They must be commended for the revision of an impressive cohort of BCa patients, 1024 individuals, who were enrolled in randomized control trials (RCTs) with either ORC or RARC between 2000 and 2022.</p><p id="par0010" class="elsevierStylePara elsevierViewall">According to a previous meta-analysis, RARC shows similar rates of overall and major complications and non-inferior surgical safety when compared to the open approach.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Interestingly, the authors report that a RARC is associated with less blood transfusion rates but did significantly longer operative time than ORC.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Fontanet et al. paper summarizes the highest quality of evidence available, and the fact that all of them reported no difference in complication rates is most noteworthy. Although the results were elegantly presented and discussed, we believe that the study presents some issues that deserve further discussion.</p><p id="par0020" class="elsevierStylePara elsevierViewall">First, no one will disagree that the higher evidence so far comes from RCTs. Nevertheless, non-RCTs are also of significant value in drawing conclusions. When RCTs and non-RCTs have been conducted on the same question, both can be included in the meta-analysis after adjustment for the biases, since there is evidence that an observational study can yield less heterogeneity than an RCT for the same therapy.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Indeed, both models can answer the same question with different but nevertheless important approaches.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> In the presented topic, the vast majority of the available data originate from non-RCTs, whereas the available RCTs are not blinded,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> so a meta-analysis of good quality prospective and even retrospective studies with a large number of patients can add significantly to the existing literature. We must remember the case of robotic radical prostatectomy where the available RCT literature remains scarce.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Second, the authors suggest that among patients treated with intracorporeal urinary diversion (ICUD), RARC had a longer length of stay (LOS) than those who underwent open surgery. This is a variable depending on both experience and caseload. For instance, RCT by Catto et al. concluded that RARC led to improved LOS in days,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> specially for patients aged 75 years or older.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Additionally, the choice of the procedure for UD, is an important point in RARC. ICUD reduces the number of incisions and can be faster and more efficient when the surgeon learning curve is completed.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> Indeed, ICUD is being performed more frequently following robotic cystectomy.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> It is possible that intracorporeal diversion may unmask the true benefit of the robotic approach; however, no conclusive data exist in this regard, and we await the longer results of the RARC with ICUD versus ORC randomized trial to throw some light on this debate.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Third, the Enhanced Recovery After Surgery (ERAS) programs increased the benefits obtained by robotic approach by decreasing LOS without increasing the number or severity of complications or 90-day readmissions/mortality.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> The authors could have further explained how the ERAS protocol affected the LOS and complication rates of analyzed RCTs.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Moreover, the rate of neoadjuvant treatment given to patients was low in all the RCTs cited. This is an important finding, as neoadjuvant therapy should be part of the multidisciplinary management of BCa patients undergoing RC now-a-days. In referral centers, collaboration among urologists, oncologists, and all the other specialists should provide rigorous and informative counseling for the administration of neoadjuvant treatments to all eligible patients either as standard-of-care chemotherapy<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> or as experimental protocols within the framework of clinical trials.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Additionally, the authors report as secondary outcomes cancer specific mortality (CSM) and disease recurrence but the time interval at which these was assessed is unclear in the article. 3 RCTs cited analyzes CSM and 4 RCTs the disease recurrence. Overall, the median follows up is might be short to generalize their results. Furthermore, one of their RCTS verified cited does not clearly specify their actual time of follow up. Meta-analysis itself is therefore underpowered and not definitive with respect to any potential differences in survival between ORC and RARC.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Considering other endpoints, not all the studies included evaluated quality of life, and none were blinded, making these results unreliable.</p><p id="par0055" class="elsevierStylePara elsevierViewall">To conclude, we respectfully suggest that RARC and ORC should be equally valid options for the treatment of non-metastatic BCa. As RARC evolves and the experience of surgeons is augmented, new randomized clinical trials with ICUD and longer follow up will be published, providing high-quality data that will surely shed light on this debate.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Furthermore, we should keep in mind systemic treatment and ERAS protocols as the standard of care for patients with BCa who candidates for surgery. As previously stated in the literature, biology underlying the disease is more important than the technique we could deploy for treatment.
