It is estimated that erectile dysfunction affects nearly 30% of men between 40 and 79 years old,<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">2</span></a> and more than one-third of men will experience premature ejaculation at some point in their lives.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">3</span></a> Peyronie's disease and hypogonadism affect nearly 6% of men,<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">4,5</span></a> and about 15% of couples have fertility issues.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Due to cultural constructs of masculinity, men tend to be more reluctant than women to consult their healthcare provider.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">6</span></a> In the particular case of sexual health disorders, shame and the perceived lack of privacy commonly act as barriers to seeking professional help, leading to low consultation rates in erectile dysfunction, premature ejaculation, Peyronie's disease, and symptoms associated with hypogonadism.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">7</span></a> The revolution of digital technologies offers now a scenario of privacy in which men can easily access a vast amount of information about almost every single health issue. It is not surprising, then, that digital initiatives and resources related to men's sexual-related disorders flourish day by day.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Objectives</span><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this review was to explore the digital environment of men's sexual disorders, defined as all digital resources used by patients and physicians (or researchers) to provide or retrieve information, communicate, and sell or purchase products and services. To achieve our goal, we established two main objectives: (1) to assess the scope of the scientific research related to the use of digital technology in men's sexual healthcare and (2) to analyze the primary sources of digital information related to this field, paying particular attention to the accuracy and quality of information.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Acquisition of evidence</span><p id="par0020" class="elsevierStylePara elsevierViewall">The review consisted of three searches: (1) a systematic search of the scientific literature, (2) an internet search, and (3) a search of mobile applications (apps).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The review of the scientific literature was performed on PubMed, using the following keywords: ERECTILE DYSFUNCTION and WEB, ERECTILE DYSFUNCTION and APP, PREMATURE EJACULATION and WEB, PREMATURE EJACULATION and APP, PEYRONIE and WEB, PEYRONIE and APP, MALE HYPOGONADISM and WEB, MALE HYPOGONADISM and APP, INFERTILITY and WEB, and INFERTILITY and APP. All articles with an English abstract available were considered for eligibility, with no restrictions regarding the publication date. After individual assessment of the content, articles not dealing with men's sexual or reproductive health were excluded from the literature review. The selected articles were allocated into one of the following categories: (1) online survey, including studies in which digital resources were used to recruit subjects and/or retrieve data for research purposes, (2) digital treatment, including studies investigating online consultation, (3) quality and safety of the digital healthcare environment, (4) digital activity, including studies assessing the extent of use of digital resources, (5) patient empowerment, including manuscripts dealing with patient information, education, and self-management (excluding treatment), and (6) online drug selling, including articles dealing with the online acquisition of therapeutic products. Although all selected articles were considered for the assessment of scope and population, those considered of greater quality regarding the study design and/or greater interest regarding the outcome were used for the description of the results.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The internet search was conducted on <a id="intr0015" class="elsevierStyleInterRef" href="http://www.google.com/">www.google.com</a> on 1st February 2016, using the following search terms: ERECTILE DYSFUNCTION, PREMATURE EJACULATION, MALE HYPOGONADISM, PEYRONIE, and INFERTILITY. Since most of the traffic is focused on the highest ranking search results, we analyzed the first 10 websites resulting from each search; advertising sites were excluded. Websites were grouped according to the searched terms, and data regarding the presence of healthcare institutions, healthcare professionals, product selling, pharmaceutical companies, advertising, references, links, and certifications were extracted from each site. The outcome of this analysis was used to rate the overall quality of the website on a 5-point Likert scale as very poor, poor, fair, good or very good.</p><p id="par0035" class="elsevierStylePara elsevierViewall">A parallel search was performed on the widely used repository of apps, play.google. Like the Internet search, we used the search terms ERECTILE DYSFUNCTION, PREMATURE EJACULATION, HYPOGONADISM, PEYRONIE, and INFERTILITY. The selected apps were investigated for the presence of identified healthcare institutions, identified healthcare professionals, in-app selling, and pharmaceutical companies. The users’ rating, the payment/free type, and the total downloads were also recorded. Based on these indicators, the overall quality of the app was rated on a 5-point Likert scale as very poor, poor, fair, good or very good.