Introduction

Popliteal artery aneurysms are the most frequent peripheral aneurysms. Their main complication is acute ischemia caused by thrombosis in the artery; rupture, however, is uncommon. The case we report here is exceptional since the rupture took place more than six years after the surgical intervention.

Case report

We report the case of a 90-year-old patient who was submitted to surgery in 1996 to treat bilateral popliteal aneurysm; ligature and a bypass were performed with an expanded polytetrafluoroethylene (PTFE) graft in the right leg and with inverted saphenous vein in the left limb. The patient visited our clinic for annual check-ups, although no arteriomegaly was detected and the reconstructions remained patent. In June 2002 the patient was admitted urgently because of the appearance, 2 or 3 weeks earlier, of a large tumour in the inner side of the left thigh. No beats or signs of haemorrhage were observed. The patient was submitted to a magnetic resonance angiography (MR-angiography), which was reported as being an ‘image compatible with a pseudoaneurysm that had possibly originated in the proximal region of the anastomosis of the graft’. Following the intervention, a false aneurysm was found that had been caused by the rupture of the wall of the aneurysm in the popliteal fossa, with non-involvement of the anastomosis. A new ligature of the distal popliteal artery was performed at the end of the aneurysm and the thrombosed false aneurysm was partially excised and a sample sent to the pathology lab. Four months after the intervention, at the same time as the anticoagulant therapy which was begun because of a suspected popliteal venous thrombosis, the tumour grew again. This time the patient was submitted to an embolisation of the distal branches of the deep femoral and superficial femoral arteries. A year later the patient is stable, and no changes were observed in a recent MR-angiography scan.

Introducción

Los aneurismas de arteria poplítea son los aneurismas periféricos más frecuentes. Su principal complicación es la isquemia aguda por trombosis del mismo; su rotura es rara. El caso que presentamos resulta excepcional porque la rotura tuvo lugar más de seis años después de la intervención.

Caso clínico

Paciente de 90 años, intervenido en 1996 de un aneurisma poplíteo bilateral; se le practicó ligadura y by-pass, con prótesis de politetrafluroetileno expandido (PTFE) en la pierna derecha y con vena safena invertida en la izquierda. Sigue controles anuales en nuestras consultas, durante los cuales no se han detectado arteriomegalias y se han mantenido permeables las reconstrucciones. En junio de 2002 ingresó de urgencia por presentar gran tumoración en la cara interna del muslo izquierdo, según el paciente, de 2-3 semanas de evolución. No se apreciaba latido ni signos externos de hemorragia. Se practicó angiorresonancia magnética (angio-RM), que informó de ‘imagen compatible con pseudoaneurisma, posiblemente originado en la zona de la anastomosis proximal del injerto’. Se intervino al paciente y se encontró un falso aneurisma por rotura de la pared del aneurisma en el hueco poplíteo, con indemnidad de las anastomosis. Se practicó una nueva ligadura de la arteria poplítea distal al final del aneurisma, se resecó parcialmente el falso aneurisma, que estaba trombosado, y se envió una muestra a anatomía patológica. Cuatro meses después de la intervención, coincidiendo con tratamiento anticoagulante por sospecha de trombosis venosa poplítea, la tumoración volvió a crecer y en esta ocasión se practicó embolización de ramas distales de la femoral profunda y de la femoral superficial. Un año después, el paciente permanece estable, sin cambios en una nueva angio-RM.

Introdução

Os aneurismas da artéria popliteia são os aneurismas periféricos mais frequentes. A sua principal complicação é a isquemia aguda por trombose do mesmo; a sua rotura é rara. O caso que apresentamos é excepcional, já que a rotura teve lugar mais de seis anos após intervenção cirúrgica.

Caso clínico

Doentes de 90 anos de idade, que em 1996 foi submetida a cirurgia de um aneurisma popliteu bilateral; fez-se a laqueação e um by-pass, com prótese de politetrafluoretileno expandido (PTFE) na perna direita e com veia safena invertida na esquerda. Seguiu controlos anuais nas nossas consultas, sem detecção de arteriomegalias, e as reconstruções mantiveram-se permeáveis. Em Junho de 2002 é internado de urgência por apresentar grande tumefacção na face interna do músculo esquerdo, segundo o doente com 2-3 semanas de evolução. Não se apreciaram latidos nem sinais externos de hemorragia. Realizou uma angio-ressonância magnética (angio-RM), que mostra ‘imagem compatível com pseudo-aneurisma possivelmente originado na zona da anastomose proximal do enxerto’. Após a intervenção encontra-se um falso aneurisma por uma rotura da parede do aneurisma na cavidade popliteia, com integridade das anastomoses. Realiza-se nova laqueação da artéria popliteia distal no fim do aneurisma e disseca-se parcialmente o falso aneurisma que está trombosado, e envia-se uma amostra para anatomia patológica. Quatro meses após a intervenção, coincidindo com o tratamento anticoagulante há suspeita de trombose venosa popliteia, a tumefacção volta a aparecer. Nesta ocasião realiza-se uma embolização dos ramos distais da femoral profunda e da femoral superficial. Um ano após, o doente encontrase estável, sem alterações na nova angio-RM.
