"apellidos" => "Sánchez-Chapado" ] ] "afiliaciones" => array:1 [ 0 => array:2 [ "entidad" => "Servicio de Urología, Hospital Universitario Príncipe de Asturias, Alcalá de Henares, Madrid, Spain" "identificador" => "aff0005" ] ] "correspondencia" => array:1 [ 0 => array:3 [ "identificador" => "cor0005" "etiqueta" => "⁎" "correspondencia" => "Corresponding author." ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Asociación entre síndrome metabólico y cáncer de próstata: efecto sobre su agresividad y progresión" ] ] "resumenGrafico" => array:2 [ "original" => 0 "multimedia" => array:7 [ "identificador" => "fig0005" "etiqueta" => "Figure 1" "tipo" => "MULTIMEDIAFIGURA" "mostrarFloat" => true "mostrarDisplay" => false "figura" => array:1 [ 0 => array:4 [ "imagen" => "gr1.jpeg" "Alto" => 1742 "Ancho" => 1652 "Tamanyo" => 134317 ] ] "descripcion" => array:1 [ "en" => "<p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">Biochemical recurrence-free survival according to the presence of MS.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">In the last few years there has been growing interest in investigating the role factors related to lifestyle and dietary patterns in the development of prostate cancer (PCa),<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> the most common type of cancer among Spanish males in 2012 according to the GLOBOCAN database.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a> Metabolic syndrome (MS) consists of a cluster of cardiometabolic risk factors, such as abdominal obesity, dyslipidemia, hypertension and plasma glycemia.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">3</span></a> The ENRICA study put the prevalence of MS in adult Spanish males at 26%.</p><p id="par0010" class="elsevierStylePara elsevierViewall">A meta-analysis published in 2012 found an association between MS and PCa among the American white population,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a> and a meta-analysis on obesity and PCa found out that the risk of PCa increased by 5% for each 5-kg/m<span class="elsevierStyleSup">2</span> increase in body mass index (BMI).<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a> A recent meta-analysis found out that patients with MS showed more advanced disease and a higher risk for biochemical recurrence (BR)<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> at diagnosis, so MS could modify the natural history of the disease after radical prostatectomy (RP).<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">8,9</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to assess the impact of MS and of its individual components on the findings at prostate biopsy, on the RP specimen and on BR.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This is an observational study on a cohort of 1319 males with an indication for a first prostate biopsy and who underwent it between January 2007 and December 2011. Non-probabilistic, consecutive sampling was performed during the recruitment period (March–May 2014).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The following variables were collected from medical records: MS and each of the components included in its definition, age, PSAt, digital rectal examination (DRE), diagnosis of PCa at biopsy and Gleason score at biopsy. We collected the anatomopathological variables from the RP specimen and the BR variable of patients with a diagnosis of PCa.</p><p id="par0030" class="elsevierStylePara elsevierViewall">With regard to the calculation of sample size, at least 202 males were required in each group (with and without MS) to find a minimum difference of 15% with statistical power of 95%.</p><p id="par0035" class="elsevierStylePara elsevierViewall">We included those males by PSAt and/or DRE had an indication for a first prostate biopsy (January 2007–December 2011) and whose information regarding MS components was known. We excluded those patients with absence of predictive data to respond to our objectives and those with prostate adenocarcinoma who had not undergone RP.</p><p id="par0040" class="elsevierStylePara elsevierViewall">MS was defined according to the criteria of the National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATPIII) Heart, Lung and Blood Institute, with waist circumference being replaced by BMI.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">BR was defined as at least two consecutive increases of PSA<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.2 after RP.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">With regard to the statistical analysis, means were compared using Student's <span class="elsevierStyleItalic">t</span>-test for independent samples and proportions using Pearson's chi-square (<span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span>) test with a level of statistical significance of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. The patients’ follow-up time was 50<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>18 months. The comparison of the degree of consistency between Gleason score at biopsy and that of the specimen was done by using Pearson's correlation test. The assessment of the predictive variables of Gleason score at biopsy and that of the specimen and sample stratification was uni- and multivariately done using logistic regression, and the assessment of predictive variables for the BR occurrence rate was done using Cox's regression model. The survival analysis for the BR variable was done using Kaplan–Meier curves and the log-rank test was used to compare the curves.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall">1319 Patients underwent prostate biopsy, out of whom 517 (40%) were diagnosed with PCa at biopsy and 802 (60%) were not. The descriptive variables and the association of the variables of patients with or without PCa are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The mean age of all patients was 66<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.1 years (64.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.3 for patients with PCa and 67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.3 for patients without PCa; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The mean PSA prior to biopsy of all patients was 7.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.7<span class="elsevierStyleHsp" style=""></span>ng/ml (9.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.08 for patients with PCa and 7.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.3 for patients without PCa; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The BMI (kg/m<span class="elsevierStyleSup">2</span>) of all patients was 25.