It is essential to know in which patients radical prostatectomy is the treatment of choice,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">5</span></a> and also which patients could benefit from adjunct therapy. The optimal therapeutic approach to adopt in patients with seminal vesicle involvement who have undergone surgery largely depends on this knowledge.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of the present work was to analyze the impact of seminal vesicles involvement on the prognosis of patients who have undergone radical prostatectomy and to design a predictive risk model for progression-free survival. As a secondary aim, we analyzed the influence of adjuvant androgen deprivation (AD) therapy on the survival of pT3b patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">We retrospectively analyzed a series of 1132 patients with T1-T2 prostate cancer according to the TNM criteria, treated with radical prostatectomy at the University Hospital of Navarra between January 1989 and December 2009. Prior to surgery, all patients had their detailed medical history taken with a physical examination including a digital rectal exam, PSA (ng/ml) and prostate biopsy. The study was completed with computed tomography until July 2000. Since then, nuclear magnetic resonance (Siemens Magnetom<span class="elsevierStyleSup">®</span> Aera or Symphony 1.5<span class="elsevierStyleHsp" style=""></span>T. Siemens AG, Germany) has been preferably used. Until the year 2000, a bone scan was also carried out in all patients. From then onwards, only in patients with PSA greater than 20<span class="elsevierStyleHsp" style=""></span>ng/ml and/or Gleason >6.</p><p id="par0025" class="elsevierStylePara elsevierViewall">In all cases, modified retropubic radical prostatectomy was done according to the technique as described by Walsh<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">6</span></a> in 1982. Until the year 2007, bilateral ilio-obturator lymphadenectomy was also systematically and subsequently practiced, in the case of PSA >15<span class="elsevierStyleHsp" style=""></span>ng/ml, clinical Gleason score ≥7 or clinical stage T2b. The laparoscopic approach was first used at our center in 2005. Since then, laparoscopic or open surgery has been conducted according to surgeons’ preferences and with the aim that all resident physicians learned about both techniques. Surgery was performed by 4 surgeons during the entire period and by a fifth surgeon who joined in for the last 3 years.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Specimen examination was performed by 2 pathologists specialized in urologic pathology according to the technique described by True.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">7</span></a> Radical prostatectomy samples were weighted, measured, inked and carved following the protocols of the College of American Pathologists. After being inked with Pelikan ink, they were fixed with acetic acid for 30<span class="elsevierStyleHsp" style=""></span>s. Once the ink had been fixed, we determined the proximal (vesical) and distal (urethral) margins by selecting samples from both areas. The prostate was subsequently carved in a serial way by making fine cuts of approximately 0.5<span class="elsevierStyleHsp" style=""></span>cm thick. We then collected representative samples of all the surgical specimen, with special emphasis on the posterior and peripheral areas. Finally, the vesicoprostatic junction was analyzed.</p><p id="par0035" class="elsevierStylePara elsevierViewall">After surgery, analytical follow-up was done through determination of PSA at months 3, 6 and 12 after completion of treatment, subsequently every 6 months up to a total of 3 years and, then, every year. In adherence with the PSA Working Group guidelines, we defined PSA biochemical recurrence as a determination of PSA ≥0.4<span class="elsevierStyleHsp" style=""></span>ng/ml<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">8</span></a> (Hybritech<span class="elsevierStyleSup">®</span>) obtained at least 30 days after surgery, and subsequently confirmed by an equal or greater value.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In the first of the study, we assessed the impact of seminal vesicles involvement on the prognosis of patients with localized cancer prostate (T1–T2) who had undergone radical prostatectomy, and designed a predictive risk model for progression-free survival. We then compared the clinicopathological characteristics of patients with pT3b lesions versus those with pT2 and pT3 disease. PT4 cases were excluded. PT2a, pT2b and pT2c patients were homogenous from the prognostic point of view (BPFS and SS), so they were grouped together. For the analysis of BPFS and SS, pT3bN+ cases and those who initiated treatment with adjuvant AD or immediate radiotherapy were excluded. We comparatively assessed the influence of seminal vesicle involvement on BPFS and SS with pT2 and pT3a tumors. We subsequently analyzed the pT3b patient group, by looking for influential factors in BPFS and SS, and designed a predictive prognostic model.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In the second part of the study, we assessed the influence of AD therapy on survival. A comparative study was also made between pT3b tumors treated with AD as a salvage regimen and those treated with adjuvant AD. In all cases, adjuvant treatment was immediately prescribed after surgery, whereas rescue treatment was administered after having evidence of biochemical recurrence. The criterion for treatment selection depended on the personal decision of each physician in charge, partly conditioned by the presence of margins or involved nodes. Androgen suppression therapy was complete and carried out with quarterly or biannual injections, for a minimum of 2 years. Some patients treated with rescue or adjuvant AD therapy also received concomitant radiotherapy. Radiotherapy was administered in 5-day sessions per week for 8 weeks, with a minimum dose of 65<span class="elsevierStyleHsp" style=""></span>Gy. Three-dimensional conformational radiotherapy (3DCRT) was administered in 4 fields on the prostate bed, including the theoretical area of the bladder neck, tissue surrounding the prostate bed and the theoretical location of seminal vesicles.