Most urothelial bladder carcinomas are initially diagnosed as non-muscle-invasive ones (75–85%) since they are confined to the mucosa (Ta or in situ carcinoma [ISC]) or invade the lamina propria (T1), but they never reach the muscular layer. In general, NMIBCs have a good prognosis, although the recurrence rate is 30–80% and progression in the form of muscular layer invasion is 1–45%.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> The overall incidence of bladder cancer in Spain is 20.8 cases per 100,000 inhabitants, this being the fourth highest rate in Europe.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Besides its clinical relevance, it is associated with high economic cost due to its high recurrence rate and a large number of follow-ups and treatments.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">It is essential to know the prognostic factors for NMIBC and to predict the evolution of the disease, since an early detection of recurrence will help to prevent complications such as infections, hematuria or urinary tract obstructions. On the other hand, early detection of tumor progression will have an implication in patient survival.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The various publications on the biological risk of NMIBC recurrence or progression were based on mathematical models using Cox's multivariate analysis. One of the most widespread and most recommended models by the European Association of Urology (EAU)<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> is the model provided by the European Organization for Research and Treatment of Cancer (EORTC).<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This model has a number of limitations since it does not consider 2 characteristics shown in multiple events: heterogeneity among individuals and the dependency of the events within a subject. Such heterogeneity occurs because some patients have a higher (or lower) rate of events than others due to unknown, and therefore unmeasured, effects. On the other hand, the occurrence of an event can make the occurrence of subsequent events more or less likely.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Various extensions of Cox's model have been proposed to deal with datasets corresponding to multiple events, as in the process of multiple NMIBC relapses, and which aim to correct the correlation between recurrence times. However, the process of multiple recurrences can be sometimes interrupted by the occurrence of a different event such as progression. Joint modeling of both processes is therefore necessary. Our aim was to use new mathematical models such as the joint frailty model,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> which takes into account this peculiarity of NMIBC and which has not been used in other analyses.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,6</span></a> We will analyze the accuracy of the model and we will subsequently validate the model with an external source. Finally, we aim at proposing an individualized scheme for follow-up and treatment adjusted to the risk of recurrence or progression.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We conducted a prospective, longitudinal, observational analysis of 960 patients diagnosed with primary NMIBC between 1995 and 2009 and who were part of a clinical trial comparing different treatment Schemes 7. Following the transurethral resection (TUR) of the tumor and the taking of random biopsies, the patients were assigned to a group with doses of early postoperative chemotherapy (doxorubicin 50<span class="elsevierStyleHsp" style=""></span>mg or mitomycin 40<span class="elsevierStyleHsp" style=""></span>mg) versus a maintenance regimen (doxorubicin 50<span class="elsevierStyleHsp" style=""></span>mg or mitomycin 40<span class="elsevierStyleHsp" style=""></span>mg) or versus the non-administration of any treatment. The latter arm was closed in the early stages of the trial since worse results were documented when compared with the treatment groups. Those patients with grade 3 tumors or presence of ISC received <span class="elsevierStyleItalic">Bacillus Calmette-Guérin</span> (BCG) 81<span class="elsevierStyleHsp" style=""></span>mg once weekly for 6 weeks. In the case of primary NMIBCs, the treatment used was: BCG with induction in 139 (14.4%) patients, maintenance chemotherapy in 470 (48.9%), single doses in 121 (12.6%), 202 (21%) with no treatment and unknown treatment in 28 (2.9%).</p><p id="par0030" class="elsevierStylePara elsevierViewall">In the event of recurrence detection, the tumor or tumors were resected and the patient was included in a new follow-up and treatment regimen. Those cases where a recurrence was diagnosed in the upper urinary tract were not included in the study. Patients were followed-up every 3 months for the first 2 years, every 6 months between the second and the fifth year following diagnosis in order to subsequently carry out annual check-ups until year 10. The diagnostic tests used were the following: bladder ultrasound alternating with cystoscopy, cytology and intravenous urography in case of suspected upper urinary tract involvement.