Some are based on the modification of keratometric power, either by means of corneal surface techniques such as those based on photoablation (laser-assisted in situ keratomileusis or photorefractive keratectomy) or photodisruption (lenticule extraction by small incision), or techniques based on corneal incisions, perforating or not (arcuate, facing incisions), or anterior or posterior chamber phakic lens implants.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> All these techniques have proven to be very effective and safe over the last 3 decades thanks to several publications and meta-analyses.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Although corneal refractive surgery has demonstrated with sufficient evidence its efficacy and safety, several studies on visual quality and dry eye showed better results in patients implanted with Visian ICL phakic lenses than with other techniques on the corneal surface.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a> To date, there is no population-based study that analyses the status and trends of refractive surgery with phakic lenses in Spain and shows the current patterns of surgical indication in the Spanish ophthalmic community.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The main objective of this study is to establish a description of the present situation of refractive surgery with phakic lenses in Spain, as well as to obtain the visual and anatomical results.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Study design</span><p id="par0020" class="elsevierStylePara elsevierViewall">This is a population-based, cross-sectional, descriptive, analytical, retrospective study in which each co-author collected data from at least their last 10 patients (up to a maximum of 30) undergoing refractive surgery with EVO Visian ICL or Visian ICL phakic lens (STAAR Surgical, Monrovia, CA, USA), regardless of their characteristics, in each of the 11 clinics participating in the study. The data analysed are as follows: patient age and gender, pre- and post-surgical uncorrected (UCVA) and corrected (CVA) visual acuities, preoperative and residual refraction, keratometry, endothelial count (ECD), anterior chamber depth from endothelium (ACD), central corneal pachymetry, intraocular pressure (IOP), white-to-white distance, implanted lens (power and size), expected spherical equivalent (SEQ), and postoperative ACD, central corneal pachymetry, ECD, IOP and vault at one month after surgery. In all cases, the lens and lens power to be implanted were calculated using the OCOS online calculator™ (Online Calculation & Ordering System, STAAR Surgical, Monrovia, CA, USA) based on white distance measurement. ) based on the white-to-white distance and ACD measurement obtained with Pentacam (Oculus, Inc., Arlington, Washington, USA) and optical biometer, as well as the subjective refraction performed at each participating centre. Descriptive statistics, analysis of results and statistical analysis with efficacy, safety, refractive predictability and correction index were performed with these data.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The data were collected in a spreadsheet prepared for this purpose, with the personal data of each subject participating in the analysis conveniently pseudo-anonymised for subsequent statistical analysis. One eye was randomly selected from each study subject using the Rand function in Excel.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Inclusion criteria were: age between 21 and 60 years, refraction between −18.00 and +10.00 D with an astigmatism up to +6.00 D, ACD measured from endothelium to anterior lens capsule higher than 2.8 mm for myopes and 3.00 mm for hyperopes and no anatomical-functional alterations or previous ophthalmological diseases.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Patients who underwent refractive surgery with EVO Visian ICLs by each of the authors between January and March 2021 were included in the study. Patients had to have at least one post-surgical check-up at least one month after ICL implantation surgery, in order to obtain comparable post-surgical results among all authors.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Subjects with unstable refraction in the last 12 months, previous ocular surgery, corneal topography with alterations in the anterior or posterior curvature, ocular or systemic disease that could alter the healing process (connective tissue diseases, diabetes mellitus, etc.) were excluded from the study. Only one eye from each subject included in the study was randomly included.