B) Intermediate D’Amico risk group.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Cryotherapy is a minimally invasive treatment option for prostate cancer (PCa) that employs freezing/thawing cycles to induce vascular and cellular damage (rupture of cell membranes, denaturation of protein), apoptosis and coagulative necrosis of tumor tissue. The technique consists in the transperineal placement of cryoprobes under transrectal ultrasound control and the formation of ice-balls in the prostatic gland with the passage of pressurized Argon though the tips of the cryoprobes.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Since 2010, cryotherapy has been adopted in our center for primary treatment of prostate cancer in highly selected patients as well as a less invasive option to salvage radical prostatectomy after radiotherapy failure. We report our experience with cryotherapy as a primary treatment option for low and intermediate risk prostate cancer by comparing oncological and functional outcomes of whole-gland cryoablation (WGC) vs hemi-cryotherapy (HC).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study population (inclusion and exclusion criteria)</span><p id="par0015" class="elsevierStylePara elsevierViewall">We conducted a retrospective study including all patients who underwent cryotherapy for prostate cancer in a single center from 2010 to 2018. Inclusion criteria for primary cryotherapy of prostate cancer included patients with ≥70 years old or younger if unfit for radical prostatectomy or radiotherapy, low-intermediate D’Amico risk PCa, estimated prostate volume of ≤60 gr. Exclusion criteria involved indication of cryotherapy for salvage treatment, follow-up of less than one year, high risk PCa (≥ T2c and/or PSA >20<span class="elsevierStyleHsp" style=""></span>ng/mL and/or Gleason ≥ 8) and metastatic patients.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Selection for cryotherapy</span><p id="par0020" class="elsevierStylePara elsevierViewall">Prostate cancer work-out included serum PSA, digital rectal examination, multiparametric MRI (mpMRI) and prostate biopsy. All patients underwent systematic biopsy plus eventual 2–3 additional targeted cores for each lesion detected at mpMRI.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Hemi/focal-ablation was offered to patient with unilateral lesion at mpMRI and proven at prostate biopsy. In case of whole gland ablation and a prostate volume 40–60<span class="elsevierStyleHsp" style=""></span>mL, androgen deprivation therapy (ADT) was administrated during 6 months prior to cryotherapy in an attempt to reduce gland volume.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Cryotherapy technique</span><p id="par0030" class="elsevierStylePara elsevierViewall">All the procedures were performed using the Precise® Cryoablation System (Galil Inc). The treatment was conducted with the patient placed in the dorsal lithotomy position. Placement of cryoprobes was guided by a 4−10<span class="elsevierStyleHsp" style=""></span>MHz biplanar transrectal ultrasound probe. In case of whole treatment ablation, a relevant number of cryoprobes were placed in the prostatic gland at a distance of 1−2<span class="elsevierStyleHsp" style=""></span>cm from one and another according to the types of cryoprobes selected (IceRod, IceSphere or IceSeeds). The safety distance from the anterior rectal and the urethral wall was >0,5<span class="elsevierStyleHsp" style=""></span>cm, in order to prevent rectal fistula and/or urethral stricture, respectively. Similar geometrical principles were followed during a hemi ablation in the targeted lobe.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Two freeze-thaw cycles –with at least -40° reached in the targeted area-were assessed in real time under transrectal ultrasound guidance. Patients were discharged one-day post-operatively in case of uneventful hospital stay and urethral catheter was removed at 7-days post-operative.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Follow-up was performed measuring PSA every 3–4 months the first year and every 6 months thereafter. During follow-up, post-cryotherapy prostate biopsies were not routinely carried out and were performed only in patients with a biochemical failure according to Phoenix criteria (elevation of PSA nadir + ≥2<span class="elsevierStyleHsp" style=""></span>ng/mL). All data were collected prospectively from an implemented institutional database.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Definition of endpoints and statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">The primary endpoint was failure of cryotherapy. Different definitions were used according to laboratory, pathological or clinical parameters as follows: 1) biochemical failure (> PSA nadir + ≥2<span class="elsevierStyleHsp" style=""></span>ng/mL), 2) histological persistency/progression of a clinically significant PCa at prostate re-biopsy (Gleason score ≥7), 3) need of re-cryotherapy or any further salvage treatment.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Secondary endpoints were: postoperative complications stratified according to the Clavien-Dindo, early postoperative and at 1-year follow-up urinary incontinence, and postoperative de novo erectile/sexual dysfunction. De novo urinary incontinence was defined as the use of any pad (including safety pad) after the cryotherapy. De novo sexual dysfunction was defined as new onset of penetration inability after cryotherapy.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Patients were stratified in 2 groups according to whether they underwent WGC or HC, and oncological, surgical and functional outcomes were compared. Data were analyzed using Xlstat® version 2018.5 (Addinsoft, Paris, France) and Graphpad Prism® version 7 (GraphPad software, California, USA). Frequencies of continuous variables were presented as median plus range [min-max] and categorical variables in percentage.