Typical deviation: 5.01 μm.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Optical coherence tomography (OCT) was developed in 1991 by a group of researchers led by Puliafito who worked together with physics and mathematicians led by ProfessorFujimoto.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> OCT was introduced in clinic practice after 5 years and presented as a market product.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Initially it was designed as a laboratory instrument having the purpose of taking a single mode A scan. However, due to its potential it developed very quickly in recent years and has become an indispensable tool in ophthalmology.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> At the present time we have several devices available in the market, with OCT Spectralis and OCT Cirrus being the most widely used devices for adult as well as pediatric populations.</p><p id="par0010" class="elsevierStylePara elsevierViewall">It is known that the <span class="elsevierStyleItalic">in vivo</span> thickness measurements obtained by OCT are reliable and exhibit good repeatability in healthy and compromised patients,<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6</span></a> although it must be mentioned that Cirrus as well as Spectralis do not include a normality database for children.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–9</span></a> On the other hand, the amount of studies in the adult population vastly outnumber those carried out with the pediatric population and in the latter group only a few have assessed differences by gender, age or ethnicity. Studies with adults having ocular pathologies have observed statistically significant differences between the mean values of the retina nerve fiber layers (RNFLm) in Cirrus and Spectralis<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> as well as between the macular thickness values (adults without ocular pathology). These differences have been attributed mainly to variations in the segmentation algorithms and the scanning pathway of each instrument,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> although said differences have not been assessed in studies with pediatric populations. On the other hand, analyses of the differences in the RNFLp readings of the same OCT device based on age in healthy pediatric subjects did not report significant differences<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12–14</span></a> and hardly any study found significant differences according to sex<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15–19</span></a> with the exception of the study by Rao et al.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> A bibliographic search carried out by the authors did not find any published study comparing the peripapillary RNFL values that assessed the degree of match between OCT Spectralis and OCT Cirrus in a sample of pediatric patients. The objective of the present study is to measure the degree of match between both OCT devices and report the normality values obtained in the sample, as well as analyzing differences on the basis of sex, age, laterality and ethnicity.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Participants</span><p id="par0015" class="elsevierStylePara elsevierViewall">A descriptive, transversal, comparative and observational study was conducted after obtaining the authorization of the Jiménez Díaz Foundation University Hospital Ethical Committee. Data obtained from patients were managed in absolute confidentiality, strictly following the norms of the Helsinki declaration. Patients over 12 years of age as well as their parents or tutors signed an informed consent to participate in the present study after receiving an explanation and reaching an understanding of the procedures to be carried out and the objectives of the study. Likewise, children under 12 years of age consented verbally and their parents or tutors authorized their participation in writing. The inclusion criteria comprised being between 5 and 15 years of age, not having previous ocular or systemic pathology that could influence ocular results, adequate gestational age and weight at birth, best corrected visual acuity adequate for their age, spherical equivalent values between −3D and +3D, and astigmatism values below 2 diopters.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Exclusion criteria comprised intraocular pressure (IOP) > 21 mmHg, images with signal lower than 5/10 dB (Cirrus, Carl Zeiss Meditec, Germany) or lower than 20 dB (Spectralis, Heidelberg Engineering, Heidelberg, Germany), and uncooperative patients. Overall, 112 patients fulfilled said inclusion criteria although 12 of these were excluded due to poor signal quality or poor cooperation, obtaining a final sample of 100 patients (n = 100). All patients underwent a complete ophthalmological examination including best corrected visual acuity, extrinsic ocular motility, stereopsis test, IOP and anterior and posterior pole observation with slit lamp. Ocular refraction was measured before refraction and after dilatation under cycloplegia administering 1% cyclopentolate at 10-min intervals totaling 3 administrations, measuring refraction at 15−20 min after the last drop. Ocular axial length was measured with IOLMaster (Carl Zeiss Meditec, Germany). The measurements of all variables of both eyes were taken but due to the similarity found in statistical analysis, the results only took into account the right eyes.