Survival outcomes are excellent in non-metastatic testicular seminoma,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> constituting the paradigm of a curable neoplasm.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In stage I seminoma patients, 5-year disease-specific survival from centers of excellence reaches 99% while 5-year relapse rates range between 10%–20% after orchiectomy.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">European Association of Urology (EAU) guidelines<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> recommends to distinguish patients with stage I seminoma according to the presence or not of poor prognostic factors: A primary tumour size ≥4<span class="elsevierStyleHsp" style=""></span>cm and <span class="elsevierStyleItalic">rete testis</span> invasion. These factors are approved by the American Joint Committee on Cancer (AJCC) to classify stage I seminoma, although they are not formally used for disease staging. Patients with one of these factors are at greater risk of disease relapse than those without them. In patients without risk factors, the five-year relapse rate under surveillance is 4%–6%, whereas in the presence of one or two risk factors, five-year relapse rate in contemporary surveillance series is 15%–20%.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Active surveillance (AS) is recommend in patients at very low-risk of recurrence (no risk factors). One dose of adjuvant carboplatin (AC) is recommend in patients with risk factors of relapse. Adjuvant radiotherapy is no longer recommend, due to an increased risk of developing secondary non-germ cell malignancies.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Objectives</span><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of this study was to evaluate the impact of tumour size and <span class="elsevierStyleItalic">rete testis</span> invasion in progression free survival (PFS) of our patients with stage I testicular seminoma, distinguishing whether an adjuvant dose of carboplatin is administered or not. A literature review is also made.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A retrospective observational study was performed including patients with the diagnosis of stage I testicular seminoma after orchiectomy between January 2010 and July 2022.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Diagnostic evaluation consist on physical examination, ultrasonography of the testes, blood test including serum tumour markers before and after inguinal orchiectomy – alphafetoprotein (AFP), β-human chorionic gonadotropin (β-hCG) and lactate dehydrogenase (LDH)–, and contrast enhanced computerised tomography (CT).</p><p id="par0035" class="elsevierStylePara elsevierViewall">The main variable was progression free survival (PFS) –measured from the day of the orchiectomy to the day of the last medical visit or the diagnosis of relapse–.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Other variables were age, serum tumour markers, tumour size (<4<span class="elsevierStyleHsp" style=""></span>cm or ≥4<span class="elsevierStyleHsp" style=""></span>cm), presence or absence of <span class="elsevierStyleItalic">rete testis</span> invasion, administration or not of adjuvant treatment and follow-up time.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Patients without factors of poor prognostic –Group A– were compared with patients with factors of poor prognostic –Group B–. In both groups, a distinction is made between those patients in AS and those treated with AC.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Follow-up is performed by measuring tumour markers, thoracoabdominopelvic CT and ultrasound of the remaining testicle every 6 months during the first three years and annually after three years.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Patients with a relapse of the disease were treated with three cycles of BEP (bleomycin, etoposide and cisplatin).</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">The <span class="elsevierStyleItalic">T</span> Student test and Mann-Whitney test were used to compare continuous variables, parametric and non-parametric, respectively. Medians and interquartile ranges or mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation were reported. The <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup">2</span> test and Fisher test were used to compared parametric and non-parametric categorical variables. Frequencies and proportions were reported.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Kaplan-Meier curves and log-rank testing were used to compare progression free survival (PFS) between these groups. Statistical significance was considered at <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>.05. Statistical analysis was performed using the IBM SPSS Statistics 25.0 database.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">55 patients with stage I testicular seminoma were included in this study.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The mean age at diagnosis was 38 years (25–73) and a mean follow-up time of 63.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>33.6 months.</p><p id="par0080" class="elsevierStylePara elsevierViewall">20 patients (36.4%) did not present poor prognostic factors, while 35 (63.6%) did. Among the patients with poor prognostic factors, 62.9% had a size ≥4<span class="elsevierStyleHsp" style=""></span>cm, 22.9% had invasion of the <span class="elsevierStyleItalic">rete testis</span>, and 14.3% had both poor prognostic factors. Of the patients with invasion of the rete testis, four patients had pagetoid invasion, four patients had stromal invasion, and five patients had not been specified in the pathology report.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The demographic and clinical characteristics of the study population of both groups are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. No statistically significant differences were found in terms of mean age of the patients –35.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.4 years for group A vs. 40.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.9 years for group B; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.07–, follow-up time –70.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>28.9 months for group A vs. 59.