Urothelial variants exhibit urothelial differentiation mixed with specific morphological phenotypes (e.g. squamous or glandular differentiation, micropapillary, plasmacytoid, sarcomatoid), while non-urothelial variants display independent features (e.g. squamous cell carcinoma, adenocarcinoma, or neuroendocrine variants).<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The management of patients with variant histology patterns may differ from that of pure urothelial carcinoma. A plethora of primary and secondary research studies suggest that variant histology patterns are associated with increased risk of disease recurrence and progression, as well as with worse cancer-specific and all-cause mortality. Additionally, they present more frequently at an advanced stage.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Therefore, the accurate diagnosis of variant histology facilitates early risk stratification, allows proper patient counseling, determines prognosis, and guides treatment decisions.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Accumulating evidence supports that an aggressive treatment strategy should be implemented in patients with variant histology of the urinary bladder, involving RC with corresponding neoadjuvant or adjuvant treatment.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Still, considering the rarity and heterogeneity of different histological variants of bladder cancer, the available evidence derives predominantly from expert opinions, retrospective series focusing on survival, or small prospective studies evaluating the use of adjunct radiotherapy, chemotherapy, or immunotherapy to RC with lymph node dissection.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8–10</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Despite the increasing interest in the management of variant histology patterns, studies evaluating the perioperative outcomes after RC in these patients are lacking. Similarly, studies focusing on long-term morbidity and mortality in patients with variant histology patterns undergoing RC are also scarce.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> In this context, we performed a propensity score-matched analysis aiming to assess the perioperative and long-term mortality and morbidity after RC in patients with histological variants of bladder cancer versus pure urothelial carcinoma.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design and selection criteria</span><p id="par0020" class="elsevierStylePara elsevierViewall">The present study was performed at the Department of Urology, Martha-Maria Hospital, Nuremberg, Germany. It was conducted in compliance with the principles of the Declaration of Helsinki, in lieu of a formal ethics committee approval. All findings were reported based on the STROBE statement for case-control studies.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Eligible for the current study were all patients who underwent open RC with lymph node dissection due to bladder cancer from November 2013 (month of the electronic health record database launch) to June 2022 and provided written informed consent for study participation. On the contrary, patients with no follow-up data were excluded.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Propensity score matching</span><p id="par0025" class="elsevierStylePara elsevierViewall">From all patients who underwent RC during this period, those with a variant histology carcinoma of the urinary bladder in the final pathology of the RC specimen were retrospectively identified. These patients were matched with patients with pure urothelial carcinoma on a 1:3 ratio.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> For this purpose, a greedy nearest neighbor propensity score matching without replacement generated by logistic regression was used to adjust for confounding factors between the two groups. Age, sex, and tumor stage at the time of RC were selected as covariates. Covariate balance was assessed with the use of a Love plot, and standardized mean differences below 0.1 indicated adequate balance in the matching.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Data collection and study outcomes</span><p id="par0030" class="elsevierStylePara elsevierViewall">For all included patients in the present study, we collected data on baseline characteristics, laboratory values, oncological status, perioperative complications, long-term outcomes, and survival from the electronic health record database of our department. Subsequently, all included patients were contacted for an additional telephonic or in-person evaluation in August 2022 for the purposes of the study.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The primary study outcome was to compare perioperative outcomes and complications after RC with lymph node dissection for bladder cancer in patients with a variant histology carcinoma versus pure urothelial carcinoma. Secondary outcomes included the comparison of patients with variant histology versus pure urothelial carcinoma in terms of i) long-term complications (clinically significant incisional hernia requiring surgery, reoperation due to urinary diversion, septicemia of any cause, and urinary tract obstruction); ii) all-cause mortality. In particular, reoperation due to urinary diversion was defined as any uretero-intestinal/uretero-cutaneous stricture or any other complication related to the applied urinary diversion that required surgery (urgent or elective) to manage urine leakage or stricture, whereas urinary tract obstruction was defined as any hydronephrosis requiring stenting.