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">9–11</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">We used contingency tables and the Chi-square or Fisher's test for the comparison of qualitative variables. The quantitative variables studied (age and PSA) did not follow a normal distribution, and we used the Mann–Whitney <span class="elsevierStyleItalic">U</span> test for its comparison. Normality of the quantitative variables was assessed by using the Kolmogorov–Smirnov and the Shapiro–Wilk tests. Actuarial survival was studied by using the Kaplan–Meier method. The log-rank test was used for the comparison of survival curves. The analysis of those influential variables in BPFS was done using Cox's regression model (univariate and multivariate study). <span class="elsevierStyleItalic">P</span> values for statistical significance and hazard ratios were calculated with their respective confidence intervals using the bootstrapping technique with 1000 replicates. All data were presented as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation. A <span class="elsevierStyleItalic">p</span> value less than 0.05 was required in order to determine statistical significance.</p><p id="par0055" class="elsevierStylePara elsevierViewall">On the basis of the predictive variables for independent influence, we designed a model risk for BPFS. A numeric value was assigned to each variable, depending on their prognostic weight, so that each patient had the numeric values of the variables involved added and the result was their value within a prognostic classification scheme. We compared several possible numeric models and chose the best through a multivariate study according to Cox's model. We used ROC curves to determine suitable cut-off points. All statistical analyses were conducted using the expanded version of IBM Statistical Product and Services Solutions software (SPSS, version 22.0, SPSS Inc., Chicago, IL, USA).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">Of the 1132 patients who comprised the overall series, 127 (11.2%) were pT3b, 770 (68%) pT2, 231 (20.4%) pT3a and 4 (0.4%) pT4. There were 90 cases of patients with pT3b tumors on whom the survival study was carried out, taking into account the procedure described in the “Materials and methods” section. The median follow-up in the overall series was 7.3 years. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> describes the main clinicopathological characteristics. In the multivariate study, seminal vesicle involvement (pT3b) independently affects BPFS (HR: 2, CI 95%: 1.4–3.3; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001), along with initial PSA, clinical stage T2, positive margins and Gleason of the specimen 8–10 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The median follow-up was 7.8 years for the pT3b group, 7.3 years for pT2 and 7.6 years for pT3a. The main clinicopathological characteristics of pT3b and its differences with pT2b and pT3a are summarized in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">5- and 10-year BPFS was 40<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% and 28<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% for the pT3b group, 84<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1% and 77<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2% for pT2 and 60<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% and 50<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% for pT3a (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 in all cases). The median BPFS for pT3b was 2.5 years (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The multivariate study on BPFS for pT3b highlighted that PSA expressed in ng/ml (HR: 1.02; CI 95%: 1.01–1.03; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.000), with best PSA cut-off point >10<span class="elsevierStyleHsp" style=""></span>ng/ml (HR: 1.9; CI 95%: 1.04–3.6; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.04) and pathological Gleason score 8–10 (HR: 2.1; CI 95%: 1.2–3.5; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.03) were independent predictive variables (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). By conducting multiple tests with the influential variables and using Cox's models, the best risk model was derived from using pathological Gleason score 8–10 and PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml, assigning the value of one point to each variable involved, as expressed in the “Materials and methods” section. Thus 2 groups were formed: (a) group 1 (0–1 variables), comprised of 66% of pT3b (HR: 1) with 5- and 10-year BPFS of 46<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7% and 27<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8%; (b) group 2 (2 variables) comprising 34% of pT3b (HR: 2.1; CI 95%: 1.3–3.6; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005) with 5- and 10-year BPFS of 14<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7% and 5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5% (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">5- and 10-year SS was significantly worse in pT3b tumors (93<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2% and 76<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4%, respectively) with regard to stages pT2 (99.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.03% and 97.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.09%) and pT3a (98.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.09% and 93.