The Charlson comorbidity index (CCI) was designed to estimate the overall risk of mortality based on the patient's comorbidity burden, and to control for comorbidities in studies assessing mortality.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">18–20</span></a> Beyond the association with the mortality risk, the CCI correlates with self-reported health<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">21</span></a> and has shown to be useful in the identification of patients who are likely to incur high health costs.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">22</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Low testosterone levels have been directly associated with a greater risk of all-cause mortality<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">17,23,24</span></a> and mortality resulting from major adverse cardiovascular events.<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">23,25</span></a> However, to our knowledge, the relationship between testosterone levels and the patient's comorbidity burden by means of the CCI score has not been explored. On the other hand, ED has been associated with an increased risk of myocardial infarct – attributed to non-detected underlying cardiovascular diseases,<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">26</span></a> but its influence on all-cause mortality has been barely investigated. Previous studies found that the ED severity significantly predicts the CCI score<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">27,28</span></a>; however, the influence of other comorbidities frequently found in patients with ED in multivariate models has not been investigated.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of this multicenter cross-sectional study was to explore the potential relationship between the CCI, which estimates the mortality risk associated with the patient's comorbidity burden, and two important indicators of sexual function decline: low testosterone levels and the ED severity, graded by means of the IIEF-5 score.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0025" class="elsevierStylePara elsevierViewall">This observational, cross-sectional study included data from all patients referred to the andrology unit of 7 Spanish centers between 13 April 2015 and 13 October 2015. Patients with previous prostate surgery, pelvic radiotherapy, ED caused by pharmacological therapy, and those whose pharmacological treatment could affect their hormone profile were excluded from the study. Other exclusion criteria were phosphodiesterase 5 inhibitors (PDE5i) therapy in the past 6 months, intracavernous injections, and 5-alpha reductase inhibitors therapy. To establish a homogeneous criterion for sexual health assessment, the International Index of Erectile Function (IIEF-5) score<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">29</span></a> was used to diagnose and grade the ED. For a greater consistency in the definition of ED, patients with normal IIEF-5 despite reporting ED were considered as healthy. All patients gave their informed consent for the use of their clinical data, and the study protocol was approved by the Institutional Research Ethics Committee of the Hospital Clínic (Barcelona, Spain). The obtained data were managed in accordance to the local law for personal data protection.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Main outcome measures</span><p id="par0030" class="elsevierStylePara elsevierViewall">Sociodemographic and clinical characteristics of patients included age, body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), and smoking history. Blood samples, drawn between 08:00 am and 10:00 am from overnight-fasting patients, were used to determine fasting serum glucose, serum cholesterol (total, HDL, and LDL), serum triglyceride, and total testosterone. The patient's comorbidity burden and the associated mortality risk were assessed using the CCI in its original version of 19 diseases.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">18</span></a> Specific categories for the comorbidities were created based on the blood parameters as follows: obesity (BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>), hypertension (either previous diagnosis or SBP<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>140 and/or DBP<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>90), diabetes (serum fasting glucose<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>110<span class="elsevierStyleHsp" style=""></span>mg/dL, which includes patients with pre-diabetes), hypercholesterolemia (total cholesterol<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>200<span class="elsevierStyleHsp" style=""></span>mg/dL), and hypertriglyceridemia (serum triglyceride<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>150<span class="elsevierStyleHsp" style=""></span>mg/dL). Information about lifestyle habits included smoking history and daily alcohol consumption. ED severity was graded according to the IIEF-5 scale as no ED (score 22–25), mild ED (score 17–21), mild-to-moderate ED (score 12–16), moderate ED (score 8–11), and severe ED (score 5–7).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">The distribution of patients among different groups was described as frequency and percentage. Quantitative variables were described as mean (standard deviation), except in the case of serum triglyceride and fasting glucose, which were described as median due to their trend to non-normal distribution and the deviation in the <span class="elsevierStyleItalic">Q</span>–<span class="elsevierStyleItalic">Q</span> plot observed in our sample.