He was an exceptional urological surgeon and a pioneer in renal vascular surgery and renal transplantation in Spain. Recently at the Milan 2023 congress on the 50th anniversary of the European Urological Association, his career and anti-reflux technique were revisited. However, the importance of the work carried out by Carlos Alférez in the creation of Actas Urológicas Españolas as the official scientific journal of the Spanish Urological Association (AEU by its Spanish acronym) deserves more recognition than is currently given.The journal Archivos Españoles de Urología, founded in 1944 by Enrique Pérez Castro (1908–1980), Antonio Puigvert Gorro (1905–1990), and Luis Cifuentes Delatte (1907–2005), was deemed the official bulletin of the AEU in 1964, during a period of crisis in terms of manuscript publication. This relationship proved beneficial for both the AEU and Archivos Españoles de Urología and lasted until 1977.<a class="elsevierStyleCrossRef" href="#bib0005">1</a> The ever-increasing scientific activity of the AEU, especially under circumstances in which congresses were held jointly with other societies (Portuguese-Spanish congress, Ibero-American congress), required the publication of a journal focused on collecting the teachings from these meetings. This journal was called Actas de la Asociación Española de Urología. The first volume (with one issue) covered the Spanish Congress of 1968 held in Málaga and the Portuguese-Spanish Congress of 1969 held in Porto (two issues). From 1970, one volume was published annually, with several issues each year. It was circulated regularly until 1977 with the intention of collecting papers, free communications, and congress chronicles. It was of enormous educational value for the associates (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A).<elsevierMultimedia ident="fig0005"></elsevierMultimedia>During the Granada Congress in 1975, the AEU approved the creation of its scientific journal, Actas Urológicas Españolas, and the board of directors was given a vote of confidence to take all necessary steps. Even so, Actas Urológicas Españolas did not come into being until January-February 1977 (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B), when it became the official scientific publication of the AEU. It never lacked manuscripts. Carlos Alférez, who served on the AEU board between 1971 and 1980 as treasurer under the presidency of Julio Pérez-Irezábal y Andrés (1918–2008), Juan Montero Gómez (1923–2010) and José Antonio Martínez-Piñeiro Caramés (1927-), was appointed Executive Director from 1975 to 1986.<a class="elsevierStyleCrossRef" href="#bib0010">2</a> He worked together with Javier Solé Balcells (1924–2014) as associate director until 1982, José Luis Insausti Cordón (1917–1989) from 1982 to 1985, and Óscar Leiva Galvis (1939–2018) from 1985 to 1986. Oscar Leiva subsequently took over as Editor in Chief of the journal and Carlos Alférez focused on directing the department at the Fundación Jiménez Díaz until his retirement in 1990.The organizational work done by Carlos Alférez between 1975 and 1977 was quite intense. First of all, he pledged to manage one of the last issues of Actas de la Asociación Española de Urología, the paper for the 1976 congress held in Leon focused on Renal Transplantation.<a class="elsevierStyleCrossRef" href="#bib0015">3</a> In the Santander congress, AEU’s Actas completed Volume IX with four issues and, from 1978 onwards, this publication became the book of the AEU national congress papers for another decade. Why did it take Carlos Alférez almost three years to create Actas? Firstly, the technical characteristics of the publication had to be included in the statutes, and its financing required advertising and membership fees. It was also necessary to organize the contents of the journal and, above all, a governance model consisting of an editorial committee and an advisory committee with international figures. It needed to become independent from Archivos Españoles de Urología, a journal of great weight and tradition at that time. Although not a great diplomat, Carlos Alférez was hard-working (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). This attitude was the main guarantor of his success. In addition, his professional prestige always accompanied him in all his decisions and facilitated the search for synergies and the trust of collaborators to publish the best series and magnificent editorials in this new journal. Actas Urológicas Españolas is an international journal dedicated to urological diseases and renal transplant. It has been the official publication of the Spanish Urology Association since 1974 and of the American Urology Confederation since 2008. Its articles cover all aspects related to urology. Actas Urológicas Españolas, governed by the peer review system (double blinded), is published online in Spanish and English. Consequently, manuscripts may be sent in Spanish or English and bidirectional free cost translation will be provided. It has high visibility and accessibility and is found in the most important data bases, Medline/Pubmed, Emabase, JCR, Ïndice Médico Español, Índice bibliográfico Español en Ciencias de la Salud (IBECS), Cuiden, Dialnet, Hinari, Scopus, Google Scholar.
