Análisis poblacional a nivel nacional: grupo GESCAP" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Among oncological diseases, prostate cancer (PCa) is one of the cancers that are diagnosed more often in Europe.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Recently, the European age-standardized rate or incidence rate of PCa has been estimated in the Spanish population at 70.75 cases per 100,000 men, and the clinical profile of patients with <span class="elsevierStyleItalic">de novo</span> diagnosis of PCa has also been published.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">2,3</span></a> Although this information is useful to assess the impact of the disease and its management in the Spanish population, it seems necessary to collect information on the characteristics of treatments applied in routine clinical practice too and evaluate compliance of the EAU guidelines in a sample of patients newly diagnosed with localized PCa, in order to obtain accurate and updated data on the major therapeutic approaches used in PCa in Spain.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In recent years, we have incorporated new alternatives to the therapeutic arsenal of PCa<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">4</span></a> through clinical practice guidelines. These are a series of systematically developed recommendations, in order to help professionals and patients make decisions about health care and to select the most appropriate diagnostic or therapeutic options for a specific clinical condition.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Due to the lack of randomized clinical trials, it is not possible to conclude that a particular treatment is clearly better than another. However, in view of the available scientific literature, some recommendations can be made based on the clinical stage or the risk group depending on the PSA or age at diagnosis.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">6,7</span></a> It is also necessary to consider the fact that each treatment option may have significantly different side effects on patients, including sexual dysfunction, infertility, or urinary incontinence, and different specific mortality rates.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">8</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">This study was conducted as part of a national registry to estimate the incidence of PCa in Spain<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">2,3</span></a> and intends to describe the therapies established in routine clinical practice in Spain and evaluate their adherence to the 2010 EAU guidelines.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">Epidemiological, observational, prospective, multicenter, and national study to observe the evolution of the disease and the therapeutic approach in patients with PCa. The study protocol was reviewed and approved by the Ethics Committee of the Hospital Virgen de las Nieves (Granada), and the study was conducted in accordance with the ethical principles of human research of the Declaration of Helsinki.</p><p id="par0035" class="elsevierStylePara elsevierViewall">This research was conducted as part of a study recently published, in which the incidence of PCa in Spain during 2010 was analyzed,<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">4,5</span></a> covering 21.8% of the Spanish male population.</p><p id="par0040" class="elsevierStylePara elsevierViewall">24 public referral hospitals were included. These hospitals were selected so that the age distribution of the population of reference for each hospital was very similar to the national average. All centers provided information on the treatment of each patient. These were followed for at least one year after diagnosis. The selection criteria of patients were: male comes to the Urology Unit with newly diagnosed PCa and histological confirmation in the period from January 1 to December 31 2010. This study only included patients with localized PCa (low, intermediate, high risk) and locally advanced.</p><p id="par0045" class="elsevierStylePara elsevierViewall">An application was created in web platform to record all available information. For each new registered case, sociodemographic and clinical basic data were specified, as well as information regarding any treatment received and control during follow-up. The figures and percentages shown were calculated obtaining some risk strata (in case of low and intermediate or high and locally advanced stage) and some treatment groups in case of RT, BT and HT and therapy alone or combined. No statistical tests were performed.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0100" class="elsevierStylePara elsevierViewall">A total of 3293 patients (84.05%) operated by any treatment were classified in the category of localized tumor according to the criteria of the D’Amico risk stratification (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the number of cases and the percentages of each treatment depending on age. The percentages for the age group and for each treatment were calculated in each risk stratum, excluding 17 patients who withdrew because age was not known.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Focusing on the treatments administered in monotherapy or in combination (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), as recommended by the EAU guidelines, 86.5% of low-risk patients received monotherapy and 43.62% of patients with high risk or locally advanced received combined treatment.</p><p id="par0115" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, in the group of patients ≤65 years RP was performed, in 66.23% of low-risk patients, in 70.76% of patients with intermediate risk, in 61.73% of high-risk patients, and in 12.50% of patients with locally advanced disease. The percentage of patients undergoing RP is noticeably lower in the group of patients >65 years (35.03% of patients with low risk compared to 66.23% in the younger age group, for example). Regarding treatment with RT, 5.00% of patients with low or intermediate risk ≤65 years were treated with RT alone, while 34.51% of patients with high risk or locally advanced disease >65 years received RT and HT. 30.36% of the group of patients with high-risk localized disease and locally advanced received only HT. This treatment was mainly used in the patients >65 years. 15.20% of low-risk patients received only BT, while 4.08% of patients with intermediate or high risk were treated with BT combined with RT and/or HT. 5.56% of the patients who were followed-up followed the guidelines of AS/observation. This management option was applied to 12.97% of low-risk patients >65 years and 3.90% of low-risk patients ≤65 years. Finally, 0.87% of all patients received ELT.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">In this paper we analyzed the main trends of treatment for clinically localized PCa in Spain, in a large cohort of patients with newly diagnosed PCa. Most patients with low or intermediate risk under 65 (almost 70%) were operated by RP, coinciding with the recommendations of the EAU guidelines and with the previously published studies.