The antibody was detected in both treated parts (A and C) and one exhibited a significant cell proliferation increase compared to the other.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Ischemic blebs are problematic after glaucoma filtration surgery due to the possibility of causing hypotony and increasing the risk of endophthalmitis. The aim of said surgery is to create a fistula enabling aqueous humor drainage from the anterior chamber to the subconjunctival space. In addition to minor modifications in the surgery technique to improve the uveoscleral drainage pathway, the formation of an operational conjunctival bleb is desirable. Said blebs involve active fibrotic phenomena in the postoperative period that could lead to the relatively immediate failure of filtration surgery. To avoid this, cytostatics are utilized, predominantly mitomycin C and 5-fluoroacil as adjuvant therapy.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Cytostatics involve a broad range of effects. Some patients exhibit hyper-responses that give rise to an ischemic conjunctival bleb with thin walls and excessive filtration, late loss phenomena and very low intraocular pressure.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> The latter could involve potential complications such as hypotension-induced maculopathy as well as increasing the risk of blebitis and endophthalmitis with disastrous results. The therapeutic approach for these patients could be medical, surgical or both. Initially, an expectant attitude should be maintained with close follow-up. However, with high infection risk or symptoms of hypotony the avascular bleb must be repaired.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">There are several repair techniques such as ischemic tissue resection with conjunctival sliding or healthy conjunctival autologous transplant. In some cases, this repair could produce conjunctival bleb failure secondary to surgical manipulation with increased intraocular pressure and risk of glaucomatous disease progression.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6–10</span></a> Techniques such as collagen CXL with riboflavin A stimulated with ultraviolet light have been utilized for several years with good results in corneal ectasia cases. The objective of this technique is to induce the formation of covalent bonds in collagen fibers that reinforce the corneal structure and increase its rigidity, thus diminishing ectasia progression. Its use has been proposed also for thin-walled blebs with the same purpose as a noninvasive and potentially repeatable alternative.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11–13</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The objective of the present paper is to retrospectively compare the results of noninvasive techniques such as conjunctival CXL with riboflavin against 2<span class="elsevierStyleHsp" style=""></span> surgical techniques, i.e., conjunctival sliding and conjunctival autologous graft.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A method was designed to collect the data of eyes that underwent glaucoma surgery and exhibited a thin-walled conjunctival bleb with hyperfiltration, hypotony or risk of infection. The study was approved by the Clinic Research Ethics Committee of the authors’ center and by the Committee for Special Medicaments of the <span class="elsevierStyleItalic">Germans Trias i Pujol</span> University Hospital. The selected eyes were treated by the Glaucoma Dept. of the hospital between 2010 and 2017. The results of the group of eyes treated with CXL with riboflavin technique versus the group of eyes treated with surgery, either conjunctival sliding or healthy conjunctival autologous graft. The method did not include a control group due to the lack of any conservative medical treatment that demonstrated efficiency in this group of patients.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In what concerns the surgical technique, conjunctival sliding consists in making a cleavage in the ischemic bleb and anteriorly displacing the posterior-most conjunctival free and detached from the subconjunctival fibrosis tissue, covering the remaining defect and stitching with Nylon® 10/0 in the margins. If the perilimbal conjunctiva does not remain in the adequate position, 2 “mattress-maker” or U-shaped stitches can be made using the same nylon.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Free autologous conjunctival graft is effective for repairing ischemic blebs regardless of bleb size. The graft is taken from the upper conjunctival area of the same eye. After measuring the defect, the free conjunctival graft is placed in the area to be covered and the conjunctival defect is closed. Autologous conjunctival graft can be used in 2 ways: either with prior cleavage of the bleb, or without resecting the bleb, performing a transposition of the conjunctiva with the tenon. Suture can be done with 2 Nylon® 10/0 stitches.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In both techniques the anesthesia was a 4<span class="elsevierStyleHsp" style=""></span>mL 2 % lidocaine and 0.75 % bupivacaine retrobulbar injection, optionally with topical anesthesia comprising 0.4 % oxybuprocaine and 0.1 % tetracaine (Colirio Anestésico Doble ®, Laboratorio Alcon Cusí, Barcelona, Spain) depending on patient cooperation and characteristics. Both surgical techniques could involve the appearance of ptosis, the necessity of performing additional surgery, diplopia, conjunctival retraction, intraocular pressure increase, addition of hypotensor medication, hypotony and repeated hyperfiltration.