Resumen

Desde diciembre de 1995 a diciembre de 1996 se han revisado todas las complicaciones mayores tras punciones femorales diagnóstico-terapéuticas. Se realizaron 5.100 punciones femorales, de las que 3.500 correspondieron a cateterismos cardiacos (68,6%); 1.200 a intervenciones radiológicas vasculares diagnósticas e intervencionistas (23,5%) y las 400 restantes a intervenciones neuroradiológicas (7,9%). Necesitaron intervención quirúrgica 10 pacientes: 8 pseudoaneurismas (0,16%); una fístula arterio-venosa (0,02%) y una infección (0,02%).

Presentamos el caso de un pseudoaneurisma micótico por Clostridium septicum tras las realización de un cateterismo cardíaco que, al principio, evoluciona sin complicaciones. A los 3 meses acude al Servico de Urgencias de nuestro hospital, presentando una tumoración ulcerada y maloliente en región femoral derecha. Pese al tratamiento antibiótico y la intervención quiriírgica, la paciente fallece por un cuadro séptico generalizado, comprobándose infección arterial por Clostridium septicum.

Se revisan los 10 casos publicados, en la literatura de infección arterial por Clostridium septicum con una tasa de mortalidad superior al 70% y se analizan las pautas de diagnóstico y tratamiento.

Since December 1995 to December 1996 we have revised all major complications after diagnostic-therapeutic femoral punctions. Were performed 5,100 femoral punctions, 3,500 of them corresponded to cardiac catheterizations (68.6%); 1,200 to vascular radiologic procedures, diagnostic and therapeutics (23.5%), and the remaining 400 were neurodiological procedures (7.9%). Ten of them needed surgical corrections: 8 pseudoaneurysms (0.6%); 1 arteriovenous fistulae (0.02%) and 1 infection.

We present a patient with a mycotic pseudoaneuiysm due to Clostridium septicum, that apparently doing well without further complications. Three months later, patient come to emergency ward with an ulcerated and bad smelling tumour in the right groin. Even after antibiotic treatment and surgical repair, the patient die due to sepsis, discovering an Clostridium septicum artheritis.

In all the revised cases of artheritis by Clostridium septicum, the prognosis is fatal with a mortality rate over 70%. In this case we try to analize the measures that can conduce to the early diagnostic and treatment.