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Synthesis of evidence</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Literature review</span><p id="par0040" class="elsevierStylePara elsevierViewall">The initial PubMed search yielded 148 references related with erectile dysfunction, premature ejaculation, hypogonadism, and Peyronie, and either apps or web. Three references were excluded for containing non-English abstracts. Of the 145 references assessed for eligibility, 9 were excluded for including only female populations, and 20 were excluded for dealing with topics not related to men's sexual health, leading to a final sample of 116 manuscripts, which were included in the literature review (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> summarizes the number of manuscripts allocated in each scope and the main populations included. The full list of references corresponding to each scope is provided in <a class="elsevierStyleCrossRef" href="#sec0095">Supplementary material</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Online survey</span><p id="par0045" class="elsevierStylePara elsevierViewall">Due to the challenge of recruiting subjects for research in medicine areas with low consultation rates or privacy concerns, the online survey has become an important alternative to the traditional methodology based on face-to-face follow-up visits. The use of online surveys, now enhanced with the arising of social media,<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">8</span></a> has enabled the study of sexual disorders such as premature ejaculation or erectile dysfunction on sample populations over one thousand men.<a class="elsevierStyleCrossRefs" href="#bib0325"><span class="elsevierStyleSup">9–12</span></a> Although the primary endpoints of studies based on online surveys lie most often on epidemiology and patients’ sociodemographic characteristics, this methodology has been successfully used for the indirect assessment of treatment efficacy by investigating the patients’ satisfaction with pharmacological treatments for erectile dysfunction.<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">13,14</span></a> Similarly, in the field of men-who-have-sex-with-men, online surveys have provided useful data on disease prevalence and risk behaviors in large study samples (up to 91,477 participants), unfeasible using the traditional face-to-face survey.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">15</span></a> Finally, in the infertility field, the aims of the online, survey-based studies include not only the investigation of couples’ perceptions and concerns<a class="elsevierStyleCrossRefs" href="#bib0360"><span class="elsevierStyleSup">16,17</span></a> but also the habits of sexual healthcare specialists in their routine clinical practice,<a class="elsevierStyleCrossRefs" href="#bib0370"><span class="elsevierStyleSup">18,19</span></a> and the retrospective assessment of in vitro fertilization treatments.<a class="elsevierStyleCrossRefs" href="#bib0380"><span class="elsevierStyleSup">20,21</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">The reliability of the online-retrieved data was investigated in the online administration of the Sexual Health Inventory for Men, a validated instrument for assessing sexual health.<a class="elsevierStyleCrossRef" href="#bib0390"><span class="elsevierStyleSup">22</span></a> The study, which compared responses of patients completing the questionnaire in person to those of patients completing it via email, found a strong correlation between groups and a high consistency in the online responses.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Digital treatment</span><p id="par0055" class="elsevierStylePara elsevierViewall">Digital treatment strategies have arisen as a new healthcare setting and provide patients and healthcare professionals with a new scenario for a relationship. Like face-to-face consultation, digital treatment interventions often start with a self-administered questionnaire, provided either by e-mail or by registration on a website specifically designed for this purpose. In addition to offering a more private environment, particularly relevant in men experiencing embarrassment for their sexual dysfunctions,<a class="elsevierStyleCrossRef" href="#bib0395"><span class="elsevierStyleSup">23</span></a> digital treatment strategies are more cost-effective than traditional, face-to-face interventions.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">24</span></a> However, there is no consensus on the actual efficacy of digital treatments, which might depend largely on the baseline sociodemographic characteristics of patients.<a class="elsevierStyleCrossRefs" href="#bib0400"><span class="elsevierStyleSup">24,25</span></a> Thus, although some authors have reported a high efficacy of these interventions in counseling and management of infertility and sexual dysfunctions,<a class="elsevierStyleCrossRefs" href="#bib0405"><span class="elsevierStyleSup">25–27</span></a> there is little homogeneity among studies, and some authors have noticed high dropout rates.<a class="elsevierStyleCrossRefs" href="#bib0405"><span class="elsevierStyleSup">25,27</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">Beyond the application of digital resources to the individual management of sexual health conditions, the use of social media and/or smartphones has shown promising benefits in preventive medicine in the field of sexual healthcare.