Md is regarded as balanced and healthy and able to provide optimum nutritional support.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> Recently, several publications relate compliance with Md to diminished cardiovascular risk factors.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">6,7</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The objective of this study is to assess compliance with Md by patients with POAG in the authors’ population area as well as analyzing its relationship with gender, education level and cardiovascular risk.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and method</span><p id="par0025" class="elsevierStylePara elsevierViewall">An observational study was carried out to assess compliance with Md by patients diagnosed with POAG who visited the Ophthalmology Dept. of the University Hospital Complex of the Canary Islands (Tenerife, Spain). The reference population of this hospital is the northern area of Tenerife (400,000 inhabitants).</p><p id="par0030" class="elsevierStylePara elsevierViewall">The study consisted in a survey that was carried out through a personal or telephone interview comprising 14 items validated by the Prevention through Mediterranean Diet (PREDIMED) study. The survey also included questions about the epidemiological and cardiovascular characteristics of participants (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The sample size was 100 surveys corresponding to 100 patients, 50 males and 50 females. One patient was excluded due to withholding informed consent for the survey. The study was carried out from January to March 2016.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The results of the study were compared on the basis of the scores obtained according to the following scale: a score of 0–6 (low compliance), 7–10 (moderate compliance), 11–14 (high compliance).</p><p id="par0040" class="elsevierStylePara elsevierViewall">The study was authorized by the Clinic Research Ethical Committee of the University Hospital Complex of the Canary Islands and was in accordance with the Helsinki declaration criteria.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The scores obtained by each patient according to the responses given were statistically analyzed with Microsoft Excel (Microsoft Office Professional 2016, Redmond, WA, USA), one Chi square test and one Mann–Whitney <span class="elsevierStyleItalic">U</span> test. Several analyses were carried out at the Research Unit of said University Hospital Complex.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Characteristics of the sample</span><p id="par0050" class="elsevierStylePara elsevierViewall">Overall, 100 glaucomatous patients who completed the survey were identified. In what concerns gender, 50% of patients were male and 50% female. The mean age was 68.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.46; in the group of males, it was 69.58<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.16 and in the group of females 67.42<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.41. Mean weight was 76.56<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.86 mean height of 166.67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.62 and mean BMI was 27.63<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.67 (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">In 82% of cases, glaucoma had developed in both eyes, in 9% the right eye and a further 9% the left eye. Mean years since diagnostic was 10.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.94 for the right eye and 10.13<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.11 for the left eye. Treatment was studied based on the number of eyedrops prescribed in each eye: 42% of all patients had one eyedrop; 50% had 2 eyedrops and 4% had 3 eyedrops. In addition, 31% of patients exhibited first and second generation familial antecedents.</p><p id="par0060" class="elsevierStylePara elsevierViewall">As regards cardiovascular risk variables, 17 cases of active tobacco smokers were identified (17%) and 34 cases of former smokers (34%). Arterial hypertension was exhibited by 58 cases while dyslipidemia was exhibited by 41 cases (58 and 41%, respectively). In addition, 22 cases had been diagnosed with diabetes mellitus (DM) type <span class="elsevierStyleSmallCaps">II</span>, with a mean time since diagnostic of 11.68<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.72 years; in treatment with ADO, 77.27% and with insulin, 22.72 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Only 14 patients (14%) took vitamin supplements for glaucoma.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">As regards education level, 68 patients indicated they had completed primary education, 20 had secondary education and 12 patients had university education (68, 20 and 12% respectively) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Questionnaire results</span><p id="par0075" class="elsevierStylePara elsevierViewall">The values of the responses to the 14 items of the PREDIMED survey are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Compliance with Md in males was low in 9 patients (18%), moderate in 37 (74%) and high in 4 of the cases (8%). In females, compliance was low in 14 patients (28%), moderate in 34 (68%) and high in 2 cases (6%). Overall, 23% with low compliance, 71% with moderate compliance and 6% of cases with high compliance (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Utilizing the PREDIMED questionnaire that comprises 14 items for measuring compliance with Md, it was not possible to demonstrate differences between males (mean: 8.04<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.564) and females (mean: 7.40<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.666), for which reason the Mann–Whitney <span class="elsevierStyleItalic">U</span> test was utilized (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.055).</p><p id="par0090" class="elsevierStylePara elsevierViewall">The Chi square test was applied to compare hypotheses, assuming equal variances with a confidence interval of 5% (<span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05) in order to determine the existence of significant differences between genders and cardiovascular risk factors. The following results were obtained:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">a)</span><p id="par0095" class="elsevierStylePara elsevierViewall">The relationship between tobacco smoking and gender (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) was statistically significant, concluding that the group of males includes more smokers.