robot-assisted laparoscopic surgery has attained a considerable momentum in the field of urology&#46; Since the US Food and Drug Administration &#40;FDA&#41; approved the platform&#44; four generations of the robotic da Vinci systems have been launched on the market<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>&#46; Noteworthy features&#44; such as precisely controlled instruments with seven degrees of freedom&#44; augmented surgeons ergonomics&#44; and three-dimensional vision has overcome the technical challenges of standard laparoscopic techniques&#46; In addition to these considerable advantages&#44; the implementation of robotic surgical platforms were also embraced by the laparoscopic single-site surgery &#40;LESS&#41; seeking to surpass the strains of the limited working space experienced with this approach&#46; Moreover&#44; a natural evolution on the field occurred leading to the development of novel robotic platforms specifically designed for laparoscopic single site surgery &#40;R-LESS&#41; aiming to reduce invasiveness&#44; refine postoperative recovery&#44; and improve cosmesis&#46; The initial series of R-LESS were reported in 2008 highlighting that the incorporation of the robotic system improved suturing and dissection compared to the standard LESS techniques<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>&#46; While acknowledging the benefits and technologic progress of R-LESS&#44; surgeons kept experiencing restricted motion within the surgical field and external clashing of the robotic instruments&#46; Most recently&#44; a novel purpose built single-port robotic platform was designed to address the previous limitations on the R-LESS approach and improve perioperative outcomes in urological surgeries&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">In this review article&#44; we aimed to analyze the current advances in single-port &#40;SP&#41; robotic surgery including the novel robotic technology available&#44; detail the features of the device&#44; describe the SP surgical technique&#44; report the available data&#44; and discuss the future directions&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">A non-systematic literature search was conducted using Medline&#47;Pubmed&#47;Embase headings &#8220;single port&#44;&#8221; &#8220;robotic surgery&#44;&#8221; &#8220;robot&#44;&#8221; &#8220;da Vinci SP&#8221;&#44; &#8220;urology&#8221; and the names of specific urologic procedures&#44; from 2008 through 2020&#46; The reference lists of selected manuscripts were checked manually looking for articles of interest&#46; Relevant original articles summarizing the existing knowledge on single port surgery&#44; including technical details&#44; description of the robotic technology&#44; procedures performed and recent comparative studies with the multi-arm robotic approach were selected for this review&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Single port robotic surgery in Urology</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Initial clinical investigation</span><p id="par0020" class="elsevierStylePara elsevierViewall">The first clinical experience using the first generation purpose-built da Vinci single-port surgical system &#40;SP999&#41; was reported in 2014<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>&#46; The study was a prospective phase 1 study that enrolled 19 patients&#46; Eleven patients underwent single-site robotic prostatectomy&#44; 8 underwent nephrectomy procedures &#40;radical nephrectomy&#44; two&#59; partial nephrectomy&#44; four&#59; and simple nephrectomy&#44; two&#41;&#46; All the procedures were successfully completed without conversion to standard approaches or major complications&#46; This initial study demonstrated that the adoption of the SP platform was feasible and safe in performing major urological procedures&#46; Further refinements on this first model were performed in the instruments&#8217; controls and optics that naturally followed the launch of the second-generation platform&#46; The SP Surgical system is a novel robotic purpose-built platform created for robotic SP surgery and received the US FDA approval on May 2018<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4&#44;5</span></a>&#46;</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">SP robot platform features</span><p id="par0025" class="elsevierStylePara elsevierViewall">The robot is designed to operate within narrow surgical fields providing the surgeon with similar capabilities as the traditional multiport robotic platform&#46; However&#44; the main difference is that instruments are introduced through an SP&#46; The novel SP system has two main components&#58; 1&#41; the first one is the surgeon&#8217;s console&#44; which houses the user interfaces panel&#44; the surgeon&#8217;s controller&#44; and the visual display system&#59; 2&#41; the second component is the patient side cart consisting of an instrument arm containing four instruments drives that allows the deployment of three double-joined articulating robotic instruments and an articulating camera &#40;<a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#41;&#46; To facilitate the robot docking&#44; there is a newly designed 25-mm port that contains 4 channels for the placement of three 6-mm double-joined robotic instruments&#44; a 