Right: zigzag suture (white) and raphe to raphe suture (blue).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Obstructive sleep apnoea (OSA) is considered a public health problem associated with decreased quality of life and productivity, along with increased morbidity and mortality from cardiovascular disease, metabolic problems, or even cancer, as well as an increase in motor vehicle accidents.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Although the standard treatment for moderate-severe OSA is overnight positive airway pressure, adherence is suboptimal in many patients, and therefore alternative treatments are needed. These alternative treatments include positional therapy, myofunctional therapy, intraoral devices, skeletal and soft tissue surgery, and hypoglossal nerve stimulation. At present, any of these could be the first choice depending on the patient's characteristics and preferences.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In recent years, there has been a paradigm shift in pharyngoplasty techniques from excision of the rim of the soft palate and uvula to remodelling techniques in which the mucosa is spared, muscles can be transposed or elevated without resection, and fat in the supratonsillar fossa can be resected or ablated.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Using barbed sutures instead of the classical resorbable sutures has the advantage of avoiding knots inside the oral cavity and distributing the tension along the thread instead of at a single point.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Different pharyngoplasty methods with barbed sutures have been described as the technique has evolved.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6</span></a> Using this suture thread does not imply a specific technique; it is a tool that can be incorporated into different pharyngoplasties depending on the anatomical characteristics of the patient and the collapse observed on videofibrosomnoscopy or drug-induced sleep endoscopy (DISE). In addition, the use of barbed sutures has been shown to have similar results to expansion sphincter pharyngoplasty, with shorter operating time and a low learning curve.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">As part of the collaboration between the two centres, variations in the technique with barbed sutures have been identified and we wanted to know whether these changes could give better outcomes, and which variables might be associated with successful surgical outcomes.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">This is a retrospective study with consecutive sampling of adult patients undergoing surgery between 2014 and 2019 with barbed sutures in the ENT departments of two university hospitals (Ospedale Morgagni-Pierantoni in Forlì, Italy, and Hospital Universitario Dr Peset in Valencia, Spain). All patients signed an informed consent form before the surgery. The study was conducted in accordance with the Declaration of Helsinki guidelines on ethical principles for medical research involving human subjects.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The Italian team used the barbed reposition pharyngoplasty (BRP) technique<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). In this technique, after tonsillectomy and weakening without complete section of the palatopharyngeus muscle in the lower part of the tonsillar fossa, using a bidirectional barbed suture, a needle is introduced at the central point (A) and passed laterally inside the palate towards the pterygomandibular raphe (C) on one side passing first through an intermediate point (B). From the raphe, the needle is introduced through the same exit point into the tonsillar fossa to embrace the palatopharyngeal muscle and return to the raphe. Up to three passes can be made to the palatopharyngeus muscle, each time returning to the raphe, applying gentle traction on the thread each time it leaves the mouth.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The Spanish team performed a modified BRP: after dissection and removal of the supra-tonsillar fat,<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> the technique begins in the same way as the classic BRP with the difference that the needle comes out of the hole created when the fat is removed, making 3 loops between the palatopharyngeus and the pterygomandibular raphe. In this technique, the palatopharyngeus muscle is weakened but not completely cut. The rest of the thread is used to close the wound in the soft palate using zig-zag sutures across the palate from the junction of the hard palate to the edge of the soft palate near the uvula.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> In addition, raphe-to-raphe suturing is possible if there is suture left over (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>a, b); if raphe-to-raphe suturing is desired, it is best planned before zig-zag suturing.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Inclusion criteria were adult patients operated for OSA, with polygraph (PG) or polysomnography (PSG) before and 3–6 months after the surgery. Exclusion criteria were the absence of a sleep test before or after the surgery. Patients who underwent multilevel surgery were also included. Multilevel surgery consisted of reduction of the lingual tonsil by robotic approach or by coblation. Nasal surgery consisted of septoplasty combined with turbinate reduction if required by the patient.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The variables analysed were age, sex, body mass index (BMI), tonsillar hypertrophy (according to Friedman scale), Epworth Sleepiness Scale (ESS), tongue base and/or nasal surgery performed in addition to barbed pharyngoplasty, and the information obtained from the PG or PSG.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Different definitions of surgical success were considered according to the postoperative apnoea-hypopnoea index (AHI) (<10/h and Sher’s surgical success criterion, AHI < 20 with greater than 50% reduction<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>). In addition, the difference (delta) of pre- and post- AHI (pre-AHI − post-AHI) and the relative delta AHI (pre-AHI − post-AHI/pre-AHI × 100) were calculated. The relative delta AHI has the advantage of showing the percentage reduction in OSA severity, providing an easy-to-understand number for both patients and clinicians, independent of the baseline AHI.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The continuous variables were dichotomised with the cut-off points described in the literature<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11–14</span></a> as determinants of each variable to analyse the association of each dichotomised variable with surgical success according to the success criteria described above.</p><p id="par0060" class="elsevierStylePara elsevierViewall">SPSS v24 software was used for the statistical analysis. Dichotomised variables were compared using the χ<span class="elsevierStyleSup">2</span> test. Finally, a multivariate analysis was performed using binary logistic regression to determine the influence of the aforementioned descriptive variables on surgical success with the variables that were significant in the bivariate analysis. A p-value of <.05 was considered significant.