Summary
Introduction

The treatment of saccular aneurysms of the renal artery, at the present time, may include various options which are both conservative (percutaneous) and surgical.

Objective

To review our experience and classify saccular aneurysms of the renal artery so as to facilitate their management and treatment.

Patients and methods

Analysis of data recorded prospectively showed that eight patients had been treated for saccular aneurysms of the renal artery situated in the main trunk, first bifurcation or lobular branch between January 1978 and June 2000. They represented 30.7% of all aneurysms of the renal artery.

Results

Three types of saccular aneurysms were differentiated mainly on therapeutic criteria: type I, aneurysm of the ostium of the renal artery, generally associated with an aneurysm of the aorta (two cases); type II, broad-basedpediculated saccular aneurysm (four cases) and type III, narrow-basedpediculated saccular aneurysm (two cases).

Conclusion

The classification of saccular aneurysms of the renal artery into the three types described may facilitate decisions regarding the different options for treatment. We believe that, at the present time, types I and II require basically surgical treatment. Type III cases may be initially treated percutaneously.

Introducción

El tratamiento del aneurisma sacular de arteria renal (ASAR) puede abarcar en la actualidad diversas posibilidades, tanto conservadoras, percutáneas como quirúrgicas.

Objetivo

Clasificar, tras la revisión de nuestra experiencia, el ASAR, con vistas a facilitar su manejo terapéutico.

Pacientes y métodos

El análisis de una base de datos recogidos deforma prospectiva demuestra la existencia de ocho pacientes tratados de ASAR localizado en el tronco principal, primera bifurcación o rama lobular, entre enero de 1978 y junio de 2000. Representan un 30,7% de todos los aneurismas de arteria renal.

Resultados

Sehan diferenciado tres tipos de aneurisma sacular, según criterios fundamentalmente terapéuticos: tipo I, aneurisma delostiumde arteria renal, generalmente asociado a aneurisma de aorta (dos casos); tipo II, aneurisma sacular pediculado de base ancha (cuatro casos); tipo III, aneurisma sacular pediculado de base estrecha (dos casos).

Conclusión

La clasificación del ASAR en los tres tipos descritos puede facilitar la decisión sobre las distintas posibilidades terapéuticas. Creemos que los tipos Iy II tienen, hoy por hoy, un manejo fundamentalmente quirúrgico. Los tipos III pueden tratarse inicialmente de forma percutánea.

Introdução

O tratamento do aneurisma sacular da arteria renal pode abranger na realidade diversas possibilidades, tanto conservadoras (percutáneas) como cirúrgicas.

Objectivo

Após a revisao da nossa experiência, classificar o aneurisma sacular da artéria renal com a finalidade de tentar facilitar a sua abordagem terapéutica.

Doentes e métodos

A análise de uma base de dados recolhida de formaprospectiva, mostra a existência de oito do entes tratados por aneurisma sacular da artéria renal localizada no tronco principal, primeira bifurcação ou ramo lobular, entre Janeiro de 1978 e Junho de 2000. Representam 30,7% de todos os aneurismas da artéria renal.

Resultados

Diferenciaram-se três tipos de aneurisma sacular, com base em critérios fundamentalmente terapêuticos: tipo I, aneurisma do ostium da artéria renal, geralmente associado a aneurisma da aorta (dois casos); tipo II, aneurisma sacular pediculado de base larga (quatro casos); e tipoIII, aneurisma sacular pediculado de base estreita (dois casos).

Conclusão

A classificacao do aneurisma sacular da artéria renal nos três tipos descritos pode facilitar a decisão sobre as distintas possibilidades terapêuticas. Cremos que os tipos Ie II tenham, hoje em dia, um tratamento fundamentalmente cirúrgico. Os tipos III podem tratar-se inicialmente de formapercutânea.