<a class="elsevierStyleCrossRefs" href="#bib0330"><span class="elsevierStyleSup">30,31</span></a> The CCI means in each ED category were compared using an ANOVA test, and the relationship between testosterone levels and CCI was assessed using a Pearson's correlation test. A chi-square test was used to compare the proportions of patients with and without a certain comorbidity in each category of ED. Total testosterone levels (mean) in patients with and without each comorbidity were compared using a <span class="elsevierStyleItalic">t</span>-test. Finally, to predict the potential influence of the ED severity in the CCI score, a multivariate model was constructed using a multiple linear regression, which included the blood parameters related to each comorbidity and the serum total testosterone as cofactors. Patients with missing values were excluded from each test; all tests were two-sided, and the significance level was set at <span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05. All analyses were performed with SPSS (IBM Corp. Released 2011. IBM SPSS Statistics for Windows, Version 20.0. Armonk, NY: IBM Corp.).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Participants/patient's characteristics</span><p id="par0040" class="elsevierStylePara elsevierViewall">The final record included 430 patients, whose main demographical and clinical characteristics, including the ED details, are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The main comorbidities among patients in our study group were diabetes, hypertension, obesity, and dyslipidemias such as hypercholesterolemia and hypertriglyceridemia. The prevalence of these comorbidities in our study sample ranged from 22% (diabetes) to 47% (hypertension); 394 patients (92%) had a CCI score of 1 or greater. Twenty-eight patients (8%) had total testosterone levels below 8.5<span class="elsevierStyleHsp" style=""></span>nmol/L, and 389 (90%) had some grade of ED. Mean (SD) total testosterone levels significantly decreased with the increase in ED severity: 18.9 (12.2) no ED, 16.5 (6.4) mild ED, 14.8 (5.5) mild-to-moderate ED, 14.6 (6.9) moderate ED, and 13.0 (4.4) severe ED (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Alcohol consumption had high variability, with most patients (70%) reporting no alcohol consumption at all, and 36 patients reporting a daily consumption of more than 30<span class="elsevierStyleHsp" style=""></span>g (maximum 150<span class="elsevierStyleHsp" style=""></span>g); 298 patients (76%) were non-smokers at the moment of their inclusion into the study.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Comorbidities and sexual health</span><p id="par0050" class="elsevierStylePara elsevierViewall">The frequency of each ED severity grade was significantly different in patients with and without hypertension and diabetes, who had a greater prevalence of ED than patients with other comorbidities. In contrast, no differences were found in patients with and without other comorbidities regarding the ED severity (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Total testosterone levels were significantly lower in patients with obesity, diabetes, hypercholesterolemia, and hypertriglyceridemia.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The mean (95% CI) CCI score significantly increased with the ED severity: 1.6 (1.0–2.2) in no ED, 3.0 (2.6–3.4) in mild ED, 3.8 (3.4–4.2) in mild-to-moderate ED, 3.7 (3.3–4.2) in moderate ED, and 4.5 (3.9–5.1) in severe ED (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). On the other hand, total testosterone levels showed a negative, very weak correlation with the CCI score (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.125; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.018).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Multivariate analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">The multivariate analysis included variables related to the most common comorbidities found in patients with ED (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The obtained model significantly predicted the CCI score (<span class="elsevierStyleItalic">F</span>(11,196)<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>13.2, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001, adj. <span class="elsevierStyleItalic">R</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.653),with the addition of all independent variables into the regression model explaining 43% of the variability of CCI score (<span class="elsevierStyleItalic">R</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.426). Regarding the individual variables included in the model, only the age, the BMI, the fasting glucose, and ED severity showed a significant contribution to the model.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">In this multicenter cross-sectional study, we observed that the health-relevant comorbidity burden, measured by means of the CCI score, significantly correlated with the ED severity, but showed a weak correlation with total testosterone levels. The significant influence of ED severity to the CCI score was maintained after adjusting for other covariates related to demographic factors and comorbidities frequently found in patients with ED.