The incidence and prevalence of each type of uveitis differs in different regions of the world. In industrialized countries, the incidence is estimated between 17 and 45 cases for 100,000 inhabitants/year with a prevalence of approximately 38–714 cases for 100,000 inhabitants.3

An additional factor of relevance is the high economic and social cost inherent in the morbidity of this disease. The study was carried out between January 2010 and March 2015. This is the reference practice for the entire Community and, according to 2014 data, it covers a population of 196,166 inhabitants of Pamplona and 640,790 inhabitants throughout said Community. In 2014, the foreign population in the region was 59,550 inhabitants (9.3% of the entire population). The study excluded patients in pediatric age (under 16) and those with episcleritis, scleritis and keratitis without ocular inflammation.

All the patients were examined by an ophthalmologist (HHM) who carried out a complete ophthalmological exploration comprising best corrected far visual acuity, applanation tonometry, biomicroscopy and indirect ophthalmoscopy. When necessary, the examination included optical coherence tomography, fluorescein angiography, campimetry and ocular echography. When intraocular infection was suspected, aqueous humor puncture was made and Polymerase Chain Reaction (PCR) and cultures were requested. When neoplasia etiology or ocular sarcoidosis was suspected, aqueous humor flow cytometry was performed.

The anatomic diagnostic was made in accordance with the diagnostic criteria established by the International Uveitis Study Group9 in 1987 and as per the criteria of the Standardization of Uveitis Nomenclature Working Group (SUN).10 Depending on the affected portion of the uvea, patients were classified as having anterior uveitis (AU), intermediate uveitis (IU), posterior uveitis (PU) and panuveitis (PAN). The group of uveitis patients with identified etiologies was divided in: infectious, systemic noninfectious, specific ocular, mixed (as defined in the previous paragraph) and other causes. The latter group included masked syndromes, uveitis induced by drugs and traumatic. Toxoplasmosis was considered in the cases of PU or PAN with hyperpigmented scar and associated to vitritis (in the acute phase). In all patients IgG serology in blood was positive. Ocular syphilis diagnostic was made when 2 typical ocular patterns were recognized, i.e., pre-retinal inflammatory deposits and posterior plaque-like chorioretinitis with positive serology. In addition, and after the series published by Llorenç et al.,8 this is the second with the largest number of included patients.

The 2 series in the comparative study were chosen due to the multidisciplinary nature, which means that the diagnostic approach should be similar in the 3 series, thus diminishing the possibility of bias. In the series by Llorenç et al.,8 the obvious angiographic and population differences are an opportunity for studying different causes of uveitis in different geographical areas. In turn, the study carried out in Castilla León6 was chosen because numerically it is the largest among available studies. In addition, it shares geographic and ethnic characteristics with Navarra, such as being inner regions of Spain and having a low percentage of immigrants.

In general, the results of the comparative study are quite similar in the 3 series. It must be taken into account that the type of practice is the same in the 3 series as they represent reference multidisciplinary uveitis consulting practices in each region. As commented above, the sample size is different, and this could explain why the differences between the Castilla León and Navarra studies were not statistically significant.

It is important to take into account the demographic diversity between the 3 series. On the one hand, the percentage of foreign patients was larger in the Barcelona series, and this can be attributed to a larger number of immigrants population and that in all likelihood the sample in this series was taken from urban and multiethnic populations. This differentiating factor could have contributed to a higher frequency of the infectious etiology in Barcelona.

The higher mean age in the Navarra series is due to the inclusion of pediatric patients in the other 2 series. AU was more frequent in the 3 series and specifically in the Navarra series, above the numbers reported in northern regions of Mediterranean countries such as Italy (51.2%)21 or Portugal (60%).22 The percentage of unclassifiable uveitis was higher in the Navarra series than in Barcelona, and similar to that of Castilla León. This could be due to the fact that the multidisciplinary practice in the Navarra hospital has less experience and history as it was established in 2010. In previous studies carried out in the 80s in Spain, unclassifiable uveitis reached 50.5%.23 This percentage has diminished considerably with the passage of time so that, in subsequent studies in the late 90s, the percentage that diminished to 33.5%.24 This is similar to the data of other more recent European series.25 The implementation of multidisciplinary uveitis practices and the improvements in microbiological diagnostic methods are probably the most important factors in said diagnostic changes.

In what concerns the most frequent etiology type, ankylosing spondylitis was first followed by herpes virus infection and ocular toxoplasmosis in the series of Navarra and Castilla León. In the Barcelona series, the 2 latter etiologies were more frequent than spondylitis. A possible reason could be the lower amount of patients with AU included in the Barcelona series. The greater prevalence of toxoplasmosis and tuberculosis in the Barcelona series could be partly explained by the higher percentage of immigrants patients, mainly from parts of the words with high prevalence of these infections, such as Latin America.8

The present study could be limited in several areas, such as its retrospective nature which means that the data depend on the quality of the information collected up prior to the study. In addition, the Barcelona series excluded masking syndromes and finally, even though the 3 practices were multidisciplinary, the organization and possibly the diagnostic approach was different in each. A further difference is that the multidisciplinary practices of Navarra and Castilla Leon are a reference in their respective regions whereas the Barcelona practice is also a national reference, which means that the complexity of the cases referred to the latter may be greater.

The diagnostic approach of uveitis patients must be individualized and directed by the clinic exhibited by the patient at the ophthalmological as well as systemic level. In general lines, it can be approached in 2 different ways: carrying out an exhaustive study in all patients or only in those exhibiting additional data in their clinical records or in the physical examination. Obviously, the first approach is more complete but is unlikely to be cost-effective and involves considerable costs and time. In addition, patients could be overdiagnosed by encountering alterations that are unrelated to the ophthalmological process. In contrast, carrying out etiological studies only on patients exhibiting systemic expressions could lead to underdiagnosing patients as in some diseases such as sarcoidosis the eye can be the first and only compromised organ.26 Therefore the decision to carry out an etiological study must be individual, taking into account the severity of the disease and the suspicions the ophthalmologist might have as well as the data collected by the other specialists when carrying out a specific clinic history and complete physical examination by means of devices.

Accordingly, it is necessary to pursue consensus on the diagnostic approach for uveitis patients. The currently applied approach in the authors’ practice, which is in accordance with the most widely accepted criteria, i.e., on the basis of the anatomic diagnostic and ophthalmological suspicion, the basic study of uveitis patients should include analysis with acute phase reactants, hemogram, biochemistry, basic urine profile, Mantoux or IGRA, syphilis serology and chest X-ray. The patients who will probably require immunosuppressants treatment should undergo serology for hepatitis B and C virus and HIV. In the case of AU, it would be convenient to add HLA-B27 and lumbar and sacroiliac spinal column X-rays.

Uveitis patients in the Navarra series are similar to those in the Castilla Leon series and this could be due to the fact that both are samples of inner regions of Spain and have a lower percentage of immigrants population when compared to the Barcelona series. The present study reaffirms the idea that, even though the differences are not very marked between the 3 regions, they do exist, making it necessary to carry out more epidemiological studies in different regions in order to evaluate said inter-regional differences.
Conflict of interests
The authors declare that they do not have any commercial or financial interests and that they have not received any economic aid for this study.