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 in all cases) (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). In the multivariate study on SS of the pT3b group, pathological Gleason score 8–10 (HR: 2.8; CI 95%: 1.2–6.6; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) was the only factor with independent prognostic value. Castration resistance-free survival (CRFS) was significantly worse in adjuvantly treated pT3b tumors (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01); however, 5- and 10-year SS was the same in both groups (95<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3% and 78<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6% in the rescue group versus 79<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9% and 67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11% in the immediate treatment group; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.2) (<a class="elsevierStyleCrossRefs" href="#fig0020">Figs. 4 and 5</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="fig0020"></elsevierMultimedia><elsevierMultimedia ident="fig0025"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">A total of 22 patients with stage pT3b disease, whose median follow-up was 8.6 years, received adjuvant AD therapy. Of the remaining 105 patients with stage pT3b disease, 65 patients were treated with a salvage regimen of AD therapy due to biochemical progression. The median follow-up for the latter was 6.7 years. When comparing the patients treated with immediate AD therapy with those who received a rescue treatment, it can be noted that both groups are homogeneous. In particular, those immediately treated showed a significantly higher proportion of positive nodes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and involved margins (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">Patients with seminal vesicle involvement have higher PSA levels, greater Gleason scores, worse clinical stage, and experience involved margins and adenopathies more frequently than patients with no seminal vesicle involvement.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">4,12</span></a> The negative prognostic value of seminal vesicle involvement on BPFS and SS in patients undergoing radical prostatectomy is therefore indisputable.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">4,13–16</span></a> In most of the studies published, it significantly worsens the prognosis of these patients, this being an independent influencing variable for BPFS.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">However, the patient group with seminal vesicle involvement was heterogeneous. It is therefore a matter of priority to stratify risk in these cases, analyzing those factors associated with prognosis and designing risk groups to indicate treatment more accurately. In our case, the independent influencing factors in these patients were pathological Gleason 8–10 and PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Salomon et al.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">14</span></a> studied 137 patients with pT3bN0M0 disease, treated with radical prostatectomy, and also noted that Gleason and PSA were in the multivariate study the influencing factors on biochemical progression. Neither extracapsular involvement nor the presence of positive margins turned out to be predictive variables for BPFS in pT3b tumors.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Many studies have assessed the influencing factors on pT3b tumor progression.<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">15–17</span></a> Epstein et al. showed in pT3bN0 patients that pathological Gleason, capsular involvement and the presence of margins were independent factors for progression.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">15</span></a> But when their analysis excluded those subjects with positive nodes, Gleason and margins were no longer predictors. In our study, patients with immediate hormone treatment and those with node involvement were excluded. Tefilli et al.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> indicated in a multivariate analysis that tumor progression in patients with pT3b disease was conditioned by surgical margin involvement, Gleason >6 and preoperative PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml, in line with the studies conducted by D’Amico et al.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">In our 127 patients, pathological Gleason 8–10 and PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml were independent influencing variables for BPFS. We designed a risk model for BPFS, which was highly simple but capable of significantly discriminating the prognosis of these patients. According to our model, pT3bN0 cases with PSA <10<span class="elsevierStyleHsp" style=""></span>ng/ml or Gleason 2–7 showed acceptable BPFS with surgery, this being 46<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7% and 27<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8% at years 5 and 10. In those who had PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml and Gleason 8–10, BPFS was only 14<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7% and 5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5% at years 5 and 10.</p><p id="par0105" class="elsevierStylePara elsevierViewall">When stratifying the risk for progression, Blute et al.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">20</span></a> also designed a predictive model after radical prostatectomy using Cox's regression model, obtaining a 5-year BPFS of 30% for those cases with negative nodes that had Gleason ≥7 combined with PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml, positive margins and vesicle involvement. Cheng et al. found similar rates of BPFS when including subjects with positive nodes.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">21</span></a> These authors consider that pT3b stage implies a poor prognosis factor which is not isolated, but conditioned by the coexistence of other unfavorable factors.