The port is placed through the advanced access platform (Gelpoint) to prevent gas leakage.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Over the past decade, robot-assisted laparoscopic surgery has attained a considerable momentum in the field of urology. Since the US Food and Drug Administration (FDA) approved the platform, four generations of the robotic da Vinci systems have been launched on the market<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. Noteworthy features, such as precisely controlled instruments with seven degrees of freedom, augmented surgeons ergonomics, and three-dimensional vision has overcome the technical challenges of standard laparoscopic techniques. In addition to these considerable advantages, the implementation of robotic surgical platforms were also embraced by the laparoscopic single-site surgery (LESS) seeking to surpass the strains of the limited working space experienced with this approach. Moreover, a natural evolution on the field occurred leading to the development of novel robotic platforms specifically designed for laparoscopic single site surgery (R-LESS) aiming to reduce invasiveness, refine postoperative recovery, and improve cosmesis. The initial series of R-LESS were reported in 2008 highlighting that the incorporation of the robotic system improved suturing and dissection compared to the standard LESS techniques<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. While acknowledging the benefits and technologic progress of R-LESS, surgeons kept experiencing restricted motion within the surgical field and external clashing of the robotic instruments. Most recently, a novel purpose built single-port robotic platform was designed to address the previous limitations on the R-LESS approach and improve perioperative outcomes in urological surgeries.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In this review article, we aimed to analyze the current advances in single-port (SP) robotic surgery including the novel robotic technology available, detail the features of the device, describe the SP surgical technique, report the available data, and discuss the future directions.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">A non-systematic literature search was conducted using Medline/Pubmed/Embase headings “single port,” “robotic surgery,” “robot,” “da Vinci SP”, “urology” and the names of specific urologic procedures, from 2008 through 2020. The reference lists of selected manuscripts were checked manually looking for articles of interest. Relevant original articles summarizing the existing knowledge on single port surgery, including technical details, description of the robotic technology, procedures performed and recent comparative studies with the multi-arm robotic approach were selected for this review.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Single port robotic surgery in Urology</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Initial clinical investigation</span><p id="par0020" class="elsevierStylePara elsevierViewall">The first clinical experience using the first generation purpose-built da Vinci single-port surgical system (SP999) was reported in 2014<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. The study was a prospective phase 1 study that enrolled 19 patients. Eleven patients underwent single-site robotic prostatectomy, 8 underwent nephrectomy procedures (radical nephrectomy, two; partial nephrectomy, four; and simple nephrectomy, two). All the procedures were successfully completed without conversion to standard approaches or major complications. This initial study demonstrated that the adoption of the SP platform was feasible and safe in performing major urological procedures. Further refinements on this first model were performed in the instruments’ controls and optics that naturally followed the launch of the second-generation platform. The SP Surgical system is a novel robotic purpose-built platform created for robotic SP surgery and received the US FDA approval on May 2018<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a>.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">SP robot platform features</span><p id="par0025" class="elsevierStylePara elsevierViewall">The robot is designed to operate within narrow surgical fields providing the surgeon with similar capabilities as the traditional multiport robotic platform. However, the main difference is that instruments are introduced through an SP. The novel SP system has two main components: 1) the first one is the surgeon’s console, which houses the user interfaces panel, the surgeon’s controller, and the visual display system; 2) the second component is the patient side cart consisting of an instrument arm containing four instruments drives that allows the deployment of three double-joined articulating robotic instruments and an articulating camera (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). To facilitate the robot docking, there is a newly designed 25-mm port that contains 4 channels for the placement of three 6-mm double-joined robotic instruments, a 12 × 10 mm articulating robotic camera, and an additional 6-mm accessory laparoscopic instrument for the assistant<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Due to the integration between the robotic arms and the camera, the system is able to rotate 360° inside the abdominal cavity. It is worth to mention that the surgical team receives live feedback of the instruments in the bottom of the screen helping to determine the spatial orientation of them in the field. A novel feature from the SP instruments is the addition of an extra joint, providing an “elbow” improving the triangulation around the target anatomy and reducing external instruments clashing that can naturally take place in restricted surgical spaces.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Technical considerations during the single-port robotic approach</span><p id="par0030" class="elsevierStylePara elsevierViewall">A thorough understanding of the SP platform can efficiently improve time for the position, docking and port placement. Patient positioning depends on the type of urological procedure to be performed; always keeping the philosophy of having the SP trocar located 20 cm from the target anatomy. Based on the rotation capabilities of the SP and unique characteristics of the single arm, the platform delineates a limit for the vertical range of motion avoiding accidental collision with the patient<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>. In addition, the platform can be docked from every angle with respect to the patient, facilitating the better space utilization inside the operating room (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">A second technical consideration includes the utilization of an advanced access platform (GelPOINT®, Applied Medical Resources, Rancho Santa Margarita, CA) that prevents CO<span class="elsevierStyleInf">2</span> gas leakage and maximizes of the depth and workspace for instruments. In our experience, we utilize a 12 mm Airseal® laparoscopic trocar placed through the GelPOINT providing a channel for the assistant to place sutures, instruments or specimens bags.