<a class="elsevierStyleCrossRef" href="#bib0420"><span class="elsevierStyleSup">28</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Quality/safety of the digital healthcare environment</span><p id="par0065" class="elsevierStylePara elsevierViewall">Due to the potential risk of bias associated with the lack of an editorial review on Internet-based information, the quality and reliability of knowledge disseminated through the Internet is a matter of major concern. The risk of biased information is particularly high when either website owners or article authors have conflicts of interest related to the information provided; still, the disclosure of conflicts of interest has not been undertaken as a regular practice in the digital environment. Amber et al. explored two important online support websites, <span class="elsevierStyleItalic">UptoDate</span> and <span class="elsevierStyleItalic">Dynamed</span>, and found that all articles in <span class="elsevierStyleItalic">UptoDate</span> but not those in <span class="elsevierStyleItalic">Dynamed</span> disclosed the financial relationship between authors and companies whose products were mentioned in the article.<a class="elsevierStyleCrossRef" href="#bib0425"><span class="elsevierStyleSup">29</span></a> Although the presence of a conflict of interest does not necessarily compromise the accuracy of information, Red et al. showed a significantly greater bias toward biological factors and drug treatment in industry-funded websites about erectile dysfunction.<a class="elsevierStyleCrossRef" href="#bib0430"><span class="elsevierStyleSup">30</span></a> Furthermore, Waack et al. found a bias in safety information of most industry-funded sites on erectile dysfunction, which contained general information about adverse events and contraindications but did not give actual percentages or referenced their materials; information on treatment alternatives was also omitted.<a class="elsevierStyleCrossRef" href="#bib0435"><span class="elsevierStyleSup">31</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">In addition to the potential bias of websites dealing with pharmacological treatments, some authors have noticed the lack of accuracy in information regarding procedures or interventions with potential impact on sexual health. Mulhall et al. analyzed 90 websites of medical centers offering services of radical prostatectomy (either open or robotic) and found that only half of them mentioned erectile dysfunction.<a class="elsevierStyleCrossRef" href="#bib0440"><span class="elsevierStyleSup">32</span></a> The authors also concluded that overall information was accurate in 17% and 30% of robotic and open prostatectomy websites, respectively. A similar approach was followed by Merrick and Marriott<a class="elsevierStyleCrossRefs" href="#bib0445"><span class="elsevierStyleSup">33,34</span></a> who analyzed the quality of online information about fertility in men diagnosed with cancer. Besides low accuracy, which varied widely among different websites, both studies concluded that most information was presented in a language difficult to understand by plain readers, with a very poor visual presentation.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In the field of fertility, several authors have also warned about the lack of accuracy of online information. Okamura reviewed 197 fertility websites and found that only 2% of them met the JAMA's minimal core standards for responsible print.<a class="elsevierStyleCrossRef" href="#bib0455"><span class="elsevierStyleSup">35</span></a> In fact, several authors have pointed out important disadvantages of digital strategies in the fertility field, stressing that fertility clinic websites are a form of advertising, rather than a channel for disseminating rigorous information on reproductive health.<a class="elsevierStyleCrossRefs" href="#bib0460"><span class="elsevierStyleSup">36,37</span></a></p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Digital activity</span><p id="par0080" class="elsevierStylePara elsevierViewall">Our literature search yielded very few articles dealing with the extent of Internet use among men affected by sex health conditions. Davis et al. performed a thorough analysis of the number of websites and google searches for “erectile dysfunction” recorded between January 2005 and December 2011.<a class="elsevierStyleCrossRef" href="#bib0470"><span class="elsevierStyleSup">38</span></a> The study showed that Internet activity experienced a significant and progressive annual increase since 2007, whereas the number of web pages began to raise in 2007 and experienced an exponential growth between 2010 and 2011.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The high level of digital activity has stimulated the transformation of physicians and healthcare centers’ websites into real markets. Speier warned on the increasing number of couples that look on the Internet for solutions to their fertility problems and the high proportion of searches that end with couples traveling abroad for cheaper fertility treatments in foreign clinics that attract customers worldwide using the Internet.<a class="elsevierStyleCrossRef" href="#bib0475"><span class="elsevierStyleSup">39</span></a></p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Patient empowerment</span><p id="par0090" class="elsevierStylePara elsevierViewall">Patients with sexual health conditions often have to deal with medical decisions they feel incompetent for, causing distress and leading to unsupported choices. Internet-based educational programs offer a channel for training patients and assisting them in medical decisions, increasing patient empowerment. Diefenchach et al. analyzed the impact of an Internet/CD-ROM-based education program in patients with prostate cancer and found that the program improved key decision-making variables and reduced the emotional implications of a difficult medical decision.