However, this procedure has complications and is associated with high incidence of reoperations.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Photoselective vaporization of the prostate with 532<span class="elsevierStyleHsp" style=""></span>nm wavelength green laser (120<span class="elsevierStyleHsp" style=""></span>W) is an efficient surgical alternative to TURP because it has a similar efficacy<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a> at lower cost.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a> This technology has evolved to achieve a power of 180<span class="elsevierStyleHsp" style=""></span>W (GreenLight XPS). This power allows managing higher-volume prostates and at the same time preserves fiber integrity, being possible to use only one fiber per patient.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The allocation of resources for health – new technologies should be accompanied by the corresponding economic assessment demonstrating its efficiency. For this reason, the objective of this study was to analyze, from the perspective of National Health System, the costs associated with surgical treatment of LUTS secondary to BPH using GL 180-W XPS and TURP in a time horizon of 3 months.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A multicenter retrospective study of costs was performed using the bottom-up methodology to estimate the average cost per patient associated with surgical treatment with the new technology GL XPS 180<span class="elsevierStyleHsp" style=""></span>W of LUTS secondary to BPH. Endoscopic surgical technology was used as a comparator. The study was performed in a time horizon of 3 months from the perspective of National Health System.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Population</span><p id="par0025" class="elsevierStylePara elsevierViewall">Data were obtained from clinical histories of patients who were operated sequentially between July 2012 and October 2012 with the surgical alternatives assessed in 4 Spanish hospitals. Inclusion criteria were: previous diagnosis of LUTS secondary to BPH, IPSS<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>15, Qmax<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>15<span class="elsevierStyleHsp" style=""></span>ml/sec and prostatic volume ≥40<span class="elsevierStyleHsp" style=""></span>ml and ≤80<span class="elsevierStyleHsp" style=""></span>ml. Exclusion criteria were the lack of follow-up of patient as well as the lack of values for necessary variables in the study.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Model</span><p id="par0030" class="elsevierStylePara elsevierViewall">Mathematical model was performed to estimate the total average cost per patient intervened with surgical alternative (<span class="elsevierStyleItalic">x</span>):<elsevierMultimedia ident="eq0005"></elsevierMultimedia>in which <span class="elsevierStyleItalic">c</span><span class="elsevierStyleInf">(<span class="elsevierStyleItalic">i</span>,<span class="elsevierStyleItalic">t</span>)</span> stands for the cost (<span class="elsevierStyleItalic">c</span>) of the resource (<span class="elsevierStyleItalic">i</span>) in process phase (<span class="elsevierStyleItalic">t</span>). Phase process is calculated by the product (<span class="elsevierStyleItalic">q</span><span class="elsevierStyleInf">(<span class="elsevierStyleItalic">i</span>,<span class="elsevierStyleItalic">t</span>)</span>*<span class="elsevierStyleItalic">p</span><span class="elsevierStyleInf">(<span class="elsevierStyleItalic">i</span>,<span class="elsevierStyleItalic">t</span>)</span>), in which <span class="elsevierStyleItalic">q</span><span class="elsevierStyleInf">(<span class="elsevierStyleItalic">i</span>,<span class="elsevierStyleItalic">t</span>)</span> stands for the quantity (<span class="elsevierStyleItalic">q</span>) of the resource (<span class="elsevierStyleItalic">i</span>) and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleInf">(<span class="elsevierStyleItalic">i</span>,<span class="elsevierStyleItalic">t</span>)</span> for its unitary price (<span class="elsevierStyleItalic">p</span>).</p><p id="par0035" class="elsevierStylePara elsevierViewall">The outcome variable was estimated as the difference between the average total cost per patient and the technologies evaluated, <span class="elsevierStyleItalic">x y x</span>′ (Δ<span class="elsevierStyleItalic">Ĉ</span><span class="elsevierStyleInf">(<span class="elsevierStyleItalic"><span class="elsevierStyleUnderline">x</span></span>−<span class="elsevierStyleItalic">x</span>′)</span>).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Costs data</span><p id="par0040" class="elsevierStylePara elsevierViewall">From the perspective of Spanish National Health System, only direct health costs were included, therefore indirect costs associated with patients’ lost work productivity were ignored. Average cost was estimated using bottom-up method,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> counting the resources consumed in hospital stay (operating room, recovery room and hospital stay), in diagnostics and analytical procedures and in differentiated equipment necessary to carry out both surgical options. Finally, the resources necessary for the management of complications were estimated. Unitary prices of resources were obtained from specialized literature,<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8–10</span></a> as well as from public<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> and professional<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> agencies. All costs were presented in 2013 euros, and, if necessary, yearly adjusted according to the General Index of Consumer Prices.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Direct costs were analyzed in a disaggregated way, depending on three temporary phases: pre-surgical phase, gathering the costs incurred from initial surgical consultation until the day preceding surgery; surgical phase, covering surgery period; and post-surgical phase, comprising the resources used since operating room exit up to 3 following months. In both surgical procedures, differences among estimated values were analyzed using Student's <span class="elsevierStyleItalic">t</span> test for samples with unequal variances at the 0.05 significance level. The degree of outcome dispersion was calculated by bootstrap method<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> or resampling with repetition, in which 500 iterations were performed and the 95% confidence interval was calculated for the trimmed mean differences,<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> in order to remove extreme values.