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Optical coherence tomography</span><p id="par0025" class="elsevierStylePara elsevierViewall">The present study utilized OCT Cirrus model 4000 (software 7.0.3.19) and specifically the protocol <span class="elsevierStyleItalic">«RNFL and ONH: Optic Disc Cube 200 × 200»</span>, that includes values within a circle having a diameter of 3.46 mm as from the center of the optic disc. The nerve fiber layer sector thickness values are shown in a circle divided in 4 quadrants (inferior-superior-nasal-temporal [ISNT]) and in a second circuit with 12 subsectors. The same protocol was utilized with OCT Spectralis (software 6.3.2.0) and the results were shown in a circle with 4 or 6 sectors depending on user preferences. In contrast with Cirrus, the OCT Spectralis device does not include 12-sector segmentation display and does not show parameters such as the horizontal or vertical ratio between the papilla cup and the overall optic disc diameter. The measurements and verification of adequate image segmentation were taken by the same examiner.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0030" class="elsevierStylePara elsevierViewall">In order to preserve the confidentiality of the data of study participants, codes were issued to each and the data were included in a database that registered the data of each variable. Statistical analysis was carried out with version 23.0 of the SPSS statistical application. First, the Kolmogorov–Smirnov test was utilized to assess the normality of variables and due to the fact that not all were normal it was decided to utilize a nonparametric test to obtain more uniform results in the hypothesis contrast process. The statistical significance level taken into account was p < 0.05, with a confidence interval (CI) of 95%. Mean, standard deviation, median and percentiles 1, 5 and 95 (for continuous variables) were obtained, and the frequencies analysis was performed for categorical variables. The absolute intraclass correlation coefficient (ICC) was utilized to calculate the degree of match between the values of both OCT devices. A value < 0.2 was considered to have a «very low match grade» while values between 0.2 and 0.5 were taken as «low match grade», between 0.5 and 0.7 as «moderate match grade », values > 0.7 and 0.9 as «good match grade» and values > 0.9 as «excellent match grade». The Bland–Altman graph was utilized to illustrate the match grade between RNFLm values of both OCT devices. Spearman correlation coefficient was used to evaluate the correlation between axial length and optic disc values. The U Mann–Whitney test was applied to assess the differences per sex and ethnicity while the Wilcoxon test analyzed differences between eyes and between the values of both devices.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Participants</span><p id="par0035" class="elsevierStylePara elsevierViewall">The study included 100 eyes of 100 pediatric patients without ocular pathology between 5 and 15 years of age. The sample comprised 60% females and 40% males between 5 and 15 years of age. The mean age was 10.2 ± 2.9 years and the range value was 10 years. The main ethnical group was European (E) (72%) followed by Latin American (LA) (20%), Asian (6%) and finally patients from northern Africa (2%).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Optic disc parameters</span><p id="par0040" class="elsevierStylePara elsevierViewall">The mean RNFLm thicknesses were 99.6 μm (Cirrus) and 106.7 μm (Spectralis) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The values of each quadrant followed the ISNT rule both for Cirrus and Spectralis. The optic disc parameters measured by Cirrus are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, including the thicknesses of each of the 12 sectors. The mean ocular axial length was 23.1 mm and the standard deviation was 0.97 mm.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Correlation between axial length and optic disc parameters</span><p id="par0045" class="elsevierStylePara elsevierViewall">Statistically significant positive correlations were observed between the values of RNFLm and disc area (<span class="elsevierStyleItalic">ρ</span> = 0.37. p < 0.01), edge area (<span class="elsevierStyleItalic">ρ</span> = 0.30. p < 0.01) and the 4 quadrants (nasal <span class="elsevierStyleItalic">ρ</span> = 0.49. inferior 0.80, temporal 0.52 and superior 0.68; p < 0.01). A poor negative correlation was observed between axial length and retina nerve fiber layer thicknesses of the inferior and superior quadrants for OCT Cirrus as well as for OCT Spectralis (p < 0.05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). This negative correlation also occurred between axial length and edge area (Cirrus: <span class="elsevierStyleItalic">ρ</span> = −0.29, p < 0.05) and disc area (Cirrus: <span class="elsevierStyleItalic">ρ</span> = −0.20, p < 0.05), and between axial length and the nasal quadrants of the nerve fiber layer (Spectralis: <span class="elsevierStyleItalic">ρ</span> = −0.22, p < 0.05).