Resultados de una encuesta nacional entre urólogos que ejercen en España" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Urethral stricture (US) disease in male adult leads to a high number of urological consultations.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The management options range from minimally invasive endoscopic procedures (EP) to open urethroplasties. US management patterns changed over time and between different countries, as previous results have evidenced.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–11</span></a> Since these practices affect the quality of care given to our patients, their assessment would seem important. The obtained results should guide, if needed, further improvement strategies.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The aim of this study is to evaluate the practice patterns on diagnostic and therapeutic approaches for adult male anterior US among urologists in Spain.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">We developed a non-validated questionnaire, based on similar surveys conducted in other countries, in order to assess the current practice on diagnosis, treatment and follow-up of patients with male anterior US disease. The questionnaire included 23 items, including demographic data and questions related to evaluation, decision process, beliefs and opinions on the specific topic (See Supplementary Material). Due to the non-validated nature of the assessment tool, previous versions of the questionnaire were evaluated by the authors, receiving also comments from experts in the field of urethral surgery. After further review, an agreement was reached on what items should be included. The electronic survey, hosted in the webpage of AEU, was tested by the authors and fellow colleagues, to ensure the proper functioning of the tool and to check the accuracy with the agreed version of the questionnaire. Invitation letters were sent by email to all members of the Spanish Urological Association (Asociación Española de Urología, AEU). Each email contained an individual link to the on-line survey. The questionnaire was anonymous and self-administered. A single questionnaire was accessible for each member, avoiding duplicate answers. Reminder invitation letters were sent to non-responders every 2 months, with up to 3 reminders across the recruiting period. Information was collected between February and September 2016. We sent 1737 invitation emails, but only 1649 reached the urologists inbox. Of those 1649 emails, 1015 were opened by users. Of those who opened the invitation email, 307 urologists (30.2%) followed the link to the survey webpage, with a total of 221 questionnaires filled, giving a response rate of 21.7%. All answers were categorized and stored in a database. All questionnaires were considered for analysis, including those with incomplete information. Frequency tables were generated for each question, summarizing the distribution of answers. Univariate comparisons between groups were conducted using the <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup">2</span> test for categorical variables. Statistical significance was considered when p value was <0.05. Statistical analysis was performed using STATA 13.0 software (StataCorp, TX, USA). When relevant, we followed the Checklist for Reporting Results of Internet E-Surveys (CHERRIES) guidelines during development of the electronic tool and evaluating the collected data.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0020" class="elsevierStylePara elsevierViewall">Data on age distribution, hospital type and level of responders are displayed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> (for further information on geographic distribution see Table 8 in the Supplementary Material). Asked whether their hospital had a unit or physician specialized in US disease, 117 (59.7%) responders answered affirmatively. Only 3 urologists (1.4%) reported not treating any patients with US during last year, while most of the urologists treated at least 11 patients per year. 40 (20.1%) urologists performed >10 uretroplasties per year, while 44 (22.1%) stated that they did not perform any (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">Preferred diagnostic tools and follow-up methods are summarized in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. Retrograde urethrogram (RUG) was the commonest diagnostic test. Uroflowmetry (UF) was the most widespread follow-up tool. RUG was performed by radiologists, according to 145 responders (71.8%), and by urologists in the remaining cases.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">Endoscopic techniques – urethral dilation (UD) and direct vision internal urethrotomies (DVIU) – were the most frequently described therapeutic approaches. The full distribution of management options is shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. Asked about the maximal length of stricture suitable for management with EP, 123 urologists (61.8%) answered <1 cm and 45 (22.6%) <1.5 cm. Only 3 urologists (1.5%) considered EP useful for strictures >2 cm. When performing EP, 170 responders (82.9%) routinely used a guidewire, 32 (15.6%) used it only in selected cases and only 3 (1.5%) never used it. Regarding the placement of a urethral catheter after EP, only 3 urologists (1.5%) did not routinely leave one, 28 (13.8%) kept the catheter 3 days or less, 34 (16.8%) between 4 and 6 days, and 61 (30.1%) one week. 59 (29.1%) maintained the catheter for 2 weeks, and 18 (8.9%) 3 weeks or longer. The preferred size for this catheter was 18 F for 79 responders (38.5%), 16 F for 61 (29.8%) and 20 F for 44 (21.5%). Larger-bore catheters (>20 F) were chosen by 6 urologists (2.9%) and narrow sizes (<16 F) also by 6 (2.9%). 