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">10–13</span></a> In 17.7% of patients treated with RP, lymphadenectomy was performed. Besides the classical clinical variables that are used to indicate the performance of lymphadenectomy, the hospital factor and technique of approach to prostatectomy used appear to have significantly influenced the decision of conducting lymphadenectomy.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">14</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">According to our data, the AS/observation is less recommended as a treatment option, especially in patients ≤65 years, probably due to lack of recommendations based on a high level of evidence. There is not much information on the use of AS in routine practice, but the data of the CaPSURE register place it around 10%, according to the period considered.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">15</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Compliance with the 2010 EAU guidelines<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a> means following their recommendations based on age and risk stratification and, in accordance with them, choosing the best therapeutic option to prevent overtreatment, minimize the incidence of mortality and ensure, as far as possible, satisfactory levels of quality of life related to health in patients with PCa. In these terms, as recommended by the EAU guidelines, patients should initially receive combination therapy if they are at high risk or locally advanced disease, while patients with low or intermediate risk should receive monotherapy as an initial approach. The RP is the standard treatment and is recommended for younger patients (≤65 years) with low or intermediate risk; and in patients with high risk or locally advanced disease, RT combined with HT is considered and, optionally, with RP (under certain criteria).</p><p id="par0135" class="elsevierStylePara elsevierViewall">The different treatment options have different effects on health and quality of life of patients, so the treatment of those with PCa should adapt to the characteristics of each particular patient following the available clinical practice guidelines. Nearly 5% of patients ≤65 years received RT in monotherapy, a percentage that is lower than other similar studies.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">11,13,16</span></a> This could be due to the fact that the RT with HT is a common practice in high-risk groups or locally advanced cancer and RP is the most common treatment among patients with low-intermediate risk. Another important aspect is that the RP could have a better prognosis than RT because patients with RP are younger.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">17–19</span></a> BT has also become an attractive option, especially for low-risk patients,<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">20–22</span></a> because it has fewer side effects related to the treatment than RT or RP.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">23</span></a> In addition, the main adverse effects are temporary urinary symptoms.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">21</span></a> In this study, 15% of low-risk patients were treated only with BT, a similar percentage to that observed in previous studies: 13.3%,<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">10</span></a> 8.5–12.3%.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">11</span></a> BT in monotherapy was also used in 5.78% of patients with intermediate risk. These results show that BT is an emerging treatment for low or intermediate-risk patients.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Regarding patients at high risk or with locally advanced cancer, the primary option should be combined RT with HT followed by HT in monotherapy, optionally with RP in young patients or even with some combination between BT, RT and HT, as recommended by the 2010 EAU guidelines,<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">6</span></a> since a multidisciplinary and multimodal therapeutic approach is essential in its management.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">11,24</span></a> The percentage of patients treated with RT combined with HT was approximately 35%, which is consistent with data from previous publications,<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">25–28</span></a> while RP was performed in a high percentage of young patients at high risk (although it was not the preferred approach until a few years ago and it has recently begun to be an optional treatment among young patients who refuse RT and meet certain criteria<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">24,29</span></a>). Approximately one third of patients with high risk or locally advanced disease received only HT. It is a higher proportion than previously reported,<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">28,30</span></a> but we must bear in mind that in men with high risk the use of HT as the main treatment has been increasing over time. The percentage of patients older than 65 who received only HT was higher than that of patients younger than 65, as shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. We must also highlight that HT has worse results and higher specific mortality of the disease<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">17,31</span></a> than other treatments, and it is adversely associated to comorbidities such as osteoporosis, cardiovascular diseases, or diabetes. The low percentage of patients receiving any combination of BT, RT and HT is explained by the high percentage of patients treated with RT combined with HT or HT in monotherapy, and also because the use of this modality is still object of analysis.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">20</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Finally, it is noteworthy that the ELTs were used only in 0.88% of patients with PCa.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The results shown in this study refer only to the main strategy; still no information is known on the incidence rates of death, the quality of life related to health, or the evolution of this cohort of patients.</p><p id="par0155" class="elsevierStylePara elsevierViewall">This is the first European national study that analyzes the therapeutic management of patients with localized PCa depending on the risk group to which they belong. Most young patients (≤65 years) with localized low and intermediate-risk PCa were treated with surgery, which is adapted to the recommendations of the 2010 EAU guidelines. The RT is indicated mainly in patients older than 65 with high-risk localized and locally advanced disease, and with concomitant androgen suppression. A third of patients with high-risk localized and locally advanced disease were treated exclusively with HT.
