He underwent endoscopic balloon dilation using a Bielefeld balloon (Tubavent®) at 10  bar for 2 min per side. Twenty-four hours after surgery, he developed facial edema during Valsalva maneuvers. Examination showed bilateral cervicofacial edema with crepitus extending to the mediastinum. A CT scan confirmed subcutaneous emphysema from the nasopharynx to the mediastinum without additional complications. He was hospitalized for five days for observation, receiving analgesia and prophylactic antibiotics. The emphysema completely resolved within two weeks (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Subcutaneous emphysema occurs in less than 1% of cases. It typically does not extend beyond the neck but can reach the mediastinum in rare instances. A CT scan is recommended to asses the extent. The suggested treatment includes observation, analgesia, prophylactic antibiotics and avoid Valsalva maneuvers for at least 2 weeks. Most cases resolve within 72 h. The insertion of the catheter may cause mucosal injury, leading to an air leak into peritubular tissues and potentially throughout the body.
