Therefore, the analysis of its influence on any disease should be conducted jointly.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Disease prevention through lifestyle modification is becoming increasingly important. Therefore, the search for scientific evidence to justify the impact of certain healthy habits is important.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,7</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The aim of this study was to analyze the influence of sedentary life and overweight, modifiable lifestyle circumstances, on the risk of PC detection and its aggressiveness in a cohort of men undergoing prostate biopsy.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0030" class="elsevierStylePara elsevierViewall">2408 men undergoing prostate biopsy (PB) between January 2006 and December 2011 due to elevated serum PSA (91%) or abnormal digital rectal examination (9%) were included. Patients undergoing treatment with 5 ARI were excluded. All patients were informed about the procedure and informed consent was obtained. The clinical characteristics of the series are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methods</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Prostate biopsy</span><p id="par0035" class="elsevierStylePara elsevierViewall">It was performed transrectally by means of ultrasound assistance. 10 additional punctures and between 2 and 8 additional ones were conducted, according to age and prostate volume following the modified Vienna nomogram.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> 77% of the PBs were initial 23% and repeat. PC was detected in 848 men (35.2%).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Assessment of tumor aggressiveness</span><p id="par0040" class="elsevierStylePara elsevierViewall">It was assessed based on the Gleason score, estimating high grade (HG) when it was greater than 7. Tumor aggressiveness was also assessed according to the risk of D’Amico,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> estimating high risk (HR) when: cT<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>3a or PSA<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>20<span class="elsevierStyleHsp" style=""></span>ng/ml or Gleason score<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>7. In 240 patients (28.3%) the PC was HG and in 297 (35%) it was HR.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Assessment of physical activity and body mass index</span><p id="par0045" class="elsevierStylePara elsevierViewall">All the patients were surveyed about their overall physical activity (PA), classified as sedentary (SE) and with moderate to high physical activity (non-SE). A total of 1264 patients (52.5%) were classified as SE, 768 (31.9%) had moderate activity and 376 (15.6%) intense activity. The body mass index (BMI) was calculated with the height and weight information obtained in the same survey. A total of 556 patients (23.1%) had a normal BMI (<25), 1200 (49.8%) were overweight (BMI: 25–29.9) and 652 (27.1%) were obese (BMI: ≥30).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Statistical analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">The quantitative variables were expressed as medians and range. The qualitative variables were expressed as percentages. Comparison between quantitative variables was performed using the <span class="elsevierStyleItalic">U</span> test of Mann–Whitney and between qualitative variables by means of the Chi-square test. The PA variable was dichotomized between SE vs. non-SE (moderate to severe PA) and the BMI between normal weight vs. OW (overweight or obesity). The analysis of independent predictors of PC and tumor aggressiveness was performed by means of binary logistic regression analysis that incorporated as covariates: age, DRE, PSA, PA, and BMI. Hazard ratios (HR) and 95% confidence intervals (95% CI) were estimated. The analysis was performed with the SPSS 20 statistical package.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall">The overall rate of patients with SE was 52.5% and the overall rate of OW patients was 49.8%. A significant association between SE and OW was observed, so a smaller PA, represented by SE, was inversely correlated with greater BMI, represented by OW, HR 0.688 (95% CI: 0.569–0.832). The chance of OW in a SE patient was 42 vs. 34.9% in one non-SE patient, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001.</p><p id="par0060" class="elsevierStylePara elsevierViewall">In order to analyze the relation between the PA and BMI with the detection of PC and its aggressiveness, several univariate analyses were conducted. A significant association between PA and risk of PC detection was observed. The PC rate was 36.7% in SE patients vs. 33.6% in non-SE patients, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.049. By contrast, we found no significant association between BMI and risk of PC detection. The PC rate was 38.1% in patients with normal BMI compared to 34.6% in OW patients, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.056 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Tumor aggressiveness was clearly associated with the BMI, so that a higher rate of HR and HG tumors was detected in OW patients. The HG tumor rate was 31.4% in OW patients vs. 18.9% in patients with normal BMI, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001. The HR tumor rate was 39.2% in OW patients vs. 22.6% in patients with normal BMI, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The association between tumor aggressiveness and PA was not so clear. The HG tumor rate was 29.3% in patients with SE vs. 27.1% in patients with moderate or high PA, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.261. In contrast, the rate of HR tumors was 39.7% in patients with SE vs. 29.4% in patients with moderate or high PA, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">In order to assess whether the PA or BMI would be independent predictors of the risk of PC detection and its aggressiveness, different binary logistic regression analyses were carried out (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). In predicting the risk of PC we found that age, digital rectal examination, serum PSA, and PA were independent predictors and not the BMI. The HR for the detection of PC in an active patient vs. a sedentary patient was 0.791 (95% CI: 0.25–0.999); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.049. With regard to tumor aggressiveness, a first analysis was performed to predict the risk of detection of a HG tumor. Age, serum PSA, digital rectal examination, PA and BMI were independent predictors. The HR of a patient with SE compared to an active one was 0.571 (95% CI: 0.456–0.759); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001. The HR of a patient with respect to another one with normal BMI was 1.635 (95% CI: 1.070–2.497); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.023. A second analysis was performed to predict the risk of detecting HR tumors. The HR of a patient with SE compared to another active one was 0.519 (95% CI: 0.349–0.771); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001. The HR of an OW patient compared to another one with normal BMI was 1.998 (95% CI: 1.282–3.115); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">This study shows, first, that there is a relation between physical activity and the degree of obesity. The proportion of overweight patients among sedentary patients was 42 vs. 35% among non-sedentary ones. Secondly, it also shows that over 50% of men with risk of PC were sedentary, and that their rates of overweight and obesity exceeded 50 and 25% respectively. These data suggest that the modification of lifestyle habits related to diet and physical activity might be relevant because of the high frequency of sedentary life and overweight.