Vaze et al.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> described an anti-VEGF treatment non-compliance rate of 10.5% in 248 AMD patients in Australia, the main reason being the perceived lack of benefit.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In another study by García-Layana et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> related to clinical practice in Spain, retinologists surveyed reported that 15% of patients discontinued treatment, mainly due to insufficient response to treatment, other clinical criteria and problems with patient follow-up.</p><p id="par0020" class="elsevierStylePara elsevierViewall">On the other hand, due to the great development and breakthroughs in technology and telemedicine in recent years, optical coherence tomography (home-OCT) devices have been developed for use at home or outside the clinical setting, as well as videoconferencing,<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a> which could be implemented in the near future. However, patients’ perception of these innovative care proposals is so far unknown.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Therefore, the main objective of this study is to identify the main barriers and preferences in health care in NAMD, as well as the perception and fears in relation to their disease and treatment with intravitreal anti-VEGF injections.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">This cross-sectional study included patients from January to November 2020 with a diagnosis of AMD in follow-up by a specialised Macula Unit. All patients gave their written consent to participate in the study, which was carried out in accordance with the ethical standards described in the Declaration of Helsinki for biomedical research, with the protocol being approved by the hospital’s Ethics Committee.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Patients with AMD in treatment with at least 3 intravitreal injections in the last 12 months were included. Exclusion criteria were established as VA < 5 ETDRS <span class="elsevierStyleItalic">(Early Treatment Diabetic Retinopathy Study)</span> letters, or any other coexisting ophthalmologic pathology that could influence VA, such as significant cataract (nuclear cataract ≥ N4/10), glaucoma or other neuropathy, diabetic retinopathy, myopia magna or severe amblyopia.</p><p id="par0040" class="elsevierStylePara elsevierViewall">For the study, an original questionnaire was developed by a group of disease specialists consisting of 26 questions divided in three blocks. The first block comprised questions regarding the disease and treatment with anti-VEGF injections. The second block comprised questions regarding healthcare for AMD, while the third block comprised questions relatied to new technologies and the future of health care. The questionnaire was completed anonymously by the patients or with the help of a family member, with only the specialist completing the VA. The age and sex, and the VA of the best and worst eye of each patient were also recorded.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Statistical analysis was performed using SPSS software® (Statistical Package for Social Sciences, v21.0; SPSS Inc., Chicago, IL, USA). The responses on a numerical scale or quantitative variables were exhibited as mean, standard deviation and range (with the score being from 1 to 5: 1 = not at all; 2 = very little; 3 = medium; 4 = quite a lot; 5 = a lot), and the qualitative variables, as frequency distribution. The median and the 10th and 90th percentiles (p10 and p90, respectively) were also calculated for VA. The t for Student test was used to analyze whether the fear of blindness and of ceasing to be independent was associated with the age, sex or VA of the patients.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">This study included 108 patients with NAMD, 59.2% of whom were women. The mean age of the patients was 80.4 ± 7.0 years (range 57–94). The mean VA in the best eye of each patient was 75.2 ± 12.4 letters ETDRS (range 34–95 letters) and 56.3 ± 20.0 (5–92) in the worst eye.</p><p id="par0055" class="elsevierStylePara elsevierViewall">In what concerns the results obtained in the first section of the questionnaire (related to the disease and its treatment), it should be noted that the patients consider that they have an average knowledge about their disease (2.9 ± 1.1%) and believe that the disease affects their quality of life in 3.2 ± 1.2%. As <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows, the highest scores were obtained in reference to fear of blindness and losing their independence (4.4 ± 0.9 and 4.3 ± 1.0%, respectively). However, there was no association between fear and VA, age, or sex (p ≥ 0.135).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">In relation to intravitreal injections, patients reported a moderate degree of anxiety (2.8 ± 1.3%), with up to 71% reporting anxiety the day before the injection. Concern about the treatment not being effective (4.0 ± 1.3%) was higher than concerns related to possible serious complications or associated pain (3.6 ± 1.3 and 3.0 ± 1.1%, respectively).</p><p id="par0065" class="elsevierStylePara elsevierViewall">With regard to the aim of treatment, most patients (78%) were clear that the aim of treatment is to stop and slow visual deterioration, being aware of the importance of check-ups and injections despite experiencing no improvement in vision (4.3 ± 1.0%).</p><p id="par0070" class="elsevierStylePara elsevierViewall">The main barriers to health care reported by the patients were the long waiting times in the waiting room (72%) followed by other pathologies (10%) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). According to the patients, said waiting time is estimated to be between 1 and 3 h for 65%, with 63% reporting that they spend between 3 and 5 h until they return. In addition, up to 28% of the patients report that they find it quite or very difficult to attend a consultation and 60% of the patients report that what would most facilitate them to attend their medical visit would be to be able to travel to a speciality centre closer to their home. It is worth noting that up to 85% of the patients attach a great deal or a great deal of importance to always being seen by the same doctor.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding the section about new technologies (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), patients with NAMD or their relatives have searched the Internet for information about their disease in 63% of the cases, contrasting with the fact that 89% have never searched for information about the ophthalmologist attending them and only 22% of the patients have requested a second medical opinion.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Overall, 61% of patients with NAMD indicated they were quite or very hopeful (≥4/5) that new treatments would be developed to control visual loss. In relation to the future of healthcare, 69% of patients would be quite or very interested (≥4/5) in having a diagnostic device at home to monitor the status of their macular pathology and the need for treatment (home-OCT type devices), while up to 68% of them showed little interest in video consultations (≤2/5).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">NAMD is a disease with a high demand for both consultation and treatment to slow down the visual deterioration it causes. Therefore, it is important to improve the knowledge of the perception that these patients have in relation to their health care and the main barriers to treatment and monitoring of the disease.