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The independent variables or possible prognostic factors included in the study were: age, gender, treatment and tumor characteristics such as grade (1973 WHO classification<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>), tumor stage (Tis, Ta, T1), number of tumors (single or multiple), size (≤3<span class="elsevierStyleHsp" style=""></span>cm or >3<span class="elsevierStyleHsp" style=""></span>cm) and the presence of ISC. The dependent variables of the study were: time to recurrence (time elapsed from the TUR of the primary tumor to the first occurrence of recurrence, time from the TUR of the first recurrence to the occurrence of the second recurrence, etc.); time to progression (time elapsed from the TUR of the primary tumor to the occurrence of progression) and disease-free time (time from the TUR of the primary tumor to the occurrence of a first event, either recurrence or progression).</p><p id="par0040" class="elsevierStylePara elsevierViewall">The statistical analysis first included a multivariate analysis using Cox's model, in order to determine the time to a first event, either recurrence or progression (disease-free time). Secondly, in order to jointly model the 2 processes arising when studying NMIBC (recurrence process and progression), we used the joint frailty model.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> This model enables, using non-parametric estimation, the joint treatment of both processes which evolve over time. A risk function is assigned for the recurrence event and a risk function for the progression event, both functions being connected through a <span class="elsevierStyleItalic">α</span> parameter, which allows quantifying the association between the recurrence event and the terminal event of progression, so that if <span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0, this means that the progression event is uninformative for the recurrence rate, and therefore both processes can be separately modeled by using traditional methods.</p><p id="par0045" class="elsevierStylePara elsevierViewall">On the other hand, and on the basis of the model's coefficients, a certain score was obtained for each independent variable or prognostic factor. With the sum of each variable, we obtained an overall score which classified the patient into a certain risk group.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">In order to assess the predictive capacity of the model, we also used an external source of 240 patients (Valencia Institute of Oncology [IVO]) with a similar follow-up and treatment regimen. We analyzed the index of concordance<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> in both series, which is the non-parametric version of the area under the curve of the Receiver Operating Characteristic (ROC), with a range between 0.5 (random discrimination) and 1.0 (perfect discrimination). The statistical analysis was performed using R software, version 2.13.0 and its applications Design, Survival, and frailty pack (<a id="intr0010" class="elsevierStyleInterRef" href="http://www.r-project.org/">http://www.r-project.org</a>).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall">The main characteristics of the 2 series analyzed are listed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The average age was 68.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.9 years, with a median follow-up of 48.1 (3–160) months. Among the 960 patients, a total of 1043 recurrences was observed. At least one tumor recurrence occurred in 468 (48.8%) patients, with a median onset time of 12.9 months, 2 recurrences in 245 (23.5%) with a median of 10.3 months, 3 in 151 (14.5%) at 10.9 months, 4 in 97 (9.3%) at 9.3 months, 5 in 57 (5.5%) at 9.4 months and 6 in 32 (3%) at 7.9 months. In 52 (5.4%) patients the tumor progressed to muscle-invasive forms, with a median time of 14.9 months. In the case of the external series analyzed, the mean age was 61.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.3 years, with 135 (56.3%) patients experiencing at least one recurrence and 4 (1.6%) experiencing tumor progression.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">When applying Cox's multivariate model, age, grade, size and the number of tumors and the treatment used were found to be significant for the occurrence of a first recurrence. Age and grade were significant for progression (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">When we analyzed the results using the extensions of Cox's model for multiple recurrences and tumor progression (joint frailty model (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>)), the following variables proved to be significant for multi-recurrence: age, grade, size, number of tumors, treatment and the number of prior recurrences. In the case of progression modeling, we included those variables identified as prognostic factors for progression. When comparing the nested models, one model with grade and the other one with grade, stage and age, we noted that the difference in the −2<span class="elsevierStyleHsp" style=""></span>Log statistic had a <span class="elsevierStyleItalic">p</span> value<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.033, which allows us to reject the null hypothesis that the coefficients of these new variables are void. Although the individual significance of these variables (grade, stage and age) was 10% when using Wald's statistic and 5% in the case of the model with only grade, we decided to keep them within the model due to the significant improvement in the overall plausibility of the model.