The efficacy of the lithotripsy is based on several parameters such as the size and location of the calculi within the pyelocaliceal system.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> Dretler first introduced the concept of stone fragility,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> the readiness with which a stone is fragmented by SWL varies in stones of different composition. Furthermore, even stones of the same composition may fragment differently.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">4</span></a> In endourology, the efficacy of intracorporeal lithotripters is dependent on stone composition. During endoscopic examinations, two types of calcium oxalate stones can be distinguished on the basis of their morphologic patterns and color – black for calcium oxalate monohydrate and yellow for calcium oxalate dihydrate. Pure monohydrate calculi are extremely dense and resistant, whereas pure or predominantly calcium oxalate dihydrate stones are much more fragile and, consequently, they are good candidates for extracorporeal shock wave lithotripsy.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Saw and Lingeman reported that, adjusted for size, cystine and brushite calculi are the most resistant to SWL, followed by calcium oxalate monohydrate, after which, in descending order of resistance to fragmentation, are struvite, calcium oxalate dihydrate, and uric acid stones.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>In 1996, the “twinkling artifact” was described by Rahmouni et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> This artifact is a color-flow ultrasonography artifact described as being behind calcifications and presenting as a random color encoding in the region were shadowing would be expected on gray-scale images. The twinkling artifact results from complex physical mechanisms related to the irregularity of the surface of calcifications, and the technical parameters used during scanning are not thought to affect it. Some studies have reported that the twinkling artifact may be useful to detect urinary stones.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–9</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Chelfouh et al. performed an <span class="elsevierStyleItalic">in vitro</span> study to determine whether the presence or the absence of the twinkling artifact behind renal calculi is correlated with their chemical composition, with the smoothness of their surface, or with changes in the primary sonography machine settings.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> They declared that an <span class="elsevierStyleItalic">in vitro</span> relationship exists between the twinkling artifact and the morphology of the urinary stones. Color-flow sonography could play a role in detecting dense calcium oxalate monohydrate calculi, which in turn may help predict fragmentability. According to a review of the literature, there is no <span class="elsevierStyleItalic">in vivo</span> study on the relationship between the twinkling artifact and the chemical composition of the stones. This study aims to determine whether the presence or absence of the twinkling artifact is correlated with the chemical composition of the stones.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Material and method</span><p id="par0015" class="elsevierStylePara elsevierViewall">This prospective study included 60 patients with complaints indicating renal calculi and/or hematuria or urinary system stone disease (left/right side pain, inguinal pain, lower-urinary system symptoms) in whose kidneys stones bigger than 0.5<span class="elsevierStyleHsp" style=""></span>cm were detected following first examination at Canakkale Onsekiz Mart University, Faculty of Medicine Urology Clinic between March 2009 and May 2010. Following a physical examination and assessment of laboratory results, all patients were examined with diagnostic radiological imaging methods such as kidney ureter bladder film, intravenous pyelography, non-contrast computerized tomography, and color and spectral Doppler US (Logiq 5, GE Medical Systems, Milwaukee, USA). During the examination, a convex probe of 3.5<span class="elsevierStyleHsp" style=""></span>MHz was used. In cases where echogenicity was detected in the urinary tract, the presence of twinkling artifact was analyzed with a color Doppler test. Since ultrasonography is an imaging technique dependent on individual judgment, the patients were assessed by two radiologists. When the artifact was present but occupied only one portion of the acoustic shadowing, it was considered grade 1, and when the artifact was present and occupied the entire acoustic shadowing, it was considered grade 2 (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1–3</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">According to the density of the patients’ stones detected by non-contrast CT before treatment, they were classified into five groups: below 100<span class="elsevierStyleHsp" style=""></span>HU; between 100 and 400<span class="elsevierStyleHsp" style=""></span>HU; between 400 and 800<span class="elsevierStyleHsp" style=""></span>HU; between 800 and 1200<span class="elsevierStyleHsp" style=""></span>HU; and over 1200<span class="elsevierStyleHsp" style=""></span>HU. According to the results of the assessment, 60 patients were found to have urinary system stones and underwent ESWL and PCNL. The patients with stones smaller than 2<span class="elsevierStyleHsp" style=""></span>cm were treated with SWL, and the patients with stones larger than 2<span class="elsevierStyleHsp" style=""></span>cm were treated with PCNL. The patients who were treated with applied SWL were followed later and asked whether they passed their kidney stones or not. Those subjects who objectively passed their kidney stones or cases in which the stones were removed comprised the study group. The stones obtained from the treatment were analyzed with infrared spectrophotometry. Results were analyzed using SPSS 15.0 for Windows. Assessment of significant difference was performed by chi-square and independent <span class="elsevierStyleItalic">t</span>-tests.