with angle closure being the second most frequent type&#46; Accordingly&#44; early detection of this type by outpatient practices using optometry and ophthalmology is important because patients have higher probabilities of losing their eyesight than with open angle glaucoma&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">1</span></a> Global prevalence of glaucoma for the 40&#8211;80 age group reaches 3&#46;5&#37;&#44; while estimates for 2040 indicate that the number will rise to 111&#46;8 million inhabitants&#44; with higher proportions affecting Asian and African populations&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Chamber angle measurement through gonioscopy with Goldmann 3-mirror lens is a highly reproducible indirect objective technique for expert observers&#44; at present being the standard test for clinically assessing angle structures such as the level of iris insertion&#44; peripheral iris shape&#44; angle width&#44; trabecular pigmentation degree&#44; anterior peripheral synechiae areas or apposition through 360&#176;&#44; as well as providing qualitative information about the anatomic condition of the structures that make up the iridocorneal angle&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">2</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Another objective technique is the Van Herick test&#44; which assesses the peripheral depth of the anterior chamber and compares it with the peripheral corneal thickness through the projection of an oblique light beam&#46; This technique correlates well with gonioscopy although it does not substitute it because it does not provide details of the above mentioned structures&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Technology developments have led to an increase of ultrasound in recent years for anterior segment examinations&#44; particularly for studying angle closure glaucoma as it provides high resolution images utilizing probes between 35 and 50<span class="elsevierStyleHsp" style=""></span>MHz&#46; Tissue penetration is of approximately 5<span class="elsevierStyleHsp" style=""></span>mm in lateral and axial resolution at 50<span class="elsevierStyleHsp" style=""></span>&#956; and 25<span class="elsevierStyleHsp" style=""></span>&#956;&#44; respectively&#46; Therefore&#44; anterior segment structures comprising the lens&#44; ciliary body and zonules can be morphologically assessed and quantitatively measured&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">4</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">A comparison of the results between said 3 tests has shown that the gonioscopy examination has a significant correlation with angle sizes measured with UBM&#44; making gonioscopy a reliable subjective method for angle interpretation&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">5</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Accordingly with the above&#44; the present study has the objective of evaluating the matches between the UBM quantitative test and the clinical assessment with gonioscopy and Van Herick as adequate examinations for estimating iridocorneal angle&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methodology</span><p id="par0035" class="elsevierStylePara elsevierViewall">The study was conducted in 2 University institutions in Bogot&#225; &#40;Colombia&#41;&#46; Candidates with ocular surgery history&#44; use of topical or systemic medication that could alter pupil size&#44; keratitis and concussion trauma were excluded from the study&#46; Before carrying out clinic tests&#44; all participants signed an informed consent which complied with Resolution 8430&#44; 1993 regulating ethical considerations for research in humans&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">Measures included intraocular pressure with Goldman tonometer &#40;Ocular Instruments&#44; USA&#41;&#44; anterior chamber depth measured through ultra-biomicroscopy UBM 840 &#40;Appasamy Associates&#47;Gantec Corporation&#44; USA&#41;&#44; indirect gonioscopy with Goldman lens &#40;Ocular Instruments&#44; USA&#41; and angle estimation through Van Herick subjective observation using a slit lamp of the <span class="elsevierStyleItalic">High Street</span> type&#46; The retina and optic nerve were also assessed by means of observation with a 90<span class="elsevierStyleHsp" style=""></span>D lens &#40;Ocular Instruments&#41; and the use of slit lamp&#46; The Van Herick and gonioscopy clinic estimates were performed by a single examiner&#44; while the quantitative assessment with UBM was performed by an experienced ophthalmologist&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">For statistical analysis purposes&#44; data distribution was assessed with the Shapiro&#8211;Wilk test&#46; Central tendency and dispersion measurements were calculated for quantitative data&#46; Dispersion graphs were analyzed to assess the correlation between relevant quantitative variables&#46; The kappa statistical test was utilized to calculate the match between UBM and gonioscopy&#44; and between UBM and Van Herick&#46;</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">The Van Herick technique</span><p id="par0050" class="elsevierStylePara elsevierViewall">Slit lamp was configured with a 16&#215; amplification&#46; An optical section in the lighting system was utilized&#44; placing the light beam on the nasal and temporal sclerocorneal limbus&#46; After identifying the anterior chamber&#44; the space between the cornea and the iris was observed&#46; In order to estimate anterior chamber depth&#44; measurements were graded as follows&#58; grade 0&#44; iridocorneal angle&#59; grade 1&#44; free space between iris and endothelial under one fourth of corneal thickness&#44; equivalent to narrow anterior chamber&#59; grade 2&#44; free space between the iris and endothelium of one fourth of corneal thickness&#44; equivalent to semi-narrow anterior chamber&#59; grade 3&#44; free space between the iris and endothelium of one half the corneal thickness&#44; equivalent to non-occludable anterior chamber&#59; grade 4&#44; free space between the iris and endothelium in a proportion of 1&#58;1 or more&#44; equivalent to a normal or deep non-occludable anterior chamber&#46; This procedure was performed by a single examiner&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">6</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Gonioscopy examination</span><p id="par0055" class="elsevierStylePara elsevierViewall">One drop of topical anesthesia &#40;0&#46;4&#37; benoxinate hydrochloride&#41; was administered it to the right eye&#44; instilling 2&#46;5&#37; methylcellulose in the gonioscopic lens in order to adhere the lens to the lower edge of the ocular sac&#44; indicating the subject to look upwards&#46; Assessment began with the mirror in the 12 o&#8217;clock position&#44; being rotated for identifying angle structures in each quadrant&#46; Angle grades were classified according to the Schaffer-Etienne scale&#44; as follows&#58; grade 0&#44; no visible structures&#59; grade 1&#44; only Schwalbe&#39;s line is visible&#59; grade 