Cirugía de la arteria renal asociada a cirugía de aorta infrarrenal

Resumen
Objetivos

Revisión retrospectiva del manejo de las lesiones de la arteria renal asociadas a lesiones de la aorta infrarrenal.

Material y Métodos

Desde enero de 1980 hasta diciembre de 1999 se han realizado 66 revascularizaciones de arteria renal asociadas a cirugía aórtica en 63 pacientes de un total de 1117 cirugías, de edad media 59±8,2 años (63/1117) (5,6%). La patología aórtica fue oclusiva en 45 pacientes y aneurismática en 16.

Presentaban insuficiencia renal 13 pacientes (21%), 8 de los cuales tenían nefropatía isquémica, e hipertensión arterial 40 pacientes (63%) en los cuales se demostró hipertensión vasculorenal en 17. Las técnicas quirúrgicas fueron 54 injertos aortorenales, 7 endarterectomías renales transaórticas, 3 reimplantaciones y una angioplastia. Todos los pacientes fueron sometidos a cirugía aórtica directa. El control de la revascularización renal se realizó con gammagrafía, arteriografía y duplex.

Resultados

Fallecieron tres pacientes en el postoperatorio inmediato (mortalidad 4,8%); 12 pacientes sufrieron complicaciones postoperatorias (morbilidad 19%). La respuesta de la tensión arterial fue favorable en el 65%. La función renal mejoró en los pacientes con nefropatía isquémica, aunque no llegó a ser estadísticamente significativa. Las causas de muerte en el seguimiento fueron por enfermedad cardiológica y neoplasia, principalmente.

Conclusión

En nuestra experiencia la cirugía renal asociada a cirugía aórtica no aumenta significativamente la mortalidad de estos enfermos y ofrece buenos resultados a largo plazo. La gammagrafía renal es un excelente método para el control tardío de estos pacientes.

Summary
Objetives

Retrospective revision about the management of renal artery lesions associated to infrarrenal aortic lesions.

Material and methods

Since january 1980 until december 1999, 66 renal artery revascularizations associated with aortic surgery have been maker in 63 patient, with medium age 59±8,2 years (63/1117,5.6%). The aortic disease was oclusive in 45 patients and aneurismatic in 16, Chronic renal insufficiency was in 13 patiens (21%), 8 of them were ischemic nephropaty; and arterial hypertension in 40 patients (63%), in them was detected renovascular hypertension in 17. The techniques were 54 renal aortic bypass, 7 transaortic renal artery endarlerectomy, 3 reimplantations and one angioplasty. All of the patients received aortic surgery. The control of the revascularization was made with gammagraphy in 68% of the patients, with arteriography in 35% and with duplex in 8%.

Results

Three patients was died in the inmediate postoperatory, a 4.8% surgery mortality. 12 patients had postoperatory complications, a 19% morbidity rate.The arterial pressure reply was good in 65%. The renal function improved in the ischemic nephropaty patients althought it was not statistically significative. The etiology of deaths in the evolution was cardiologic disease and neoplasic principally.

Conclusions

In our experience the renal artery surgery associated with aortic surgery don’t increase the patients mortality and have good results for a long time. The renal gammagraphy is an excellent method for later control of this patients.
