≥50%, while “complete recovery” was considered as 100% or within 10<span class="elsevierStyleHsp" style=""></span>dB of the contralateral ear.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">All statistical analyses were carried out using GraphPad Prism 9.0.1. A descriptive analysis of all data was performed, being reported as means, percentages, medians and standard deviations. For the data that was not normally distributed, non-parametric testing was used. The Kruskal–Wallis test was used to compare overall differences between the different groups. The Mann–Whitney <span class="elsevierStyleItalic">U</span> test was used to assess differences between two groups. Post-hoc testing involved the Wilcoxon signed-rank test. A <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall">For the reviewed period, 211 cases met the inclusion criteria. See <a class="elsevierStyleCrossRef" href="#sec0080">Supplementary Table 1</a> for demographic characteristics. Clinically, most patients reported hearing loss, aural fullness or muffled hearing (97.6%), followed by tinnitus (71.1%), vertigo (17.5%) or dizziness (15.2%). When looking at the most frequent combination of symptoms, we observed that hearing loss accompanied by tinnitus was reported in half of the cases (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>103, 48.8%). Only three patients reported isolated tinnitus. In 30.8% of the cases, patients reported vertigo or dizziness as well as hearing loss or tinnitus. Eighteen percent of all patients reported vertigo and 15% reported dizziness. The ear involved was the right ear in 93 cases, left ear in 116 cases, and bilateral in two cases only. On average, patients consulted on day 9 since the beginning of the symptoms, with a median of 6 days before receiving treatment.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Patients were mostly treated with steroids (96%), only eight patients were not treated (<a class="elsevierStyleCrossRef" href="#sec0080">Supplementary Table 1</a>). The most frequent treatment was prednisone PO followed by TSI if complete recovery was not observed following PO treatment (44.5%). Thirty-seven percent of patients were treated with steroids PO, and 14.7% were treated with TSI only; these patients were diabetic or had difficult-to-treat hypertension.</p><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Hearing loss patterns</span><p id="par0065" class="elsevierStylePara elsevierViewall">Because there is considerable variability in the literature regarding how to calculate the initial hearing loss, and what is considered as recovery,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> we hereby provide our results with different criteria. The average hearing thresholds for the affected ear at the initial consult can be seen in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. As for the audiometric curves for all ears (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>213), 34.3% were downward-sloping, 30% were flat, 25.4% were upward-sloping 5.2% were trough shaped and 5.2% were reverse U-shaped.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">We then calculated the different PTAs by group, with a special emphasis on the affected area of the cochlea. Average hearing thresholds for the affected ear at initial presentation as well as the different PTAs and SDS can be seen in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. As can be expected, upward-sloping cases had worse hearing in the L-M frequencies which was mild (38.8<span class="elsevierStyleHsp" style=""></span>dB). Downward-sloping cases had worse hearing for the high frequencies which was severe (74.2<span class="elsevierStyleHsp" style=""></span>dB). Flat loss cases had on average profound hearing throughout all frequencies (L-M, speech, high). Trough shaped cases had worse hearing in the speech frequencies which was moderate (53<span class="elsevierStyleHsp" style=""></span>dB) while reverse U-shaped cases had worse hearing at the high frequencies which was moderate (43.5<span class="elsevierStyleHsp" style=""></span>dB) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The SDS were the lowest for the flat loss cases (24.6%), and highest for the reverse U-shaped cases (98.2%). This is reasonable as the speech frequencies were mostly affected in the flat losses (83.1<span class="elsevierStyleHsp" style=""></span>dB), while least affected for the reverse U-shaped cases (31<span class="elsevierStyleHsp" style=""></span>dB).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The degree of hearing loss at the initial presentation, based on speech PTAs, is decreasing order: mild (33.3%), profound (28.6%), moderate (19.5%), severe (11.9%), and normal (6.7%). Degree of hearing loss based on the type of curve can be seen in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Contralateral ear</span><p id="par0080" class="elsevierStylePara elsevierViewall">The aforementioned results were based on the initial presentation PTAs, however, comparing the thresholds to the contralateral ear as a baseline has been recommended for calculating recovery rates.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> For this part, we have eliminated the non-treated and bilateral cases. The difference between both ears at the initial presentation varied based on the type of curve, these changes were statistically significant (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.00001). The biggest differences for the speech frequencies were seen between the upward-sloping and reverse U-shaped curve cases vs flat loss curves (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.00001) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). As for the SDS, differences between both ears were statistically significant (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.00001) with the biggest differences being observed between the upward-sloping and downward-sloping vs flat losses (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.00001).