It may be used alone with curative intention, synergically with systemic agents, or as an adjuvant to surgical procedures. Radiotherapy also has a well-established role in the palliative treatment of different types of tumors. Pelvic radiotherapy techniques have improved during the years to minimize the exposure of healthy organs to radiation, but urologic complications still emerge.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The incidence and severity of such complications may vary with location and size of the exposed tissue, dose and type of radiation and with the comorbidities of each patient.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Radiotherapy related complications may be classified as acute (during or immediately after radiation exposure), subacute (four to twelve weeks after radiation) or chronic (from three months to many years after radiation). Genito-urinary complications include cystitis, fistula and stenosis formation and secondary neoplasms.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> One of the most threatening adverse effect of radiotherapy is an obliterative endarteritis that results in hypoxia, hypovascularity and hypocellularity.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> These histological changes are intimately associated with bladder mucosa atrophy, ulceration and subsequent bleeding.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Bladder complications may be seen in 5-12% of patients treated with pelvic irradiation and acute hemorrhagic cystitis may occur in 2-8% of patients.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> This is one of the most challenging complications that the urologist must manage.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Initial management for radiation cystitis and its hemorrhagic component is usually conservative. There are several treatment options available ranging from intravesical or systemic treatments to radical procedures (arterial embolization, supra vesical urinary diversion and cystectomy).</p><p id="par0025" class="elsevierStylePara elsevierViewall">Hyperbaric oxygen therapy (HBOT) is used as primary or adjuvant therapy for a diverse range of medical conditions, its use in radiation cystitis is reported in several small series.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> It seems to improve tissue oxygenation resulting in neovascularization and capillary growth into hypoxic and scarred submucosal tissue. HBOT also increases fibroblast concentration, induces healing of tissues and decreases edema, necrosis and leukocyte infiltration.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">The aim of this study was to evaluate the efficacy of HBOT in radiation cystitis and to identify the predictive factors for a successful outcome.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients and methods</span><p id="par0035" class="elsevierStylePara elsevierViewall">We analyzed 105 patients diagnosed with radiation cystitis treated with Hyperbaric Oxygen at the Hyperbaric Medicine Unit of Hospital Pedro Hispano between January 2007 and December 2016.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Informed consent was obtained and all patients fulfilled a questionnaire documenting symptom severity before treatment using just the subjective part of Late Effects of Normal Tissues – Subjective, Objective, Management, Analytical (LENT-SOMA) scale.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> Patients with a urinary catheter were only evaluated for hematuria.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Median follow-up was 64 months (interquartile range: 31–100) and ranged from 2 to 150 months. Follow-up period was defined as the time from the end of HBOT to the time of cystectomy or last review of patients. All patients had a phone interview to assess their final LENT-SOMA scale subjective response.</p><p id="par0050" class="elsevierStylePara elsevierViewall">We reviewed patients demographic characteristics, previous pelvic cancer, dose and date of radiotherapy, other late radiation complications, history of previous chemotherapy, cystoscopic appearance, prior or concomitant treatments, number of transfusions given before the HBOT, time to the first episode of hematuria and time to HBOT.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Our protocol consisted of multiple sessions (median of 40) of 100% oxygen delivery in a multiplace hyperbaric chamber at 2.4 atm pressure for 80 min for five days a week. Complications were described and extracted from the medical records.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Descriptive and analytical statistics were applied using SPSS Statistics version 25·0. (IBM, Armonk, NY, USA) using the exact Wilcoxon signed rank test to compare pre and post-HBOT late radiation cystitis morbidity scores. The Pearson χ2 test, the Student’s <span class="elsevierStyleItalic">t</span>-test and analysis of variance were performed to detect possible determinants of success. Through the Kaplan-Meier method, hematuria free survival curves were obtained, applying the log rank test for comparison between the study groups. Response was evaluated in terms of total or partial resolution of bleeding and degree of improvement of the other symptoms evaluated by the LENT-SOMA scale.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">Patients’ characteristics are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. In 105 patients undergoing HBOT for radiation cystitis, 51,4% were male and the mean age at the beginning of HBOT was 65,4 years (±12,6).