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">By simultaneously analyzing the influence of the PA and the degree of obesity on the risk of PC detection and its aggressiveness, we have observed that while sedentary life was associated with increased risk of PC detection, sedentary life and overweight were associated with increased risk of aggressive tumors.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The influence of PA on the risk of PC and its aggressiveness has been analyzed in 3 large population and prospective studies conducted in the United States<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> and in Europe.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> In the 3 of them we found a protective effect of exercise on tumor aggressiveness, but not of the risk of PC detection. A recent meta-analysis including 88,294 men suggests a slight association between physical activity and PC risk. However, the same meta-analysis found no association between physical activity and tumor stage. In order to discriminate the association between sedentary life and obesity, the authors of this meta-analysis examined the influence of PA, stratifying patients according to their BMI, concluding that sedentary life and obesity were independent factors. Therefore, they suggest that a sedentary man would have a higher risk of being diagnosed with PC regardless of his BMI.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> In another study with a similar design to ours, Antonelli et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> analyzed 190 men undergoing prostate biopsy of which more than half were sedentary. The authors found that moderate exercise was associated with lower risk of PC detection and also to lower Gleason score. This study did not consider the influence of the degree of obesity. The biological mechanisms through which physical activity can reduce the risk of PC are not clear.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> However, it was found how PA can reduce endogenous levels of testosterone, serum insulin, and insulin-like growth factor IGF-1, whose impact on prostate carcinogenesis is well-known.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Moreover, physical exercise would improve the immune function and antioxidant defense mechanisms, which may also influence the risk of PC.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Currently, there is abundant literature analyzing the influence of obesity on the risk of PC.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Three meta-analyses have reported a positive association between obesity and risk of PC.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–18</span></a> However, the findings of each of the studies included in these meta-analyses differ drastically, and while some report that obesity is a risk factor, others find a protective effect. However, the diversity in the sample size of these studies should be considered, and it should be noted that a study conducted in Norway contributed with more than 50% of cases in 2 of the 3 meta-analyses,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> and one carried out in Sweden contributed with more than 50% of the cases in the other meta-analysis.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> A recent meta-analysis, conducted in the U.S., found an association between obesity and increased risk of advanced disease.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> The mechanisms by which obesity might be associated with an increased risk of PC or greater aggressiveness are multiple.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Insulin and its axis with the insulin-like growth factor IGF-1 are widely documented in prostate carcinogenesis.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> Decreased testosterone levels, associated with obesity, have been associated with a phenotype of increased tumor aggressiveness.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> The increase of estradiol linked to obesity, due to the aromatization of testosterone in adipocytes, has also been associated with development and progression of PC.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> The decrease in adiponectin linked to obesity has been related to a phenomenon of migration and it appears to be associated with an increased incidence of spread PCs.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> Elevated levels of interleukin IL-6 also appear to be associated with greater aggressiveness<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> and, eventually, it has also been suggested that obesity might be associated with the genetic signature myriad of PC.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The findings of our study are consistent with the above mentioned ones. We think having jointly considered PA and the degree of obesity has added value, since this has allowed us to confirm the protective effect of exercise on the risk of PC detection and its aggressiveness, while overweight would only be related to the aggressiveness of PC. Concerning the design of our study, in a cohort of men with risk of PC undergoing prostate biopsy, another occasion has been reported in which only the influence of physical activity was analyzed and the conclusions were similar.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> This type of design has the advantage that the suspected diagnosis is established with uniform criteria, although it may have the limitation of the influence of the predictors, obesity and PA, on PSA levels.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusions</span><p id="par0100" class="elsevierStylePara elsevierViewall">We suggest that sedentary life appears to increase the risk of detection of PC and that both sedentary life and overweight would be associated with increased tumor aggressiveness.
