A recent Cochrane metanalysis comparing the MRI pathway to systematic PBx, observed that the MRI pathway outperformed systematic PBx by detecting 12% more csPCa. This advantage increases to 44% when considering only patients with a previous negative PBx, but decreases with biopsy naïve patients to 5%.16 This might explain the current stronger recommendation for performing MRI prior to repeat PBx than for biopsy naïve patients.12

In our study mpMRI was performed before every repeat PBx.