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">Categorical variables were summarized as frequencies with percentages and continuous variables as mean with standard deviation or median with interquartile range (IQR). The assessment of normality and the comparisons between the two groups were performed with the corresponding statistical tests. The Kaplan–Meier estimator was used to plot survival curves for all-cause mortality from the date of RC up to the last available follow-up stratified by the histology (variant histology or pure urothelial carcinoma). A univariate Cox regression analysis was undertaken in this population to estimate hazard ratios (HRs) with corresponding 95% confidence intervals (CIs) for the risk of early- and long-term all-cause mortality. All analyses were conducted in the R software (version 3.6.3) and a two-sided <span class="elsevierStyleItalic">P</span>-value of less than .05 was considered statistically significant.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Patient characteristics</span><p id="par0045" class="elsevierStylePara elsevierViewall">Of all patients undergoing RC with lymph node dissection for bladder cancer in our department, 37 displayed variant histology in the final RC specimen. In 32 (86%) cases, the variant histology was diagnosed from the TUR-B specimen. We identified 12 (32%) patients with squamous differentiation, 1 (3%) with glandular differentiation, 1 (3%) with micropapillary carcinoma, 6 (16%) with sarcomatoid carcinoma, 4 (11%) with pure squamous cell carcinoma, 8 (22%) with neuroendocrine carcinoma, and 5 (13%) with adenocarcinoma of the urinary bladder. These patients were matched in a 1:3 ratio to 111 patients with pure urothelial carcinoma of the urinary bladder. The two groups were well balanced based on the propensity-score–matched samples.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Overall, 148 individuals with a mean age of 72 ± 10.8 years were included in the present study. At the time of RC, the two groups did not present any differences in their baseline characteristics and intraoperative outcomes. In particular, the medical history, the type of applied urinary diversion, the oncological status, the renal function, and the operative time did not differ between patients with urothelial versus variant histology patterns. Still, it should be highlighted that the proportion of patients with metastases at the time of RC was higher in patients with variant histology (9, 21.6%) versus urothelial carcinoma (10, 9%, <span class="elsevierStyleItalic">P</span> = .081). Moreover, a high number of cases with concomitant incidental prostate or gynecological (ovarian or uterine) cancer was detected both in patients with variant histology (14, 37.8%) and urothelial carcinoma (36, 32.4%, <span class="elsevierStyleItalic">P</span> = .69). The baseline characteristics and intraoperative outcomes of all included patients are presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Perioperative morbidity and mortality</span><p id="par0055" class="elsevierStylePara elsevierViewall">Patients were discharged after 21.6 ± 6.6 days from RC (21.7 ± 6.8 in the urothelial carcinoma group and 21.3 ± 5.9 days in the variant histology group, <span class="elsevierStyleItalic">P</span> = .061). Importantly, 107 (72.3%) individuals presented at least one perioperative complication based on the Clavien-Dindo classification. This proportion was similar between patients with urothelial versus variant histology carcinoma (<span class="elsevierStyleItalic">P</span> = .22). In both groups, for grade 1 complications, paralytic ileus and lymphocele formation occurred more often, for grade 2 complications, administration of intravenous antibiotics and blood transfusion occurred more often, while, for grade 3 complications, VAC-placement occurred more often. Even though statistically insignificant, the proportion of Clavien-Dindo 4 or 5 complications was about 20% in the variant histology group versus approximately 7% in the pure urothelial carcinoma group, which might be clinically relevant. In particular, septicemia was the commonest grade 4 Clavien-Dindo complication, whereas, during the hospital stay, two (1.8%) deaths were observed in patients with urothelial carcinoma and four (10.8%) deaths in patients with variant histology carcinoma. Accordingly, no significant differences were observed in the perioperative complications between patients diagnosed with variant histology directly after TUR-B versus those diagnosed with variant histology only after RC. Importantly, the estimated GFR was similar in patients with variant histology and pure urothelial carcinoma both at the one-month- and at the three-month evaluation (<span class="elsevierStyleItalic">P</span> > .99 and <span class="elsevierStyleItalic">P</span> = .064, respectively). The perioperative outcomes on morbidity and mortality are available in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Long-term morbidity and mortality</span><p id="par0060" class="elsevierStylePara