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Finally, for suction device, a nasogastric tube (14Fr) can be placed through the Airseal® or alongside the access platform with a string attached on the tip which can be grabbed by the surgeon on the console and manipulated on the field. For radical or simple prostatectomies, the bladder catheter coming from the urethra can be used as a suction attaching it to an outside suction and assist through the procedure.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">SP pre-clinical experience</span><p id="par0045" class="elsevierStylePara elsevierViewall">Before the application of the SP robotic platform in humans, the system was rigorously assessed in particular pre-clinical scenarios to determine the technical feasibility during urological procedures<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>. Based on the distinctive features of the new system, extraperitoneal and transperitoneal surgical approaches were completed successfully with no conversion to standard techniques<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. In addition, the platform expanded the limits on minimally invasive surgery with contemporary interventions such as transperineal<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> and transvesical<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>. These approaches may represent advantages in terms of morbidity reduction, avoiding the risk inherent to intraperitoneal access, decreasing the need for steep Trendelenburg positioning, and finally improving the cosmetic outcomes.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">SP clinical experience</span><p id="par0050" class="elsevierStylePara elsevierViewall">In 2018, after the release of the SP system, the Cleveland Clinic group conducted a prospective data collection during the first days after the system was installed. A total of nine major urological procedures were performed (3 robot-assisted radical prostatectomies, 3 transperitoneal robot-assisted partial nephrectomies, 1 simple cystectomy with pure intracorporeal ileal conduit urinary diversion, and 2 ureteral implantations). No intraoperative complications were reported with six cases completed by a pure single-site approach. Two complications were recorded including one minor (Clavien I) and one major (Clavien IIA) with no conversions to standard approaches<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,13</span></a>.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Other centers have been describing their early experiences with the SP system, especially in robot-assisted radical prostatectomy (RARP)<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a>. A retrospective study published by the Mayo Clinic group described an initial cohort of 49 patients that underwent SP RARP<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>. From the entire cohort, 7 Retzius-sparing cases were performed with an overall median blood loss of 200 ml. Four Clavien II complications occurred (8.1%) with an operative time below 200 min. Dobbs et al. from the University of Illinois at Chicago reported similar results with 10 consecutives patients that underwent SP RARP<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>. Perioperative outcomes related to this series described no intraoperative complications and minimal blood loss (20−150 ml).</p><p id="par0060" class="elsevierStylePara elsevierViewall">Based on the versatility of the SP, Kaouk et al. published the first 10 cases of extraperitoneal SP RARP demonstrating several advantages including patient positioning (Trendelemburg avoidance), postoperative pain and same-day discharge<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>. This report was followed by a comparative study between transperitoneal vs extraperitoneal approach for SP RARP on 98 patients<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>. The retrospective analysis concluded that the extraperitoneal approach was associated with a significantly shorter hospital stay (4.2 vs 25.7 h, p < 0.0001) and decreased need for postoperative narcotics compared to the standard transperitoneal approach.</p><p id="par0065" class="elsevierStylePara elsevierViewall">In terms of other urological procedures, there have been series describing the technique for perineal prostatectomy<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>, radical cystectomy<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>, urinary diversion<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>, ureteroneocystostomy<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a>, pyeloplasty<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a>, simple prostatectomy<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a>, and partial/radical nephrectomy<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27,28</span></a> using the SP system with outcomes comparable to the standard multiport robotic approach (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>). Data review of the perioperative outcomes from these studies are detailed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. While these studies represent small series from single center experiences, the safety and feasibility of using the SP robotic platform during urological surgeries is demonstrated.</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Recently, Kaouk et al. published a study with the experience of the first 100 patients that underwent various SP robotic procedures<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a>. Different urological approaches, such as, extraperitoneal (n = 53), trans peritoneal (n = 37), and trans vesical (n = 10) were used to access the target organs. The total cohort included 73 (73%) patients operated with different oncological indication, such as radical prostatectomy with pelvic lymph node dissection (n = 60), cystectomy with intracorporeal urinary diversion (n = 6), partial nephrectomy (n = 6), and retroperitoneal lymph node dissection (n = 1). Ninety-nine procedures were successfully completed without the need for any supplementary procedure/maneuvers. In 1 patient with ureteroileal anastomosis stricture after cystectomy, the surgery was converted to open due to failure to progress and multiple dense adhesions. Neither intraoperative nor postoperative blood transfusions were required in this cohort. Nine patients (9%) from the entire cohort presented with Clavien grade II (3%) and grade IIIa (6%) complications. From the entire cohort of patients with SP extraperitoneal prostatectomy (n = 60), 70% required no opioids for pain control after their surgeries and more than half (35, 58.3%) were discharged a few hours after the surgery.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusions</span><p id="par0075" class="elsevierStylePara elsevierViewall">Currently, the majority of the data of SP robotic platform in the urological field comes from single center, cases series and retrospective studies with undoubted limitations. However, the reports established the safety and feasibility of the adoption of the novel robotic platform to successfully complete urologic procedures. Certain specific contributions, such as new approaches (transvesical and transperineal), avoidance of peritoneal cavity with extreme patient positioning, and decrease reabsorption of gas had been described due to the remarkable characteristics of the single site approach.