</p><p id="par0070" class="elsevierStylePara elsevierViewall">ED is a complex and multifactorial disorder,<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">2</span></a> and there is growing evidence that it could be a manifestation of non-treated underlying comorbidities, particularly cardiovascular and metabolic diseases.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">3,10–12</span></a> In agreement with this rationale, we observed a higher prevalence and severity of ED in patients with hypertension and diabetes than in those without these comorbidities. In a further step to explore the potential influence of ED in a broader range of health-relevant comorbidities, we investigated the relationship between the CCI and the severity of ED. The CCI is an easy-to-use clinical tool to evaluate the patient's comorbidity burden and has shown to be particularly useful in the assessment of survival in patients with health-relevant comorbidities.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">18,20,21</span></a> In addition to survival, the CCI score has proven to be a useful tool to predict the severity of urogenital conditions such as vesical tumors.<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">32</span></a> Although the presence and severity of cardiovascular conditions contribute to the final CCI score, other comorbid conditions such as malignancies, disabilities, and renal and liver diseases have a greater weight in the assessment of the CCI; thus, the information provided by this index is beyond the cardiovascular risk. The relationship between the ED and the CCI was previously investigated by Salonia et al., who found a significant influence of the ED severity (addressed by the IEFF-EF score) on the CCI score in two multivariate models including different parameters of the dynamic penile color Doppler ultrasonography in patients with suspicion of ED.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">27</span></a> More recently, Favilla et al. observed that the prevalence of ED progressively increased with the CCI score and that the OR for ED (IIEF-5 < 22) increased with the CCI score.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">28</span></a> In line with the previously reported results, in our study population, the CCI score significantly increased with the severity of ED. In addition to the relationship between these two indicators, ED patients typically present with a clinical condition encompassing ED, low testosterone levels, and metabolic syndrome.<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">33</span></a> This condition has also been associated with an increased risk of atherosclerotic plaques.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">34</span></a> We therefore decided to investigate the individual influence of hypertension and conditions of the metabolic syndrome, such as diabetes, dyslipidemias, and obesity. The comparative analysis revealed significant differences regarding the ED severity in patients with hypertension and diabetes/pre-diabetes, but not in those with dyslipidemias. In the multiple regression model, the severity of the ED significantly predicted the CCI, and so did individual comorbidities such as diabetes and hypertension. Conversely, covariates related to dyslipidemias (triglyceride and total, HDL, and LDL cholesterol) did not show a significant contribution to the model. Although reducing these biochemical parameters remains a therapeutic goal in real-life clinical practice, guidelines on dyslipidemias tend not to consider cholesterol and triglyceride levels as absolute risk factors per se, but to integrate them into a comprehensive risk assessment for cardiovascular disease that also includes lifestyle habits, blood pressure, and age. Hence, rather than the different parameters separately, the comprehensive cardiovascular risk might have a greater contribution to a multivariate model for predicting the CCI score. Another unexpected finding was the significant, negative contribution of the BMI to the multiple regression. Notwithstanding the moderate statistical power of our multivariate model, the BMI has a stronger influence in comorbidities related to cardiovascular disease, which – as discussed above – have a moderate influence on the final score of the CCI. In fact, some authors in other areas have considered that the CCI does not fully represent the risk associated with obesity, and have proposed combined models including both the CCI and the BMI.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">35</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">Large prospective cohort studies observed a relationship between low testosterone and an increased risk of all-cause mortality, admitting the uncertainty about the influence of the genetic factors.<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">23–25,36</span></a> Nonetheless, the threshold for an increased risk of mortality has not been established, and the differences in testosterone levels of survivors and dead subjects observed in these studies are narrow. A previous prospective study (187 subjects followed up for 10 years) reported testosterone levels of 23.1 (9.2) and 20.2 (6.6) nmol/L for survivors and dead subjects, respectively<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">24</span></a>; whereas in a larger prospective record (2314 subjects followed up for 10 years), the corresponding values were 16.7 (5.7) and 15.8 (5.7) nmol/L for survivors and dead subjects, respectively.