9 colleagues (4.4%) stated not having any preferred caliber.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The preferred urethroplasty technique for bulbar US was end-to-end anastomosis, followed by dorsal augmentation using grafts (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>). After urethroplasties, 83 urologists (49.7%) routinely performed RUG before or immediately after removing the catheter, while 46 (27.5%) claimed doing it depending on each individual case and 38 (22.8%) never performing it.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The participants were asked about how they would manage two different patients. First, a 35 year-old uncircumcised male, with a 3.5 cm idiopathic bulbar US, complaining of poor flow, with maximum flow rate (Qmax) of 6 ml/s. Nearly half of responders (100, 48.8%) would offer him a dorsal augmentation urethroplasty using grafts, while only 12 (5.9%) considered him a candidate for EP (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>). The second scenario was a 24 year-old male, with a 1 cm idiopathic proximal bulbar US, with 2 previous DVIU (last one 6 months ago), complaining of poor flow, with Qmax of 7 ml/s. In this case, anastomotic urethroplasty would be the choice for 131 responders (64.2%), with only 5 (2.5%) offering him EP (<a class="elsevierStyleCrossRef" href="#tbl0035">Table 7</a>).</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia><elsevierMultimedia ident="tbl0035"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Participants were also asked about their opinion as to how US should be managed. The so-called “therapeutic ladder”, starting with minimally invasive procedures (UD, DVIU) and considering urethroplasty only after repeated failure of these EP, was supported by 59 urologists (29.1%). The remaining 70.9% considered urethroplasty as the primary option, when indicated, as the adequate approach.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Regarding the need for referral centers for treatment of male anterior US disease, 175 urologists (88.4%) considered them as necessary. Only 102 (52.9%) rated their specific training on US as adequate. Asked about courses and workshops on this topic, 193 (95.1%) considered both theoretical and hands-on-courses as useful, while 10 (4.9%) preferred only hands-on ones. Nobody considered the courses on this topic as useless.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Responders from teaching hospitals (private or public) stated that their centers had a person or unit dedicated to US in a higher percentage than responders from non-teaching hospitals (RR:1.5; 95%IC:1.1–2.1 p < 0.005). Likewise, tertiary centers had this person or unit more frequently than secondary ones (RR:1.3; 95%IC:1.1–1.8 p < 0.05).</p><p id="par0060" class="elsevierStylePara elsevierViewall">Urologists performing higher number of urethroplasties (> 10 per year), compared with low volume surgeons, use more frequently patient reported outcome measure (PROM) questionnaires for diagnosis (RR:7.6; 95%IC:2.1–27.2 p < 0.001) and follow-up (RR:4.0; 95%IC:1.3–11.8 p < 0.01). They also performed “non-transecting” urethroplasties in a higher percentage (RR:3.6; 95%IC:2.3–5.8 p < 0.001), and would choose more frequently a urethroplasty as a first line therapy, if indicated, instead of a “therapeutic ladder” (RR:1.3; 95%IC:1.2–1.6 p < 0.005). Regarding specific training in US disease, urologists performing >10 urethroplasties per year rated it as adequate in a higher percentage than low volume surgeons (RR:1.5; 95%IC:1.2–1.9 p < 0.005).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">Male US disease is a common problem, with an estimated prevalence between 229 and 627 cases per 100.000 adult males.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,13</span></a> In 2000, US disease accounted for >191 million USD in expenses, with 69% of them related to ambulatory surgery visits.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> EP are the most common treatment options for US, both primary and recurrent.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> This approach is certainly minimally invasive but has been shown to be curative only in selected cases, with little chances for success when repeated more than twice.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15–17</span></a> Moreover, some authors evidenced a negative impact of repeated intervention on the outcome of further urethral reconstructions<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a> and this has also been proved as less cost-effective.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,20</span></a> Open urethroplasty is the definitive treatment for most anterior US, with excellent long-term outcomes,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> and should be offered as a primary treatment option in certain cases.