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">The main pelvic malignancies of our patients were prostate cancer (46.7%) and cervix cancer (41.9%) with a median radiation dose delivered of 70 and 50 Gy, respectively.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Bowel morbidity or perineal inflammation other than radiation cystitis were present in 25 patients (23.8%). Sixty-one per cent of the patients had undergone other treatments prior to HBOT: systemic agents (oral aminocaproic acid or pentosan polysulfate); intravesical agents (alum or hyaluronic acid irrigation); cystoscopy and clot evacuation with or without fulguration under anesthesia; arterial embolization.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Patient´s baseline symptoms (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) demonstrate that 71.4% of patients had persistent and refractory hematuria with clots. Thirty-eight patients (36.2%) needed multiple blood transfusions before or during HBOT. A significant number of patients irradiated due to prostate cancer had weaker stream compared to other pelvic malignancies (p < 0,001). Twenty-three patients had an urethral catheter before HBOT.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The median number of HBOT sessions was 40 (range: 10–110 sessions). Regarding HBOT complications, there were 3 cases of barotraumatic otitis treated with myringotomy, subsequently completing their treatments with HBO. All these patients continued their treatments subsequently. No other complications were observed.</p><p id="par0090" class="elsevierStylePara elsevierViewall">At the end of treatments, hematuria was completely resolved in 59 patients (56,1%) and partly resolved in 38 patients (36,2%), accounting for a hematuria success rate of 92.4%. During the follow-up period, 24,7% patients presented with recurrence of hematuria, 5 (20,8%) of which were successfully treated with subsequent HBOT.</p><p id="par0095" class="elsevierStylePara elsevierViewall">From the 8 patients who did not respond to the initial treatments, 6 were submitted to a cystectomy and 2 were submitted to angioembolization as a salvage resource.</p><p id="par0100" class="elsevierStylePara elsevierViewall">During our follow up period, other 8 patients were performed with cystectomy: 2 due to recurrence of hematuria, 4 due to vesicovaginal fistula and 2 due to recurrent infection.</p><p id="par0105" class="elsevierStylePara elsevierViewall">From our sample, the mean score of each subjective variable of LENT-SOMA scale (dysuria, frequency, hematuria) was significantly lower after the follow-up period (p < 0,05) with the exception of decreased stream (p = 0,14) and incontinence (p = 0,54). The sum of all subjective scores of LENT-SOMA scale was also significantly lower after the follow-up period (p < 0,05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Regarding hematuria response and recurrence, the time between the first episode of hematuria and the beginning of HBOT sessions had statistically significance (p < 0,05). Patients submitted to HBOT within 12 months of the first episode of hematuria had a better response and less recurrences.</p><p id="par0115" class="elsevierStylePara elsevierViewall">However, the success rate was not related to patient gender, age, hypocoagulation or antiaggregation.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Hematuria free-survival is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. Our results demonstrate that patients who had a complete response to HBOT have a longer lifespan without recurrence of hematuria requiring medical assistance when comparing to those with only partial response (71,4 months vs 36,4 months; log rank test p < 0,001 ). Likewise, women submitted to RT due to cervical cancer presented a higher hematuria free survival time (75,2 months vs 45,9 months; log rank test p = 0,036) when comparing to patients submitted to RT for prostate cancer.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion and conclusions</span><p id="par0125" class="elsevierStylePara elsevierViewall">Radiation cystitis is a challenging condition for the urologist. It has a great impact in patient’s quality of life due to the lower urinary tract symptoms (hematuria, frequency, incontinence, dysuria and decreased stream).<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Numerous studies have reported a positive effect of HBOT on the radiation-induced inflammatory condition of the bladder. Increased oxygen tension stimulates neovascularization that reverses tissue hypoxia and induces collagen production, culminating in wound healing and tissue regeneration.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">According to a recent metanalysis, our study is the second largest series of patients undergoing HBOT for radiation cystitis and has the third longest period of follow-up.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> With a median follow-up of 64 months, our immediate response rate was 92,4% which is compatible with previous studies.</p><p id="par0140" class="elsevierStylePara elsevierViewall">De Oliveira et al.,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> reported the effect of HBOT in 176 patients, the largest series in literature. With a median follow up of 12 months, their immediate hematuria resolution rate was around 90%, which is like ours. On the other hand, the study of Nakada et al.,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> offers retrospective data from a series of 34 patients with history of prostate cancer followed for an average of 11,6 years, the longest series described, and 74% of patients obtained nonrecurrent outcome which is similar to our study.