<a class="elsevierStyleCrossRef" href="#bib0480"><span class="elsevierStyleSup">40</span></a> Likewise, Symonds et al. observed that patients with erectile dysfunction were able to assess their own suitability for treatment with 50-mg sildenafil after being provided with information about PDE5i treatments.<a class="elsevierStyleCrossRef" href="#bib0485"><span class="elsevierStyleSup">41</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">In the field of fertility, offering expert forums to couples has proven to be a useful complement to usual care in both providing information and addressing patients’ concerns, with a positive impact on healthcare quality<a class="elsevierStyleCrossRefs" href="#bib0490"><span class="elsevierStyleSup">42,43</span></a> and cost reduction.<a class="elsevierStyleCrossRef" href="#bib0500"><span class="elsevierStyleSup">44</span></a> Although some patients have highlighted negative aspects of online communities on fertility,<a class="elsevierStyleCrossRef" href="#bib0505"><span class="elsevierStyleSup">45</span></a> a randomized controlled trial specifically assessing the patients’ perception about online management concluded that these programs are overall well perceived and succeed in reducing levels of both distress and depression.<a class="elsevierStyleCrossRef" href="#bib0510"><span class="elsevierStyleSup">46</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">Despite the actual and potential benefits of digital resources in patient empowerment, the vast amount of information available on the Internet is a double-edged sword which might hamper the decision-making process. In this regard, Heart et al. observed that, although the use of the Internet improved the patient–practitioner relationship, many patients were overwhelmed by the excess of information, which prevented them from making informed decisions about their care.<a class="elsevierStyleCrossRef" href="#bib0515"><span class="elsevierStyleSup">47</span></a></p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Online drug selling</span><p id="par0105" class="elsevierStylePara elsevierViewall">One of the major drawbacks of the digital transformation is the limited control over drug access. While the purchasing of natural products has been considered convenient – despite the advice for cautiousness with information regarding safety and efficacy<a class="elsevierStyleCrossRef" href="#bib0520"><span class="elsevierStyleSup">48</span></a> – drug selling seems to jeopardize the patients’ health seriously. As Cordaro et al<a class="elsevierStyleCrossRef" href="#bib0525"><span class="elsevierStyleSup">49</span></a> concluded after analyzing websites about anabolic-androgenic steroids, selling sites have “<span class="elsevierStyleItalic">misleading information and deceiving practices”</span>. However, more than providing deceitful information, online drug selling is potentially hazardous because of the lack of guaranteed safety of the products sold. An example of this hazard is the finding of non-allowed substances such as sildenafil, vardenafil, tadalafil, and testosterone in cosmetic creams sold on the Internet with promising results on erectile dysfunction.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Among all products commercially available on the Internet, sildenafil has become a leading brand in online selling (as an example, at the moment of preparing this review, the Google search using the combined terms “Viagra” and “purchase” yielded more than thirty thousand results). Gallagher and Chapman analyzed the first 100 Google hits for searches related to the supply of either Viagra or generic versions containing sildenafil, and found that only 6.8% were legitimate online pharmacies, indicating a serious safety concern with the online acquisition of Viagra tablets.<a class="elsevierStyleCrossRefs" href="#bib0530"><span class="elsevierStyleSup">50,51</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Finally, it is worth mentioning that the unsafe environment of online selling has an important exception in the state-regulated Internet prescribing system (e-medicine), which has proved to be overall safe and effective, even in studies specifically assessing PDE5i drugs.<a class="elsevierStyleCrossRefs" href="#bib0540"><span class="elsevierStyleSup">52–54</span></a></p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Digital surfing</span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Websites</span><p id="par0120" class="elsevierStylePara elsevierViewall">The searched terms yielded an average of 10,619,200 Google results, being “erectile dysfunction” and “infertility” the terms retrieving a greater number of sites. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> summarizes the ownership and presence of quality indicators in the websites included in our analysis. Overall, 31.6% and 51.6% of websites belonged to a healthcare institution or a healthcare professional, respectively. Regarding the commercial characteristics, 23.3% of sites sold products or services, and 46.7% displayed advertisements. Only 33.3% of sites provided relevant references or external links, and 55.0% exhibited a seal of quality from an external validator. Pharmaceutical companies were present in 26.6% of sites.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">According to the reviewers’ criteria, 29 (58.0%) websites were rated as good (13 sites) or very good (16 sites), whereas the rest (42%) had score 3 or lower on the 5-item Likert scale.