Tumor regression with a calcified pattern <span class="elsevierStyleItalic">(white arrow)</span> and multiple new lesions in middle periphery <span class="elsevierStyleItalic">(white arrow heads)</span>.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Anophthalmic sockets are the result of severe ophthalmological traumas or diseases that require the eye globe’s removal.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Retinoblastomas play an important role in anophthalmic cavities in children as, although there are numerous therapeutic options (systemic, intra-arterial or intravitreal chemotherapy, transpupillary thermotherapy, cryotherapy…), in some cases enucleation is inevitable in order to avoid tumour spreading which is the main goal in retinoblastoma treatment.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Orbital implants are used after enucleation to help restore volume and the aesthetic appearance of a normal eye. Implant extrusion remains an important concern as although different materials have been studied in order to prevent it or fix it, there is still no consensus regarding the most optimal material for these cases.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case report</span><p id="par0015" class="elsevierStylePara elsevierViewall">A six months old Caucasian female patient with an unremarkable medical history was referred to our department for left eye leukocoria. Ophthalmological examination of the latter eye revealed an intraocular mass with an exudative retinal detachment associated in the infero-temporal quadrant (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>b). Right eye examination was normal.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">Ultrasonography showed a 14 by 11 mm intraocular mass with microcalcifications within the tumor, which was highly suggestive of retinoblastoma. Further imaging techniques confirmed it was a single mass with no spreading outside the eye (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>a).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The patient was diagnosed with grade E left eye retinoblastoma.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Treatment with etoposide, vincristine and carboplatin chemotherapy associated to selective chemoembolization of the left ophthalmic artery with melphalan was started. Tumor regression characterized by the presence of calcium in the remaining lesion was observed during the following examinations and transpupillary thermotherapy was carried out to consolidate the favourable results.</p><p id="par0035" class="elsevierStylePara elsevierViewall">However, four months after diagnosis multiple lesions were observed in middle periphery, which increased over the following weeks despite chemotherapy treatment (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>c). After discussing the case with the Oncology Committee, left eye enucleation was decided. A 16 mm hydroxyapatite prosthesis was placed in the anophthalmic cavity and sutured.</p><p id="par0040" class="elsevierStylePara elsevierViewall">During post-operative check-ups a satisfactory anatomic result of the remaining anophthalmic cavity was observed. However, two years after prosthesis implantation surgery, signs of prosthesis extrusion were appreciated (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">We decided to use a semisynthetic dura mater substitute patch as a graft to cover the defect in order to obtain the better possible long-term results with the minimum number of surgical interventions as the patient´s age made general anaesthesia necessary for every procedure.</p><p id="par0050" class="elsevierStylePara elsevierViewall">First, we cut the four by four centimetre Dura-Guard® (Synovis®) sterile semisynthetic xenograft (bovine origin) which acts as a dura mater substitute, stored at room temperature in a sodium hydroxide solution, to the correct size to cover the conjunctival buttonhole. The surrounding conjunctiva was partially dissected in order to fit the graft underneath the defect. Overlying conjunctiva was then sutured using Vicryl® 6/0 (Videoclip 1).</p><p id="par0055" class="elsevierStylePara elsevierViewall">Postoperative results were satisfactory, achieving correct healing of the conjunctiva. There is no evidence of prosthesis extrusion six months after the procedure.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Conclusion</span><p id="par0060" class="elsevierStylePara elsevierViewall">The use of dura mater and its substitutes in ophthalmology has been reported specially linked to glaucoma surgery.