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The continuous variables were analyzed with the <span class="elsevierStyleItalic">t</span>-test and categorical variables with the chi<span class="elsevierStyleSup">2</span> test or Fischer test as appropriate. Survival rates was assessed by the Kaplan-Meier technique and compared by Log-rank test. Significance differences were considered when <span class="elsevierStyleItalic">p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05.</span></p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">After applying inclusion/exclusion criteria, 66 patients were included: 40 (61%) whole gland cryoablation and 26 (39%) hemi-cryoablation. Baseline clinical and tumor characteristics are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. At diagnosis, median age was 74 years [42–81], median ASA score was 2 [1–3] and median PSA was 6.7<span class="elsevierStyleHsp" style=""></span>ng/mL [1.2–11.7]. According to the D’Amico classification, low and intermediate risks were respectively 13 (20%) and 53 (80%) in the whole cohort, with no significant difference in proportion between whole-gland and hemi cryoablation groups (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.75) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Intra- and peri-operative outcomes are reported in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. Median surgical time of cryoablation was 120 [60–180] min in whole gland vs 90 [60–140] min in hemi-cryoablation (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.42). Less number of cryoprobes were employed in the hemi-ablation group with 4[4–6] versus 7 [4–10] respect to the whole-gland ablation (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01). Median hospital stay and duration of indwelling bladder catheter were not significantly different (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The prevalence of complications Clavien ≤ 2 were significantly lower in the HC group (23% vs 48%; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04). The most frequent postoperative complications were acute urinary retention (18%) and gross hematuria (9%). There was no complication Clavien ≥ 3 and no rectal fistula.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Concerning functional results, pre-operative, 1-month and 12-month urinary continence rates were respectively 100%, 65% and 83%, with no significant difference between hemi vs whole-gland cryoablation in all time-points (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.99) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Pre-operative erectile dysfunction was 80% in the whole gland cryoablation group vs 57% in the hemi-ablation group (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04). Patients who had a normal preoperative sexual function and developed a de-novo erectile dysfunction postoperatively were 75% in the whole-gland ablation groups vs 46% in the hemi-ablation (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.33) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">During a median follow-up of 36 months, 18 patients (27%) experienced a biochemical failure, 10/40 (25%) in the WGC group vs 8/26 (37%) in the HC group (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.76) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Overall, a histology persistency/progression was confirmed in 10/66 patients (15%) 7 (18%) in the whole-gland ablation group, and 3 (12%) in the hemi-ablation group (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.73) - with a median Gleason score in the prostate re-biopsy of 8 [7–9].</p><p id="par0090" class="elsevierStylePara elsevierViewall">In the WGC group, 5 patients were deemed eligible for an ancillary/salvage treatment: 3 patients had a second/salvage cryotherapy, 1 patient had an external beam radiotherapy (EBRT), and 1 patient started androgen deprivation therapy (ADT). Finally, one patient did not have a second line treatment because of a Gleason score 6 in post-cryotherapy prostate biopsy and stable serum PSA. In the hemi-ablation group, first line failure occurred in 3 patients: two patients undertook a whole-gland re-cryotherapy, and 1 patient received ADT.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Biochemical failure free survival, positive prostatic re-biopsy free survival and second line treatment free survival according to Kaplan Meier, are reported in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. Four years cryotherapy failure free survival in WGC vs HC were 69% vs 53% according to definition 1 (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.24), 82% vs 80% according to definition 2 (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.95), 83% vs 77% according to definition 3 (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.73). There was no significant difference in the incidence of treatment failure, according to the type of cryoablation (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) and the D’Amico risk group stratification (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> A, B).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">In this retrospective single-center study of cryotherapy as first line treatment of low or intermediate prostate cancer, we found that patients selected for hemi-cryoablation had comparable oncological outcome, less perioperative complications, and similar functional results compared to whole-gland cryoablation.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Although ablation therapies –including high-intensity focused ultrasound, cryotherapy, vascular-targeted photodynamic (VTP) therapy, etc. - are still considered as treatment options under investigation in most guidelines, cryotherapy is an established alternative to prostatectomy and radiotherapy in the treatment of low-intermediate risk prostate cancer in high-volume tertiary centers. In 2010, Donnelly et al. reported the results of a prospective study that randomized 244 patients with localized prostate cancer of low to intermediate D’Amico risk, between whole gland cryoablation and EBRT.