La prevalencia en España<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> es del 21,5% (25,6% en mujeres y 17,4% en varones). Su tratamiento supone un reto para los profesionales implicados, ya que raramente es curativo y la adherencia a dichos tratamientos es baja<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Los antimuscarínicos están disponibles para el tratamiento de la vejiga hiperactiva no neurogénica refractaria a la terapia no farmacológica desde hace años. Sin embargo, sus efectos secundarios (sequedad de boca, estreñimiento, visión borrosa, mareos y problemas cognitivos) limitan la tolerabilidad de la terapia y son responsables de la falta de adherencia.</p><p id="par0015" class="elsevierStylePara elsevierViewall">La vía de administración mayoritaria para los anticolinérgicos sigue siendo la clásica vía oral, aunque actualmente existe también la vía transdérmica.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Un estudio comparativo de los antimuscarínicos (Cochrane) concluye que si se dispone de preparaciones de liberación prolongada de oxibutinina o tolterodina, éstas serían preferibles a las de liberación inmediata, debido al menor riesgo de sequedad de boca<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">La vía de administración también se ha demostrado clave para el aumento de la adherencia. Recientemente, Herbison P et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">4</span></a>, publicó un metaanálisis donde todos los anticolinérgicos evaluados (trospio, oxibutinina oral, oxibutinina transdérmica, fesoterodina, propiverina, darifenacina, solifenacina, tolterodina, imidafenacina) fueron mejores que el placebo y tuvieron un efecto similar. Ninguno de ellos fue claramente superior. Sin embargo, la oxibutinina transdérmica causó menos sequedad de boca que los otros tratamientos, por lo que vale la pena considerarlo como tratamiento de primera línea.</p><p id="par0030" class="elsevierStylePara elsevierViewall">En pacientes de edad avanzada preocupa la posible afectación cognitiva. Se ha visto que en pacientes sometidos a tratamiento con oxibutinina oral, la afectación cognitiva pasaba de un 24,1% antes del tratamiento a un 41,1%<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">5</span></a>. Müller-Arteaga <span class="elsevierStyleSmallCaps">C</span> et al.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">6</span></a> evaluó el tratamiento con oxibutinina transdérmica en 70 pacientes mayores con SVH. Hubo una mejoría significativa en todos los síntomas de almacenamiento (excepto la incontinencia urinaria de esfuerzo) y mejoría del cuestionario de autoevaluación de control de vejiga, con un 84,3% de adherencia al tratamiento. No observaron deterioro cognitivo alguno tras un mes de tratamiento con oxibutinina transdérmica.</p><p id="par0035" class="elsevierStylePara elsevierViewall">La oxibutinina es un potente antagonista del receptor muscarínico que tiene efecto anticolinérgico, lo que consigue uniéndose a los receptores M3 y bloqueando la activación colinérgica<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">7</span></a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">El metabolito primario de la oxibutinina después del metabolismo hepático de primer paso, la N-desetiloxibutinina (N-DEO), se considera en gran parte responsable de sus efectos secundarios anticolinérgicos asociados, a causa de su elevada afinidad por los receptores muscarínicos M3 de las glándulas salivares. Por este motivo, se han buscado otras vías de administración que obvien este paso hepático. La vía transdérmica ofrece esta característica. Además, a diferencia del resto de anticolinérgicos, el bajo peso molecular de la oxibutinina y su carácter lipofílico permite su absorción por vía transdérmica, mejorando la biodisponibilidad. Con esta vía de administración se ha conservado su eficacia, disminuyendo los efectos secundarios a nivel sistémico, si bien puede aparecer algún efecto anticolinérgico secundario, como reacciones adversas a nivel local<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">8</span></a>. De hecho, según Zobrist RH et al.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">9</span></a>, los niveles de N-DEO plasmáticos son bajos con esta forma de administración, ya que se evita el metabolismo gastrointestinal y hepático de primer paso, lo que reduciría los efectos secundarios anticolinérgicos. Otra ventaja de la vía transdérmica es que la oxibutinina pasa directamente a la circulación sistémica, proporcionando unos niveles séricos mucho más estables.