They are usually congenital and diagnosed in childhood. Other causes are infectious, inflammatory, and iatrogenic.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> They are usually asymptomatic.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> When they present symptoms, they are non-specific and typical of the lower urinary tract, including irritative symptoms, recurrent infections, terminal drip, microhematuria, and hematuria.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The most widespread classification is that of Maizels,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a> which distinguishes 4 types:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">1.</span><p id="par0020" class="elsevierStylePara elsevierViewall">Simple syringocele: distal dilation of the duct communicating with the urethra through a hole that may not be visible in endoscopy.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">2.</span><p id="par0025" class="elsevierStylePara elsevierViewall">Imperforate syringocele: dilation of the duct that does not drain to the urethra, a cystic formation appearing.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">3.</span><p id="par0030" class="elsevierStylePara elsevierViewall">Perforated syringocele: the syringocele communicates through a large orifice, easily visualized at endoscopy. This orifice gives it a diverticular aspect.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">4.</span><p id="par0035" class="elsevierStylePara elsevierViewall">Broken syringocele: the rupture of an imperforate syringocele, appreciating a membrane that separates the light from the syringocele.</p></li></ul></p><p id="par0040" class="elsevierStylePara elsevierViewall">In most cases, the symptoms are mild or the patient is asymptomatic,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> so the treatment is conservative.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> The most widespread intervention for symptomatic cases is endoscopic marsupialization, and there must be ample communication to avoid relapse.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">We present a 14-year-old male with increased diameter of the penile root. In ultrasound, we observed an anechoic and fusiform image of 50<span class="elsevierStyleHsp" style=""></span>×17<span class="elsevierStyleHsp" style=""></span>mm (L<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>AP), intimately related to the bulbar urethra. The genitourinary examination was normal, but the expression of the body of the penis that was sometimes performed at the end of urination attracted attention.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Urethrography was performed showing a narrowing without irregularities of the light of the bulbar urethra, suggesting extrinsic origin. In the retrograde urethrosonography, a smooth urethral wall was seen, without lesions, as well as the cystic cavity that thinned the bulbar urethra. Confirmation of imperforate syringocele of Cowper's gland was performed by retrograde urethrosonography with ultrasound contrast diluted in serum, which ruled out the communication of the cavity with the bulbar urethra (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Traditionally, the diagnosis of syringocele has been made by urethrography and urethroscopy. Retrograde urethrosonography has the advantage of being radiation-free and non-invasive. This technique makes it possible to study the urethral lumen, its wall, the corpora cavernosa, and the spongy. It needs a lubricated probe that is inserted into the urethra, with its tip slightly surpassing the balanopreputial sulcus. This probe is connected to an infusion system with serum placed in a drip. It is required, to keep the urethra relaxed, to clamp the penis in the balanopreputial sulcus with a material that acts as a clamp, to prevent the serum from flowing back to the outside of the penis.