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> All patients had a 3–6 months ADT prior cryotherapy and EBRT. After a mean follow-up of 100 months, the authors reported no significant difference between cryotherapy and EBRT in biochemical recurrence free survival, disease-specific survival and overall survival. However, the incidence of a positive biopsy post-treatment was in favor of cryotherapy (7.7% vs 28.9% for cryotherapy vs EBRT group, respectively; p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0004).</p><p id="par0110" class="elsevierStylePara elsevierViewall">The COLD registry accrued 2427 cases of cryoablation of localized prostate cancer and stratified free-biochemical recurrence survival according to the D’Amico classification and the PSA nadir.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The 5-year biochemical disease-free survival rates of low, intermediate and high D'Amico risk were respectively of 92%, 76% and 71% in case of PSA nadir <0,1<span class="elsevierStyleHsp" style=""></span>ng/mL, versus 86%, 67% and 51% in case of PSA nadir between 0,1 and 0,5<span class="elsevierStyleHsp" style=""></span>ng/mL.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Nevertheless, whole gland cryoablation did not prove to have superior functional results compared to radiotherapy. Robinson et al. from a prospective randomized control study of cryoablation vs EBRT reported high levels of quality of life with both treatments.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> However, cryotherapy was associated to more acute urinary dysfunction and a poorer sexual function compared to EBRT. Authors concluded that, although cryotherapy was associated to favorable global quality of life, men who wish to increase their odds of retaining sexual function should be counselled to choose EBRT over cryoablation.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> However, the follow-up was limited to 36 months post-treatment, which is the time-point since when erectile dysfunction rates have been shown to raise in EBRT patients in the Protect study.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Hemi or focal cryoablation have been proposed as a potential solution to significantly decrease the morbidity of cryotherapy respect to EBRT and radical prostatectomy.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> In a recent systematic review, focal cryoablation was associated to 0–3.6 % of de novo urinary incontinence, and 71 to 91% of preservation of spontaneous erection.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> On the other side, a correct selection of patients is crucial to first and foremost provide optimal oncological outcomes. Advances in imaging and targeted biopsy have improved the ability to differentiate intermediate- and high-grade cancers from indolent ones. Due to the widespread use of PSA and the detection of prostate cancer at earlier stage, 15% of men are diagnosed with biopsy proven truly unifocal;<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> on the other side, many studies have demonstrated that <span class="elsevierStyleItalic">index lesion</span> drives the disease progression, so that multifocality of the disease should not affect oncologic outcomes.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a> Thus hemi or focal ablation could be an alternative to whole-gland radiation or prostatectomy, as well as an option for patient on active surveillance who wish to be even more active in their surveillance.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">The 2014 international multidisciplinary consensus on inclusion criteria for focal therapy candidates selection for trials included patients with PSA<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>15<span class="elsevierStyleHsp" style=""></span>ng/mL, clinical stage<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>T2a, GS<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3 or GS<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>4 (10). According to this consensus, tumor volume was not considered as a limit for focal therapy. Patients with a single focal Gleason 7 (3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>4) prostate cancer with limited surrounding tissue of Gleason 6 (3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3) and who are particularly motivated to receive a low-morbidity treatment could be suitable candidates.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> On the other side, patients should be informed about the lack of long-term oncological outcome of focal therapy.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">In 2011, Ward et al. reported results of focal cryoablation in 1160 patients of the COLD registry. Functional outcomes were similar to our results in terms of urinary continence (defined as use of “no pad”) in 98.4% and maintenance of spontaneous erection in 58.1%, no rectourethral fistula but lower rate of postoperative acute urinary retention (1.1%). Biochemical recurrence free rate according to the ASTRO definition was 75.7% at 36 months but the relevance of such a result is questionable in focal treatment. Studies on focal cryoablation consist mainly in small retrospective series with two main methodological issues.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11–18</span></a> Most studies took into account the ASTRO or PHOENIX definition of biochemical recurrence while they were neither created nor evaluated for focal treatments. Systematic prostate biopsy during follow up was missing in most of the studies and relevance of residual tumor was undetermined considering shortness of follow-up.