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">A total of 126 patients were included in the study, whose characteristics are detailed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Success rates were analysed according to the different definitions of success explored, yielding 39.7% for a postoperative AHI < 10/h and 51.6% success according to Sher's criteria. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> summarises the prevalence of the descriptive variables of interest according to each definition of success.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Multivariate analysis was performed using binary logistic regression to determine the influence of descriptive variables with statistical significance on surgical success as defined by a postoperative AHI < 10 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">In this multicentre study, no differences were found in terms of surgical success according to the number of loops performed in the soft palate. Nor were significant differences found with respect to other demographic or clinical variables.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Non-resective pharyngoplasty with barbed sutures has been shown to be useful in reducing the severity of OSA and patient drowsiness. Regardless of the barbed pharyngoplasty technique used, BRP achieves a surgical success rate according to Sher's criteria of more than 50% of the sample.</p><p id="par0090" class="elsevierStylePara elsevierViewall">In this study, the difference in the number of loops performed in barbed pharyngoplasty was not a predictor of improved surgical success in multivariate analysis, although in an initial analysis it appeared to be important in improving outcomes. This is paradoxical when we look at the intraoperative changes that occur when passing the suture that can be seen in Video <a class="elsevierStyleCrossRef" href="#sec0035">1</a>. The zigzag suturing in the soft palate seems to help open the velopharynx in its anteroposterior diameter even more than raphe-to-raphe sutures. Surgical success may be due to factors that do not depend on increasing upper airway (UAW) diameters postulated by Masiyev et al.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> These authors measured postoperative changes with acoustic pharyngometry after expansion sphincter pharyngoplasty. They found that, in both successful and unsuccessful patients, the UAW increased significantly to the same extent, with no significant difference between the two. However, Barbieri et al. did find an improvement in their success rates when they modified their technique by increasing the suture passage through the soft palate with two horizontal sutures.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Despite the low number of female patients in our sample, they seem to have better results than males. To our knowledge, this point has not been previously described and will have to be validated in studies with larger samples.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Age also seems to be important in the results of the surgery, since, although there is no difference in success rates according to the classical Sher criteria, the chance of achieving an AHI < 10/h is reduced. With age there are changes in the upper airway, including a lowering of the hyoid which has been associated with worse surgical outcomes.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Another surprising finding is that patients who underwent nasal surgery have worse outcomes than those who did not require nasal surgery. This is contrary to findings in a larger series, in which patients who underwent nasal surgery had a much higher success rate than those who did not.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> This association is not significant in the multivariate analysis, so there may be other unknown variables influencing the association. The resolution of nasal problems has been associated with a decrease in airway resistance with a consequent improvement in patients' quality of life,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> therefore we believe it unlikely that nasal surgery is associated with a worse prognosis.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Although the long-term effect of these two types of pharyngoplasty with barbed sutures has not been evaluated, it has been suggested that pharyngoplasties that do not resect the free border of the soft palate have better long-term success rates than classic uvulopalatopharyngoplasty (UPPP).<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> Both expansion pharyngoplasty and barbed pharyngoplasty appear to be equal in short-term success rate.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> It may be plausible that the better results of barbed pharyngoplasty are maintained in the long term as is the case with expansion sphincter pharyngoplasty. This will be the focus of future studies.</p><p id="par0115" class="elsevierStylePara elsevierViewall">This study has some limitations, the Friedman Tongue Position (FTP), also known as the Modified Mallampati Grade, could not be obtained in all patients, and therefore a comparative analysis according to this characteristic could not be assessed, even with previous series. The FTP is an important variable to assess in OSA patients, as a high FTP is associated with lower success rates.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Furthermore, as described by Sundman et al.,<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a> measurement of the tonsils with the Friedman scale does not show a high concordance between different examiners even from the same department, which might also have been the case in our study. A measurement of the actual volume of the resected tonsils could have solved this limitation, but this data was not collected either. We did not perform a statistical size calculation beforehand to know if we had the statistical power, we simply reviewed the patients undergoing surgery with barbed sutures who had pre- and postoperative sleep studies in both centres.</p><p id="par0120" class="elsevierStylePara elsevierViewall">This study has strengths, such as the relatively high number of patients undergoing pharyngoplasty with barbed sutures combined or not with multilevel surgery, which reflects the wide variety of problems in the UAW of patients with OSA. As this study was conducted in two centres, its results are more generalisable. However, it is precisely the heterogeneity of the groups, reflecting the real life of OSA patients, which prevents us from obtaining more significant results.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0125" class="elsevierStylePara elsevierViewall">Non-resective pharyngoplasty with barbed sutures is an effective technique for the treatment of OSA in selected patients. Both the classic and modified techniques are alternatives with good postoperative results in both single and multilevel surgery.