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">Sixty subjects (18 females, 42 males) with an average age of 51 (34–70 age range) were managed prospectively. According to the findings, 36% of the subjects had stone in the left kidney, and 24% in the right one. Average stone burden was 29<span class="elsevierStyleHsp" style=""></span>mm (5–42<span class="elsevierStyleHsp" style=""></span>mm). Twenty-one subjects, those with stones smaller than 2<span class="elsevierStyleHsp" style=""></span>cm and without a diagnosis of obstruction in intravenous pyelography were treated with ESWL. Thirty-nine subjects with stones larger than 2<span class="elsevierStyleHsp" style=""></span>cm were treated with PCNL.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Through the ultrasonographic diagnosis carried out before treatment, the stones were classified into three groups in terms of their level of twinkling artifact: 11 subjects in whom no artifact was detected were considered as grade 0; 25 subjects were detected to have grade 1 artifact, and another 24 subjects were detected to have grade 2 artifact. Statistically, a significant relationship was found between the increase in the twinkling artifact and the size of the stones (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). When the patients were distributed into two groups, those with artifact (grade 1<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>2) and those with no artifact (grade 0), no difference was found between the two groups in terms of the size of the stone (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.051) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). When the patients with the twinkling artifact and the patients with no twinkling artifact were compared in terms of stone density measured through CT, statistically no significant difference was found between the two groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.314) (<a class="elsevierStyleCrossRefs" href="#tbl0010">Tables 2 and 3</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">In the cases where the stones were obtained following treatment, the metabolic analysis of the stones found 22 cases were calcium oxalate monohydrate stones; 26 cases were calcium oxalate dihydrate stones; 9 cases were calcium phosphate stones; and 3 cases were uric acid stones. When the relation between the composition of the stones and the twinkling artifact was analyzed, the artifact was detected in nearly all the calcium oxalate dihydrate and calcium phosphate stones, whereas the artifact was detected in more than half of the calcium oxalate monohydrate and uric acid stones (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). No grade 2 artifact was detected in any of these stones. The distribution of the chemical structure of the stones in terms of the twinkling artifact and stone density is summarized in <a class="elsevierStyleCrossRefs" href="#tbl0005">Tables 1 and 2</a>.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Posterior acoustic shadowing was not found in just 3 cases. These cases had stones between 0.5 and 1<span class="elsevierStyleHsp" style=""></span>cm and no twinkling artifact. In 21 cases treated with ESWL, when the twinkling artifact and number of the ESWL treatment sessions were analyzed, it was observed that as the grade of the twinkling artifact increases, the number of required ESWL sessions decreases (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>). Out of 39 patients treated with PCNL, only 6 had stones with smooth surface. Five of these 6 stones were monohydrate stones and no twinkling artifact was found in these cases (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>). The twinkling artifact was found in all the patients with roughly surfaced stones.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Discussion</span><p id="par0045" class="elsevierStylePara elsevierViewall">‘Clean’ and ‘dirty’ acoustic shadows, which are important parameters for the ultrasonographic diagnosis of kidney stones, were first defined by Sommer et al. in 1980.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Clear acoustic shadows are observed behind the structures absorbing sound waves (e.g., stones), whereas dirty acoustic waves are observed behind the structures reflecting the sound waves (e.g., intestinal gases). While diagnosing kidney stones, imaging the clear acoustic shadow supports the diagnosis. However, these clear acoustic shadows cannot always be imaged.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Kimme-Smith et al. have stated that it is extremely difficult to image both stones smaller than 5<span class="elsevierStyleHsp" style=""></span>mm and their acoustic shadows.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> In our study, the acoustic shadows of 3 stones out of 7, measuring between 0.5 and 1<span class="elsevierStyleHsp" style=""></span>cm (42.8%), could not be imaged. There are also some studies indicating that the chemical structure of the stones does not affect or change the acoustic shadow.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> Therefore, in an attempt to better assess the echogenic images which might indicate urinary system stones, researchers have recently started to dwell upon the phenomenon of the twinkling artifact.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The twinkling artifact was first described by Rahmouni et al. in 1996, and until now researchers have suggested different reasons for its formation. Rahmouni et al. found that roughly surfaced compositions creating multiple reflections lead the US beam to diffuse and make it a sound beam with a complex structure. They asserted that this leads to an increase in the wave interval of the reverberating signal, and this is perceived as movement. Aytac and Ozcan stated that this artifact is observed more widely in examinations carried out with new generation US devices with fully digital technology than in examinations carried out with US devices with analogue technology, and that the color sensitivity and acoustic strength of the US device used in the examination has an effect on the formation of this artifact.