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Evaluation of oncological outcome of focal treatment should rely on a hybrid definition taking into account not only the rising PSA, but also the pathological (positive prostate re-biopsy) and the clinical (need for a second line treatment) aspects.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> As there is a lack of consensus on how to define a biochemical failure in the setting of focal therapy, currently stronger indicators of treatment failure are the persistency/progression of PCa Gs ≥ 7, as well as the need of second line treatment.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> In our cohort, due to the patients selection criteria, the clinical decision to retreat patients for an elevation of PSA was based on multiple clinical factors -including age, comorbidities and prostate cancer risk of progression- and debated at the multi-disciplinary team meetings; this is also the reason why not all patients undertook prostate re-biopsy on default during the follow-up, but rather in the case a potential benefit for patients with eventual further actions/treatments could be foreseen.</p><p id="par0140" class="elsevierStylePara elsevierViewall">In a recent consensus panel, Donaldson et al. defined small volume Gleason 3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3 (Prognostic grade group 1) or very small volume (< 0.2 cc or < 7<span class="elsevierStyleHsp" style=""></span>mm in diameter) Gleason 3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>4 (Prognostic grade group 2) as acceptable pathological outcomes at re-biopsy within the treated zone, based on latest pathological data from the International Society of Urological Pathology (ISUP).<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> Significant volume (≥ 0.2 cc or ≥ 7<span class="elsevierStyleHsp" style=""></span>mm in diameter) of Gleason 3<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>4 (Prognostic grade group 2) within the treated zone would be considered as failure (20). Moreover, they suggest to monitor low-risk disease in untreated zone as such as it is applied in most of the active surveillance protocols.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> Our retrospective study is strongly limited by a small cohort sample, the lack of a standardized evaluation of functional results and inevitable selection biases. Nevertheless, the hemi-ablation has shown a good oncological outcomes and reliable functional results, in accordance to the most recent evidence in literature.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> The finer selection of patients with the aid of modern mpMRI and targeted biopsy is expected to improve outcomes, by identifying more accurately the targeted lesions, by applying only focally the energy sources, and by standardizing follow-up and failure definitions.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusion</span><p id="par0145" class="elsevierStylePara elsevierViewall">For patients with unilateral mpMRI and biopsy proven low to intermediate localized prostate cancer, hemi-cryoablation seemed to provide favorable oncological outcomes and less early complications compared to whole-gland cryoablation. All patients had a low-intermediate risk prostate cancer according to D’Amico risk classification. Hemi-ablation was proposed in case of biopsy and prostate MRI proven unilateral prostate cancer. Primary endpoint was Cryotherapy Failure for which 3 definitions were considered and compared: 1) biochemical failure (> PSA nadir + <span class="elsevierStyleBold">≥</span>2ng/mL), 2) positive prostate re-biopsy with Gleason score≥7, 3) initiation of further prostate cancer treatment.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Median patients age at treatment was 74 [42–81] vs 76 [71–80] years in WGC vs HC group, respectively (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.08). Todos los pacientes tenían cáncer de próstata de riesgo bajo-intermedio según criterios D'Amico. Se propuso crioablación hemiglandular en caso de cáncer de próstata unilateral comprobado por biopsia y RM. La variable principal de evaluación fue el fracaso de la crioterapia, para el que se consideraron y compararon tres definiciones: <span class="elsevierStyleBold">1)</span> fallo bioquímico (> PSA nadir + <span class="elsevierStyleBold">≥</span>2ng/mL), <span class="elsevierStyleBold">2)</span> re-biopsia positiva de próstata Gleason ≥7, y <span class="elsevierStyleBold">3)</span> inicio de un tratamiento adicional para el cáncer de próstata.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">La edad media de los pacientes durante el tratamiento fue 74[42–81] vs. 76[71–80] años en el grupo de CT vs. CH, respectivamente (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,08). Los grupos de riesgo bajo e intermedio (D'Amico) fueron 15% y 85% frente a 23% y 77% (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,75), respectivamente. El tiempo medio de seguimiento fue de 41[1,5-99,0] vs. 27[0,9-93] meses (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,03). La supervivencia libre de fracaso de la crioterapia a cuatro años en CT vs. CH fue de 69% vs. 53% con la definición <span class="elsevierStyleBold">1</span> (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,24), 82% vs. 80% con la definición <span class="elsevierStyleBold">2</span> (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,95), y 83% vs. 77% con la definición <span class="elsevierStyleBold">3</span> (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,73).</p><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">La continencia urinaria postoperatoria y al año fue de 60% y 83% en CT frente a 72% y 83% en CH (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,26).