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The CXL technique consists in prior instillation of topical anesthesia and subsequent application of ribolavin drops (0.1 % with dextrane, Mediocross © M, Avedro, Boston, USA) every 2<span class="elsevierStyleHsp" style=""></span>min during 20<span class="elsevierStyleHsp" style=""></span>min, followed by UVA irradiation with an energy of 9<span class="elsevierStyleHsp" style=""></span>mW/cm<span class="elsevierStyleSup">2</span> during 10<span class="elsevierStyleHsp" style=""></span>min (CCL Vario System, Ophtec ®, Peshcke Trade, Huenenberg, Switzerland).</p><p id="par0050" class="elsevierStylePara elsevierViewall">In what concerns the histopathological analysis of the fistula treated with CXL, it should be mentioned that previously 2 ischemic blebs were cleavaged for <span class="elsevierStyleItalic">ex vivo</span> treatment with the same procedure in order to assess by means of histological methods the cell effects after applying CXL treatment. after said cleavage, one part was treated with CXL and riboflavin <span class="elsevierStyleItalic">ex vivo</span> and the other was maintained as control. The samples were stained with hematoxylin-eosine and immunohistochemistry by means of the Ki67 antibody (a monoclonal antibody that detects a nuclear antigen that expresses in the cells that are part of the cell cycle).<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">It was expected that after the application of CXL the marker would show a significant reduction. However the Ki67 epithelial cell proliferation marker was detected in both treated parts, with a significant increase in proliferative epithelial cells in one case but not in the other<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">After studying the effects of CXL on the ischemic blebs <span class="elsevierStyleItalic">ex vivo,</span> it was decided to apply the technique <span class="elsevierStyleItalic">in vivo</span> to 4<span class="elsevierStyleHsp" style=""></span>patients. The study comprised best corrected visual acuity, intraocular pressure measured with Goldmann applanation tonometry prior to treatment, after 24<span class="elsevierStyleHsp" style=""></span>h, 1 and 6 months and 1 year. Data on biomicroscopy, anterior segment tomographies, and endothelial corneal density before and after treatment were collected. The number of hypotensor drops before and after carrying out the procedure were compared.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Postoperative treatment in the 3 techniques was nocturnal occlusion the first day followed by combined treatment with cortisone and antibiotic (ciprofloxacin and tobramycin) 3 times a day during the following 2 months.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">The nonparametric Kruskal-Wallis tests were utilized for comparing mean values and for studying all the quantitative variables between different groups. The Wilcoxon test was used for comparing pre- and post-surgery values at 12 months and to determine the existence of variations.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">Sample characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Half of the patients underwent trabeculectomy and 41.67 % nonperforating deep sclerectomy. The antibiotic utilized in all cases was 0.02 % mitomycin C while the exposure time depended on the characteristics of the patient’s eye. In what concerns lens condition, 41.67 % were phakic whereas 58.33 % were pseudophakic.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Statistical analysis was made to determine whether the groups were homogeneous, applying the Kruskal-Wallis test: the value of <span class="elsevierStyleItalic">p</span> for age was 0.376 and for sex 0.497, without significant differences.</p><p id="par0085" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a> shows microscopic images of ischemic blebs prior to treatment with CXL (left columns) with their respective results obtained with anterior segment optical coherence tomography (OCT-SA VISANTE-Zeiss ® Carl Zeiss Meditec, Inc., Dublin, AC, USA) and the same eyes at 12 months (right columns).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The endothelial count prior to CXL and after 12 months was taken. Mirror microscopy was used to determine the following morphological parameters: variation coefficient, endothelial density and percentage of variation. The results are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> without significant differences (p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.715). Data were compared with OCT-SA, studying the presence of cysts, bleb wall thickness, height and volume. The results are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> without significant differences.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">The CXL group exhibited previous intraocular pressure of 12.11<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.14; at 24<span class="elsevierStyleHsp" style=""></span>h, 11.13<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.61 at 1 month, 9.36<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.32; at 3 months, 10.95<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.88; at 6 months 14.22<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.72 and at 12 months 12.32<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.29. In eyes treated with autologous conjunctival graft, prior intraocular pressure was 11.65<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.76; at 24<span class="elsevierStyleHsp" style=""></span>h 5.58 ± 6.34; at 1 month 12.13 ± 5.22; at 3<span class="elsevierStyleHsp" style=""></span>months 11.56 ± 6.26; at 6 months 13.21 ± 10.21 and at 12 months 14.78 ± 7.21. In the case of the group treated with conjunctival sliding, prior data were 9.32 ± 5.34; at 24<span class="elsevierStyleHsp" style=""></span>h 7.41 ± 12.73; at 1 month 16.21 ± 5.74; at 3 months 13.16 ± 5.57; at 6 months 14.28 ± 5.88 and at 12 months 15.16 ± 9.24.