elsevierViewall">Overall, 59 (40.1%) patients underwent adjuvant oncological treatment due to the urinary bladder carcinoma. At a median follow-up of 12 months (IQR: 5.8–36), the number of patients with clinically significant incisional hernia requiring surgery [14 (12.7%) versus 3 (8.3%), <span class="elsevierStyleItalic">P</span> = .68], uretero-intestinal/uretero-cutaneous strictures or any other complication related to the applied urinary diversion [15 (13.6%) versus 7 (19.4%), <span class="elsevierStyleItalic">P</span> = .56], as well as the number of patients presenting with septicemia of any cause [17 (15.5%) versus 10 (27.8%), <span class="elsevierStyleItalic">P</span> = .16] or with urinary tract obstruction [12 (10.9%) versus 4 (11.1%), <span class="elsevierStyleItalic">P</span> > .99] at follow-up did not differ between urothelial versus variant histology carcinoma. The corresponding long-term outcomes on morbidity and mortality are also displayed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Accordingly, no significant differences were observed in the long-term morbidity and mortality between patients diagnosed with variant histology directly after TUR-B versus those diagnosed with variant histology only after RC. The survival analysis with Kaplan-Meier curves for the all-cause mortality suggested that the risk of death from any cause was increased in patients with variant histology compared to urothelial histology (log-rank test = 0.045, <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Based on the univariate Cox regression model, variant histology was significantly associated with increased mortality compared to pure urothelial carcinoma (HR: 1.7, 95% CI: 1.01–2.87, <span class="elsevierStyleItalic">P</span> = .047).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Our findings indicate that patients diagnosed with variant histology carcinoma at the time of RC display similar perioperative complications in terms of number and severity. Intraoperative outcomes, operative time, hospital stay, postoperative renal function, as well as perioperative complications based on the Clavien-Dindo classification did not differ significantly between the two groups. Importantly, the perioperative mortality seems to be equal between the two groups. Additionally, long-term complications after RC seem to occur at a similar rate in patients with variant histology carcinoma compared to urothelial carcinoma. Still, it should be highlighted that variant histology carcinoma was significantly associated with an about two-fold increased all-cause mortality compared to urothelial carcinoma in the long-term. The latter may also be attributed to the higher number of patients with variant histology and metastatic cancer at the time of RC. Nevertheless, this reflects the real-world scenario, where patients with variant histology present with more aggressive tumors at the time of diagnosis.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Our findings are in line with prior high-quality retrospective cohort studies assessing the mortality of variant histology carcinoma. Nagumo et al. in a high-volume cancer registry from Japan demonstrated that the prognosis of variant histology patterns is worse than that of pure urothelial carcinoma. The authors proposed a more aggressive surgical approach, even in the early stages of bladder cancer, to improve patients’ oncologic outcomes and survival.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> Similarly, Martin et al. evaluated survival trends for different bladder cancer subtypes from the California Cancer Registry. The authors concluded that variant histology patterns are associated with worse oncologic outcomes compared to pure urothelial carcinoma. Based on a Kaplan–Meier analysis and a multivariable Cox regression model adjusting for age, gender, stage, and grade, variant histology patterns presented worse disease-specific and overall survival. In particular, the 5-year overall survival was 21.7% for neuroendocrine carcinoma, 22.2% for squamous cell carcinoma, 33.8% for adenocarcinoma, and 32.4% for other variant histology patterns. On the contrary, pure urothelial carcinoma displayed a 5-year overall survival of 57.2%.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Estimations of perioperative outcomes after RC have been previously attempted in the literature. Our results corroborate and strengthen the trend that patients receiving RC are relatively old and display multiple comorbidities.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Furthermore, it seems that urinary tract infections, surgical wound infections, acute renal failure and transfusion are among the commonest perioperative complications, as reported in the present cohort study.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,17</span></a> More specifically, available evidence from high-volume studies suggests that urinary tract infections and surgical wound infections might affect up to 40% of patients undergoing open RC, while renal failure and transfusions might exceed 50%.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18–20</span></a> Importantly, even though we included only patients undergoing open RC, some of these complications such as transfusion might be lower in patients undergoing robotic RC.