</p><p id="par0045" class="elsevierStylePara elsevierViewall">La oxibutinina transdérmica en formato de parche permite una liberación prolongada con una dosis habitual de dos parches a la semana, aplicados en abdomen, nalgas o cadera, y proporciona un suministro continuo, minimizando las fluctuaciones máximas y mínimas en los niveles plasmáticos.</p><p id="par0050" class="elsevierStylePara elsevierViewall">En la administración por esta vía transdérmica, es importante que los pacientes sean instruidos en el correcto manejo del parche, ya que un uso adecuado (alternar los lugares de aplicación y no repetirlos durante al menos una semana) reduce la posibilidad de desarrollar una reacción en el sitio de aplicación.</p><p id="par0055" class="elsevierStylePara elsevierViewall">En el estudio OSCAR, se observaron mejorías significativas desde el inicio del tratamiento en la frecuencia urinaria, nocturia, número de episodios de urgencia y número de incontinencia de urgencia, según el diario miccional. Un 38,1% de los pacientes tuvieron eventos adversos, principalmente en el sitio de aplicación (27,6%), de los cuales, la mayoría de ellos fueron de intensidad leve o moderada. Hubo una baja incidencia de reacciones adversas sistémicas, de las cuales ninguna fue considerada grave. Tan solo seis pacientes (5,7%) comentaron la presencia de sequedad de boca tras un año de tratamiento. El 55,2% de los pacientes se mantuvo con el tratamiento tras 12 meses, siendo percibido por los pacientes como cómodo o muy cómodo en un 92,9% de los casos<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">10</span></a>.</p><p id="par0060" class="elsevierStylePara elsevierViewall">En el estudio MATRIX, se encontraron mejorías significativas y clínicamente relevantes con la administración del parche de oxibutinina transdérmica. El tratamiento fue bien tolerado, encontrando pocos efectos adversos anticolinérgicos (sequedad de boca 2,6%, estreñimiento 1,5% y mareos 0,7%). Se encontraron reacciones en el lugar de aplicación (prurito, eritema, dermatitis e irritación) en el 14,0% de los pacientes<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">11</span></a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">En la revisión de Salinas-Casado J et al.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">12</span></a> se muestra que los pacientes tratados con oxibutinina transdérmica mejoran la sintomatología, siendo esta eficacia similar a la observada en pacientes tratados con oxibutinina oral o tolterodina. La mejoría sintomática se produce desde la segunda o tercera semanas de tratamiento y de manera sostenida hasta el final del tratamiento. Además, la oxibutinina transdérmica, en comparación con la oxibutinina oral o la tolterodina, presenta un mejor perfil de seguridad a nivel de reacciones adversas sistémicas.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Tras una exhaustiva revisión de la literatura nacional e internacional, podemos afirmar que los fármacos transdérmicos en general, y la administración de oxibutinina por vía transdérmica en particular, tienen una eficacia clínica similar a los fármacos de administración oral y provocan una menor frecuencia de efectos adversos, en particular la boca seca, produciendo así una menor tasa de abandonos del tratamiento.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Todo ello se traduce en un beneficio clínico y una mejora en la calidad de vida para los pacientes, por lo que estos fármacos transdérmicos pueden y deben ser considerados como de primera línea para el tratamiento de la vejiga hiperactiva.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "bibliografia" => array:2 [ "titulo" => "Bibliografía" "seccion" => array:1 [ 0 => array:2 [ "identificador" => "bibs0015" "bibliografiaReferencia" => array:12 [ 0 => array:3 [ "identificador" => "bib0065" "etiqueta" => "1" "referencia" => array:1 [ 0 => array:2 [ "contribucion" => array:1 [ 0 => array:2 [ "titulo" => "Prevalencia de vejiga hiperactiva en España: estudio poblacional [Prevalence of overactive bladder in Spain: a population-based study]" "autores" => array:1 [ 0 => array:2 [ "etal" => false "autores" => array:4 [ 0 => "D.
