In trauma centers, the management of patients with ORT has varied from the emergency surgery to expectant management. The rate of surgical intervention in RT for guns ranges between 20% and 30%. Stab and gunshot wounds differ greatly in the severity of the injuries produced, and in the energy made on the tissue and nearby organs, with greater probability of associated lesions in the second case. The high velocity projectiles must be considered contaminated, as they can carry foreign bodies (such as clothing) in the entry site. To date, there are no studies on RT comparing the different injuries from knives, guns, and their respective caliber and speed ratings.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Objectives</span><p id="par0015" class="elsevierStylePara elsevierViewall">This paper aims to describe the characteristics, management, and outcomes of the patients with ORT admitted to the Emergency Department of our hospital.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Material and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">We retrospectively identified the total of patients with chest, abdominal, and thoracoabdominal trauma admitted to the Emergency Department of the <span class="elsevierStyleItalic">Hospital Padre Hurtado</span> in a period of 24 months, between July 2009 and June 2011. We identified a total of 14 patients with diagnosis of ORT and evaluated their medical records, lab tests, and images. We examined the variables of age, sex, mechanism of injury, imaging findings, treatment, laboratory tests, and evolution. The RTs are classified according to the scale of the American Association for Surgery of Trauma Organ Injury Severity Scale for the Kidney. The classification of the American Association for Surgery of Trauma (AAST) was obtained from the information provided in the radiology report, and depending on the case, corroborated in surgery. The information was processed in a database created with the SPSS program, where the descriptive statistical analysis of the different variables was performed.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">The demographic and clinical characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The age ranged from 16 to 37 years, with an average of 24.5. We studied 13 male patients and one female patient. The mechanism of the traumatism was 71% (10/14) by guns and 29% (4/14) knives. The RT was classified according to the AAST (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), with the information provided by the abdominal computed tomography (CT) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A–D) or the surgery data (where we will not count on CT). It is observed that 3 out of 14 patients (21%) were grade II, 4 out of 14 (29%) grade III, 4 out of 14 (29%) grade IV, and 3 out of 14 (21%) grade V. In the laboratory controls, there was a drop in the hematocrit ranging between 1% and 27%, with an average of 13.9%. There was need for blood transfusion in 12 out of 14 (86%) patients, with an average of 3.4 units per patient transfused (range: 2–8). There were no changes in overall renal function; the last plasma creatinine registered on average is 1.04<span class="elsevierStyleHsp" style=""></span>mg/dl.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">Conservative management was decided in 6 out of 14 patients. In this group, the surgeries performed by non-urological organ damage are not considered. A total of 8 patients underwent urological examination, 6 of them (43%) requiring nephrectomy. Of the nephrectomies performed, 3 corresponded to traumatism grade IV, and 3 to grade V of the AAST. Among the associated lesions, the thoracoabdominal ones were observed in 11 out of 14 patients (79%), the most frequent being in the liver (4), the stomach (4), the diaphragm (4), the pleura (3), the spleen (2), the colon (2), and the duodenum (2). The average hospital stay was 19 days, with a range of 1–81 days (SD<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>20). The minimum follow-up of each patient was 4 months, with one death in the study group. It must be emphasized that the deceased patient had multiple injuries on admission, and he could not be revived in the block.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">The experience of our department reflects the data published on the subject, considering the age, sex, etiology, and associated injuries.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4</span></a> The AAST classification and the need for nephrectomy relate directly, showing that 50% of the nephrectomies (3/6) were classified as grade V, and that there were no nephrectomies in grades II and III. It is observed that there is a high percentage (79%) of the cases associated with another organ injury, demonstrating the need for the urologist-surgeon work in this pathology. This, associated with a high rate of blood transfusion requirements (86%) and a high number of bed days (average of 19 days), reflects the cost to the health center. Despite this, the observed mortality is low (8%).<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8–10</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">The number of patients obtained in this study is still small to draw major conclusions. The ORT, despite being rare in the general population, is increasing in our population probably because of the sociocultural conditions thereof.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The AAST grade and the need for nephrectomies share relation, as described in studies of more patients. It is mainly caused by gunshot wounds or knives. Gunshot wounds are classified as high energy trauma and are usually associated with other organ injuries. We present our experience in opened renal trauma in the last 24 months.</p> <span class="elsevierStyleSectionTitle">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Retrospective study: patients with thoracic, abdominal and thoracoabdominal trauma admitted to the emergency room between July 2009 and June 2011 were studied. Fourteen patients were identified with opened renal trauma, with diagnostic confirmation by imaging study or during surgery.</p> <span class="elsevierStyleSectionTitle">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Age ranged from 16 to 37 years, with a mean age of 24.5 years. Thirteen patients were male. The mechanism of injury was produced by gunshot in 71% (10/14) and by knife in 29% (4/14). The opened renal traumas were classified according to the American Association for the Surgery of Trauma. Of these, 3/14 (21%) belonged to grade II, 4/14 (29%) to grade III, 4/14 (29%) to grade IV and 3/14 to grade V (21%). Fall in hematocrit ranged from 1% to 27%, with an average of 13.9%. Expectant management was done in six patients, however, this management did not take into account those surgeries performed due to non-urologic organ injuries. Six patients (42%) required nephrectomy. Se evaluaron los pacientes con traumatismo torácico, abdominal y toracoabdominal ingresados entre julio de 2009 y junio de 2011. Se identificaron 14 pacientes con diagnóstico de traumatismo renal abierto con confirmación diagnóstica por imágenes o durante el intraoperatorio.</p> <span class="elsevierStyleSectionTitle">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Las edades oscilan entre 16 y 37 años, con un promedio de 24,5; 13 casos de 14 fueron hombres. El mecanismo del traumatismo es en un 71% (10/14) por arma de fuego y en un 29% (4/14) por arma blanca. Se clasificó el traumatismo renal según la <span class="elsevierStyleItalic">American Association for the Surgery of Trauma</span>, encontrándose que 3/14 (21%) corresponden a grado II, 4/14 (29%) a grado III, 4/14 (29%) a grado IV y 3/14 (21%) a grado V. El descenso de hematocrito varió entre el 1 y 27%, con un promedio de 13,9%. Se decidió un manejo conservador en 6/14 pacientes, considerándose como tal las cirugías efectuadas por lesión de órgano no urológico. Un total de 6 pacientes (42%) requirió nefrectomía. Hubo lesiones toracoabdominales asociadas en el 79%.</p> <span class="elsevierStyleSectionTitle">Conclusiones</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">A pesar de la baja incidencia de traumatismo renal abierto, se observa un gran número de pacientes considerando otras series nacionales.