12&#8239;&#215;&#8239;10&#8239;mm articulating robotic camera&#44; and an additional 6-mm accessory laparoscopic instrument for the assistant<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6&#44;7</span></a> &#40;<a class="elsevierStyleCrossRef" href="#fig0010">Fig&#46; 2</a>&#41;&#46; Due to the integration between the robotic arms and the camera&#44; the system is able to rotate 360&#176; inside the abdominal cavity&#46; It is worth to mention that the surgical team receives live feedback of the instruments in the bottom of the screen helping to determine the spatial orientation of them in the field&#46; A novel feature from the SP instruments is the addition of an extra joint&#44; providing an &#8220;elbow&#8221; improving the triangulation around the target anatomy and reducing external instruments clashing that can naturally take place in restricted surgical spaces&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Technical considerations during the single-port robotic approach</span><p id="par0030" class="elsevierStylePara elsevierViewall">A thorough understanding of the SP platform can efficiently improve time for the position&#44; docking and port placement&#46; Patient positioning depends on the type of urological procedure to be performed&#59; always keeping the philosophy of having the SP trocar located 20&#8239;cm from the target anatomy&#46; Based on the rotation capabilities of the SP and unique characteristics of the single arm&#44; the platform delineates a limit for the vertical range of motion avoiding accidental collision with the patient<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>&#46; In addition&#44; the platform can be docked from every angle with respect to the patient&#44; facilitating the better space utilization inside the operating room &#40;<a class="elsevierStyleCrossRef" href="#fig0015">Fig&#46; 3</a>&#41;&#46;</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">A second technical consideration includes the utilization of an advanced access platform &#40;GelPOINT&#174;&#44; Applied Medical Resources&#44; Rancho Santa Margarita&#44; CA&#41; that prevents CO<span class="elsevierStyleInf">2</span> gas leakage and maximizes of the depth and workspace for instruments&#46; In our experience&#44; we utilize a 12&#8239;mm Airseal&#174; laparoscopic trocar placed through the GelPOINT providing a channel for the assistant to place sutures&#44; instruments or specimens bags&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">Finally&#44; for suction device&#44; a nasogastric tube &#40;14Fr&#41; can be placed through the Airseal&#174; or alongside the access platform with a string attached on the tip which can be grabbed by the surgeon on the console and manipulated on the field&#46; For radical or simple prostatectomies&#44; the bladder catheter coming from the urethra can be used as a suction attaching it to an outside suction and assist through the procedure&#46;</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">SP pre-clinical experience</span><p id="par0045" class="elsevierStylePara elsevierViewall">Before the application of the SP robotic platform in humans&#44; the system was rigorously assessed in particular pre-clinical scenarios to determine the technical feasibility during urological procedures<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>&#46; Based on the distinctive features of the new system&#44; extraperitoneal and transperitoneal surgical approaches were completed successfully with no conversion to standard techniques<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>&#46; In addition&#44; the platform expanded the limits on minimally invasive surgery with contemporary interventions such as transperineal<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> and transvesical<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>&#46; These approaches may represent advantages in terms of morbidity reduction&#44; avoiding the risk inherent to intraperitoneal access&#44; decreasing the need for steep Trendelenburg positioning&#44; and finally improving the cosmetic outcomes&#46;</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">SP clinical experience</span><p id="par0050" class="elsevierStylePara elsevierViewall">In 2018&#44; after the release of the SP system&#44; the Cleveland Clinic group conducted a prospective data collection during the first days after the system was installed&#46; A total of nine major urological procedures were performed &#40;3 robot-assisted radical prostatectomies&#44; 3 transperitoneal robot-assisted partial nephrectomies&#44; 1 simple cystectomy with pure intracorporeal ileal conduit urinary diversion&#44; and 2 ureteral implantations&#41;&#46; No intraoperative complications were reported with six cases completed by a pure single-site approach&#46; Two complications were recorded including one minor &#40;Clavien I&#41; and one major &#40;Clavien IIA&#41; with no conversions to standard approaches<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5&#44;13</span></a>&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">Other centers have been describing their early experiences with the SP system&#44; especially in robot-assisted radical prostatectomy &#40;RARP&#41;<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14&#8211;16</span></a>&#46; A retrospective study