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.1 (26<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.1 for patients with PCa and 25<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.03 for patients without PCa, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Two hundred and seventy-five patients (21%) met the diagnostic criteria for MS. We found a higher proportion of PCa diagnoses among patients with MS than among those without MS (130 out of 275 patients [47%], versus 387 out of 1044 patients [37%]) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002; OR: 1.5 [1.1–1.9]). The mean age (years) of those patients with MS was greater than that of those patients without MS (67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.4 versus 65<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.2; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02). PSA (ng/ml) prior to biopsy was higher in patients without MS than in patients with MS (7.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.8 versus 7.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.1; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.9). The BMI of those patients with MS was greater than that of those patients without MS (27<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.8 versus 24.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.7; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). A higher percentage of patients with BMI<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>30 was diagnosed with PCa versus those patients with BMI<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>30 (88 out of 190 patients [46%] versus 429 out of 1129 patients [38%]) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03; OR: 1.4 [1.08–1.9]).</p><p id="par0065" class="elsevierStylePara elsevierViewall">Out of the 517 patients with a diagnosis of PCa at biopsy and who underwent RP, 130 (25%) showed MS and 387 (75%) did not. The most common MS variable among patients with PCa and MS was arterial hypertension (AHT) (106 out of 130 patients, 82%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001; OR: 9.2 [5.6–15]). We did not find statistically significant differences neither in the age nor in the PSA of patients with PCa with regard to the occurrence or not of MS, although both variables were higher in patients with MS (65<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.4 versus 64<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.3, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1; 9.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.3 versus 8.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.3; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.3 respectively). The BMI of those patients with PCa and MS was greater than that of those patients with PCa and without MS (28<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.7 versus 25<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.5; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the clinical and anatomopathological aggressiveness of the PCas diagnosed in patients with regard to the presence or absence of MS. There was a total of 78 BRs, 30 (24%) in patients with MS and 48 (13%) in patients without MS (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003; OR: 2.1 [1.2–3.5]). The mean follow-up time of all the patients who underwent RP was 50<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>18 months, and there was no difference between those patients with and without MS (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.6). The mean time (months) elapsed to BR was 13.1<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.9 for patients with MS and 17.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.2 for patients without MS (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">The multivariate study to predict a Gleason score at biopsy<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>7 showed that only clinical stage behave as a predictor (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01; OR: 1.7 [1.1–2.8]), whereas MS and the PSA prior to biopsy lost their statistical significance (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1; OR: 1.4 [0.91–2.1] and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.3; OR: 1.01 [0.9–1.03] respectively).</p><p id="par0075" class="elsevierStylePara elsevierViewall">The results of the predictive study on the Gleason score of the specimen are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. We tested the interactions BMI*HDL cholesterol; BMI*triglycerides and BMI*glycemic status, with none of them being significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.7; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.4; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.5 respectively). The occurrence of MS independently predicted a worse Gleason score of the specimen (≥7) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001; OR: 3.6 [2.4–5.6]).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The results of the predictive study on the stage of the specimen are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. We tested the interactions BMI*HDL cholesterol, BMI*triglycerides and BMI*AHT, with none of them being significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.7; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.5; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.4 respectively). MS predicted in the multivariate analysis a worse stage of the specimen (≥T2c) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001; OR: 3.2 [1.9–5.3]).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows the results of the prognostic study for BR. MS behaved as an independent predictor for BR (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03; HR: 1.7 [1.08–2.9]) and we found a positive interaction between the Gleason score of the specimen and MS.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The estimation of biochemical recurrence-free survival by using Kaplan–Meier curves is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">The ENRICA study established a prevalence of MS in adult Spanish males of 26%, with AHT and obesity being the two most common aspects of MS.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">4</span></a> Although our study was a retrospective one and focused on patients with suspected PCa, our prevalence of SM was 21%, similar to that in the ENRICA study,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">4</span></a> and the prevalence of MS in males diagnosed with PCa was 25%; the most common MS component in our cohort who underwent biopsy was AHT.