<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a> Adequate management of US patients requires appropriate education of the urological community, exposing the poor outcomes of repeated EP and current non-justification of the “therapeutic ladder” belief, as primary urethroplasty would lead to the best outcome in most patients.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">Knowledge of practice patterns in US started to be evaluated a few years back. A first attempt was made in the UK and Ireland, with a mailed survey assessing practices on pelvic-fracture related urethral injuries.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Focused in anterior US, another mailed survey was conducted among board certified urologists in the USA.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Both studies suggested the non-familiarity with urethral surgery and the lack of knowledge of literature on this topic as the reasons for an excess of EP. The Netherlands,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Italy,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> and more recently, Germany,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> conducted national surveys on anterior US practices, based on the questionnaire of the American study.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">In our survey, responders were mainly from public teaching hospitals and tertiary centers, representing a higher percentage than in previous surveys. In USA, only 10.7% were from Academic practices, 18% in Netherlands from Academic teaching hospitals, and 20% in Germany from Academic/University Centers. The number of US managed per year is higher in Spain compared with other countries. 38.7% of urologist treated more than 20 US yearly, while 30.1% in Netherlands, 5.9% in Italy and 20% in Germany.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The diagnostic tools in Spain are similar to those in the contemporary German survey, with RUG and UF as preferred choices. For follow-up, UF is the commonest test, as in all previous surveys, but RUG is also frequent in our country, as opposed to others (17.4% in Italy, 9.6% in Netherlands, 13.2% in USA).</p><p id="par0085" class="elsevierStylePara elsevierViewall">EP (DVIU 95.7% and UD 94%) are the most widespread management options, as in all previous surveys. In Spain, blind internal urethrotomy is uncommon, compared with the Netherlands (81.4%), Germany (57.5%) or Italy (42.4%). The selection of US suitable for EP seems to be appropriate, with 84.4% of responders using them for strictures <1.5 cm. This percentage is higher than other countries like Italy (71.5%), Netherlands (50.2%) or USA (44.1%). It is particularly low the number of urologists employing EP for strictures >2 cm (1.5%), compared with 28.1% of Dutch and 13.8% of German colleagues. Spanish practitioners tend to keep the catheter after EP longer than others, with only 13.8% removing it in the first 72 h, as advocated by current evidence.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> Conversely, 38% maintain the catheter for 2 weeks or longer, compared with only 8.2% in USA and 1.6% in Germany.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Spanish urologists report performing urethroplasties in a higher percentage than other surveyed countries. Only 22.1% stated not performing urethroplasties in Spain, compared with 77% in Netherland, 73.2% in Germany, 60.8% in Italy and 57.8% in USA. The Public National Health System in Spain is divided into communities with their own budgets, making referral of patients to other hospitals difficult. This could be the reason for our high percentage of urologists performing urethral surgery. Also, our Health System lacks any economic reimbursement for performing urethroplasty or DVIU, so no economic bias seems to affect the management decision. More than 3 out of 4 responders used end-to-end anastomosis, compared with 28.9% in Germany, 16.4% in Netherlands or 15.3% in USA. In line with previously published data, in Spain, grafts are more frequently used than flaps. However, the preferred location seems to be different. In other countries, ventral graft augmentation is the commonest, while Spanish urologists preferred dorsal placement for grafts.</p><p id="par0095" class="elsevierStylePara elsevierViewall">As in previous surveys, two clinical cases assessed the management of different situations. In a long (3.5 cm) bulbar US in a young patient, Spanish responders would perform a dorsal graft augmentation, with only 6% offering EP. In other surveys, EP were chosen for a case like this by between 20.5 and 43.8%. And, when augmentation techniques were offered, they tend to prefer ventral grafting. A second case presented a young male with a bulbar stricture that recurred after 2 failed DVIU. End-to-end anastomotic urethroplasty would be offered by most of responders, as in previous surveys.