A significant proportion of these cases are iatrogenic but also associated with sickle cell disease, anticoagulants, psychiatric medications, α-adrenergic antagonists, malignancy, total parenteral nutrition, recreational drug use such as cocaine, heavy alcohol abuse and trauma.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Ischaemic priapism is an acute urological emergency and management should follow a stepwise approach. The aim of the treatment is to restore penile detumescence in order to minimise long-term damage to the corpora cavernosa. Due to the rarity of this condition, there is a limited experience among trainees, therefore, a structured training approach during simulation setting is required. There is limited evidence on the training models for priapism in the literature.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a> We developed a simpler model to teach junior urology residents corpora aspiration as initial management of priapism.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0010" class="elsevierStylePara elsevierViewall">An old synthetic catheterisation model was used as the basis of our priapism model. The original core of the penis was removed from this model to allow insertion of the newly devised ‘corpora’. These were created by wrapping a foam core in synthetic foreskin used in circumcision models (Limbs & Things, Bristol, UK). The distal end of the foreskin was sealed. The open end of an intravenous giving set was embedded into the proximal end of the foam core. An attached bag of saline, coloured with red dye, was used to provide pressurised ‘blood’. A small plastic rod was also inserted into the model to allow the instructor to change the angle of the penis to simulate detumescence (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The model allows repeated aspiration attempts.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Urology Boot Camp is a 5-day innovative, practical, hands-on interactive simulation rich course which follows the Urology curriculum for the 1st-year urology resident in the UK divided into 8 modules. The pilot course was delivered to 16 urology trainees in 2015 and the course now has 48 places.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> The course is aimed at current and newly appointed urology trainees. Nearly thirty hours of technical skills simulation training in cystoscopy, ureteroscopy, transurethral resection skills, botox, scrotal surgery, bowel anatomosis and laparoscopy is provided during the course. The emphasis is on one-to-one hands-on training. The course also offers ten hours of non-technical skills training delivered in the form of simulated scenarios using state-of-the-art technology, followed by interactive structured debriefing sessions. On the fifth day, participants are assessed on five technical skills and their assessment score is compared with the baseline score to assess progression. During the course, the focus is on the development of professionalism and excellence in clinical performance required at the registrar level and above. Penile and scrotal emergencies (penile fracture, priapism, acute scrotum, difficult catheterisation) are included in one of the modules of the boot camp. Each module is delivered in four hours. All participants are expected to complete pre-course reading materials on the previously mentioned topics to have a good basic knowledge base. During the module, 40 min are devoted to the management of priapism with a step-by-step explanation of the aspiration technique. This includes 10 min presentation followed by aspiration and irrigation practice on the model for 30 min. The practical part covers the setting up for aspiration, explaining the procedure to the simulated patient, calculating the dose of phenylephrine if required, and performing aspiration on the model.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">A simple questionnaire was developed following consultation with expert faculty to assess the fidelity and effectiveness of our priapism aspiration task-trainer. We assessed the anatomic appearance, realism in task performance and potential for the educational tool. Face validity of the priapism model was measured by experienced urologists teaching on a urology boot camp (2018, 2019). In addition, during the 2019 course, 7 trainees also assessed the validity. All experts had managed >5 cases of acute priapism. Responses were collected using a 5-point Likert Scale. Most trainees had seen or managed <5 cases. Data were analysed using IBM SPSS Statistics V25. The intra-class correlation was calculated using a “One-way Random model”.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">The model has been used successfully to demonstrate, teach, and practice corpora aspiration. Eleven consultant urologists with special interest in andrology participated in the evaluation. Seven trainees evaluated the model but one decided not to complete the evaluation form. Tactile feedback from experts on needle insertion for aspiration was also felt to be realistic with 72.6% reporting it as “good” or “very good” (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>) and 85.71% of trainees reported “good” feel during aspiration (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Nine experts agreed and 2 strongly agreed that the model can be used for training. The intra-class correlation amongst experts was 0.552 (n = 11). All were receptive of the concept of the model and suggested its regular use during the Urology Simulation Boot Camp.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">All trainees (N = 7) reported the appearance of the priapism model as “good”. Surgical treatment of ischemic priapism is indicated after repeated penile aspirations and injections of sympathomimetics have failed.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,6</span></a> These patients usually present to the accident and emergency department and require urgent assessment and treatment to minimise long-term damage to the erectile tissue. In spite of the fact that urology residents are responsible for specialist management, the resident in the accident and emergency department may eventually be the first person to encounter these patients. Therefore, it is essential that both urology trainees and emergency department trainees are suitably trained in the initial management of priapism. Several authors have highlighted the importance of simulation-based training.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–9</span></a> A survey of Emergency Department residents reported need for training on the management of priapism (75%) and only 36% felt confident in managing a patient with priapism. Interestingly, 17% of senior residents were not at all confident. Furthermore, although 80% of training program had a teaching session on priapism treatment but only 20% of the program involved use of simulation.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> One limitation of the study is the lack of assessment of trainees on the model. We did not set up any assessment criteria as our first intention was to assess the usefulness of the model. Once the model is established, we plan to develop a checklist type assessment for the exercise. Another limitation is that we did not explore the possibility of simulation of other treatment options for priapism like a Winter shunt.</p><p id="par0055" class="elsevierStylePara elsevierViewall">A limited number of simulation-based teaching on priapism is delivered for urology residents.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> We believe that our model is ideal for teaching corporal aspiration technique to new residents in developed and low- and medium income countries with a high incidence of sickle cell disease induced priapism.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> The model’s penis consistency, size and shape reasonably mimic priapism. In addition, the production and set-up of the model are simple and quick. The model can be developed from a catheterisation model by creating a cavity in the penile shaft of a catheterisation model. Male catheterisation models are available all over the world and a local catheterisation model can be used to create the model. The model can be used by multiple trainees. The foreskin used to cover the model gives realistic needle insertion feel and the foam provides lifelike aspiration impression. The limitation of the model is the lack of options to simulate training in other techniques to treat patients with priapism.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0060" class="elsevierStylePara elsevierViewall">Our model is particularly useful as it is realistic, life-like, and easy to set-up. Early treatment is required to prevent irreversible loss of erectile function. Corporal aspiration is the first step in management. Currently, there are no satisfactory training models to develop skills in a controlled environment. We have therefore developed a novel training model to teach trainees the steps of penile aspiration in a safe and representative way.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">We have developed a priapism model using an old catheterisation teaching model. Face validity of the model was assessed by participants and experienced urologists teaching on a urology boot camp. All had managed at least 5 cases of actual priapism. Responses were reported using a 5-point Likert Scale. Data were analysed using IBM SPSS Statistics V25. The intra-class correlation was calculated using a «One-way Random model».</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">Eleven urologists and seven trainees participated in the evaluation. The model appearance was reported as the best simulation trait of the priapism model. Tactile feedback from needle insertion for aspiration was also felt to be realistic with 72.6% reporting it as «Good» or «very good» and 85.7% reported the model to be realistic for needle insertion. Intra-class correlation amongst experts was 0.552. Majority of trainees (83.3%) reported a realistic simulation. All evaluators agreed or strongly agreed that the model provided a good simulated experience that would be useful in training.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Our model provides a realistic simulation of corporal aspiration.