2&#44; Schwalbe&#39;s line and trabecular mesh are visible&#44; although not the spur&#59; grade 3&#44; 3 structures can be observed&#44; i&#46;e&#46;&#44; up to the scleral spur&#44; and grade 4&#44; all structures are visible from Schwalbe&#39;s line to the ciliary band&#46; This procedure was carried out by a single examiner&#46;<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">7</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Ultrasound biomicroscopy examination</span><p id="par0060" class="elsevierStylePara elsevierViewall">In this procedure&#44; the patient had to remain horizontal with the back to the floor on a stretcher&#46; The UBM 840 device was utilized &#40;Appasamy Associates&#47;Gantec Corporation&#44; USA&#41; with a 35<span class="elsevierStyleHsp" style=""></span>MHz probe in a darkened room after applying topical anesthesia &#40;0&#46;4&#37; benoxinate hydrochloride&#41;&#46; An ocular made of plastic was used to gently separate eyelids&#44; filling a silicone container with distilled water avoiding exerting pressure on the ocular globe&#46; Gaze remained constant and the patient was asked to look at an object in the roof with the fellow eye&#46; The probe was maintained at right angles to the ocular structures in the limbar region being examined&#44; and each eye was assessed in the axial section with radio images of the limbus area in all 4 quadrants &#40;superior&#44; inferior&#44; nasal and temporal&#41;&#46; Angle images thus obtained were quantitatively classified on the basis of the following scale&#58; grade 0&#44; closed&#44; angle with under 10 degrees&#59; grade 1&#44; between 10 and 15 degrees&#59; grade 2&#44; between 15 and 25 degrees&#59; grade 3&#44; between 25 and 35 degrees and grade 4&#44; over 35 degrees&#46;<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">8</span></a> The examination was performed by an ophthalmologist previously trained for the task&#46;</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">The 60 eyes of 30 participants were included in the analysis&#44; 67&#37; of which were female &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>20&#41;&#46; The mean age was 50<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>11&#46;51 years&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">The quantitative assessments measured through UBM indicated an angle for all quadrants between 15 and 25 degrees&#46; &#40;<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">Objective and subjective examinations for estimating iridocorneal angles constitute a valuable tool for diagnosing closed angle glaucoma&#46; A range of studies have reported similar angle results between UBM and gonioscopy&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">9</span></a> In addition&#44; the clinical assessment of angle structures with practical and simple subjective examinations such as the Van Herick technique is important&#46; In turn&#44; even though UBM quantifies angles with precision&#44; it is not the first diagnostic tool in clinic practice for identifying iridocorneal opening&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">9</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The results of the present study indicate an adequate match between the test of reference &#40;UBM&#41; and gonioscopy &#40;kappa 0&#46;96&#41; and between UBM and Van Herick &#40;kappa 0&#46;83&#41;&#46; Similar values were reported by Liu et al&#46; between UBM and gonioscopy &#40;kappa 0&#46;88&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">9</span></a> Even though in the view of the authors gonioscopy could be in the first instance a decisive clinical assessment for diagnosing glaucoma&#44; Cui et al&#46; pointed out that UBM examination is indispensable to confirm angle closure&#44; emphasizing that none of said tests should be substituted&#46;<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">The present results indicate that age is not an exclusive factor that promotes angle closure measured by means of UBM&#44; which leads the authors to think that other variables could have a stronger relationship&#46; Even though the angle closure prevalence in Asian subjects over 40 years of age could rise up to 70&#37; with greater closure frequency in the superior quadrant&#44;<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">11&#44;12</span></a> we consider that other factors could intervene in this clinic finding such as family history&#44; female sex&#44; ametropia and ocular globe and anatomy&#46;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">13</span></a> Small ocular sizes and hypermetropia are considered to be strong associated factors that could give rise to angle closure&#46;<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">14</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">At present&#44; supplementary tests based on images of the ocular structures can be utilized for assessing anterior chamber angles&#46; Anterior segment optical coherence tomography shows the structures with high velocity and resolution&#46; The classification of said devices is determined by the wavelength used for scanning&#46; For example&#44; Fourier-domain optical coherence tomography quantifies the angle with 840<span class="elsevierStyleHsp" style=""></span>nm wavelength and provides values that determine the detection of occludable angle&#46;<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">15</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">Other devices operate with wavelengths of 1310<span class="elsevierStyleHsp" style=""></span>nm&#44; transversal resolution of 60<span class="elsevierStyleHsp" style=""></span>&#956;&#44; and axial resolution of 10&#8211;20<span class="elsevierStyleHsp" style=""></span>&#956;&#46; Said characteristics provide valuable quantitive information on dynamic changes in angle configuretion&#44; which is not provided by standard gonioscopy&#46; In comparison with UBM&#44; optical coherence tomography is not invasive while being reproducible&#44; quick and well-tolerated&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">The most significant limitations of the present study are the low number of subjects&#44; convenience in the selection of participants due to the fact that for the majority of studied subjects their inclusion could have been biased by clinical interest and a presumptive glaucoma diagnostic&#46;</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0120" class="elsevierStylePara elsevierViewall">UBM&#44; gonioscopy and Van Herick tests are clinically compatible&#46; Gonioscopy and Van Herick can indicate the anatomic and clinic shape of the iridocorneal angle structures&#46; The results of said observation constitute a valuable tool to adequately request diagnostic confirmation tests&#44; together with appropriate ophthalmological management&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">Even though the UBM is not generally included in a routine examination&#44; it would be appropriate to request it to confirm the diagnostic and define the surgical procedure&#44;
