(C) Operative field view once the Omni-Tract FastSystem® and the wound ring retractor are placed.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Renal Cell Carcinoma (RCC) represents 2–3% of adult malignancies, being the third most frequent and most lethal genitourinary cancer.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> The American Society for Cancer has estimated a total of 73,750 new cases and 14,830 RCC related deaths for 2020.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Currently, most RCC cases are diagnosed incidentally. However, up to 30% of them are still found in locally advanced or metastatic stages given its initial long asymptomatic period.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> This entity, also presents unique proclivy for vascular involvement, extending inside the surrounding veins and giving rise to a unique form of locally advanced disease so-called tumor thrombus, which is present in up to 4–10% of all cases (even reaching the right atrium in 1% of them).<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Despite the prognostic significance of vascular involvement in RCC is still in debate, most authors agree in the absence of tumor thrombus detrimental effect on survival if a complete removal of neoplastic tissue is accomplished. However, higher tumor thrombus levels may correspond to more advanced stages, and thus shorter survival rates and worse prognosis.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In fact, aggresive surgical treatment remains the only potentially curative strategy for these patients, obtaining overall survival rates reaching 68% in the best surgical candidates.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Definitely, vascular involvement greatly increases the technical difficulty of radical nephrectomy(RN) and tumor thrombectomy(TT), given the presence of collateral veins developed in response to inferior vena cava(IVC) occlusion (that may bleed profusely during surgery), and the potential for tumor thrombus dislodgement (that may migrate and embolize in the pulmonary circulation) with surgical excessive manipulation.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Therefore, in spite of the promising results of surgical treatment, the morbidity and mortality rates associated to RN and TT are still non-negligible, with complication rates exceeding 50%, and related-mortality rates reaching 23.5% and 40% for infra- and supradiaphragmatic tumor thrombi, respectively.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–11</span></a> Hence, surgical innovation in this context has pursued a reduction in morbi-mortality, via describing equally oncologically effective but safer approaches to provide the best outcomes to these patients.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,11</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">We have developed a set of surgical maneuvers aiming to gain complete IVC control in cases of RCC and level IIIa tumor thrombus(i.e., extending in the retrohepatic IVC up to the ostium of the major hepatic veins),<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> that would avoid the development of intraoperative PE while preventing massive hemorrhage during the procedure. Our technique is based on the previously described “non-touch technique”,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> through which the major hepatic veins(MHVs) are approached from their anterior aspect before mobilizing the liver for IVC exposure. A step-by-step description of the procedure and its surgical outcomes in an initial series of patients is provided in this report.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0025" class="elsevierStylePara elsevierViewall">RCC and level IIIa tumor thrombus cases that underwent RN and TT at our institution in the period Oct/18–Oct/19 were prospectively included in this series, regardless other demographic, cormorbidity, and oncological features. The diagnosis of IVC involvement was based on tumor thrombus level observed in computed tomography (CT) imaging (always performed in the period of 2 weeks before the intervention). In cases of unclear depiction of tumor thrombus upper limit, magnetic resonance imaging (MRI) or transthoracic echocardiography completed the initial study. A radiologist with expertise in the field was consulted for a thoroughly review of the images until a certain diagnosis was accurately achieved in every case.</p><p id="par0030" class="elsevierStylePara elsevierViewall">TT was always performed after gaining early and complete proximal vascular control using the “Retrohepatic IVC control through an Anterior approach” (RIVCA) technique. The decision of using this technique was made preoperatively, always ensuring enough space (>0.5 mm) between the thrombus craneal limit and the inferior aspect of MHVs ostium (i.e., minimum of space required for Rummel tourniquet placement) (Supplementary Fig. 1).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Surgical technique</span><p id="par0035" class="elsevierStylePara elsevierViewall">The patient is placed in the supine position. Slight anti-Trendelemburg flex of the operating table provides better access to the diaphragmatic domes, increasing the exposure at this level. The abdomen is entered using a J-shaped incision. For those tumors located in the left side, or whenever additional space is required (i.e., bulky mass extending bilaterally), a bilateral subcostal with/without vertical extension (Chevron or Mercedes type) incision is preferred.