Paciente masculino de 30 años de edad acudió pocas horas después de sufrir un traumatismo ocular mientras martillaba, recibiendo un impacto con una astilla de metal en el ojo izquierdo (OI). Por cuadro clínico desfavorable se decide manejo quirúrgico, el cual fue inicialmente rechazado por el paciente. Regresó seis meses después con una profunda disminución de la agudeza visual del OI, llegando a cuenta dedos a un metro a pesar de la corrección óptica. El segmento anterior muestra un leucoma cicatricial inferior, asociado a Tyndall (++), pigmento retroquerático, sinequias posteriores inferiores y catarata total. Se indicó cirugía de catarata con implante de lente intraocular y vitrectomía posterior con extracción de cuerpo extraño intraocular (CEIO). En el control posoperatorio se demostró que la siderosis ocular (SO) postraumática no afectó significativamente su visión central, que mantiene hasta el momento.

Due to an unfavorable clinical picture, surgical management was decided, which was initially rejected by the patient. He returned six months later with a profound decrease in left eye visual acuity, reaching counting finger at one meter despite optical correction. The anterior segment shows a lower scarring leukoma, associated with Tyndall (++), retrokeratic pigment, lower posterior synechiae and a total cataract. Cataract surgery with intraocular lens implantation and a posterior vitrectomy with intraocular foreign body extraction were indicated. At postoperative control it was shown that post-traumatic ocular siderosis did not significantly affect his central vision, which remains until now.