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Mobile apps</span><p id="par0130" class="elsevierStylePara elsevierViewall">Our search yielded 40 apps, with an average of 19,426 downloads. The characteristics of the apps are summarized in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The most downloaded apps were those dealing with hypogonadism, erectile dysfunction, and premature ejaculation. The identity of the health center or the healthcare professional appeared in only 1 (2.5%) and 2 (5.0%) of the apps, respectively. Users rated the apps with a mean score of 3.79 on a 5-point scale. No app showed a seal of quality from an external validator, a center, a scientific society, or any other source.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Discussion</span><p id="par0135" class="elsevierStylePara elsevierViewall">The digital revolution has changed the way we communicate, the way we generate and access knowledge, and the way we relate to each other; in other words, the way we live our lives. The healthcare field is not immune to this change, and it is being transformed in a revolutionary manner by the Internet, social media, apps, wearable devices, and an endless list of digital strategies and concepts which make the healthcare system evolve toward unknown scenarios. Overall, the potential contribution of digital resources in healthcare and medical research is beyond doubt. Applications identified in this review such as transmitting information to – and retrieving scientific data from – a huge amount of people in an almost costless way are some examples of a list of advantages that grows day by day.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The particular field of men's sexual healthcare is strongly influenced by two important factors not affecting other medical areas: (1) the psychological barrier – arising from cultural constructs of masculinity –, which prevents many patients from seeking help, and (2) the lack of education in sexual field among general healthcare professionals, which often leads to disregard treatable sexual dysfunctions. Digital resources make it possible to overcome these limitations by providing patients with anonymity and disseminating knowledge worldwide, regardless of the characteristics of the healthcare system and the educational interventions implemented in each country. Nonetheless, in addition to the advantages in sexual healthcare, both anonymity, and availability of information have prompted the use of the Internet as a big market in the field of men's sexual disorders, leading to a large growing of online resources in this field. The results of our Google searches revealed the presence of more than 20 million of sites covering erectile dysfunction, more than 2 million covering premature ejaculation, and more than 1 million dealing with Peyronie's disease and testosterone; the field of fertility exceeded 25 million of sites. However, our quality analysis shows a worrisome percentage of websites (42%) not covering minimum quality standards such as the identification of the author or institution providing the information, the presence of references, or the presence of a quality seal supporting the accuracy of the published information. These findings should be read on the basis that we only analyzed the top-10 sites of every search, as we considered them the most visited ones. Other authors performing a more thorough assessment of the quality of websites dealing with specific fields of sexual health found poorer standards of quality.<a class="elsevierStyleCrossRefs" href="#bib0440"><span class="elsevierStyleSup">32,35–37</span></a> The low quality seems to be related to the overall commercial tone of most sites, which either sell products or services or receive funding from advertisements or pharmaceutical companies sponsorship. It is worth mentioning that some scientific societies are making remarkable efforts for providing patients with an accurate source of information (e.g. the Internet portal of the European Association of Urology).<a class="elsevierStyleCrossRef" href="#bib0555"><span class="elsevierStyleSup">55</span></a> However, none of these websites came up in the first ten results of our Internet search.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The development of apps has a remarkably recent trajectory. Thus, it is not surprising that our search yielded only 40 apps dealing with men's sexual disorders. More than in the website search, we found poor quality standards on apps, with only 7.5% of them identifying a center or a professional supporting the app and none of them reporting any quality seal. These results may indicate that healthcare institutions and centers are still missing the potential of reaching patients through this technology, and therefore the apps arena is currently conquered by commercial initiatives.