</p><p id="par0145" class="elsevierStylePara elsevierViewall">We registered a median of 40 sessions of HBOT. There were 5 cases of recurrent hematuria in which repeated treatments were needed to achieve success. One of them was submitted to cystectomy 54 months after the recurrence due to recurrent infections. Repeated HBOT sessions can provide additional benefit,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> however, we were not able to establish any relationship between the number of treatments and patient outcome when evaluating hematuria or all symptoms score during our long period of follow-up.</p><p id="par0150" class="elsevierStylePara elsevierViewall">In this study, the median time interval between the first episode of hematuria and HBOT was relatively short (8 months) and 61% of all patients had received prior treatments to control symptoms. In our series we documented significant differences in hematuria response with longer interval lags from beginning of symptoms and the first session of HBOT. Starting HBOT earlier seems to favor total resolution of hematuria in opposition to delayed treatments which seem to result in only a partial improvement. Chong et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> demonstrated that treatment with HBOT within 6 months of hematuria onset was associated with a greater therapeutic response rate. Furthermore, we could demonstrate that patients that start HBOT earlier have less recurrences.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Many studies do not use a bladder toxicity scale to compare the results pre- and post-HBOT, making their comparison difficult. Most studies focus on hematuria alone while we used the LENT-SOMA scale to evaluate other debilitating symptoms. Data on the effects of HBOT on these symptoms are scarce. Oliai et al.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> evaluated 15 patients retrospectively according to the LENT-SOMA scale, as we did. Comparing our mean pre-HBOT score (sum of subjective scores divided by five) with the one of Oliai et al., our patients were on average much more symptomatic (2,04 versus 0,72) and with more severe hematuria before HBOTs (71.4% persistent or refractory versus 53.3% persistent). The mean post-HBOT score between our study and Oliai’s was also different (1.16 versus 0.2, respectively).</p><p id="par0160" class="elsevierStylePara elsevierViewall">Less invasive procedures, such as selective/superselective vesical arterial embolization, can be tried in patients with refractory and life-threatening hematuria prior to surgical therapy. There are few series reporting the results of embolization to control radiation-induced hematuria with few patients or without long-term results. Delgal et. al concluded in their series that clinical success rate seems to be higher for hematuria due to pelvic malignancy than for hemorrhagic cystitis.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> Mohan et al. evaluated 9 patients with intractable hematuria due to hemorrhagic cystitis: 7 had bladder cancer and had undergone transurethral resection of bladder tumor and pelvic RT; 1 had carcinoma cervix post-pelvic radiotherapy; 1 received cyclophosphamide due to Takayasu’s arteritis.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> Their success rate was 100% and there was only one recurrence with a mean follow-up of 14,45 months. Indeed, embolization controls the immediate problem of life-threatening hemorrhage and provides sustained bleeding control. In our opinion, this approach can be consolidated with HBOT.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">Cystectomy and urinary diversion are reserved for the treatment of intractable hematuria when other treatments have failed. It is an aggressive procedure and one of the main goals of HBOT is to avoid it. In our study, 13.3% patients were submitted to cystectomy: 5.7% after HBOT due to its ineffectiveness in resolving hematuria or other symptoms and 7.6% during our long follow up period. Few studies report these data. In the study of Mougin et al.,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> the rate of cystectomy was 23.9%.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Due to the absence of randomized studies comparing treatments, it is difficult to set definitive rules about management of these patients. HBOT seems to be disease modifying and maybe can cure some patients.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> Dellis et al.,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> reported a prospective series of 38 patients with severe radiation cystitis that received HBOT as primary treatment. They concluded that starting therapy within 6-months of hematuria onset was beneficial, assuming that the early setting of HBOT breaks the vicious circle of chronic inflammation and scaring of bladder tissues. In our clinical experience, initial management of patients with active bleeding due to diagnosed radiation-induced cystitis involves stabilization, clot evacuation and continuous bladder irrigation. Cystoscopy with fulguration can be performed if required, as well as oral and intravesical therapies. Patients should start HBOT as soon as possible as a consolidation measure. If the hematuria is refractory, selective or superselective arterial embolization or urinary diversion should be considered. Cystectomy should be considered as a salvage resource. No management strategy is 100% successful and a stepwise progression with multimodal treatment seems to be more accurate.