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Hearing improvement</span><p id="par0085" class="elsevierStylePara elsevierViewall">For calculating the recovery rates (percentage of the maximum possible improvement), we used the contralateral ear as baseline. We used the PTAs that were mostly affected in each group for this purpose: L-M frequency PTA for upward-sloping; speech frequency PTA for the trough shaped, and high frequency PTA for downward-sloping, flat and reverse-U shaped cases (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The percentage of patients recovering to at least 50% of the maximum possible recovery, using the unaffected contralateral ear as a baseline by 3 months of follow-up were: 65% for upward-sloping, 35.6% for downward-sloping, 28.1% for flat, 45.5% for trough-shaped and 60% for reverse U-shaped cases, respectively. For the upward-sloping cases, 47.3% showed improvement between both periods. For downward-sloping cases, 60% did not show any improvement from 14 days till 3 months. A similar pattern was seen for the flat loss cases.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Speech discrimination score</span><p id="par0095" class="elsevierStylePara elsevierViewall">The worst SDS were seen for the flat loss curves followed by the downward-sloping curve cases. The upward-sloping and reverse U-shaped cases had the best scores at all three testing periods (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Statistically significant changes were seen between the initial and 14 days follow up for all cases, except reverse U-shaped cases seen as they had normal SDS from the initial period (upward-sloping <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0001; downward-sloping <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0008; flat <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>0.0001, trough shaped <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0267; reverse U-shaped <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.2451). Although most improved in time, there were no significant changes between 14 days and 3 months follow-up.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">When the degree of hearing loss was mild, the SDS was excellent and mostly over 80%. When the degree of hearing loss was profound, SDS was poor and tended to be less than 24%, however, four cases with profound hearing loss still managed to have SDS over 50%. Great variability in SDS was observed for cases of moderate or severe hearing loss, varying from 0 to 100% (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>A). As SDS are related to hearing thresholds, we evaluated changes in hearing thresholds with changes in SDS from the initial presentation until 14 days follow-up; reverse U-shaped cases were excluded for this part as the initial SDS were within normal limits (98.2%). The speech frequency PTA was used. For the upward-sloping cases, a difference of 14.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.5<span class="elsevierStyleHsp" style=""></span>dB was seen, with 10.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>22.1% for the SDS; downward-sloping: ΔPTA 17.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17.6 and ΔSDS 13.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>25.2%; flat: ΔPTA 19.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>22.3<span class="elsevierStyleHsp" style=""></span>dB and ΔSDS 29.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>40%, and trough shaped ΔPTA 19<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.5 and ΔSDS 16.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>16.3%. As can be observed in <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>B, changes in speech PTA do not always correlate with changes in SDS.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">We then evaluated differences between patients that had ≤50% SDS initially versus patients with SDS<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>51% SDS. There were no significant differences in sex (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.2525), age (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.3177), hypertension (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1068), or diabetes mellitus (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1954). However, patients with SDS<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>50% initially, presented with vertigo more frequently (30% vs 10%), and this difference was significant (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0003).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Possible etiologies of SSNHL</span><p id="par0110" class="elsevierStylePara elsevierViewall">The etiology of SSNHL commonly remains unknown in the majority of cases, and is therefore classified as idiopathic. In this series, the etiology was identified in 18% of cases (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>37). The most common cause was determined to be the <span class="elsevierStyleItalic">début</span> of Ménière's disease (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>24) as evidenced by their follow-ups. Other causes identified included: infectious disease (meningitis <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2, herpes zoster <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1, HIV <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1), recurrent SSNHL (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3, all cases recovered completely and did not present with dizziness or vertigo), autoimmune ear disease (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2), demyelinating process (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2), labyrinthine hemorrhage (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1), and cerebellopontine angle tumor (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1). It is important to mention that only 56% of patients had MRI scanning of the internal auditory canal and cerebellopontine angle; hence, the identifiable causes are most probably underestimated. It is difficult to state when the cause is vascular; we observed that four patients had a history of a stroke, five had a coronary artery bypass surgery, 25.4% had hypertension and 11.8% had diabetes mellitus. However, we cannot state with any certainty if these factors were involved in the incidence of SSNHL.