There were 399,964 cases with prostate cancer diagnosed in Europe in 2012, with an incidence and mortality rates of 92.1 and 19.3 respectively, per 100,000 men and year.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> It is estimated that up to 30% of diagnosed patients will develop castration resistance at some time. Castration-resistant prostate cancer (CRPC) is defined as the state of the disease in which the patient has castrate serum levels of testosterone (testosterone <50 ng dl−1 or <1.7 nmol l−1) in addition to biochemical or radiographic progression.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Biochemical progression is defined by three consecutive rises of PSA, 1 week apart, resulting in two 50% increases over the nadir, with PSA > 2 ng ml−1; while radiographic progression is the appearance of two or more bone lesions on bone scan or enlargement of a soft-tissue lesion using the Response Evaluation Criteria in Solid Tumors (RECIST).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">CRPC has prognostic as well as therapeutic implications. The clinical trials that used docetaxel in combination with hormonal blockade, CHAARTED and STAMPEDE, contributed with evidence of considerable improvement regarding overall survival rates.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> With the results of the LATITUDE and an additional study of the STAMPEDE trial arm, the combination of androgen deprivation therapy (ADT) plus abiraterone/prednisone has become the first-line treatment for this type of patients.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> It has shown improved results in terms of overall survival, radiographic progression-free survival and time to chemotherapy initiation.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Regarding prognosis, it has been described that the mean survival rate ranges between 18 and 24 months after the onset of castration resistance phase. This is because the CRPC progresses independently to androgen levels, which leads to the development of metastasis in the medium term.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> The chances of changing the prognosis of these patients with the new therapeutic options depend on an early detection of this phase of the disease. The group of patients with metastases at diagnosis is one of the most susceptible for close follow-up in order to diagnose the castration resistance phase early. Some series report up to 18% of patients already metastatic at diagnosis.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The considerably inconsistent clinical course of metastatic prostate cancer has led to the evaluation of several prognostic factors regarding their role in determining the treatment strategy and the ability to predict the patient’s response to it.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The objective of our study is to establish the factors that are independently related to the development of early castration resistance in patients diagnosed with metastatic prostate cancer.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Sample</span><p id="par0025" class="elsevierStylePara elsevierViewall">Observational, multicenter and prospective study conducted to observe and assess the evolution of patients with prostate cancer within the National Health System. The investigation begins with the data obtained in the National Prostate Cancer Registry, including patients diagnosed with prostate cancer with histopathological confirmation between January 1, and December 31, 2010. The registers belonged to 24 Spanish reference centers, representing 20% of the population. This sample of patients was subsequently followed until December 31, 2013. There was a total number of 3,843 patients included in the National Prostate Cancer Registry at the end of the first phase (December 31, 2010), and there were 7 deaths (9%) during the 36-months follow-up. 2.9% of these deaths were caused by the disease (24).</p><p id="par0030" class="elsevierStylePara elsevierViewall">There were 155 patients included for our study: they had been diagnosed with metastatic prostate cancer and had undergone hormone therapy. All patients underwent prostate biopsy and extension imaging tests such as computed tomography and bone scintigraphy for metastases evaluation at diagnosis. They had a 39-month follow-up.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Analyzed variables</span><p id="par0035" class="elsevierStylePara elsevierViewall">The analyzed quantitative variables were age of the patients at diagnosis, level of basal serum prostate specific antigen (PSA) (ng / ml) and nadir PSA. The qualitative variables included were Gleason score (categorized in 2 groups, Gleason ≤ 7 and Gleason > 7), the presence of perineural invasion in the sample, T local stage (categorized in 2 groups, T1- T2 and T3-T4), the existence of lymph node involvement (N stage), metastasis (M stage) and the type of hormonal blockade administered (intermittent or continuous). The event of death and its cause were also recorded, in addition to the development -or not- of castration resistance and it’s time to onset during our study period.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the clinical and demographic characteristics of our sample. We must emphasize on the high prevalence of tumors with high-grade Gleason and perineural invasion in the pathology reports. The mean follow-up of the patients in the study was 26,2 ± 13,4 months, with a maximum follow-up of 39 months. From the 155 patients included for the study, 82 (52.9%) developed castration resistance during follow-up, with a mean time until the onset of the event of 12,2 ± 8,7 months.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The univariate analysis (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) shows significant differences regarding PSA values at diagnosis, mean age, mean nadir PSA, Gleason score and T stage between CRPC and non-CR patients. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the Kaplan–Meyer curves for the categorical variables with significance p < 0,05 in the univariate analysis.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">The multivariable analysis (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>) demonstrated that younger patients developed castration resistance, with higher nadir PSA and with more advanced T stages (T3-T4).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0055" class="elsevierStylePara elsevierViewall">Patients with metastatic CRPC improve with specific treatments such as abiraterone plus prednisone,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> enzalutamide<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> or docetaxel.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Therefore, it is reasonable to initiate life-prolonging treatment as soon as possible, since it has shown to increase their survival. For all these reasons, it is essential to determine the factors associated with the progression to castration resistance, in order to improve its detection among patients. This is why it is very important to analyze the early incidence of castration resistance in patients with metastatic prostate cancer at diagnosis, which was observed in more than half of the patients in our study. In addition, we evaluated the predictive factors of resistance in men primarily treated with hormone therapy, finding serum levels of nadir PSA, T stage and the patient's age at diagnosis as useful indicators. Therefore, these predictive factors can help us assess the risk of developing castration resistance, and high-risk patients can be evaluated more frequently than those with lower risk, which will result in earlier diagnoses.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Several authors such as Cooper et al. and Morote et al., have proposed that PSA level at diagnosis can predict ADT response,<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a> unlike the results obtained in our study. However, in accordance to our study, other authors found that PSA at diagnosis is not related with the development of resistance.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> Nadir PSA after hormonal blockade is generally evaluated in relation to progression to CRPC,<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13–16</span></a> and according to Newling et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> the ability to reach an undetectable PSA level as nadir is the most significant predictor for metastatic prostate cancer. In addition, other authors point out that the impossibility of reaching a nadir PSA of  < 1 ng/ml after hormonal blockade treatment predicts early progression to CRPC.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,17</span></a> However, our study shows that Gleason score is not a predictive factor for the development of castration resistance, while Choueiri et al.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> include Gleason score as an independent factor for shorter overall survival in patients with metastatic prostate cancer. Smaletz et al. expose that older ages are associated with better survival rates, as stated in our study. However, other articles refer to older age as a marker of poor survival. In spite of these, there are also studies that have not found age as a predictive variable for survival,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18–20</span></a> which does not clearly establish the effect of age as a predictive factor with the exposed results.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The present study has several strengths, such as the collection of data at a national level as well as the inclusion of multiple centers in Spain, which make this series a faithful representation of this type of disease in our system. However, the study is not devoid of limitations. The main weakness is based on the non-establishment of a joint follow-up protocol, so the diagnostic tests were established based on the doctors' considerations, which could be the cause of variability in their requests. Another limitation is the number of patients included in the study, which probably decreases the power of our results. Finally, the lack of data regarding some variables, such as the N stage, could have affected our results.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusion</span><p id="par0070" class="elsevierStylePara elsevierViewall">The nadir PSA in blood and a T3-T4 tumor stage at diagnosis appear to be associated with an increased risk of developing early resistance to castration in patients with metastatic prostate cancer at diagnosis. Furthermore, the age at diagnosis is shown as a variable that decreases this risk.