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">The statistical analysis of the database, in addition to descriptive and correlational statistics between variables, analysed the residual spherocylindrical refraction and the UCVA and CVA, establishing the safety and efficacy indices, as well as the correction index, defined as the ratio between the ametropia to be corrected compared to the ametropia actually corrected. The analytical software SPSS Statistics (IBM, v. 20.0) was used for the statistical analysis and representation of the results.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">A total of 140 eyes were included from 140 subjects with a mean age of 31.35 ± 7.28 (range 19–55) years, with 46.4% of the subjects being male (65/75), and 61 eyes being right eyes, 43.6% of the sample. The mean preoperative SEQ was −6.33 ± 3.69 D, with 5.7% of the sample being hyperopic (8/140). Of the sample of myopic patients, 50% had an SEQ of less than −7.25D. The rest of the descriptive parameters of the sample, as a whole and in the group of myopic eyes, are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The mean power of the implanted ICL lens in the whole sample was −7.81 ± 4.09 D (range −18.00 to +10.00), and -8.33 ± 3.26 D (range −18.00 to −2.00) in the lenses of the myopic group, 32.4% being toric lenses, with a mean cylinder of +2.27 ± 1.23 D (range +0.50 to +6.00). The mean SEQ in the myopic sample was −6.85 ± 2.91 D (range −1.13 to −16.50 D), which corresponds approximately to −5.58 D in corneal plane considering a mean vertex distance of 12 mm. The target SEQ chosen in the power calculation was +0.04 ± 0.27 D (range −0.89 to +0.89).</p><p id="par0060" class="elsevierStylePara elsevierViewall">Regarding the size of the implanted lenses, 45.69% were sized 13.2 mm, followed by 37.93% of lenses sized 12.6 mm, 12.93% sized 12.1 mm and 3.45% sized 13.7 mm (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">As for the myopic sample, the refractive, functional and anatomical results at one month after surgery are described in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The SEQ and cylinder accuracies are described in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>. Regarding refractive predictability, a correction index of 0.96 ± 0.25 was obtained and a correlation between corrected and preoperative SEQ of R<span class="elsevierStyleSup">2</span> = 0.993. For the cylinder in the implanted toric lenses, the accuracy showed a correction index of 0.90 ± 0.19 and a correlation between the preoperative and corrected cylinder of R<span class="elsevierStyleSup">2</span> = 0.882. The change in UCVA went from 0.05 ± 0.11 to 1.02 ± 0.18, a change of 0.95 ± 0.23 decimal places. Regarding CVA, the change was from 0.95 ± 0.17 to 1.05 ± 0.15, an increase in CVA by 0.07 ± 0.13. 75% of the eyes have UCVA of 1.0 or better and more than 85% of the eyes have a CVA of 1.0 or better (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). These data indicate an efficacy index of 1.06 ± 0.18 and a safety index of 1.10 ± 0.19. Refractive and visual outcomes according to the laterality of the eye showed no significant differences in UCVA, CVA, SEQ, efficacy and safety indices (p = 0.828, 0.540, 0.899, 0.880 and 0.632, respectively). The mean vault at one month was 508.80 ± 198.40 (58–1100) µ.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">In 70% of the cases the vault was in theoretically ideal parameters between 300 and 700 µm (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>). The mean ECD at one month after surgery was 2,802.93 ± 273.30 cells/mm<span class="elsevierStyleSup">2</span>, which was 0.11% (23.09 ± 273.3; p = 0.922) higher than preoperatively. Similarly, mean IOP was 14.04 ± 2.59 mmHg, with a change of -0.92 ± 2.80 mmHg from preoperative.</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">There were no intraoperative complications and no eye required lens explantation or repositioning. There were no cases of anterior subcapsular opacity, pigment dispersion, pupillary block or other potentially sight-threatening complications.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">Much has already been said about the good refractive and visual residuals with phakic lens implantation in pre-presbyopic patients.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,6–8</span></a> In all these articles the efficacy and safety indices are higher than 1, as in our work where no patient lost visual acuity lines (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The study by Tang and Liao<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> showed that the mean SEQ at 1 day, 1 week, 1 month and 3 months after V4c ICL implantation was −0.23 ± 1.35, −0.26 ± 1.18, −0.25 ± 1.18, and −0.26 ± 1.16, respectively. Notably, the mean prior SEQ in the myopic group was −6.84 ± 2.91 D, clearly lower than that observed in other recent work,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,8,13</span></a> reflecting a higher degree of confidence with the procedure and that the technique is no longer exclusive to high myopia.