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Our review of the scientific literature revealed a remarkable unbalance between manuscripts using digital resources for research purposes, which accounted for nearly half of all manuscripts, and those dealing with other topics such as digital treatment, quality and safety of the digital healthcare environment, patient empowerment, and online drug options. This finding suggests that medical researchers do take advantage of the potential of digital technology for clinical research, but they might underestimate the potential of digital resources for improving the healthcare quality (i.e. patient empowerment and digital treatment). Accordingly, the low percentage of manuscripts dealing with online drug options and the quality and safety of online information indicates that the medical research community is barely watching the potential drawbacks of digital technology in the field of men's sexual health. Another issue missed in the articles retrieved was confidentiality in patient–practitioner relationship. Again, the digital world has moved more quickly than laws, and while data protection is widely covered by regular patient–practitioner relationship, most countries and scientific societies lack specific legislation regarding confidentiality in the digital scenario. Fortunately, the rising interest in providing patients with a framework for a safer Internet browsing has prompted remarkable initiatives in this direction.<a class="elsevierStyleCrossRefs" href="#bib0555"><span class="elsevierStyleSup">55,56</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">Our review is limited by the search terms used. This restriction is particularly important in the Google search, as websites are not indexed in the same way as scientific articles. We performed searches based on key terms and analyzed the ten first results assuming they would encompass most of the Internet traffic related to sexual health topics. However, the way our patients surf the Internet is unpredictable and the actual environment they face when seeking help on the Internet could have even poorer quality. Another limitation in this regard is the use of a single search engine and app repository, which may not cover all searches in some countries. However, considering the difficulties of balancing the Internet surfing behaviors worldwide, we chose the most used search engine. In the case of the app repository, our previous experience treating patients potentially interested on sexuality-related apps suggested that Google Play was most frequently chosen as an app source.</p><p id="par0160" class="elsevierStylePara elsevierViewall">We feel that we are facing a unique opportunity to develop a new model of relationship in the men's healthcare field, and we are currently living a critical moment in which our actions may have significant consequences in the building of this new relationship. To the light of our findings, the construction of this new scenario deserves some considerations:</p><p id="par0165" class="elsevierStylePara elsevierViewall">First, we should be able to create resources to adequately inform people who look for accurate, veracious information, including the development of quality seals and tools to identify – and eventually recommend closure – of poor quality sites. In the field of online selling, we should be able to educate our patients (and ultimately the overall population), so they understand the usefulness but also the hazards of acquiring products on the Internet.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Second, in addition to watching digital activity, we should be able to participate actively in the digital arena by designing digital strategies to empower patients and improve treatment using apps, websites, online communication, and wearables.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Third, digital media provide medical researchers with massive retrieval data capacity and guaranteed anonymity. Those facts should be taken into consideration to optimize the knowledge in some specific areas in our field. However, we believe that these strategies should complement – but not substitute – traditional clinical research.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Conclusions</span><p id="par0180" class="elsevierStylePara elsevierViewall">This review analyzes various issues regarding the use of digital resources related to men's sexual disorders. Although each of the topics addressed should be thoroughly analyzed in future reviews, our work provides a global perspective regarding how physicians and patients interact with the digital resources in this field. The digital era is shifting the way we live, and we should take advantage of this precise moment to design the future of our specialty. However, there is little information on how digital resources are used and what their advantages and disadvantages are.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Objectives</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">To explore the scope of the scientific research on the use of digital technology related to men's sexual disorders and to analyze the primary sources of digital information related to this field.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Acquisition of evidence</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Systematic searches of the scientific literature, websites (10 first results in each google search) and mobile applications (apps). The searches combined the keywords “web” and “app” with “erectile dysfunction”, “premature ejaculation”, “Peyronie”, “male hypogonadism”, and “infertility”. Websites and apps were assessed for quality according to predefined indicators.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Synthesis of evidence</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">The qualitative analysis of the scientific literature included 116 manuscripts; 47% were clinical studies based on online survey, 9% dealt with digital treatments, 11% with quality/safety of digital healthcare environment, 3% with digital activity, 21% with patient empowerment, and 9% with online drug selling. Of 50 websites assessed for quality, 29 (58%) scored 4 or 5 on a 5-point Likert scale.