published by the Mayo Clinic group described an initial cohort of 49 patients that underwent SP RARP<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>&#46; From the entire cohort&#44; 7 Retzius-sparing cases were performed with an overall median blood loss of 200&#8239;ml&#46; Four Clavien II complications occurred &#40;8&#46;1&#37;&#41; with an operative time below 200&#8239;min&#46; Dobbs et al&#46; from the University of Illinois at Chicago reported similar results with 10 consecutives patients that underwent SP RARP<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>&#46; Perioperative outcomes related to this series described no intraoperative complications and minimal blood loss &#40;20&#8722;150&#8239;ml&#41;&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Based on the versatility of the SP&#44; Kaouk et al&#46; published the first 10 cases of extraperitoneal SP RARP demonstrating several advantages including patient positioning &#40;Trendelemburg avoidance&#41;&#44; postoperative pain and same-day discharge<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>&#46; This report was followed by a comparative study between transperitoneal vs extraperitoneal approach for SP RARP on 98 patients<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>&#46; The retrospective analysis concluded that the extraperitoneal approach was associated with a significantly shorter hospital stay &#40;4&#46;2 vs 25&#46;7&#8239;h&#44; p&#8239;&#60;&#8239;0&#46;0001&#41; and decreased need for postoperative narcotics compared to the standard transperitoneal approach&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">In terms of other urological procedures&#44; there have been series describing the technique for perineal prostatectomy<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>&#44; radical cystectomy<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>&#44; urinary diversion<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>&#44; ureteroneocystostomy<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22&#44;23</span></a>&#44; pyeloplasty<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24&#44;25</span></a>&#44; simple prostatectomy<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a>&#44; and partial&#47;radical nephrectomy<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27&#44;28</span></a> using the SP system with outcomes comparable to the standard multiport robotic approach &#40;<a class="elsevierStyleCrossRef" href="#fig0020">Fig&#46; 4</a>&#41;&#46; Data review of the perioperative outcomes from these studies are detailed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#46; While these studies represent small series from single center experiences&#44; the safety and feasibility of using the SP robotic platform during urological surgeries is demonstrated&#46;</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Recently&#44; Kaouk et al&#46; published a study with the experience of the first 100 patients that underwent various SP robotic procedures<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a>&#46; Different urological approaches&#44; such as&#44; extraperitoneal &#40;n&#8239;&#61;&#8239;53&#41;&#44; trans peritoneal &#40;n&#8239;&#61;&#8239;37&#41;&#44; and trans vesical &#40;n&#8239;&#61;&#8239;10&#41; were used to access the target organs&#46; The total cohort included 73 &#40;73&#37;&#41; patients operated with different oncological indication&#44; such as radical prostatectomy with pelvic lymph node dissection &#40;n&#8239;&#61;&#8239;60&#41;&#44; cystectomy with intracorporeal urinary diversion &#40;n&#8239;&#61;&#8239;6&#41;&#44; partial nephrectomy &#40;n&#8239;&#61;&#8239;6&#41;&#44; and retroperitoneal lymph node dissection &#40;n&#8239;&#61;&#8239;1&#41;&#46; Ninety-nine procedures were successfully completed without the need for any supplementary procedure&#47;maneuvers&#46; In 1 patient with ureteroileal anastomosis stricture after cystectomy&#44; the surgery was converted to open due to failure to progress and multiple dense adhesions&#46; Neither intraoperative nor postoperative blood transfusions were required in this cohort&#46; Nine patients &#40;9&#37;&#41; from the entire cohort presented with Clavien grade II &#40;3&#37;&#41; and grade IIIa &#40;6&#37;&#41; complications&#46; From the entire cohort of patients with SP extraperitoneal prostatectomy &#40;n&#8239;&#61;&#8239;60&#41;&#44; 70&#37; required no opioids for pain control after their surgeries and more than half &#40;35&#44; 58&#46;3&#37;&#41; were discharged a few hours after the surgery&#46;</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusions</span><p id="par0075" class="elsevierStylePara elsevierViewall">Currently&#44; the majority of the data of SP robotic platform in the urological field comes from single center&#44; cases series and retrospective studies with undoubted limitations&#46; However&#44; the reports established the safety and feasibility of the adoption of the novel robotic platform to successfully complete urologic procedures&#46; Certain specific contributions&#44; such as new approaches &#40;transvesical and transperineal&#41;&#44; avoidance of peritoneal cavity with extreme patient positioning&#44; and decrease reabsorption of gas had been described due to the remarkable characteristics of the single site approach&#46;