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">On the basis of the resulting model, we developed a rating scale for both recurrence and progression using a non-parametric study (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). The possibility of being relapse-free at 1, 3 and 5 years is shown in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>, with 3 risk groups: low, medium or high risk of recurrence. Regarding progression, we calculated the probability of being progression-free at 6, 12 and 24 months, with 3 risk groups: low, medium and high risk of progression. Depending on the risk group, we developed a treatment and follow-up regimen according to the EAU<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> recommendations (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><elsevierMultimedia ident="tbl0030"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding the internal validation of the model, the resulting concordance index was 0.64 for the multiple relapse process and 0.85 for tumor progression. As to its external validation, the concordance index in the IVO series was 0.57 for multiple relapse and 0.87 for tumor progression.</p><p id="par0080" class="elsevierStylePara elsevierViewall">When contrasting the null hypothesis <span class="elsevierStyleItalic">H</span><span class="elsevierStyleInf">0</span>: <span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0 by using Wald's test, we obtained a <span class="elsevierStyleItalic">p</span> value<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.15, so we cannot reject the null hypothesis and accept that <span class="elsevierStyleItalic">α</span> is not equal to zero and that the risks of both processes (multi-recurrence and progression) are associated.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">The descriptive analysis of both series revealed that the recurrence percentage in both centers (48.8 and 56.6%) is similar to that of other published series<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10–16</span></a>; however, the progression percentage was lower.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,14,16–18</span></a> The prognostic factors for a first recurrence are equivalent to those of other series, but both the presence of ISC and stage were not significant,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> as well as in other published series.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,19</span></a> In the resulting model, grade 2 had a higher risk of recurrence than grades 1 or 3. In a recent multicenter study in 26 Spanish hospitals and with a total of 4285 cases of bladder tumors, the recurrence percentage of grade 2 tumors (29.5%) was higher than that in the case of grade 3 ones (27.7%).<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> A possible cause of these differences might be the subjectivity of the anatomopathological analysis when determining the cell differentiation degree between medium-grade and high-grade tumors. In the case of multiple recurrences, having a recurrence increases the risk of new events in 45% of cases. As to tumor progression, age is a crucial factor, since the risk of progression increases by 5%<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,18</span></a> for every additional year of age. Another factor involved in our patients’ progression rates was tumor stage<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,17</span></a> and tumor grade.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,14,16–18</span></a> In a patient with a grade 2 tumor, the risk of progression increases by 4-fold compared to grade 1 and in the case of grade 3 tumors the risk increases by 14-fold. On the other hand, the presence of ISC is not a significant variable for progression. This finding might be contradictory, since ISC has traditionally been associated with tumor progression.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,16</span></a> Nevertheless, other series have not identified ISC as an independent factor for progression.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,19</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">The information provided by these models is complex and clinical tools that can facilitate its use are required, for example nomograms.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,21</span></a> Another mechanism to simplify its use is the scoring system based on the resulting coefficients for each of the variables analyzed.</p><p id="par0095" class="elsevierStylePara elsevierViewall">In order to be able to use these models, it is necessary to know their reliability and their predictive capacity. For this purpose, we used discrimination methods which are quantifiable by using the concordance index.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,22</span></a> This method per se does not guarantee the reliability of the model, and this is why we used data from an external source and thus avoid the center bias. There are few references to the validation of the tables provided by the EORTC and CUETO groups with external data sources.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,23</span></a> In the various studies trying to validate these risk tables, the concordance indexes ranged from 0.60 to 0.66 for recurrence, these values being similar to those obtained in our model. Regarding tumor progression, our study is one of the first in validating this event with an external database. The resulting concordance index for progression was high and similar in the two series analyzed (0.85 versus 0.87). Other mathematical models used in urological tumors, such as prostate cancer,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> and validated with external sources obtained a concordance between 0.67 and 0.83. On the other hand, other studies have proven the limitations of the models with a greater impact such as those provided by the EORTC<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and CUETO<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> groups. A retrospective review where 4689 patients with NMIBC were analyzed and where the discrimination of the CUETO and EORTC models was assessed, identified that when the patients treated with BCG were specifically analyzed, discrimination of the CUETO model increased (CI 0.597 for recurrence and 0.645 for progression). However, the authors of this study concluded that both models overestimated the risk of progression in high-risk patients.