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Kamaya et al. reported that the formation of the twinkling artifact is largely dependent on adjustments and settings like ‘color-write priority’, gray scale gain and pulse repeating frequency (PRF), and device type. The study in question asserts that the underlying reason for the formation of the artifact is an intrinsic noise in the Doppler circuits of the device, which is called ‘phase jitter’, and reports the rough surface to expand the noise spectrum only secondarily.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The relationship between the chemical composition of the urinary system stones and the twinkling artifact has also been researched. Studies have found that the twinkling artifact is frequently observed in calcium oxalate dihydrate and calcium phosphate stones, and it is hardly ever observed in calcium oxalate monohydrate and urate stones. The twinkling artifact has been reported to help in the detection of the resistant calcium oxalate monohydrate stones, which are of importance in terms of extra-corporeal lithotripsy.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">While calcium oxalate (60%) and calcium phosphate stones (20%) comprise the majority of kidney stones, uric acid (7%), struvite (7%), and cystine (1–3%) stones are observed less often.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> In this study, 80% of the patients had calcium oxalate, 15% had calcium phosphate, and another 5% had uric acid stones.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Based upon the literature review, this is the first study to research the effect of kidney stone density, measured with non-contrast CT, on the formation of the twinkling artifact, and in conclusion, to reveal that there is no relation between stone density and the twinkling artifact. Ulusan et al. have found that there is a strong statistical relation between the absence/presence of posterior acoustic shadow in USG and the size and density of a stone detected in non-contrast CT.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> There are also studies reporting that there is a close relation between the twinkling artifact and posterior acoustic shadowing, and furthermore, that the twinkling artifact has a better diagnostic value in kidney stones.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In light of these former findings, this study might seem contradictory by attributing a lesser diagnostic value and irrelevance to stone density with respect to the twinkling artifact. One of the main reasons for this contradiction might be the fact that the majority of the former studies included stones smaller than 5<span class="elsevierStyleHsp" style=""></span>mm, and that, except for just one <span class="elsevierStyleItalic">in vitro</span> study, none of the studies have examined the chemical components of the stones. Another reason for the difference in the findings might be the USG device used.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> One should never forget that ultrasonography findings might change with the radiologist. In order to reduce this unfavorable effect to the minimum, all cases were assessed by two radiologists together.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Following the <span class="elsevierStyleItalic">in vitro</span> study of Chelfouh et al., this is the second study to research the relation between the twinkling artifact and stone composition. It study examined the twinkling artifact before any treatment was applied to the patients, and it assessed the conformity between the data obtained and the success of ESWL. In the PNCL process, before the surface integrity of the stones was broken down, the surface appearance of the stones was also assessed. This study might be the first in the literature to take these different aspects into consideration. This study investigated the relation between the surface appearance of the PCNL applied stones and the artifact, instead of the stone fragments obtained as a result of the ESWL process as these stones were not able to provide reliable information about their surface appearance. This study supports the findings of Chelfouh et al. and detected the artifact in nearly all the roughly surfaced stones, while no artifact was found in 83.3% of the smooth surfaced stones. As different lithotriptors were used in the PCNL process, these stones could not be assessed in terms of breakability. This study has also found that, in the cases where SWL was applied, as the grade of the artifact increases, the number of SWL sessions necessary for the treatment decreases.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Dretler and Polykoff<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> reported that the radiographic patterns of stones can be correlated with four types of calcium oxalate stones. However, these patterns were found to be reliable only for stones larger than 1<span class="elsevierStyleHsp" style=""></span>cm in diameter. According to Rahmouni, the twinkling artifact with color flow sonography is not limited by the size of the calcification and can be found even when it is sometimes slight and spread out within tissues.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> However, this study found grade 1 artifact in 4 stones out of 7 smaller than 1<span class="elsevierStyleHsp" style=""></span>cm, and no artifact in the other 3 stones. While the average number of ESWL sessions applied to these 3 cases was 3.3, it was 2.2 in the cases where the artifact was detected. At first sight, according to the findings of this study, one might suggest that the twinkling artifact is of no use in stones smaller than 1<span class="elsevierStyleHsp" style=""></span>cm. In the cases where there is no artifact imaging of a stone smaller than 1<span class="elsevierStyleHsp" style=""></span>cm, which is to be treated with applied SWL, its absence can be of clinical use to anticipate that the stone will be more difficult to break.