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>35.9 months for group B; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.246– or laterality of the lesion –predominantly right in both groups, 65% vs 62.9%; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.55–.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">A higher percentage of patients with positive tumour markers was observed in Group B –10% in group A vs 42.9% in group B; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.01–. 64.7% of the patients with positive markers raised βhCG, 11.8% LDH –associated with large size of the tumour– and 17.6% with both.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Adjuvant chemotherapy was not administered to any patient in group A. Two patients presented relapse (10%), with a mean time to relapse of 8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.4 months. One patient presented a relapse 7 months after orchiectomy, consisting of a round left paraaortic retroperitoneal lymph node with a minor axis of 17<span class="elsevierStyleHsp" style=""></span>mm, with a complete response to three cycles of BEP. Another patient presented a relapse 9 months after orchiectomy, consisting of two interaortocaval lymph nodes of 18 and 13<span class="elsevierStyleHsp" style=""></span>mm in minor axis, with a complete response to three cycles of BEP.</p><p id="par0100" class="elsevierStylePara elsevierViewall">26 patients (74.3%) of the patients in group B received adjuvant chemotherapy with one cycle of cisplatin. 9 patients (25.7%) did not received adjuvant chemotherapy due to comorbidities or by decision of the patient –fully informed of the advantages and disadvantages of AS or adjuvant treatment–. One of this nine patients suffered a relapse, consisting of a retroperitoneal lymph node of left paraaortic location, with an axis smaller than 15<span class="elsevierStyleHsp" style=""></span>mm, with a complete response to three cycles of BEP.</p><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows a comparison of the patients based on adjuvant treatment –AS vs adjuvant cisplatin–. None of the patients with adjuvant cisplatin presented a relapse of the disease.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">A survival analysis is performed to assess whether there are differences in PFS, comparing according to risk factors (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>, log Rank <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.317) and according to the adjuvant treatment administered (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>, log Rank <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.114), without finding statistically significant differences in any of the cases.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">The importance of stage I seminoma lies in its high frequency, approximately 80% of patients with seminomatous testicular cancer are diagnosed as stage I, constituting 40% of all testicular tumours.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> The probability of relapse depends on several factors, such as tumour size and <span class="elsevierStyleItalic">rete testis</span> invasion, with relapse rates of around 16%.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> It is under discussion whether testicular microlithiasis constitutes a risk factor for the development of testicular cancer. Betancourt et al.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> made a literature review including four systematic reviews and they found that the combination of testicular microlithiasis with other risk factors further increased cancer development. However, the likelihood of testicular cancer risk is similar to that of the general population in patients without other risk factors.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Historically, for several decades adjuvant radiotherapy to the paraaortic retroperitoneal and ipsilateral pelvic lymph nodes was considered to be the standard treatment for patients with stage I seminoma, leading to recurrence rates of less than 5%–10%. Patients who relapseafter adjuvant radiation therapy are almost always cured with three cycles of BEP.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> However, radiotherapy is no longer recommended as an adjuvant treatment for seminoma, due to it has been associated with an increased long-term risk of cardiovascular toxicity and second malignancies.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> These toxicities have also been reported in patients treated with platinum-based chemotherapy, but it is unknown if a single cycle of carboplatin would have these effects.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">The optimal treatment strategy for stage I testicular seminoma is highly controversial and generates debate, because of there are no randomized trials to show the superiority of AS or adjuvant treatment.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> This is partly due to the excellent overall survival (OS) of these patients even when relapse of the disease occurs,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> as we can see in our patients, who present an OS of 100%. Some groups recommend AS when possible, even in some patients with risk factors, to avoid overtreatment and its side effects.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> AS is preferred in countries like Canada and Denmark, chemotherapy is more frequently used in Norway, Sweden, and Australia.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> A risk-adapted approach seems to be a rational strategy, because it avoids treatment-related morbidity for patients whose disease was cured by orchiectomy and restricts adjuvant therapy for those at a higher risk of relapse.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Azizi et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> performed an analysis of patients diagnosed with stage I testicular seminoma using the National Cancer Data Base (NCDB). They observed that a tumour size ≥4.5<span class="elsevierStyleHsp" style=""></span>cm was an independent risk factor for overall mortality. Patients with large T1 seminomas managed with adjuvant radiation or chemotherapy vs. AS had a 41% decrease in overall mortality translating into a 3.5% and 4.5% absolute improvement in OS at 5 and 10 years, respectively.