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Although we have always used an Omni-Tract FastSystem® (Omni-Tract Surgical/Minnesota Scientific, Inc., St. Paul, MN, USA) self-retaining retractor, a different liver surgery retractor (i.e., Rochard or Thompson) would serve perfectly this purpose. The use of a wound-ring retractor (Alexis®-O-Wound/Applied Medical Resources Co., Rancho Santa Margarita, CA, USA) of adequate caliber facilitates the further placement of the self-retaining retractor (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The dissection begins with the division of the round ligament of the liver. The falciform ligament is then divided in craneal direction until the anterior aspect of the hepatocaval confluence is encountered. By doing so, the anterior liver surface is completely exposed thus permitting a thorough exploration for eventual metastases. Using digital disection, the hepatocaval confluence is embraced between the first and index fingers of the surgeon’s dominant hand, and the connective tissue behind the IVC is thinned sufficiently as to feel the contact between the finger’s tips. A right angle clamp (i.e., Guyon clamp) is then gently passed behind the IVC communicating the spaces created with the fingers (in both sides of the IVC), and a cotton band placed at this level, thus providing for complete suprahepatic IVC control. Gentle liver traction in caudal direction permits full visual guidance during the performance of this maneuver. The soft connective tissue located between the left and middle MHVs in the anterior aspect of the hepatocaval confluence, is blunt dissected using the tip of a Metzenbaum scissor (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Once appropriately delimitated, complete circunferential control over left MHV is gained by means of delicate disection maneuvers (otherwise the MHV can be teared and a profuse hemorrhage developed) using a vascular right angle clamp. The left end of the band encircling the IVC is passed behind the left MHV, thus excluding this structure from the initial vascular control obtained. The procedure is now repeated for the middle MHV in a similar fashion. Depending on the configuration of the hepatocaval confluence, sometimes both left and middle MHVs share a single venous trunk entering the IVC. In these cases, the common trunk may be dissected, encircled, and excluded in only one step. The right MHV is also disected, and encircled with a right angle clamp. The right end of the cotton band is then passed between the posterior aspect of the right MHV and the anterior surface of the IVC, thus excluding this structure from the vascular control initially gained (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The placement of a Rummel touniquet on both cotton band ends at this moment, allows the selective control of blood flow in the upper portion of the retrohepatic IVC below the entrance of the MHVs. Closing the tourniquet, the blood stream in the IVC is completely interrupted, while the flow coming from the portocaval natural bypass is left intact, thus preserving cardiac preload with practically no variation (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Once excluded the MHVs from vascular control, a trial of temporary tourniquet closing is made, and the patient is checked for hemodynamic stability. If the patient tolerates flow interruption at the IVC well, then the Rummel tourniquet is left closed, and IVC circumferential dissection is finished by means of “piggy-back” liver mobilization.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a> Briefly, the right triangular ligament is divided and the right lobe of the liver gradually rotated towards the middline. The short veins between the anterior aspect of the IVC and the posterior surface of the right and caudate lobes are ligated and divided. Commonly, these veins are quite engorged due to inadequate drainage (i.e., by IVC occlusion) and must be considered high flow vessels. For safe control, 3-0 silk transfixion sutures (close to the vein take-off in the IVC) may be necessary. Otherwise, bleeding may be controlled with difficulty due to vein stump retraction inside the liver parenchyma. To complete liver rotation, the Makuuchi (i.e., hepatocaval) ligament is divided using a vascular endo-GIA stapler, favoring the exposure of the right MHV, and the Rummel tourniquet occluding the IVC below this level. The retrocaval space is then dissected, and all the venous vessels crossing from the posterior body wall to the posterior surface of the IVC are ligated and divided to gain full circumferential control. Finally, vascular control on the IVC is completed by placing a Rummel tourniquet in the infrarenal IVC below the thrombus caudal limit and the renal vein not affected by the disease.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Variables and ethical issues</span><p id="par0065" class="elsevierStylePara elsevierViewall">Demographic, comorbidity, and disease (imaging and pathological reports) information was recovered from the clinical files of every patient included. For surgical outcomes assesment, intra- and postoperative complications in the period of 30 days after the date of the intervention (and their underlying causes) were recorded and categorized regarding their severity according to the Clavien-Dindo Classification System.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Cardiac output and hemodynamic changes (including hypotension development) during the procedure were monitored. Estimated blood loss (EBL), intra-/postoperative transfusion requirements, and postoperative lenght of stay (LOS) were also registered.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">A total of 6 patients with RCC and level IIIa tumor thrombus were included in this study. Demographic, comorbidity, and disease characteristics of these patients are included in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The RIVCA technique described was used in every case before mobilizing the liver. TT was complete in all cases. In 1 out of 6 cases (16.6%), was not possible to encircle the right MHV due to extremely difficult dissection and high risk for venous rupture. In this case, we opted to proceed with liver mobilization directly, while keeping the exclusion previously gained over the middle and left MHVs. In this case, the cardiac preload was also maintained although the exclusion was incomplete. For subsequent liver mobilization we followed the piggy-back technique previously described.