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Most of our responders acknowledge that urethroplasty could be the first line therapy according to current evidence, instead of climbing a “therapeutic ladder”. In previous surveys the commonest opinion was the opposite, with only 21.3% supporting urethroplasty as initial treatment in the USA (survey performed in 2002). Surprisingly, even in the recent German study (2015–2016), this percentage was still low (26.2%), supporting the need for further education programs on the limits of EP and the indications for early urethroplasty.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Comparing these beliefs by the number of urethroplasties performed annually showed significant differences. Urologists performing higher number of procedures are more prone to choose urethroplasty as first option in selected patients. This conclusion was also demonstrated in the previous American survey,<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a> and supports the need for high volume specialized centers. Teaching hospitals have US disease units more frequently than non-teaching ones. In the German survey, teaching hospitals were also more likely to select open reconstructive treatments, instead of EP. Likewise, high volume surgeons use better diagnostic and follow-up tools, as PROM questionnaires in our survey. For the first time, we are able to demonstrate that high volume surgeons are also using new urethroplasty techniques (as “non-transecting” ones) in a significantly higher percentage than low volume ones. Most of our responders (88.4%) agree with the need for referral centers for treatment of US disease. This was also previously suggested for both anterior<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a> and posterior urethral injuries,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> although that was not asked directly to the urologic community, as we did in our survey.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Our study presents some limitations that should be discussed. The response rate is low (21.7%), which could bias our outcomes. However, our response rate is in line with previous US-related on-line surveys.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> The reply rate to surveys tends to be variable, depending on the targeted population. The American study, performed on a randomly selected sample of board-certified urologists, had only a 34% response rate.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> More recently, the German survey was returned by only 14.6% of urologists.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> To date, the highest response rate was achieved by Dutch and Italian studies (74% and 74.7%), but one was conducted over all the urologists in The Netherlands – which is a small population-, while the other was distributed among a randomly selected group of Italian urologists, with no information about how the authors selected them. We targeted all members of the AEU, not only a sample of them, so a low response rate was expected. Certainly, some degree of selection bias would exist towards a higher percentage of academic centers and reconstructive urologic surgeons answering. Being this group of tertiary centers the most represented one could overestimate their current practices.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Another potential limitation could be the different computer literacy and access to emails among members, leading to age bias. As the age distribution of responders is uniform, it is unlikely that younger urologists were selectively targeted.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusions</span><p id="par0120" class="elsevierStylePara elsevierViewall">Anterior US in Spain are managed by a high percentage of urologists, with EP being the commonest. The selection of patients for these procedures is better than previously reported in other countries. A high percentage of Spanish urologists performs urethral surgery, but in a low volume. Urethroplasties are mainly anastomotic repairs, with an increased tendency to dorsal approach when augmentation procedures are performed. High volume urologists use a wider and more exhaustive array of diagnostic and follow-up modalities as well as more recently developed urethroplasty techniques. Demography data and practices on evaluation and treatment of US were included. 1737 invitation letters sent by email, with 21.7% response rate. Data were prospectively collected during 2016. Descriptive analysis and univariate comparisons conducted using <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup">2</span> test. Statistical significance considered when P ≤ 0.05.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0085" class="elsevierStyleSimplePara elsevierViewall">Responders were mainly from Tertiary and Teaching University Hospitals. 63.2% treated ≥10 patients/year with US. Retrograde urethrogram (RUG) was the commonest diagnostic tool followed by uroflowmetry (UF), and internal urethrotomy under direct vision (DVIU) the most frequent treatment. 84.4% limited DVIU for US ≤ 1.5 cm. 62.3% performed ≤5 urethroplasties/year. Anastomotic urethroplasties were performed by 75.7% and graft repairs by 68.9%. Dorsal grafting was preferred rather than ventral. Non-transecting techniques were used by 23.9%. UF was the most common follow-up tool. 88.4% felt that referral units were required. Tertiary hospitals used Patient Reported Outcome Measure (PROM) questionnaires more frequently than secondary centres.
