class="elsevierStylePara elsevierViewall">Case 1: a 75 year-old woman with pulsatile tinnitus of the left ear that evolved over 2 years. Otoscopy showed a retrotympanic lesion compatible with grade 1 glomus tympanicum, confirmed by CT and NMR imaging studies (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). Tonal audiometry was normal.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Case 2: a 70 year-old woman with a history of serous otitis media and tinnitus in the right ear that evolved over 8 months. Otoscopy revealed an anterior retrotympanic lesion with a vascular appearance that led to suspicion of glomus tympanicum, confirmed by TCand angio-NMR imaging, grade 1 (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B–D). There was mixed hypoacusia with a difference of 25 dB between the airway and bone thresholds at low frequencies.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The cases were operated under general anaesthesia in MOS regime, in a tertiary hospital, from June to September 2018, respectively. A rigid 0° (18 cm long, 4 mm ø) endoscope was used for the transcanal endoscopic approach. No specific material for endoscopic ear surgery was used. The surgical phases were: 1) Exposure of the tumour: creation of a tympanomeatal flap with an anterosuperior base with elevation of the hammer handle membrane; 2) tumour reduction: coagulation of the tumour surface by CO<span class="elsevierStyleInf">2</span> laser fibre (CW 5 W, 0.30 s/pulse) and otology handpiece (AcuPulse™, MicroLase™ CO<span class="elsevierStyleInf">2</span> fibre, Micro-H™ handpiece, Lumenis Ltd., Yokneam, Israel); 3) Tumour dissection and exeresis; 4) Haemostasis and vaporisation of tumour bed residues by laser (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A–D). During surgery surgical patties were used with a preparation of lidocaine B. Braun 20 mg/mL (10 mL) and epinephrine 1 mg/mL (0.5 mL). In the last 2 phases of the procedure two-handed work was performed. The procedure ended with replacement of the flap, reinforcing the triangular fossa underlay cartilage in case 2. The patients were discharged, without incident, a few hours after the intervention.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">The Pathology Department confirmed the diagnosis of paraganglyoma in both cases. A surgical complication occurred in the first case, with residual perforation of the tympanum and transmissive hypoacusia, which required myringoplasty in a second operation. At follow-up after 6 months no tumour relapse was found in either case.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Discussion</span><p id="par0025" class="elsevierStylePara elsevierViewall">Exclusively endoscopic transcanal surgery of the glomus tympanicum confined to the middle ear is feasible, with results that are comparable to those that may be obtained using a transcanal or microscopic retroauricular approach. Previous studies with wider series such as those by Killeen et al.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and Daneshi et al.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> describe similar results to those of this work.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Glomus tympanicum is the most common benign tumour of the middle ear.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> It originates in the Jacobson nerve or Arnold nerve neural crest cells, and its extension is determined according to the Glasscock-Jackson classification.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Tumours of the middle ear and, among them, glomus tympanicum confined to the middle ear (grades I and II), may be approached using endoscopic transcanal surgery.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> This technique offers a broader view of the ear structures, making it possible to exclude canaloplasties or retroauricular incisions, so that it is minimally invasive.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Nevertheless, glomus tympanicum is a bleeding lesion, and this is one of the main limitations of endoscopic surgery. This is due among other reasons to the fact that only one hand is working. Additionally, transcanal surgery is performed in a limited space, and this determines which instruments may be used. As well as the aforesaid drawbacks there is also the contraindication against the use of monopolar electrocautery on the promontorium because of the risk of damaging the neuroepithelial cells of the cochlea and causing neurosensorial hypoacusia. In our experience the “almost contact” use of a CO<span class="elsevierStyleInf">2</span> fibre laser to coagulate the lesion and vascular bed is suitable, due to the existence of single-use handpieces of an ideal size for transcanal work. Other alternatives described in the literature are the diode laser or bipolar pincers designed for ear surgery, such as the Calvian endo-pen®.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,6</span></a> Finally, it has to be pointed out that in our cases 2 hand working (2 surgeons) was possible, and this aided the operation during the bloodiest parts of the same, although it is wise to practice and train beforehand.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Conclusion</span><p id="par0040" class="elsevierStylePara elsevierViewall">In our experience, the exclusively endoscopic transcanal approach is a valid alternative for the treatment of grade I glomus tympanicum.