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">In cases of complete IVC obstruction, high suspicion of IVC wall invasion, uncontrollable associated bland thrombus, and enough colateral development verified, we decided to perform a complete circumferential IVC resection between the level of the MHVs ostium and the IVC bifurcation, but preserving the lumbar connecting network with the azygos-hemyazygos system (3/6 patients; 50%). No IVC reconstruction was considered necessary in these cases. Otherwise, the cavotomy was extended from the diseased renal vein ostium over the anterior IVC surface as far as it was required to complete tumor thrombus withdrawal. Thereafter, a tangential cavectomy was performed (i.e., compromising less than 50% of IVC lumen in all cases), and IVC reconstruction finished by means of closing the defect with simple or double running 3-0 Gore-tex CV5® or Prolene® sutures. Main operative characteristics are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Median EBL was 325 cc (range: 250–400 cc). No hemodynamic inestability was registered after IVC clamping in any case. The RIVCA technique did not increase operative time significantly (range: 14–22 min). Blood trasfusion was not required intraoperatively in any of the cases. Median postoperative transfusion rate was 1.3 red blood cell packed (RBCP) units (range: 0-2). There were no cases of intraoperative PE. Overall complication rate (excluding transfusion) was 16.6% (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). No major complications (i.e., Clavien-Dindo III-V) or perioperative deaths were registered. Median postoperative LOS was 8 days (range: 5–11).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">This study includes the description and the preliminary report regarding the outcomes of a surgical technique aimed to increase the safety of RN and TT in cases of RCC and level IIIa tumor thrombus.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Based on the original description provided by Takasaki,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Ozawa,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> and Shimahara,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> and later popularized Lai et al.,<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> the anterior approach to the MHVs, enables liver anatomic segmental resection in difficult oncologic cases without mobilizing the liver as a first step. Using this concept, the RIVCA technique pursues the individualization of each of the MHVs as they enter in the IVC. The main objective is to exclude them from the vascular control gained on the IVC above the tumor thrombus cranial limit (that is mandatory for tumor thrombus withdrawal). In this way, the natural bypass though the portocaval circuit is preserved avoiding the hemodynamic changes secondary to an abrupt decline in cardiac preload threatening patient’s life. At the same time, flow interruption at the IVC prevents the rise of massive hemorrhage and potential embolization of thrombus dislodged fragments into the pulmonary circulation until IVC circumferential control is completed by piggy-back liver mobilization and IVC posterior detachment.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">RN and TT remains a technical challenge for the surgeon due to the occurrence of potentially lethal complications.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Despite intraoperative PE is not frequent (1.5–3.4% of cases), mortality rates are extremely high (60–75% of cases) when this complication occurs. In addition, exanguinating massive hemorrhage has been reported in up to 7.5% of the cases undergoing RN and TT.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Main factors determining the occurence of these complications are associated to the disposition of the surgical field for a particular case, and include the amount of vascular involvement (i.e., tumor thrombus level and degree of IVC occlusion), extent and location of venous collaterals in response to IVC obstruction, surrounding visceral invasion, and presence of associated bland thrombus.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21–23</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">As a general rule, the extent of the dissection required for the control of the vessels involved, is predicated on the location of the upper thrombus limit, dictating the latter, the type and sequence of maneuvers required for its complete withdrawal. Hence, the classification systems used for this purpose are based on the proper identification of the tumor thrombus burden and upper limit location inside the IVC in cross-sectional imaging. Currently, the most widely used surgical classification system is that described by Neves and Zincke in 1987.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> However, in our opinion this system is somehow incomplete to aid in the decision-making process among the different level III tumor thrombus types. Conversely, the classification system described by Ciancio and coworkers<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> provides valuable additional information for accurate surgical planning in these cases. According to this classification system, level IIIa (thrombus upper limit located in the retrohepatic IVC below the ostium of the MHVs) is the easiest scenario among level III tumor thrombi. Ideally, in such cases a vascular clamp should be early placed to interrupt the blood stream in the retrohepatic IVC between the inferior limit of the MHVs ostium and the location of the tumor thrombus cranial end. In this way, it is possible to maintain the natural bypass through the portocaval circuit (and thus the cardiac preload), while massive hemorrhage from the IVC and/or eventual PE and their devastating effects are prevented.