</p><p id="par0175" class="elsevierStylePara elsevierViewall">The major limitation of our study is the retrospective nature of its design, not allowing an accurate measurement of the effect of HBOT in urinary symptoms after pelvic radiotherapy. Bevers et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> reported a prospective but non-randomized study of 40 patients with 75% of complete response rate. With a median follow-up of 23 months, 7.5% patients had no response to HBOT and 18.9% had recurrence of hematuria. There is a lack of randomized or prospective studies conducted on the effects of HBOT on late radiation cystitis, which weakens recommendations for its use. As any prospective study has a control group not receiving HBOT, one cannot exclude the possibility that some element in the improvement is a result of chance or a placebo effect. Recently, a Scandinavian prospective, controlled, multicenter trial, RICH-ART (radiation induced cystitis treated with hyperbaric oxygen – a randomized controlled trial), has been completed and its results may support the use of HBOT (ClinicalTrials.gov Number: NCT01659723).</p><p id="par0180" class="elsevierStylePara elsevierViewall">Another limitation of our study is the lack of information about radiotherapy characteristics. Information on the total dose of radiation was only available in 22% patients. Some patient’s records date back more than 10 years and all of them performed radiotherapy in different institutions. However, it is important to emphasize that success rate was not related to primary pelvic tumor or type of radiation therapy (combined or isolated external beam radiotherapy and/or brachytherapy).</p><p id="par0185" class="elsevierStylePara elsevierViewall">The tolerance of HBOT by the patients was extremely good, with limited side effects being reported.</p><p id="par0190" class="elsevierStylePara elsevierViewall">Until the presentation of these outcomes, our long-term improvement rate of hematuria of 92.4% issued the capacity of HBOT to decrease the number of emergency episodes, inpatient admissions and consultations in our sample of patients. The similar impact observed on urinary frequency and dysuria represented an expected improvement of quality of life in our patients. Hyperbaric oxygen therapy (HBOT) is an option for the management of radiation-induced hemorrhagic cystitis (RIHC). The aim of this study was to evaluate the efficacy of HBOT in radiation cystitis and to identify the predictive factors for a successful outcome.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and Methods</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">We retrospectively reviewed 105 patients diagnosed with RIHC which were treated with HBOT between 2007 and 2016 in our institution. Patients received 100% oxygen in a multiplace hyperbaric chamber at 2.4 atm for 80 min. All patients fulfilled a questionnaire documenting symptom severity pre-HBOT and at the end of the follow-up period.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">After a median of 40 HBOT sessions, there was success rate of 92,4% in the control of hematuria. During our follow-up period (median of 63 months) 24,7% patients presented with recurrence of hematuria. The mean score of the questionnaire-assessed variables: dysuria, urinary frequency and hematuria, was significantly lower after the follow-up period (p < 0,05).</p><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">Our data shows that the sooner HBOT is delivered after the first episode of hematuria, better response rates are achieved and lower recurrences concerning hematuria were registered (p < 0,05). La terapia de oxígeno hiperbárico (TOHB) es una opción para el manejo de la cistitis hemorrágica inducida por radioterapia (CHIR). El objetivo de este estudio fue evaluar la eficacia de la TOHB para tratar la cistitis por radioterapia e identificar factores predictivos para un resultado exitoso.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Material y métodos</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">Revisamos retrospectivamente 105 pacientes diagnosticados con CHIR que recibieron un tratamiento de TOHB entre 2007 y 2016 en nuestro centro. Los pacientes recibieron oxígeno al 100% en una cámara hiperbárica multiplaza a 2,4 ATA durante 80 minutos. Todos los pacientes cumplimentaron un cuestionario en el que se documentaba la gravedad de los síntomas previos a TOHB y tras el período de seguimiento.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Después de una media de 40 sesiones de TOHB, hubo una tasa de éxito del 92,4% en el control de la hematuria. Durante nuestro período de seguimiento (mediana de 63 meses), el 24,7% de los pacientes presentaron recurrencia de la hematuria. La puntuación media de las variables evaluadas en el cuestionario –disuria, frecuencia urinaria y hematuria– fue significativamente menor después del período de seguimiento (p < 0,05).</p><p id="spar0080" class="elsevierStyleSimplePara elsevierViewall">Nuestros datos muestran que cuanto antes se administre la TOHB después del primer episodio de hematuria, se logran mejores tasas de respuesta y se registran menores recurrencias en relación con la hematuria (p < 0,05).