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The difference between SEQ 3 months after lens implantation and SEQ before implantation was statistically significant (p < 0.001). A study by Li et al.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> showed that the SEQ of the V4c ICL 6 months after surgery was slightly lower than that of the previous model without central hole, but the difference was not statistically significant. This was consistent with the studies of Shimizu et al. and Kamiya et al.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,14</span></a> Seventy-five per cent of eyes have UCVA of 1.0 or better and more than 85% of eyes have CVA of 1.0 or better (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><p id="par0090" class="elsevierStylePara elsevierViewall">In relation to mean IOP, there was a slight decrease from 14.92 ± 2.57 to 13.98 ± 2.57 mmHg, with no patient with IOP above 20 mmHg postoperatively. The results of this study match those of other authors, such as Kamiya et al. (13.1 mmHg, 0 eyes > 21 mmHg) and Lisa et al. (12.4 mmHg, 0 eyes > 20 mmHg).<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a> No substantial difference in IOP in any eye could be observed in this study. As already demonstrated, the AquaPORT central orifice offers the advantage of not requiring iridotomy or iridectomy to avoid pupillary block or pigmentary dispersion syndrome.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> This work has a short follow-up, but in other studies with a longer follow-up period, the results are very similar.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">There were also no statistically significant differences in ECD. The mean ECD was 2,758.00 ± 395.62 cells/mm<span class="elsevierStyleSup">2</span> preoperatively and 2,802.93 ± 362.49 cells/mm<span class="elsevierStyleSup">2</span> postoperatively. Our results match those of other studies where the mean value ranged between 2574 and 2799 cells/mm<span class="elsevierStyleSup">2</span> with a percentage of cell loss between 1.7 and 5.4% with a longer follow-up period.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,8,13,15</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Cell losses in the first months after surgery are often related to the surgery. The experience of the surgeons in this group and the reduced manipulation with the V4c model have helped to achieve these good results.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The mean vault at one month was 508.80 ± 201.04 (58–1100) µm. Kamiya et al., in 46 eyes with a follow-up period of one year, found that the vault of the central hole lens is very similar to that obtained with the previous models, suggesting that the presence of the port did not affect the vault or the refraction obtained.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> This mean vault is higher than that observed in other studies due to the short follow-up period in our case. We know that there is a progressive decrease in vault during the first 6 months after surgery. This decrease attenuates later and is greater in those cases with high vault.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In 2 eyes (0.7%) we had a very low vault (≤100 µm) and in 6 eyes (4.4%) a low vault (101−200 µm). We consider the separation with the lens to be the key parameter to be assessed.</p><p id="par0105" class="elsevierStylePara elsevierViewall">One of the main risks associated with the development of anterior subcapsular cataract is low vault. In a review of early ICL models, Fernandes et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> reported that in 33.8% of eyes with ICL-related cataract the vault was less than 200 µm. In contrast, in a recent study with the longest follow-up period (7 years) with the new V4c model, there were no cases even though 20% of the cases had a vault smaller than 200 µm. Gonzalez-Lopez et al. analysed only those cases with vault less than 100 µm in miosis and presented only one eye (4.17%) with anterior subcapsular opacities in a 45-year-old patient operated on 5.7 years earlier.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> This confirms that the central port of the V4c model could prevent or at least delay the onset of lens opacities. Following this evidence, it was decided not to perform ICL explant in the eyes with reduced vault, recommending only annual check-ups. In 4 eyes (3%) we had a high vault (≥900 µm) that could be easily solved by rotating the lens to the vertical meridian.