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Match grade and differences per age, sex, ethnicity, laterality and type of OCT</span><p id="par0050" class="elsevierStylePara elsevierViewall">The match grade between RNFLm values was moderate (ICC: 0.67; CI 95%: −0.02; 0.87), and also in the rest of quadrants with the exception of the nasal quadrant (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). The Bland–Altman graph is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The RNFLm thickness was larger in females than in males (Cirrus: females = 99 μm, males = 97 μm, p > 0.05; Spectralis: females = 107 μm, males = 106 μm, p > 0.05) without reaching statistical significance. In what concerns age, the sample was divided in 2 groups; group 1 (G1) that included subjects between 5 and 10 years of age (n = 48), and group 2 (G2), between 11 and 15 (n = 52). No statistically significant differences were observed in the RNFLm between said groups (p > 0.05. Cirrus and Spectralis) excepting for the EE inferior quadrant (G1 = 138 μm, G2 = 129 μm, p < 0.01) and in disc area (G1 = 2.16 mm<span class="elsevierStyleSup">2</span>, G2 = 1.98 mm<span class="elsevierStyleSup">2</span>; p < 0.01) for Cirrus and in the inferior quadrant for Spectralis (G1 = 134 μm, G2 = 117 μm, p < 0.01). In what concerns ethnicity and RNFLm thickness, Latin American patients exhibited greater thickness than their European peers (Cirrus: LA = 101 mm, E = 97 mm, p < 0.05; Spectralis: LA = 111 mm, E = 106 mm, p < 0.05).</p><p id="par0060" class="elsevierStylePara elsevierViewall">No statistically significant differences could be observed in RNFL thickness between right and left eyes (p > 0.05. Cirrus and Spectralis). When calculating the RNFLm thickness differences of both OCT devices, the values of OCT Spectralis were higher than those of OCT Cirrus (p < 0.05). Analysis based on axial length produced a difference per age group in RNFLm values (G1 = 22.6, G2 = 23.5, p < 0.01) but not in sex (p > 0.25), ethnicity (p > 0.21) or laterality (p > 0.12).</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">Previous studies with OCT Cirrus in healthy children with large samples included 358<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> and 344 eyes,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> while the study with OCT Spectralis having the largest sample included 162 eyes.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> The highest values for RNFLm were found in North American<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> and Mexican children,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> while the lowest were found in Indian children.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,25</span></a> In general, the samples of the present study could be considered to be between said previous studies. This could be related to the present sample that included patients of different origins. When comparing the present values for the optic disc clock sectors (Cirrus), these were generally thicker (excepting the nasal and nasal inferior sector) when compared to those of Indian ethnicity.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Yanni et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> included Caucasian, African-American and Asian patients and did not find significant differences in RNFLm values. In contrast, the present study found significantly higher RNFLm values in Latin American children than in European children. The present study followed the ISNT rule, matching the findings of other studies.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,17,19,22</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">The correlation between axial length and RNFLm thickness was negative, with low degree and statistically significant, matching other studies in children without ocular pathology.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16,19,20</span></a> Likewise, the present study observed a significant negative correlation between axial length and disc and edge areas. When correlating axial length with age a positive relationship was obtained, following the trend indicated by other authors.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,16,17</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The literature includes reports with higher RNFLm values in males than in females (p < 0.05).<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> This was not observed in the present results as females had greater RNFLm than males, although the difference was not statistically significant. Other authors did not report significant RNFLm differences.