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Effectiveness data</span><p id="par0050" class="elsevierStylePara elsevierViewall">The result of effectiveness was healing at 3 months, defined as unobstructed patient (a post-operative reduction ≥5 points according to International Prostate Symptom Score [IPSS] and an increase in maximum flow ≥5<span class="elsevierStyleHsp" style=""></span>ml/s. measured in flowmetry).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Sensibility analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Finally, results obtained were analyzed by univariate sensitivity analysis of the most relevant variables: hospital stay and prostate size.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">During the period from July 2012 until October 2012, 79 patients randomly underwent prostate surgery, 40 of them using TURP and the other 39 with laser new technology GL XPS. Baseline characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The clinical outcome of healing was equivalent for both the procedures (94.9 and 92.5% for GL XPS and TURP, respectively) and did not show statistically significant differences (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.67).</p><p id="par0070" class="elsevierStylePara elsevierViewall">Bootstrap result after 500 iterations showed a statistically significant difference of –121<span class="elsevierStyleHsp" style=""></span>€ (CI 95%: −91; −151; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01) between total average cost per patient treated with GL XPS (3277€; DE:6.56) and TURP (3398€; DE: 13.80) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). In patients who underwent GL XPS, cost disaggregation according to temporary phases of surgical process was higher (difference: 1209€; CI 95%: 1200; 1217; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), whilst in post-surgical phase it was lower (difference: −1351€; CI 95%: −1322; 1380; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRefs" href="#tbl0015">Tables 3 and 4</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Sensibility univariate analysis for hospital stay time after surgery reports that in GL XPS under outpatient regime or surgery without admission compared with TURP. the cost reduction can reach up to 698€ per patient in the first 3 months (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Prostate size is also a cost moderator variable. Linear regression model correlating both variables shows that total average cost per patient is slightly dependent on size in GL XPS procedure (<span class="elsevierStyleItalic">R</span><span class="elsevierStyleSup">2</span>: 0.248) and independent in TURP (<span class="elsevierStyleItalic">R</span><span class="elsevierStyleSup">2</span>: 0.007). Final outcomes are strongly influenced by variations in clinical practice.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">Total average cost per patient in the first 3 months has been associated with a statistically significant saving of 121€ per patient who underwent to GL XPS (CI 95%: 91; 151). This difference is caused by a significant cost reduction (−1351€) in the post-surgical phase, which is associated with a shorter hospital stay in GL XPS patients (this fact had already been shown in cases involving GreenLight HPS 120<span class="elsevierStyleHsp" style=""></span>W laser<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>). This cost reduction offsets the increase in the same in the surgical phase (+1209€) associated with the allocation of the necessary equipment for surgery by GL XPS.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Dispersion in outcomes is associated with variation in clinical practice, mainly with hospital stay, set in a range from 0.1 to 2.2 days (mean: 1.31 days) for GL XPS, and from 3.5 to 5.5 days (mean: 4.05 days) for TURP. A study performed in Spain<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> showed that the GL XPS carried out in outpatient surgery regime had a lower cost. Additionally, allocated cost to the technology – responsible for the increase in cost in surgical phase – depends on the price thereof, its utilization level and amortization time as well as the usage of fibers. In our study, the equipment price (90,000€) has been decreased compared to that in the preceding years. The same happens with the price of fiber (950€). All of this increases the efficiency. When utilization level of GL XPS is higher than 100 patients per year, its cost decreases and, consequently, its efficiency enhances. On the other hand, fiber usage has been reduced to almost one per patient (mean: 1.03 fibers per patient).</p><p id="par0095" class="elsevierStylePara elsevierViewall">According to our knowledge, this is the first economic study published comparing the new technology GL XPS 180<span class="elsevierStyleHsp" style=""></span>W versus TURP. Outcomes obtained newly report a similar efficacy and the lower cost for GL XPS and are in line with previously published findings for GL HPS 120<span class="elsevierStyleHsp" style=""></span>W.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">When health resources are limited, it is necessary to allocate them in the most efficient way so that better outcomes are obtained at lower cost. Thus, opportunity cost will reduce and, as a consequence, it might be able to use surplus resources for other purposes. Outcomes obtained show that surgery using GL XPS in patients with LUTS secondary to BPH might allow a cost reduction, even in the short term, maintaining the health outcomes achieved by using a common procedure of TURP. This cost reduction will be more important for the higher proportion of patients who undergo outpatient surgery.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The study has some limitations. First, the size of the sample is limited, reducing statistical power; however, given the size of the phases, detailed phase function analysis of costs shows significant differences. In order to resolve this limitation, a bootstrap resampling analysis was performed, showing statistically significant total cost differences. Second, the variability in medical practice results in a remarkable dispersion in economic performance (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).