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6–10</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">In general, the different series of patients identify similar causes of therapeutic noncompliance reported by patients. Vaze et al.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> highlight the perception of lack of improvement. Polat et al.,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> in a series of 314 patients and an inadequate compliance rate of 39.8%, described that 21.6% of patients refused intravitreal treatment due to doubts about its benefit. These authors determined that patient response to treatment in terms of VA (both improvement and deterioration) was the factor that could have the greatest effect on patient compliance with follow-up and treatment. As our results show, patients are aware of the importance of coming for consultation and receiving treatment despite not experiencing visual improvement. Polat et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> found a correlation between patient compliance and VA in the affected eye at the time of diagnosis, VA after treatment, sociocultural level and age. In the present series, patients were highly concerned about losing their eyesight and therefore their independence as well as the lack of efficacy of the treatment. However, fear of blindness was not related to VA, age or sex. According to other studies, patients reported moderate anxiety to intravitreals, reaching up to 71% the day before. Other studies showed that the causes of such anxiety may or may not depend on the procedure itself, including fear of vision loss and unfamiliarity with the procedure.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–19</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">In the present study, patients reported that the main barrier was the waiting time to be seen, which was estimated, as the most frequent option, to be between 1 and 3 h, dedicating between 3 and 5 h to visit the doctor. For this reason, it is important to carry out agile exploratory circuits that bring together the essential tests, such as VA and OCT, to make therapeutic decisions. In patients undergoing treatment with intravitreal injections of antiangiogenic agents, Sivaprasad et al.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> observed that each appointment takes an average of 4.5 h, determining that the greatest desire of patients is to achieve the same visual results with the least number of visits and injections.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Thus, different authors have described the importance of the proximity of the hospital centre to the home in therapeutic compliance. In the series by Vaze et al.,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> in which 7.6% of patients with poor therapeutic compliance were observed, the determining factor was the difficulty in attending the consultation. Polat et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> found that compliance rates improved in proportion to the decrease of distance between the patient’s home and the hospital. This aspect has been endorsed by our patients, who consider that what would make it easier for them to attend a consultation would be to be seen in a speciality centre close to home. Therefore, providing the speciality centres with diagnostic methods could facilitate therapeutic adherence. In addition, due to age and other comorbidities, up to 28% of patients report that it is quite or very difficult for them to attend a consultation, a factor also described in several series.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Another factor described as essential in therapeutic compliance is the patients’ knowledge of AMD and the importance of treatment. In the present population most patients considered that they had a good knowledge of their disease. Proof of this is that 78% of patients understand that the aim of intravitreal treatment is to slow down the disease, as well as the importance of attending check-ups despite not experiencing improvement in VA.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Regarding new technologies, previous studies have shown that the vast majority of patients (89%) prefer to receive information about AMD from their ophthalmologist rather than from any other source, such as the Internet.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,18</span></a> In the present sample, AMD patients or their relatives searched the internet for information about their disease in 80% of the cases, to supplement the information received in the practice. This contrasts with the fact that 89% never searched for their ophthalmologist on the Internet. Given the importance of close follow-up, 85% of AMD patients attach great importance to always consulting the same ophthalmologist, in whom they seem to place great trust, given that only 22% of the patients surveyed reported having sought a second medical opinion. Therefore, the fact that patients prefer to be seen by the same specialist at each visit should be taken into account in the organization of AMD consultations. Senra et al.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> described that most patients are satisfied with the support received from the hospital and with the information received about their disease and its treatment.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Interestingly, portable OCT devices (home-OCT) have recently been developed that could herald the future of AMD management.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,21</span></a> These devices allow to assess the need for treatment, thus reducing the number of medical visits to patients. Early studies have shown promising results, but patient opinion has not yet been evaluated.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> In the present sample, 69% of patients would be quite or very interested in having this type of diagnostic device at home. On the other hand, in other systemic diseases the use of telemedicine has shown good results in Spain,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> but 68% of our patients showed little interest in video consultations. One reason could be the need for specific supplementary tests for the follow-up of NAMD. This data should be evaluated in the context of tele-ophthalmology initiatives, whose interest has been renewed in the context of the COVID-19 pandemic.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Lastly, as mentioned above, new technologies can help to facilitate diagnosis and follow-up in patients with AMD, but their application in the administration of treatments or in reducing the need to receive them, in this case intravitreal injections, is still limited. Facilitating this treatment in specialty centers could contribute to better accessibility of NAMD treatment. On the other hand, it should be noted that the new antiangiogenic drugs that are being developed (due to their longer duration and lower frequency of injection) could facilitate adherence and contribute significantly to reducing barriers to treatment.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Although the present study answers many questions about patients' perceptions, our work has several limitations. It is a relatively small sample and a single-center study. However, it describes the results of patients in routine clinical practice in our setting. Our study points to a series of elements to be taken into account in the future planning of developments in Ophthalmology Services in terms of AMD therapy: greater proximity to the treatment centre and face-to-face consultations with shorter waiting times and with the same doctor, in detriment of video consultations.