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Ondrusova et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> published a single center retrospective study with stage I testicular seminoma patients who were stratified into two groups according to risk-adapted therapeutic approaches. Low-risk patients underwent AS and high-risk patients were treated with adjuvant chemotherapy. OS was 100% and PFS was 90.5% in low-risk patients and 87.5% in high-risk patients. The statistically significant difference in PFS between these two groups was not found.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The highest level of evidence has been published by two groups:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0145" class="elsevierStylePara elsevierViewall">The Spanish Germ Cell Cancer Group (SGCCG).</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0150" class="elsevierStylePara elsevierViewall">The Swedish and Norwegian Testicular Cancer Group (SWENOTECA).</p></li></ul></p><p id="par0155" class="elsevierStylePara elsevierViewall">The aim of the prospective study published by SGCCG<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> was to analyse prognosticfactors for relapse in stage I seminoma managed by either AS or AC, in order to reinforce the efficacy of this individualized management approach. This study included 396 patients in AS and 348 patients treated with AC. Relapse occurred in 11.1% of patients in AS and 14.8% in AC group. Overall 5-year disease free survival was 88.3% for AS versus 96.8% for AC, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0001. Based on these results, a nomogram was developed from surveillance patients that includes two independent, predictive factors for relapse: <span class="elsevierStyleItalic">rete testis</span> invasion and tumour size.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The prospective study published by SWENOTECA<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> included 897 patients (422 in AS and 469 with AC), observing 69 disease relapses. <span class="elsevierStyleItalic">Rete testis</span> invasion [hazard ratio (HR) 1.9, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.011] and tumour size >4<span class="elsevierStyleHsp" style=""></span>cm (HR 2.7, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) were identified as risk factors predicting relapse. In patients without risk factors, the relapse rate (RR) was 4.0% for AS-controlled patients and 2.2% in patients treated with AC. In patients with one or two risk factors, the RR was 15.5% in patients in AS and 9.3% in patients who received AC. They did not find an increased RR in patients receiving carboplatin <7<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>AUC compared with patients receiving ≥7<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>AUC, therefore they conclude that the efficacy of AC in high-risk stage I testicular seminoma is relatively low and further adjuvant treatment options need to be investigated. Based on these results, a SWENOTECA-ABC trial is currently recruiting patients with the aim of comparing the efficacy of a cycle of adjuvant carboplatin with a cycle of adjuvant cisplatin, etoposide and bleomycin (BEP).<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">The Testicular Cancer Guidelines Panel, evaluating 20 studies, had published a recent systematic review. They concluded that there was a lack of evidence to justify a cut-off point of 4<span class="elsevierStyleHsp" style=""></span>cm, although the authors did not perform a meta-analysis due to the heterogeneity of the studies.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0170" class="elsevierStylePara elsevierViewall">Based on these results, the authors highlight the importance of a risk-adapted approach for stage I testicular seminoma. However, the most important thing in decision-making strategy is to thoroughly inform the patient about treatment options, including AS and AC, relapse probabilities and the adverse effects derived from these treatments.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">Relapse rate for stage I testicular seminoma is 15%–20%. In our study, we observed a relapse rate of 10.3% in patients in AS and 0% in patients who received AC (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>,092).</p><p id="par0180" class="elsevierStylePara elsevierViewall">The average time until recurrence is usually around 12–18 months, data that agrees with what was observed in our patients, 17.2 months.</p><p id="par0185" class="elsevierStylePara elsevierViewall">Para-aortic lymph nodes (82%) are the most frequent location for relapse, in our experience was 100%. No statistically significant differences in PFS have been observed between patients undergoing AS vs. AC. Relapse was successfully treated with three cycles of BEP in all of our patients.</p><p id="par0190" class="elsevierStylePara elsevierViewall">The OS of our patients is 100%, similar to that recorded in other studies.</p><p id="par0195" class="elsevierStylePara elsevierViewall">Although our study has limitations (retrospective single-center study), it contributes to reinforce the importance of individualization of adjuvant treatment in stage I testicular seminoma, the paradigm of a curable neoplasm.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0200" class="elsevierStylePara elsevierViewall">Current clinical guidelines strongly recommend active surveillance in stage I testicular seminoma as a safe alternative to adjuvant treatment after orchiectomy, making it possible to avoid the side effects of adjuvant treatments. However, this recommendation is limited to some criteria, including the absence of poor prognostic factors.</p><p id="par0205" class="elsevierStylePara elsevierViewall">Our group recommends following the AJCC staging system in decision-making, although we emphasize the importance of individualization of adjuvant treatment. We included patients with stage I seminoma between January 2010 and July 2022. Patients without factors of poor prognostic –Group A– were compared with patients with factors of poor prognostic –Group B–. Kaplan-Meier curves and log-rank testing were used to compare progression free survival (PFS) between these groups. Veinte pacientes (36,4%) tenían un pronóstico favorable -Grupo A- y 35 (63,6%) presentaban factores de pronóstico desfavorable -Grupo B-.