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,16,19,23</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> Similarly,<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,17,19,20</span></a> the present study did not observe differences in RNFLm values in both OCT devices on the basis of age, with the exception of disc area values (Cirrus; p < 0.05). In what concerns laterality, significant differences were not reached for RNFLm values, matching the results of previous studies,<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,24,26</span></a> However statistical significance was found in the nasal, temporal and superior sectors with Cirrus (p < 0.05) and in the nasal and temporal sectors with Spectralis (p < 0.05). The difference (d) of RNFLm values between both OCT devices was significant (d = 7.3 μm, p < 0.01), and this value was higher than those reported in other studies.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,27,28</span></a> Despite an extensive bibliographic search, the authors were unable to find an adequately substantiated explanation for the differences between RNFL values produced by said OCT devices. It is known that both utilize virtually the same circular diameter to obtain optic disc measurements and that the resolution is very similar. Even so, the RNFL thickness values differ and for this reason more studies would be necessary to reach a satisfactory explanation.</p><p id="par0080" class="elsevierStylePara elsevierViewall">To date and on the basis of the bibliographic search, the authors were unable to find an article comparing the peripapillary RNFL values of both OCT devices in the pediatric population. By means of ICC, the smallest found match value was 0.45 (nasal sector) why the highest was 0.67 (RNFLm). These values differ from the findings in adult population reported by Tan et al.,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> who mention and ICC of 0.66 (nasal sector) and 0.91 (superior and inferior sector). Patel et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> reported lower values for RNFLm (ICC = 0.62) in an adult population, although the rest of their values were higher than those of the present study. It is assumed that the discrepancy between both values could be associated to structural differences between adults and children. In any case, the grade of match between both instruments in what concerns RNFLm thickness values was low and moderate.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conclusions</span><p id="par0085" class="elsevierStylePara elsevierViewall">On the basis of the findings for reliability and variability between the OCT devices of the present study, it is suggested that their values should not be used indistinctly for diagnostic interpretation as well as for management and follow-up of pediatric populations. On the basis of these results, the authors provide normality values in children between 5 and 15 years for both OCT devices which do not include a database for said population. We used Spectralis OCT and Cirrus OCT to measure optic disc values of both eyes in one hundred pediatric patients (age 5–15 years), with no previous ocular pathology or systemic disease that could affect the eye. Due to the similarity between eyes, only right eyes were considered (n = 100).</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">The average peripapillary retinal nerve fiber layer (pRNFL) values were 99.6 μm with Cirrus and 106.7 μm with the Spectralis system. The four quadrants followed the ISNT rule (inferior-superior-nasal-temporal). The average pRNFL measures correlated negatively with the AL (P < 0.01) in both OCTs, and the degree of agreement between OCTs for pRNFL and inferior RNFL was moderate (CCI: 0.67 and 0.61, respectively), and low in the rest of the sectors. The average pRNFL values of both OCTs were higher in women than in men (P > 0.05) and there were also no statistical differences in the pRNFL according to age or laterality (P > 0.05). Debido al alto grado de semejanza en los valores de ambos ojos al llevar a cabo el análisis estadístico, y para evitar sesgos, en la muestra final solo consideramos los ojos derechos (n = 100).</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0080" class="elsevierStyleSimplePara elsevierViewall">Los valores de CFNRm fueron de 99,6 μm con Cirrus y de 106,7 μm con Spectralis. Las mediciones de CFNRm mostraron correlación negativa con los valores de la longitud axial (p < 0,01) para ambas OCT. Mediante el coeficiente de correlación intraclase (CCI), el grado de concordancia entre los valores de CFNRm y entre los valores de CFNR inferior fue moderado (CCI: 0,67 y 0,61, respectivamente), y bajo en el resto de los sectores. Los valores de CFNRm para ambas OCT fueron mayores en mujeres que en varones (p > 0,05), y tampoco se obtuvo significación estadística según la edad o la lateralidad (p > 0,05).