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">23</span></a> The mean testosterone level in our study group (15.2<span class="elsevierStyleHsp" style=""></span>nmol/L) was in the low range of the one observed in similar studies.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">6</span></a> As observed by different authors, in a previous study, we observed a significant relationship between low testosterone levels and the presence of risk factors for cardiovascular disease, such as hypertension, dyslipidemia, or overweight.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">4</span></a> In line with these findings, our results showed that patients with metabolic syndrome had significantly lower levels of total testosterone. However, the CCI score showed a weak correlation with testosterone levels, which did not significantly contribute to the multivariate model for predicting the CCI score. To our knowledge, this is the first study that investigates the potential correlation between testosterone levels and the CCI score. Considering the proven association between the CCI and the mortality risk, we expected a stronger correlation between testosterone levels and the CCI score. However, besides the narrow differences in testosterone levels discussed previously, a time lag between the rise in the CCI score and the decline in testosterone levels cannot be ruled out. In this regard, the results obtained in a cross-sectional study do not allow to explore the relationship between testosterone levels and the CCI over time.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The cross-sectional design is the main limitation of our study. Our results display a ‘static picture’ of the overall and sexual health status of our patients. However, we did not collect data on the duration of the ED or previous testosterone levels, which would be of greater interest in the assessment of the relationship between patients’ sexual health and their comorbidity burden. In this regard, prospective studies recruiting patients without ED and normal testosterone levels at the moment of their inclusion in the study would provide an opportunity to explore the health status of patients during the onset of ED and/or testosterone declining. Finally, the missing values in some variables did not allow the inclusion of a comprehensive cardiovascular risk score, as suggested by the European Society of Cardiology, in the multivariate analysis.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Summarizing, we observed a significant relationship between the ED severity and the patient's comorbidity burden, as well as a positive correlation between the ED severity and the CCI score, which remained significant after adjusting for different variables related to comorbidities common among patients with ED. Conversely, testosterone levels showed a weak correlation with the CCI and did not significantly contribute to the multivariate model predicting the CCI score. As a cross-sectional study, our analyses do not allow to investigate how the low testosterone levels and the ED severity are influenced by the patient's comorbidity burden. The ED was diagnosed and graded using the International Index of Erectile Function (IIEF-5) score. Total testosterone, the prevalence of each comorbidity, and the CCI were compared between patients with different grades of ED. Besides, the correlation between total testosterone and the CCI score, the influence of each comorbidity, and the ED severity on the CCI was assessed in a multiple linear regression.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The study included 430 men with a mean age of 61 years. The mean CCI was 3.5, and mean total testosterone 15.2<span class="elsevierStyleHsp" style=""></span>nmol/L; 389 (91%) subjects had some grade of ED: 97 (23%) mild, 149 (35%) mild-to-moderate, 86 (20%) moderate, and 57 (13%) severe. Los niveles de testosterona, la prevalencia de cada comorbilidad y el ICC se compararon entre pacientes con distintos grados de DE. Además de la correlación entre la testosterona total y el ICC, la influencia de cada comorbilidad y de la severidad de la DE en el ICC se evaluaron mediante una regresión lineal múltiple.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">El estudio incluyó 430 hombres con una media de 61 años de edad. El ICC medio fue 3,5 y la testosterona total 15,2<span class="elsevierStyleHsp" style=""></span>nmol/l; 389 (91%) de los sujetos tenían algún grado de DE: 97 (23%) leve, 149 (35%) leve a moderada, 86 (20%) moderada y 57 (13%) severa. La severidad de la DE se asoció a un nivel menor de testosterona (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,002) y a un mayor ICC (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001). Los niveles de testosterona fueron significativamente menores en pacientes con obesidad, diabetes, hipercolesterolemia e hipertrigliceridemia (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,05). Sin embargo, únicamente la diabetes y la hipertensión mostraron una relación significativa con la DE. El modelo multivariado, que incluía variables relacionadas con todas las comorbilidades evaluadas, los niveles de testosterona y la severidad de la DE, predijo el ICC (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001, R<span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,426).