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Concomitant vertigo</span><p id="par0115" class="elsevierStylePara elsevierViewall">As it has been described that patients with SSNHL and vertigo have worse outcomes, we compared the initial speech PTA between patients with and without dizziness or vertigo. Patients experiencing dizziness or vertigo had worse initial speech PTAs (68.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>35.2<span class="elsevierStyleHsp" style=""></span>dB) compared to the rest (51<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>29.5<span class="elsevierStyleHsp" style=""></span>dB) (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>0.0001). We then evaluated how many patients had no recovery in speech PTA at 14 days follow-up. In the group of patients without dizziness or vertigo, 26% had no recovery while 40% of patients experiencing dizziness or vertigo had no recovery; this difference was significant (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0007). Percentages of patients that experienced vertigo and/or dizziness for each type of curve can be seen in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">SSNHL is still a widely debated condition. There is variability with regards to potential etiologies, pathophysiology, audiometric patterns, recovery rates and prognostic factors. While it may not be such a common clinical presentation, patients can be left with an important hearing loss and/or tinnitus which limits their quality of life. It may also be the first manifestation of an underlying disease and should not be taken lightly.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Not all patients report the same clinical presentation at onset. We observed that the most frequently reported symptoms were hearing loss accompanied by tinnitus (48.8%). Only three patients consulted because of an acute unilateral tinnitus, but they had not perceived they also had hearing loss. This was confirmed by audiometry testing. Overall, we observed a very high incidence of tinnitus (71.1%). Other authors have also reported tinnitus, varying between 60–97%.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">15,17,18</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Patients with SSNHL can also exhibit vertigo or dizziness; 30.8% of our patients reported vertigo and/or dizziness. However, we do not have any vestibular testing to clarify which vestibular organs, and to which degree they may have been involved. This is not a rare occurrence, and our results are consistent with the literature.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">15,19–21</span></a> We observed that vertigo and/or dizziness were present for all types of audiometric curves, but was more frequent for flat and downward-sloping cases, which tend to have a greater degree of hearing loss.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Variability also exists with regards to the initial frequencies affected. We observed that 34.3% were downward-sloping, 30% were flat, 25.4% were upward-sloping, 5.2% were trough shaped, and 5.2% were reverse U-shaped. Byl also described that the most commonly found audiometric pattern was downward-sloping (27.6%) followed by flat (25.4%); upward-sloping was observed in 17.1% of cases (USA).<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> Chang et al., on the other hand, found that a flat loss was the most common presentation (48–53%). However, in this study from Southern Taiwan, all included patients were hospitalized. We do not know whether they hospitalize all patients with SSNHL or if patients with a more severe presentation are referred to the hospital.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">14</span></a> Xie et al. reported that the most common presentation was profound hearing loss (45.2%) followed by flat (34.3%); they also included all hospitalized cases,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a> while Pecorari et al. described that 48% were downward-sloping, 32% were flat and 20% were upward-sloping, these results are more consistent with our findings.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> With regard to the degree of hearing loss, we found that overall, the most common initial presentation was mild or profound, yet this varied extensively depending on the type of audiometric curve observed. Xie et al. observed that 57% of their cases were severe to profound losses, only 13.3% were mild.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a> On the other hand, Pecorari et al. report that 56% were mild followed by 18% were severe,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> however, the criteria used for severe hearing loss was 71–90<span class="elsevierStyleHsp" style=""></span>dB HL while we used 61–80<span class="elsevierStyleHsp" style=""></span>dB HL for that category as they used the International Bureau for Audiophonology recommendations.