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Intraoperative dislodgement and secondary pulmonary embolization of both bland and tumor thrombi fragments, may occur as a consequence of excessive surgical manipulation.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> In level III tumor thrombus cases, IVC exposure requires liver mobilization. Medial rotation of the right liver lobe may twist the right MHV facilitating its accidental tear, thus giving rise to a profuse bleeding in an already stasic liver that may be difficult to control. In addition, the dissection of the anterior surface of the IVC requires the control of the short veins crossing from the right and caudate liver lobes. Gaining enough space to allow ligation and division of these veins usually requires certain degree of traction. Last but not least, the Makuuchi ligament division required to complete liver mobilization may induce the dislodgement of tumor thrombus fragments and the development of a PE.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">The RIVCA technique applied to RCC and level IIIa tumor thrombi meet all the abovementioned goals. In one hand, this technique favors patient hemodynamic stability by keeping inaltered the flow through the natural bypass provided by the portocaval circuit. In the other hand, the vascular control is early achieved, before attempting liver mobilization, thus preventing massive hemorrhage from the IVC or PE secondary to excessive surgical manipulation. The only mandatory condition for its performance is to assure the location of the cranial end of the tumor thrombus below the MHVs ostium, leaving at least 0.5 cm distance to allow the safe placement of the cotton band for the Rummel tourniquet. This condition must be guaranteed preoperatively in cross-sectional imaging. However, sometimes the arterial flow deprivation provided by the ligation and division of the main renal artery of the deseased kidney promotes the loss of turgidity within the thrombus causing its shrinkage, thus making a level IIIb-d to IV tumor thrombus (i.e., upper margin located at the MHVs ostium or beyond up to the right atrium) turn into a level IIIa.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> In these cases, intraoperative transesophageal ecocardiography may help in the intraoperative decision-making process in order to attempt the RIVCA technique.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Surgical innovation has revolutionized the management of RCC with tumor thrombus in the last decades, providing for the aggresive oncological procedures required with increasingly less invasive approaches. Innovation is defined as the introduction of something new, wether an idea, concept, method, or device. Currently, this process is mediated by a sequence of events including original thinking, surgical ingenuity, and rigorous research. While the best of these innovations portrends the potential to induce major shifts in surgical paradigms, most of them do not reach this objective. Although, the latter do not attract the same attention that paradigmatic shifts they are of outmost importance for the evolution and progress of a subspecialty.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Surgical planning is crucial to limit complications. Knowledge regarding the management of complications is important, but the ability to avoid them is prudent and intelligent. Our RIVCA initial series shows no intra- or postoperative major complications with more than aceptable EBL and transfusion rates (325 cc [250–400], and 1 RBCP unit [range:0–2], respectively), enabling us to affirm that this technique may represent a safe approach in cases of RCC and level IIIa tumor thrombus.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Nevertheless, this study is not without limitations. This is an initial series, and therefore the number of cases included is still short. The scarcity of presentation of this entity, even in referral centers, may have prevented a higher volume. In addition, RIVCA technique performance requires a solid expertise in liver surgery. In one of our cases, it was not posible to encircle the right MHV but the maneuver was considered effective even in this scenario. Sometimes, the dissection of the MHVs required may be tedious and difficult. In the face of a probable imminent bleeding, our recommendation is to desist and abort the performance of the complete maneuver maintaining the exclusion gained(i.e., if any) and proceed with liver mobilization by using the transplantation techniques previously described.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a> Despite these limitations, it is our belief that the RIVCA technique may become a helpful adjunct to be added to the surgeon’s armamentarium under the proper experience.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0135" class="elsevierStylePara elsevierViewall">Currently, the ultimate goal of “innovative” strategies in the context of RCC and tumor thrombus is probably to provide for the best patient surgical care by means of decreasing the morbidity and mortality associated to RN and TT. Se registraron prospectivamente los rasgos de la enfermedad, así como las características y los resultados quirúrgicos.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">La nefrectomía radical y la trombectomía tumoral se completaron en todos los casos. La técnica RIVCA no aumentó significativamente el tiempo quirúrgico (rango: 14–22 min). La media de sangrado estimado fue de 325 cc (rango: 250–400). No se requirió transfusión de sangre intraoperatoria en ningún caso. La media de unidades de concentrados de hematíes transfundidos por paciente en el período postoperatorio fue de 1,3 (rango: 0–2). No hubo casos de embolia pulmonar intraoperatoria ni se produjeron complicaciones mayores (Clavien-Dindo III-V) en el período postoperatorio a 30 días.
