It occurs when the tunica albuginea of the rigid penis ruptures, usually during vigorous sexual intercourse. It can also occur during masturbation, rolling over or falling onto the erect penis and a number of other scenarios.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The tunica albuginea is 2 mm thick in the flaccid state and becomes 0.25 mm thin when the penis is erect making it vulnerable to injury. The diagnosis is often clinical and can be made by history and physical examination. However, imaging, especially Magnetic resonance imaging (MRI) has its role mainly in equivocal cases. There is usually a sudden ‘snap’ described, followed by pain, detumescence and swelling/bruising of the penis, occasionally resembling an aubergine. If the Buck fascia is intact, these changes are limited to the penile shaft, whereas if it is breached the haematoma will extend to the suprapubic area, scrotum, and perineum. Patient allowing, a palpable defect can be felt over the site of the tear. If there is urethral involvement, blood at the urethral meatus can be present or haematuria (visible or non-visible) and sometimes retention of urine. A retrograde urethrogram can be performed may be useful in such cases.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Treatment usually consists of surgical exploration and repair. There has been a trend away from conservative management as prompt surgical correction results in fewer postoperative complications and lower rates of penile curvature.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">At present, there are no satisfactory training models for penile fracture repair in order to develop the required skills in a controlled environment. We propose a novel penile fracture simulation model for teaching in a safe and risk-free setting.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A validated circumcision model<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> (Limbs and Things, Bristol, UK) was used to create a new penile fracture simulation model. Synthetic foreskin for circumcision was divided into two halves. A “simulated fracture” was created using a transverse slit on one half of the foreskin (called “tunica”) and was applied over the penile model. A red jelly tablet was placed underneath the cut; this was to simulate the “clot”. A second full-length foreskin was applied to cover the defect. Construction of the model is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">The model was assessed by participants and expert faculty (andrologists and urologist with special interest in andrology) at the UK Urology Simulation Boot Camp.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> This annual event is a week-long, fully hands-on interactive simulation course for newly appointed urology residents in the UK. The focus is on one-to-one training and employs a variety of simulation methods and practical modules, including non-technical skills. During one module, 40 min were dedicated to the penile fracture repair station. A questionnaire was designed after consultation with expert faculty (supplementary file). We assessed the model usefulness for training, overall appearance of the model and of the penile fracture in particular (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), the realism in localisation of fracture and the ability of the model to represent realistic simulation. Data was collected using a 5-point Likert scale and was analysed using Microsoft Excel and IBM SPSS Statistics V25. The intra-class correlation was calculated using a ‘One-way random model’.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0030" class="elsevierStylePara elsevierViewall">Twenty-two urology trainees (n = 22) and four experts completed the evaluation. Half of the residents (n = 11) did not see a case of penile fracture beforehand and 45% reported seeing less than 5 cases. Sixty-percent (n = 13) have not managed the case themselves. Experts strongly agreed (75%) that the model is useful for training with trainees similarly agreeing in 59% of cases. The intra-class correlation coefficient amongst experts was 0.889 showing very high agreement. The overall appearance of the model was considered excellent by 40% of trainees whilst 55% found it good (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Half of trainers also agreed the overall appearance was good. Experts felt the appearance of penile fracture was good (75%) but agreed in the same proportion a neutral position with regards to realism in localisation of the fracture site. Eighteen percent of trainees found the simulator to be excellent in realism in localisation of fracture whilst 64% thought it was good. The ability of the model to represent realistic simulation was considered excellent by 23% of trainees and good by 64% (n = 14) thus establishing face validity. Also, 64% of residents felt their confidence increased after simulation.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">Concerning qualitative data, the free text comments were balanced, with positive reviews but also points for improvement. For example, ‘excellent model for training’, ‘very useful’, ‘difficult to appreciate the layers, mainly around urethra’, ‘more realistic-lateral placement of clots’.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0040" class="elsevierStylePara elsevierViewall">The incidence of penile fracture is 1 in 100 000 men,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> making it a rare urological emergency and so clinical exposure, especially in the first years of practice might be limited. To our knowledge there is no published simulation model to teach this particular skill. We have developed a novel bench model using a previously validated circumcision simulator.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The modern management of penile fracture favours early repair as opposed to a conservative approach.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,7,8</span></a> Both the American Urological Association and the European Association of Urology recommend prompt surgical exploration and repair. Conservative management has immediate (penile abscess, missed urethral injury, persistent haematoma, penile curvature) and late complications (fibrosis, angulation - in 35% and erectile dysfunction- up to 62% of cases).<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a> Even with surgical repair there is a risk of curvature (2.8%) and erectile dysfunction (1.9%), albeit smaller.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> As such having a simulation model to train and practice this skill is particularly useful.</p><p id="par0050" class="elsevierStylePara elsevierViewall">As penile fracture is a rare event, our data shows the limited experience trainees will have at the beginning of their training with the majority having never seen such a case. The simulation model we propose was found to be useful and realistic for teaching penile fracture repair. The set-up of the model is simple, quick, and relatively cheap provided that the circumcision model (£175) is already purchased. A pack of five synthetic circumcision skins costs £55.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> The limitations, we acknowledge, are mainly in fine discrimination of penile fascial layers, urethral dissection and/or mobilisation, and lack of blood supply. Initially, the position of the “simulated fracture site” created a bit of confusion as it was more dorsally placed. This was mainly due to the lack of knowledge among the nonmedical technical staff. In addition, because of the shape of the penile base model a ventrolateral lesion causes damage to model during rough handling. We are looking to add an extra-layer to improve anatomical dissection and external validation of the model.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0055" class="elsevierStylePara elsevierViewall">The new bench model we have developed provides a good simulation method for penile fracture repair and has demonstrated face validity. Foreskin for a circumcision was divided into two halves. A transverse slit (“simulated fracture”) was created on one part of the first half of the foreskin (mimicking “tunica”) and was applied over the penile model. A red jelly tablet (“clot”) was placed underneath the cut. A second full-length of foreskin was applied over it to cover the defect. The model was assessed by participants and expert faculty at the Urology Simulation Boot Camp. Evaluation was performed using a 5-point Likert Scale questionnaire. Data was analysed using Microsoft Excel and IBM SPSS Statistics V25. The intra-class correlation was calculated using a “One-way random model”.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">Twenty-two urology trainees and four experts participated in the evaluation. The majority of trainees strongly agreed (59%, n = 13) the model is useful for training with experts similarly agreeing in 75% of cases. The appearance of penile fracture was considered good by both trainees (68%, n = 14) and faculty (75%). Overall, the ability of the model to represent a realistic simulation of the task was considered excellent by 23% of participants and good by 64%. Personal confidence after simulation in managing a similar situation was considered high among trainees. El modelo fue evaluado por los participantes y docentes expertos del boot camp de simulación de urología. La evaluación se realizó mediante un cuestionario con escala de Likert de 5 puntos. Los datos se analizaron mediante Microsoft Excel e IBM SPSS Statistics V25. La mayoría de los aprendices estuvieron muy de acuerdo (59%, n = 13) en la utilidad del modelo para la formación, y los expertos también estuvieron de acuerdo en el 75% de los casos. Tanto alumnos (68%, n = 14) como docentes (75%) consideraron correcta la apariencia de la fractura del pene. En general, la capacidad del modelo para representar una simulación realista de la tarea fue considerada excelente por el 23% de los participantes y buena por el 64%. Tras la simulación los alumnos afirmaron tener una mayor confianza en el manejo de este tipo de casos.