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Accordingly, the length of hospital stay of our series is comparable to other published series from Germany, suggesting that robotic RC might both improve length of hospital stay and other perioperative outcomes such as transfusion rates.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">The optimal management of different variant histology patterns remains controversial due to the lack of high-volume, prospective studies. Nevertheless, it is anchored on RC with pelvic lymph node dissection.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Chemosensitivity varies widely among different variant histology patterns. Neoadjuvant chemotherapy with etoposide and cisplatin is the mainstay of treatment in patients with neuroendocrine carcinoma.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> Accordingly, muscle-invasive bladder urothelial carcinoma with squamous or glandular differentiation, as well as micropapillary or plasmacytoid variants may be treated with cisplatin-based neoadjuvant chemotherapy.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">25–27</span></a> On the contrary, pure squamous cell carcinoma, adenocarcinoma, and sarcomatoid carcinoma are generally associated with poor response to chemotherapy.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,28,29</span></a> Importantly, the role of adjuvant chemotherapy or radiotherapy for some specific morphological phenotypes or non-urothelial variants remains a matter of debate.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">It should be highlighted that, despite the plethora of available studies focusing on the diagnosis and treatment of variant histology patterns in patients with bladder cancer undergoing RC, no studies have evaluated the perioperative and long-term complications after RC in this population. Based on the previous notion, we provide, to our knowledge, the first study that suggests that variant histology patterns exhibit similar intraoperative and perioperative outcomes, as well as short- and long-term complications compared to patients with pure urothelial carcinoma. The rationale behind our analyses on complication rates is that patients with variant histology present more aggressive disease compared to patients with the same tumor stage but with pure urothelial carcinoma.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> Therefore, we hypothesized that this increased aggressiveness of variant histology in combination with the presence of potential micrometastases might increase the risk of complications.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a> It should be stressed that the proportion of perioperative complications was higher in patients with variant histology although it did not reach statistical significance. Nevertheless, further research in the field is mandatory. Moreover, given that the initial studies on the role of immune checkpoint inhibitors in patients with variant histology report promising findings,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> additional studies are necessary in an attempt to expand our therapeutic armamentarium in patients with variant histology patterns of the urinary bladder.</p><p id="par0095" class="elsevierStylePara elsevierViewall">It should be noted that the present study is not devoid of some limitations that need to be considered. Our findings were tempered by the heterogenous and relatively small number of included participants with variant histology. Given that our study had a single-center design and, thus, a rather small sample size, we could not perform a multivariable cox regression analysis to adjust for risk factors that may negatively affect mortality and morbidity. Due to the retrospective design of the study, no data on cancer specific mortality and on the cause of death could be retrieved. Importantly, considering that some variant histology patterns may be more aggressive and lead to higher mortality and morbidity, it might have been an oversimplification to group patients with variant histology patterns and compare them with matched patients with pure urothelial carcinoma. Moreover, given that there is a low interobserver agreement among pathologists for variant histology, the lack of a central pathology review might have further reduced the generalizability of our findings.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">34,35</span></a> Of note, taking into account that patients with increased expression of variant histology in the specimens of the urinary bladder may present worse survival outcomes after RC, we could not retrospectively retrieve these data from the specimens of the included patients.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">36</span></a></p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conclusions</span><p id="par0100" class="elsevierStylePara elsevierViewall">The present propensity score-matched analysis suggests that patients with variant histology of the urinary bladder display similar perioperative and long-term morbidity compared to pure urothelial carcinoma. Accordingly, the perioperative mortality seems to be equal between the two groups. Still, in the long-term, variant histology patterns harbor a two-fold increased all-cause mortality compared to pure urothelial carcinoma.