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Accordingly, no statistically significant differences were found comparing prior intraocular pressure and after 12 months (p = 0.655) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Prior visual acuity was compared against 12 months of treatment in each of the 3 groups without finding statistically significant differences: CXL p = 0.083; autologous conjunctival graft p = 1.000 and conjunctival sliding p = 0.317. Therefore, no significant differences were observed for visual acuity in any of the 3 groups before or 12 months after the procedure. In what concerns intraocular pressure and number of medicaments before the procedure and 12 months later, no differences were found in the CXL group of the autologous graft group. However, differences were found in the conjunctival sliding group (p = 0.021 and p = 0.046. respectively) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">Tannenbaum et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> proposed to study the results obtained after resection of the ischemic bleb and conjunctival sliding in patients with hypotonic hypo-infiltrating blebs. To this end they analyzed a retrospective case series treated with said technique and observed intraocular pressure prior to the procedure of 3.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.4 and 13.8 ± 4.8 at the end of the follow-up (p < 0.0001). For this reason they stated it was a good technique for repairing said fistulas because it could safely control glaucoma. Even though the intraocular pressure they found was significantly lower than in the present series, the results at 12 months are similar (15.6<span class="elsevierStyleHsp" style=""></span>mmHg), with a significant increase (p = 0.021).</p><p id="par0115" class="elsevierStylePara elsevierViewall">A retrospective study published by Panday et al.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> comparing a series of 51 cases of patients with hyper-filtrating ischemic blebs treated with autologous conjunctival graft and a mean follow-up of 80 weeks reported intraocular pressure values that increased from 4.41 ± 4.61<span class="elsevierStyleHsp" style=""></span>mmHg to 12.67 ± 4.83<span class="elsevierStyleHsp" style=""></span>mmHg (p < 0.001), with a failure percentage of 20.6 %. Said authors concluded that autologous conjunctival graft could be an effective technique for managing hyper-filtrating ischemic blebs. Compared with the present results, prior intraocular pressure was much lower but the results 12 months after surgery were similar, i.e., in the present study the value was 14.78<span class="elsevierStyleHsp" style=""></span>mmHg (p = 0.273).</p><p id="par0120" class="elsevierStylePara elsevierViewall">Recently, Choy et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> carried out a prospective study with a single CXL session on 5 ischemic blebs that exhibited a significant pressure increase at the end of the follow-up. They concluded that a single session of CXL was effective to limit the process of a cystic bleb, maintaining the effect from week 1 post-CXL up to 5 months later without adverse effects. In a retrospective study of a 7-patient series with ischemic bleb, Wang et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> reported that one year after treatment with CXL 5 cases exhibited complete resolution of excessive filtration and went from an intraocular pressure of 3.9 ± 1.0–9.6 ± 3.0<span class="elsevierStyleHsp" style=""></span>mmHg without adverse effects. The results of both studies did not correlate with the present results as in the present study intraocular pressure increased only 0.22<span class="elsevierStyleHsp" style=""></span>mmHg (p = 0.655).</p><p id="par0125" class="elsevierStylePara elsevierViewall">The surgical review of ischemic filtration blebs could produce an immediate postoperative intraocular pressure increase as well as persistence and repetitions of leaks, conjunctival dehiscence, dysesthesia and requirements for additional subsequent surgery. In the experience of the authors, conjunctival sliding facilitates pressure increase but is associated to less treatment requirements whereas autologous transplant and CXL do not appear to have less effect on intraocular pressure even though in the former case it could help diminish the ischemic conjunctival area. If hypotony is severe and compromises visual acuity or involves scleral melting risk, it is necessary to carry out an examination under the scleral flap.</p><p id="par0130" class="elsevierStylePara elsevierViewall">In the experience of the authors, the positive results obtained by other authors might not be attributed to CXL treatment<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> because the untreated hyper-filtrating ischemic bleb could exhibit some tendency towards resolution without self-limited Seidel phenomena and fluctuating visual acuity. No differences were found at the end of treatment in bleb structure and morphology at the biomicroscopic level or with OCT-SA examinations.</p><p id="par0135" class="elsevierStylePara elsevierViewall">By way of conclusion, ischemic bleb management is difficult and challenging. Even though we agree with Choy et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> in that CXL could be a noninvasive, affordable, safe and repeatable alternative, in the present small series a single session was not effective in what concerns macroscopic changes nor in OCT-SA. The same can be said about intraocular pressure changes.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The present study exhibits the limitations inherent to its retrospective nature, the lack of a control group and the small number of subjects. However, it does not seem that CXL is an effective method for managing ischemic blebs. Perhaps a larger group could provide more definitive conclusions.