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">22</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Recovery rates were calculated with the contralateral ear as a baseline. Overall, 24.8% and 28.7% of cases did not recover their hearing by 14 days and 3 months, respectively. On the other hand, 19.4% and 18.5% recovered their hearing completely by 14 days and 3 months, respectively. However, not all patients had test results available for both time periods. Forty percent and 37% recovered 50% or more of their hearing by 14 days and 3 months, respectively. A flat curve was an ominous sign of no hearing recovery (38.6%), while an upward-sloping or trough shaped curve were a favorable prognostic factor for a full recovery (25% and 27.2%, respectively). It is difficult to compare recovery rates as different studies have used different criteria to establish was is considered as recovery.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">8,13,17</span></a> We believe that the contralateral ear should be used as a baseline to assess recovery rates, as long as that ear has no known pathology; and also, using the frequencies affected for this calculation.</p><p id="par0145" class="elsevierStylePara elsevierViewall">With regards to improvement over time, we can only comment on recovery rates up to 3 months follow-up. When removing cases that had already recovered by day 14, and including patients that had audiometry tests available at 14 days and 3 months, we observed that hearing did improve over time in 42% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>25/60). Therefore, we suggest that patients with SSNHL be followed up for at least 3 months, as a minimum. The latest guideline from the AAO-HNS actually recommends that clinicians obtain follow-up audiometry at the conclusion of treatment and within 6 months of completion of treatment<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">5</span></a> while the Spanish Consensus states a 12 month follow-up.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">11</span></a> Yeo et al. also observed mild improvement in hearing over three months.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">23</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">While hearing recovery has been the primary outcome in most studies, we wanted to assess what happens to SDS in SSNHL. SDS not only depends on the degree of hearing loss; there is evidence that certain patients have relatively mild degrees of hearing loss on pure tone audiometry, yet may exhibit poor speech discrimination, for instance, patients with vestibular schwannomas.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> Another factor that has been described to affect speech discrimination is the audiometric pattern, with flat curves tending to result in poorer scores.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">25</span></a> Most importantly, speech perception is a complex process and various structural levels of auditory processing are necessary for understanding speech.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a> For mild cases, the SDS were mostly over 90%. For moderate and severe cases, great variability was observed in SDS, and most profound cases had poor SDS, usually less than 20%. The lowest SDS scores were seen for patients with flat curves, however, these cases also showed the worse PTAs. Also, for these cases, significant improvements in SDS were seen at 14 days and 3 months. Excepting the reverse U-shaped cases (because they had excellent initial SDS), all other audiometric patterns showed significant improvements in SDS over time. When comparing improvements in speech PTA at 14 days with changes in SDS, we observed that there is no clear predictive pattern. Cases with large speech PTA improvements, over 60<span class="elsevierStyleHsp" style=""></span>dBs, had dramatic changes in SDS. However, cases with improvements of 20<span class="elsevierStyleHsp" style=""></span>dB or less in speech PTA had variable changes in SDS, varying from 0 to 100%. This obviously depends on the initial speech PTA. We observed that changes in speech PTA do not always correlate with changes in SDS. We note that: (1) there are cases with mild changes in speech PTA that had important changes in SDS, and (2) cases with great changes in speech PTA that had mild changes in SDS. Hence, speech discrimination recovery is complex, and involves other various factors besides hearing thresholds.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Limitations of this study include the retrospective nature of the study and the lack of vestibular testing.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conclusion</span><p id="par0160" class="elsevierStylePara elsevierViewall">SSNHL is still a controversial topic, and as audiovestibular testing is continually evolving, we are observing different patterns of abnormalities that can provide clues to underlying etiologies. Downward-sloping was the most frequent audiometric pattern. Patients with concomitant vertigo and/or dizziness had worse initial speech PTAs and also exhibited inferior recovery rates. Overall, 40% of the patients had recovered 50% or more of their hearing by 14 days. Hearing improved over time, and this was more common for the upward-sloping cases. As for speech discrimination, we observed that patients that had a SDS of 50% or less, had vertigo more frequently. Also, for cases that showed moderate and severe hearing loss, great variability was observed in the SDS. Outcome measures included audiometric patterns, recovery rates, improvements over time for hearing loss as well as for SDS at 14 days and 3 months follow-up. The Kruskal–Wallis test and Mann–Whitney <span class="elsevierStyleItalic">U</span> test were used to compare differences between the different groups. Post-hoc testing involved the Wilcoxon signed-rank test. A <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">We included 211 patients, 64.3% showed downward-sloping or flat audiometric curves. Overall, 40% of the patients had recovered 50% or more of their hearing by day 14. We observed that hearing did improve over time, and this was more common for the upward-sloping cases, with 65% recovering to at least 50% of the maximum possible recovery by 3 months follow-up.