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> The sulcus has an oval shape, with the vertical meridian larger than the horizontal meridian. Therefore, lens rotation is expected to reduce postoperative vault and the complications associated with high vault without the need to exchange the lens.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">These good results with the new model at the crystalline lens level have allowed us to select smaller lens sizes than usual. In the 7-year study, none of the implanted lenses were 12.1 mm (the smallest size manufactured in myopic lenses).<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> In contrast, even though the present study has a percentage of 12.1 mm patients, the most frequent was still 13.2 mm, and the percentage of 13.7 mm has clearly diminished (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><p id="par0115" class="elsevierStylePara elsevierViewall">In conclusion, it should be noted that surgery with ICL phakic lens implantation is an effective, safe and predictable procedure in the short term with high refractive predictability. The present study represents a multicentre study with different decision algorithms and surgical techniques which, as seen in the good results obtained, have no influence on the parameters usually analysed. One eye from each subject was randomly selected for study inclusion, and the variables analysed were: age, gender, visual acuities with and without correction pre and post-surgery, preoperative and residualrefraction, keratometry, endothelial cellular density, anterior chamber depth from endothelium, central corneal pachymetry, intraocular pressure, white-white, implanted lens (power and size), expected and achieved spherical equivalent (SEQ) post-surgery at one month after surgery.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">A sample of 140 eyes was obtained from 140 patients with a mean age of 31.35 ± 7.28 years. Mean preoperative SEQ was −6.33 ± 3.69D, mean CDVA was 0.96 ± 0.16, ACD was 3.30 ± 0.29 mm, WTW 12.02 ± 0.40 mm, CCT 535.03 ± 37.68 µm, ECD 2684.37 ± 313.74 cels/mm<span class="elsevierStyleSup">2</span> and IOP 14.84 ± 2.59 mmHg. 66.4% of the implanted lenses were spherical ICLs and 33.6% toric, with a mean power of −7.81 ± 4.09D and a mean cylinder of 2.27 ± 1.23D. 5.7% of the implanted lenses were hypermetropic. The mean SEQ target selected was +0.04 ± 0.27D. 48.92% of the implanted lenses were of size 13.2 mm, with the remaining 36.69, 10.79 and 3.60% being of sizes 12.6, 12.1 and 13.7 mm, respectively. At one month after surgery a residual SEQ of −0.01 ± 0.31D was obtained. The efficacy and safety indices were 1.06 ± 0.18 and 1.10 ± 0.19, respectively. The mean central vault was 508.80 ± 201.04 µm. The accuracy between the calculated SEQ and the obtained SEQ was 99.3% for spherical lenses and 88.2% for Cyl correction. The mean ECD at 1 month after surgery was 2805.53±273.30céls/mm2, which was a difference from preoperative of 0.11% (p = 0.922), similarly, the mean IOP was 13.98 ± 2.57 mmHg, which was a difference of −0.92 ± 2.80 mmHg.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">ICL phakic lens surgery is an effective, safe and predictable procedure. Se elegió aleatoriamente un ojo de cada sujeto para incluirlo en el estudio, y se analizaron las variables: edad, género, agudezas visuals sin corrección y con corrección pre y postquirúrgica, refracción preoperatoria y residual, queratometrías, contaje endothelial, profundidad de cámara anterior desde endotelio, paquimetría corneal central, presión intraocular, blanco-blanco, lente implantada (potencia y talla) y equivalente esférico esperado y obtenido (SEQ) esperado y obtenidoposquirúrgico al mes de la cirugía.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Se obtuvo una muestra de 140 ojos de 140 pacientes con una edad media de 31,35 ± 7,28 años. El 66,4% de las lentes implantadas fueron ICL esféricas y el 33,6% tóricas, con una potencia media de −7,81 ± 4,09 D y un cilindro medio de 2,27 ± 1,23D. El 5,7% fueron hipermetrópicas. El 45,69% de las lentes implantadas eran de la talla 13,2 mm, siendo el 37,93, 12,93 y 3,45% restante de las tallas 12,6, 12,1 y 13,7 mm, respectivamente. Al mes de la cirugía se obtuvo un SEQ residual de −0,01 ± 0,31 D. Los índices de eficacia y seguridad fueron de 1,06 ± 0,18 y 1,10 ± 0,19, respectivamente. El vault central medio fue de 508,80 ± 201,04 µm. La precisión entre el SEQ calculado y el SEQ obtenido fue del 99,3% en el caso de las lentes esféricas y del 88,2% para la corrección del cilindro.