</p><p id="par0080" class="elsevierStylePara elsevierViewall">As for the limitations of this study, 60 patients might seem insufficient. More patients were not included in the study due to reasons such as scarcity of patients admitted to our center; non-volunteering of some patients; disobedience of some patients treated with applied SWL or their failure to bring the stones they passed to our center; or inability to make proper macroscopic analysis of the surfaces of the stones obtained from SWL treatment. Another limitation is the failure to assess the breakability of stones during the PCNL procedure. As we only had a pneumatic lithotriptor in the center at the beginning of the study and later on got an ultrasonographic lithotriptor, we were not able to assess the level of breakability of the stones during the PCNL procedure.</p><p id="par0085" class="elsevierStylePara elsevierViewall">In summary, the roughness of the stone surface is the most determinant factor of the twinkling artifact in kidney stones. It was found that the twinkling artifact detected by color Doppler US in the diagnosis of kidney stones larger than 0.5<span class="elsevierStyleHsp" style=""></span>cm is not superior to posterior acoustic shadowing. However, the twinkling artifact can be of use in anticipating the breakability of the stones in patients to be treated with applied ESWL. One might anticipate that the cases with stone size larger than 2<span class="elsevierStyleHsp" style=""></span>cm but without twinkling artifact are calcium oxalate monohydrate, which is one of the stones with highest level of breakability. Some studies have reported that the twinkling artifact (color-flow ultrasonography artifact) may be useful to detect urinary stones. This study aims to determine whether the presence or absence of the twinkling artifact is correlated with the chemical composition of the stones.</p> <span class="elsevierStyleSectionTitle">Material and method</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Patients with renal stones ≥0.5<span class="elsevierStyleHsp" style=""></span>cm were included in a prospective study. Sixty patients were examined with X-ray film, intravenous pyelography, non-contrast computerized tomography, and color and spectral Doppler ultrasonography. The artifact was considered grade 1 when it occupied only one portion of the acoustic shadowing and when the artifact occupied the entire acoustic shadowing was considered grade 2. Patients with stones smaller than 2<span class="elsevierStyleHsp" style=""></span>cm were treated with SWL and patients with stones larger than 2<span class="elsevierStyleHsp" style=""></span>cm were treated with PCNL.</p> <span class="elsevierStyleSectionTitle">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">No artifact (grade 0) was detected in 11 subjects, grade 1 in 25 and grade 2 in 24. Significant relationship was found between the increase in twinkling artifact and stone size (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). When the relation between the composition of the stones and the twinkling artifact was analyzed, artifact was detected nearly in all of the calcium oxalate dihydrate and calcium phosphate stones, whereas the artifact was detected in more than half of the calcium oxalate monohydrate and uric acid stones. In ESWL group it was observed that as the grade of the twinkling artifact increases, the number of required ESWL sessions decreases (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). En algunos estudios se ha publicado que el artefacto de centelleo (artefacto de ecografía de flujo en color) puede ser de utilidad en la detección de piedras en el riñón. En este estudio se pretende dilucidar si la presencia o ausencia del artefacto de centelleo tendría alguna relación con la composición química de los cálculos.</p> <span class="elsevierStyleSectionTitle">Material y método</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">En un estudio prospectivo se incluyó a pacientes con cálculos renales de ≥ 0,5<span class="elsevierStyleHsp" style=""></span>cm. Se examinó a 70 pacientes mediante rayos X, pielografía intravenosa, tomografía computarizada sin contraste y ecografía doppler espectral y a color. El artefacto se consideró de grado 1 si sólo ocupaba una parte de la sombra acústica, considerándose de grado 2 si ocupaba la totalidad de la sombra. Se trató a los pacientes con cálculos de menos de 2<span class="elsevierStyleHsp" style=""></span>cm con LEOC, y a aquellos con piedras de mayor tamaño se les derivó a tratamiento con NLP.</p> <span class="elsevierStyleSectionTitle">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">No se detectó artefacto alguno (grado 0) en 11 sujetos, detectándose el grado 1 en 25 y el grado 2 en 24. Se encontró una relación significativa entre el aumento en artefactos de centelleo y tamaño del cálculo (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001). Al analizar la relación entre composición de los cálculos y artefacto de centelleo se detectó el artefacto en prácticamente todas las piedras de oxalato de calcio —dihidrato y fosfato de calcio, mientras que en el caso de los cálculos de oxalato de calcio— monohidrato y ácido úrico se detectó el artefacto únicamente en algo más de la mitad de ellos. En el grupo LEOC se observó que, al aumentar el grado del artefacto de centelleo, descendía el número de sesiones de LEOC necesario (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001). En el grupo NLP se encontró el artefacto de centelleo en todos los pacientes (100%) con piedras de superficie irregular.</p> <span class="elsevierStyleSectionTitle">Conclusión</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">La irregularidad de la superficie lítica es el factor más influyente en la formación del artefacto de centelleo en piedras de riñón. Dicho artefacto puede tener utilidad a la hora de prever la predisposición a la fragmentación de los cálculos en pacientes derivados a tratamiento con aplicación de LEOC.