<a class="elsevierStyleCrossRefs" href="#bib0305"><span class="elsevierStyleSup">20–22</span></a> In our overall series, both pT3b per se and in combination with other predictive variables (pathological Gleason 8–10 and PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml) negatively affected survival. Our work highlighted that the pT3b patient group was heterogeneous in prognosis and that a patient group likely to show good results after radical prostatectomy could be selected by using clinicopathological variables (Gleason <8 and PSA <10<span class="elsevierStyleHsp" style=""></span>ng/ml).</p><p id="par0110" class="elsevierStylePara elsevierViewall">There has been a lot of discussion about whether immediate complementary treatment improves SS.<a class="elsevierStyleCrossRefs" href="#bib0320"><span class="elsevierStyleSup">23–29</span></a> Zincke et al.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">29</span></a> published in 2001 that adjuvant AD therapy versus rescue AD improved SS in pT3b. They studied 707 prostatectomized pT3bN0M0 cases between 1966 and 1994. Out of these, 157 patients were treated with adjuvant AD and 550 only after progression. Both groups were homogeneous in terms of age, Gleason and preoperative PSA. Although patients with immediate treatment showed a significantly higher proportion of positive margins (66 vs. 33%); SS was significantly better (95% vs. 87%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.04). Since then, multiple studies have been published with conflicting results, and we can only say that to this day it is unknown if hormonal adjuvant therapy benefits these patients. It seems to delay progression, but it does not modify survival,<a class="elsevierStyleCrossRefs" href="#bib0355"><span class="elsevierStyleSup">30–36</span></a> so the indication for immediate treatment is not justified today.</p><p id="par0115" class="elsevierStylePara elsevierViewall">It is not currently possible to set out definitive recommendations with regard to starting immediate or deferred AD, unless results from randomized studies are brought about. In our experience, treatment with adjuvant AD shortened time to castration resistance; however, it equalized SS to that of the group with the best prognosis treated with a salvage regimen. Nonetheless, the low number of patients, non-random choice in the indication for complementary treatment, and the evidence of more adenopathies in the adjuvant group prevent us from making strong assertions. Besides, a third of the adjuvant and rescue group received concomitant radiotherapy.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Among other constraints, our work presents a non-centralized, unicenter study analyzing a historical series where the following aspects cannot be assessed: the so-called Will Rogers phenomenon,<a class="elsevierStyleCrossRef" href="#bib0390"><span class="elsevierStyleSup">37</span></a> the evolution in the type of patient candidate for radical prostatectomy over the last 20 years,<a class="elsevierStyleCrossRef" href="#bib0395"><span class="elsevierStyleSup">38</span></a> the changes in the pathological qualification criteria with a tendency to upgrade the Gleason score<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a> or the clinicopathological stage migration phenomena of this disease.<a class="elsevierStyleCrossRefs" href="#bib0405"><span class="elsevierStyleSup">40,41</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusions</span><p id="par0125" class="elsevierStylePara elsevierViewall">In patients with prostate cancer treated with radical prostatectomy, seminal vesicle involvement severely affects BPFS and SS. With pathological Gleason 8–10 and PSA >10<span class="elsevierStyleHsp" style=""></span>ng/ml, which are the independent influencing variables, we have been able to design 2 risk groups. A first group, which approximately accounted for two thirds, with no or only one variable, showed a 5- and 10-year BPFS of 46 and 27%. A second group, which accounted for around a third of the patients, with 2 variables involved, showed a BPFS of 14 and 5% at years 5 and 10, respectively.</p><p id="par0130" class="elsevierStylePara elsevierViewall">This risk model highlights that pT3b cases are a heterogeneous group, within which a majority group implies better prognosis with surgical treatment. We assessed the variables that influenced BPFS and designed a risk model.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 127 out of 1.132 patients who underwent surgery (11%) presented seminal vesicle invasion (i.e., pT3b). In the multivariate study of the entire series (Cox model), pT3b affects the BPFS (HR: 2; 95% CI: 1.4–3.3; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001). Other influential factors were the affected borders, initial prostate-specific antigen levels, pathological Gleason score and the presence of palpated tumor. The pT3b tumors have poorer clinical–pathological variables when compared with pT2 and pT3a tumors. Sixty-five percent of the patients evidenced biochemical progression. The BPFS was significantly poorer for pT3b (40<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% and 28<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% at 5 and 10 years, respectively) than for pT2 and pT3a (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.0001). Se evalúan las variables influyentes en la SLPB y se diseña un modelo de riesgo.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Un total de 127 pacientes sobre 1.132 intervenidos (11%) mostraron invasión de vesícula seminal; es decir, fueron pT3b. En el estudio multivariado de toda la serie (modelo de Cox) pT3b influye en la SLPB (HR: 2; IC 95%: 1,4-3,3; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,001). Otros factores influyentes fueron márgenes afectos, PSA inicial, Gleason patológico y presencia de tumor palpable. Los tumores pT3b presentan peores variables clinicopatológicas cuando se comparan con pT2 y pT3a. El 65% ha evidenciado progresión bioquímica. La SLPB es significativamente peor en pT3b (40<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% y 28<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4% a 5 y 10 años) que en pT2 y pT3a (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,0001).