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Some of the risk factors for the development of MS also play a role in the development of PCa,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">12</span></a> but there were racial differences described in the American population regarding the risk of developing both conditions.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,14</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Obesity and insulin resistance seem to play a key role in the development of MS, although there is an inverse relationship in the association between diabetes mellitus and PCa.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">15,16</span></a> Various endogenous substances related to insulin resistance and obesity, such as IGF-1, IGF-II, leptin and adinopectin, might mediate in the influence of these clinical factors on PCa development,<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">17–19</span></a> although they were not the object of this study.</p><p id="par0110" class="elsevierStylePara elsevierViewall">In the most recently published meta-analysis on the risk of PCa in patients with MS, MS is associated with a 12% higher risk of developing PCa, but this association was statistically significant only in European studies and for the AHT and waist circumference variables.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> In this regard, the likelihood of finding PCa among our patients with MS was of 60%, close to that published in a Finnish study (56%),<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a> and the variable for which the risk was higher in our study was AHT.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In our study, patients with MS who underwent biopsy showed lower PSA levels than those patients without MS, although, as this was a study which focused on patients at risk of developing PCa, these figures are higher than those in other epidemiological studies.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">22,23</span></a> Those patients with a diagnosis of PCa in our cohort and MS occurrence had a higher PSA than those patients without MS, just as the patients in Jeon's study,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> which could partly explain the more aggressive profile of PCa in patients with MS.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Patients with PCa and MS in our study showed more adverse characteristics both at biopsy and in the RP specimen, which established a positive association between MS and a more aggressive PCa that had also been seen in other studies.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> In our study, MS did not behave as a predictor for developing high-risk PCa (≥7) at biopsy in the multivariate analysis and, although it is not shown in the results, we conducted a concordance study between the Gleason score at biopsy and that of the specimen, this being low (0.37 [0.27–0.46]). There are discrepancies in the literature in this regard,<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">24–27</span></a> and thus in they study carried out by Jeon et al.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> the presence of MS was associated with a decreased risk of developing high-risk PCa at biopsy (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004; OR: 0.1 [0.02–0.4]) and in that by Monforte et al.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> MS was associated with an increased risk of high-risk tumors (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001; OR: 1.75 [1.26–2.41]).</p><p id="par0125" class="elsevierStylePara elsevierViewall">In our study, MS was capable of predicting in a multivariate model a Gleason score in the specimen ≥7, and both BMI and HDL cholesterol individually behaved as predictors when adjusted by the rest of variables. It was also capable of predicting a specimen stage<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>T2c, and both BMI and HDL cholesterol as well as triglycerides behaved as predictors when adjusted by the rest of variables. A meta-analysis published in 2013<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> established an association between MS and a high Gleason score, with an overall OR of 1.36 (0.90–2.06) and a risk for more advanced stage with an overall OR of 1.37 (1.12–1.68), so in this regard our study provided a new relationship in the predictive behavior of MS in the findings in the RP specimen in a cohort of white Caucasian males.</p><p id="par0130" class="elsevierStylePara elsevierViewall">The present study shows that MS is an independent prognostic factor for BR adjusted by the traditional clinical and pathological variables, and besides we found a positive interaction between the Gleason score of the specimen and MS, so that MS behaves as an effect modifier in the association between the Gleason score of the specimen and BR. However, none of the individual MS variables behaves in isolation as an independent prognostic factor for BR, which implies the need for the presence of at least three MS factors to predict a higher BR rate. Other authors have found that MS is an independent prognostic factor for BR,<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">28,29</span></a> and a study by Asmar found out that obesity and AHT increased the risk of BR after RP.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">We can conclude that in our population MS increases the risk of developing PCa, increases its aggressiveness and increases the risk of BR after RP, so future research enabling to modify MS occurrence in our patients might contribute to reduce the percentage of BR and cancer-specific mortality in patients with PCa.</p><p id="par0140" class="elsevierStylePara elsevierViewall">This study has some constraints, such as the replacement of waist circumference by BMI as a diagnostic variable in the criteria of the NCEP-ATPIII, which can be explained by the retrospective nature of the study, so the relationship between BMI and androgen metabolism is less consistent. Besides, our population represents a selected population of patients with a high risk of developing PCa (elevated PSA and pathological DRE), so the results cannot be generalized to the entire population. The impact on the biopsy findings, the radical prostatectomy specimen and biochemical recurrence was evaluated using logistic regression and Cox regression.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Of the 1319 patients, 275 (21%) had metabolic syndrome, and 517 prostate cancers were diagnosed. A greater percentage of metabolic syndrome was found among patients with prostate cancer than among patients without prostate cancer (25% versus 18%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.002).
