"apellidos" => "Valentín-Bravo" "email" => array:1 [ 0 => "fjvb90@icloud.com" ] ] ] "afiliaciones" => array:2 [ 0 => array:3 [ "entidad" => "The Retina Clinic London, London, United Kingdom" "etiqueta" => "a" "identificador" => "aff0005" ] 1 => array:3 [ "entidad" => "Hospital Germans Trias i Pujol, Barcelona, Spain" "etiqueta" => "b" "identificador" => "aff0010" ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Opacidades vítreas: un fenómeno común pero poco comprendido" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">The vitreous is an extracellular matrix composed mainly of water, collagen and hyaluronic acid.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> These elements are organised in such a way that they form a homogeneous, transparent gel.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Liquefaction of the vitreous and aggregation of its collagen fibrils, mainly type II, generate vitreous opacities commonly known as "floaters".<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0005">Vitreous opacities</span><p id="par0010" class="elsevierStylePara elsevierViewall">Vitreous opacities are small particles in the vitreous that cause light scattering and/or cast shadows on the retina.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Despite their high prevalence, the impact of vitreous opacities has traditionally been underestimated by healthcare professionals. This may be because, in most cases, they are imperceptible and have little effect on standard visual function tests such as visual acuity. However, we must not forget that for many patients, these vitreous opacities can lead to a significant deterioration in quality of life and even psychological state.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Factors such as the ageing of the population, the increasing prevalence of myopia and the constant use of backlit screens (computers, mobile phones, tablets) have contributed to an increase in the perception of these opacities and an increase in the patient's search for a solution.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In addition, the development of new vitreous and retinal imaging technologies has enabled an improved visualisation of the vitreous and a more objective perception of patient symptoms.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The study of the vitreous and vitreous opacities goes beyond the search for a better understanding of their nature, as the use of new imaging and functional techniques can provide valuable information in other ocular and/or systemic conditions. An application of said techniques for diagnostic purposes is the assessment of activity and monitoring of response to treatment of tumours (primary ocular lymphoma) or inflammatory processes (uveitis).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">The role of imaging techniques in the study of vitreous and vitreous opacities</span><p id="par0025" class="elsevierStylePara elsevierViewall">Despite being the largest structure in the eye, the study of the vitreous and non-invasive imaging thereof has been a great challenge due to its characteristics.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">The development of new imaging techniques such as wide-field optical coherence tomography (WF-OCT) and infra-red confocal scanning ophthalmoscopy (IRcSLO), is allowing better visualisation of the vitreous, the vitreoretinal interface and its opacities.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">The greater image depth on OCT enables the assessment of vitreous opacities as hyper-reflective spots, as well as the evaluation of posterior vitreous detachment (PVD) and its extent.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> On the other hand, dynamic IRcSLO imaging also allows assessment of the behaviour of vitreous opacities with eye movement and provides a more complete and objective assessment of their impact on patient vision.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,7</span></a> It appears that the characteristics of the opacities such as size, shape, density, kinetics and the presence of posterior vitreous detachment may be determining factors in the degree of clinical symptoms perceived by the patient.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Similarly, more complex functional tests are being developed that assess not only visual acuity and contrast sensitivity, but also the light scattering that can be caused by these opacities on an experimental and clinical level.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Recently, Stanga et al. demonstrated the usefulness of a new methodology in the assessment of vitreous opacities using the described techniques.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> They observed the absence of these opacities in WF-OCT, in IRcSLO and an improvement in light scattering using three different measurement methods after vitrectomy.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Treatment of vitreous opacities</span><p id="par0050" class="elsevierStylePara elsevierViewall">Historically, "floaters" have not been considered to be amenable to therapeutic intervention, mainly as a result of limited options and diagnosis based on patient symptomatology.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">However, technological advances in both diagnosis and treatment enable the development of more effective and personalised therapies to eliminate symptomatic vitreous opacities.</p><p id="par0060" class="elsevierStylePara elsevierViewall">At present, treatment options for vitreous opacities include vitrectomy, either partial or complete, and Nd:Yag laser vitreolysis.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,9</span></a> Both options present significant advances but are not free of risks and complications. Therefore, it should be taken into account that this is an elective treatment and the patient should make the ultimate decision after careful and personalised assessment.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Outlook for the future</span><p id="par0065" class="elsevierStylePara elsevierViewall">The future of the study of the vitreous and its opacities is promising. With the ageing population, the increasing prevalence of myopia and the constant use of backlit screens, the demand for effective solutions for vitreous opacities is on the rise. It is a reality that "floaters" generate increased disability in today's world and observation alone is not the solution in all patients.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Therefore, there is a clear need to assess objectively the anatomy of the vitreous and its opacities, as well as their visual involvement and intermittent effect on visual function.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> The refinement of diagnostic techniques and the implementation of a methodology is essential to understand and classify vitreous opacities.