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> Therefore, the use of these models in patients with NMIBC identified as high-risk ones by these models might lead to overtreatment or follow-up excess.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The main limitations in our study were the fact of not having analyzed the variable of the cystoscopy result at 3 months after TUR. Several studies have identified this variable as a predictor of tumor evolution.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Our aim was to establish the prognosis and treatment once TUR had been performed and the anatomopathological study was available. Other prognostic models, as well as our study, did not include this variable in their analysis.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,27</span></a> Using a retrospective series with different treatment regimens may have modified the natural history of recurrence or progression, since their percentages may vary depending on the treatment.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> On the other hand, the series used for external validation was not completely homogeneous regarding the proportions of some variables such as Tis, size and multiplicity. This may lead to a bias. However, several studies have performed external validations of the EORTC and CUETO tables by using their patients without checking homogeneity with respect to the group of origin for the model. These studies have focused on calculating the reliability of the models within a specific series by using various statistical methods such as the ROC curves<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> or the Kaplan–Meier method for overall scores.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> In our case, the study has been based on the similarity of the concordance index. Another limitation is determined by the frailty variable from the joint frailty model. The value of this variable is determined by a median equal to 1 and an unknown variance. It is a hypothetical variable that either has not been assessed or with an unknown value. One of the possible frailty variables would be tumor markers<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> or certain genetic mutations.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> The inclusion of these variables might increase the predictability of progression and multiple relapse models. If these measures were not available, a recommendation would be to use the traditional clinical and pathological parameters associated with the frailty variable by using the extensions of Cox's model.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0105" class="elsevierStylePara elsevierViewall">Biological characteristics, such as individual heterogeneity and the presence of multiple events in the form of NMIBC recurrences, have been incorporated into these new mathematical models. This allows us to calculate tumor progression in a given patient with greater reliability and to establish individualized follow-up and treatment regimens on the basis of the biological risk of progression or recurrence. We have confirmed that the risks of recurrence and progression are associated, and therefore must be modeled together. The classical Cox model is not valid for the assessment of this kind of events because the time between recurrences in the same patient may be strongly correlated. These new models for risk estimation of recurrence/progression lead to individualized monitoring and treatment plan.</p> <span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">960 patients with primary NMIBC were enrolled. The median follow-up was 48.1 (3–160) months. Results obtained were validated in 240 patients from other center. Transurethral resection of the bladder (TURB) and random bladder biopsy were performed. Subsequently, adjuvant localized chemotherapy was performed. The variables analyzed were: number and tumor size, age, chemotherapy and histopathology. The endpoints were time to recurrence and time to progression. Cox model and its extensions were used as joint frailty model for multiple recurrence and progression. Model accuracy was calculated using Harrell's concordance index (c-index).</p> <span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">468 (48.8%) patients developed at least one tumor recurrence and tumor progression was reported in 52 (5.4%) patients. Variables for multiple-recurrence risk are: age, grade, number, size, treatment and the number of prior recurrences. All these together with age, stage and grade are the variables for progression risk. Concordance index was 0.64 and 0.85 for multiple recurrence and progression respectively.</p> <span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">The high concordance reported besides to the validation process in external source allows accurate multi-recurrence/progression risk estimation.