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">b)</span><p id="par0100" class="elsevierStylePara elsevierViewall">AHT and gender (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.685), which means the absence of significant differences.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">c)</span><p id="par0105" class="elsevierStylePara elsevierViewall">Dyslipidemia and gender (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.422), no significant differences.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">d)</span><p id="par0110" class="elsevierStylePara elsevierViewall">Diabetes and gender (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.009), significant differences were found, with diabetes being more frequent in males than in females.</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">e)</span><p id="par0115" class="elsevierStylePara elsevierViewall">Education level for each gender gave a <span class="elsevierStyleItalic">p</span> value of 0.215, which means the absence of significant differences.</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">f)</span><p id="par0120" class="elsevierStylePara elsevierViewall">BMI (kg/m<span class="elsevierStyleSup">2</span>) was also compared with gender, and compliance with Md vis-à-vis gender, utilizing the Mann–Whitney <span class="elsevierStyleItalic">U</span> test, which produced a p value of 0.56 and 0.055, respectively, which demonstrates the absence of significant differences.</p></li></ul></p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0125" class="elsevierStylePara elsevierViewall">The analysis carried out in said reference population demonstrates a moderate compliance with Md in the glaucomatous population without gender differences.</p><p id="par0130" class="elsevierStylePara elsevierViewall">A study carried out in 2014 at the University Hospital Center of Virgen de la Candelaria (Tenerife) assessed compliance with Md in patients affected by diabetes macular edema compared to healthy controls (University students) among the population of the Canary islands. Adherence to Md and diabetes was low in 6 patients (9.68%%), moderate in 44 (70.97%%) and high in 12 cases (19.54%). In the control group of patients, compliance was low in 29.12%, moderate in 67.96% and high in 2.91%.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> In the present study 23% of the subjects exhibited low compliance with Md, 71% exhibited moderate compliance and 6% high compliance. Accordingly, when comparing diabetes macular edema with glaucomatous patients, compliance with Md is higher in the first group. This is not the case with healthy patients who demonstrated lower compliance when compared to glaucomatous patients and even more so compared to diabetic patients. This leads to the conclusion that, in addition to receiving specific medical treatment, diabetes patients receive information about the best diet as in adjuvant, and this information should be also given to glaucomatous patients.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In 2012 a nationwide study was carried out in order to determine compliance with Md and the level of physical activity of users of the Plan for Physical Activity Sports and Health website (PAFES) in the general population. A PREDIMED study questionnaire was adapted for said website, which demonstrated that compliance was high (11–14 points) in 12.5%, moderate (7–10 points) in 67.2% and low (0–6 points) in 20.3%.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> Apart from said PREDIMED questionnaire, other surveys have been developed for assessing compliance with Md.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">9–11</span></a> However, as is the case with a questionnaire utilized in the present study, said surveys must be certified.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The potential benefits of Md, nutritional supplementation, antioxidants and neuroprotective medicaments for glaucoma has been described by different authors.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">4,12–14</span></a> However, the authors believe that diet and nutritional supplementation guidelines are not sufficiently implemented in daily clinical practice, in the light of the benefits these guidelines could provide not only for ocular but also for general health.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">15–19</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">The increased availability of refined food products with high concentrations of sugar, fat and energy (such as sweet drinks and bakery products, among others), the destructuring of meals, the low level of knowledge and education (68% of the present sample completed only primary school), and skills for buying and preparing food together with the high technological level entailing diminished physical activity at work, daily life and leisure time, are factors that explain low and moderate compliance with Md. The study included completing a 14-item questionnaire validated by the PREDIMED Study, in person or by telephone.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 100 questionnaires were completed successfully by 50 males and 50 females. The mean age was 69.58 years for the males and 67.42 years for women. The men had more comorbidities than women (tobacco 14 vs. 3%), arterial hypertension, and diabetes (30 vs. 28%, and 16 vs. 6%, respectively). Adherence to the Mediterranean diet in males, was low in 9 patients (18%), moderate in 37 (74%), and high in 4 (8%) cases. In women adherence was low in 14 patients (28%), moderate in 34 (68%), and high in 2 (6%) cases. Consistió en la realización de una encuesta —personal o telefónica—, compuesta de 14 ítems validados por el estudio PREDIMED.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Se encuestó a 100 pacientes, 50 hombres y 50 mujeres. El grupo de los hombres tenía una edad media de 69,58 años y el de las mujeres, de 67,42 años. Los hombres presentaban mayores comorbilidades que las mujeres (tabaco 28 vs. 6%), hipertensión arterial (60 vs. 56%), diabetes (32 vs. 12%). La adherencia a la dM en hombres fue baja en 9 pacientes (18%), moderada en 37 pacientes (74%) y alta en 4 pacientes (8%). En las mujeres la adherencia a la dM fue baja en 14 pacientes (28%), media en 34 pacientes (68%) y alta en 2 pacientes (6%). El total de adherencia a la dM de los pacientes es baja en el 23%, moderada en el 71% y alta en el 6%.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusiones</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Los pacientes diagnosticados de GPAA en nuestra área de atención sanitaria presentan en su mayoría una adherencia moderada a la dM.
