This modifiable risk factor is linked to the disease in several ways,<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">4–6</span></a> including the development of Graves’ disease (GD) in healthy patients, the development of GO in patients with GD,<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">7</span></a> the reactivation of GO after orbital decompression<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">8</span></a> and a worse and slower response to treatment of GO with oral glucocorticoids and orbital radiotherapy.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">9,10</span></a> Furthermore, the severity of GO is dose-related to the history of tobacco consumption,<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">10,11</span></a> and there is also evidence that GO is exacerbated by passive smoking.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">12</span></a> In the light of this evidence, smoking cessation is recommended for prevention and treatment of GO,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">4</span></a> and is now viewed as almost mandatory given the fact that current smoking has a stronger influence than lifetime tobacco consumption on the progression of GO and GD.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">5,11,13,14</span></a> Advice to stop smoking has been successful in relation to cardiovascular diseases,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">15</span></a> lung diseases,<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">16</span></a> and cancer,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">17</span></a> and also in the healthy population.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">18</span></a> However, as far as we know, the effectiveness of smoking cessation recommendations has never been studied.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients, inclusion and exclusion criteria</span><p id="par0010" class="elsevierStylePara elsevierViewall">A retrospective cohort was studied. The patients were identified through an electronic database. All the patients included had GO and were referred by the endocrinologist to a single ophthalmology consultant (MSS) at our institution between January 2013 and December 2014.</p><p id="par0015" class="elsevierStylePara elsevierViewall">During their first appointment in ophthalmology all the current and former smokers were strongly advised on the importance of not smoking, by means of a detailed explanation of the relation between smoking and GO. The counselling emphasized the greater risk associated with current tobacco smoking versus accumulated lifetime consumption.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">11</span></a> Patients were shown photographs of cases with severe GO in order to reinforce the advice. The endocrinologist also counselled all the patients, but the advice did not include showing photographs.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The selected group of participants in this study were aged 18 years or more and had to give a written informed consent. The study design met the Declaration of Helsinki standards.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">19</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">All the patients were followed and treated medically or surgically as necessary, regardless of their smoking status.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In December 2015 all patients in the database were asked by telephone to answer a questionnaire evaluating their smoking habits and other social variables at the time of the initial ophthalmology consultation (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Patients who reported smoking cigarettes regularly or occasionally in the year prior to development of GD or GO were considered as smokers and were included in the study. Patients who had never smoked or who reported that they had not smoked during the year prior to development of GD or GO were considered non-smokers and former smokers respectively and were excluded from the study. Patients who could not be contacted were also excluded from the study.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Main outcome measures</span><p id="par0035" class="elsevierStylePara elsevierViewall">Patients were asked about their concurrent or former smoking habits by a telephone questionnaire. The main outcome of the study is to assess the differences in cigarette consumption before and after the specific counselling of smoke cessation provided by an ophthalmologist.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The main questions were related to the average number of cigarettes smoked per day in the year before the diagnosis of GD and GO and the number of daily intake of cigarettes after medical consultation with the endocrinologist and 12 months after the consultation with the ophthalmologist. The patients who smoked at the moment of the consult in ophthalmology, but referred that they had not smoked for at least 12 months after the consultation in ophthalmology were considered as former smokers. Other questions were adapted from other tobacco use questionnaires.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">20–22</span></a> These are the ages when the patients start smoking daily, the time without smoke since awakening, the number of attempts to stop smoking before the development of GD or GO and the use of adjuvant pharmacotherapy to avoid the relapse.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In order to adjust the intervention with other variables, patients were also asked about their demographic characteristics such as their annual income, their formal education, their working status and their family situation. The social environment was analyzed by asking about the number of close friends and relatives that smoke, the number of alcoholic drinks they had per week or the number of caffeinated drinks per day. All these questions were included in the questionnaire (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0050" class="elsevierStylePara elsevierViewall">Clinical characteristics such as severity or activity of GO at the presentation, the treatment received for GD or GO or the presence of other diseases were obtained from their medical records.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Parametric tests (<span class="elsevierStyleItalic">T</span>-test paired and ANOVA) were used because normality tests (Kolgomorov–Smirnov and Shapiro–Wilk) and histograms showed that cigarette smoking had a normal distribution.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Dataset and statistical analysis were performed with STATA v.13® (STATA Corp., College Station, TX 77845, USA).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Patients studied</span><p id="par0065" class="elsevierStylePara elsevierViewall">A total of 110 patients were found in our database. It was impossible to contact 33 patients. The remaining 77 patients agreed to answer the telephone questionnaire but 44 patients were excluded (24 were nonsmokers and 20 were former smokers). The remaining 33 patients were current smokers at the time of developing GD and formed our sample for the study.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The mean age was 54.7 (SD 9.65), 26 were female (78.8%) and the mean age when they became daily smokers was 19.78 (SD 7.11). Just 1 patient had diabetes mellitus and 9 had other associated diseases such as hypertension or lipid disorders. The disease activity and severity of GO patients is described in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Tobacco consumption</span><p id="par0075" class="elsevierStylePara elsevierViewall">15.39 (standard deviation, SD 10.31) and 13.6 (SD 9.66) were the mean number of cigarettes smoked per day before and after the consultation in endocrinology respectively. 13.6 (SD 9.66) and 6.30 (SD 7.73) were the mean number of cigarettes smoked before and after the consultation in ophthalmology respectively. The change in smoking habit was significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0003 in ANOVA). In order to clarify which consultation was responsible for this change, a <span class="elsevierStyleItalic">T</span>-paired test was performed between means. The change in cigarette smoking was not significant (<span class="elsevierStyleItalic">p</span> value<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.235) before and after the endocrinology consultation but it was significant before and after the ophthalmology consultation (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0006).</p><p id="par0080" class="elsevierStylePara elsevierViewall">After the endocrinology consultation 2 patients quit smoking (6.1%), 3 patients decreased the number of cigarettes consumed per day (9.1%) and 28 patients smoked the same or more cigarettes than before (84.8%). However, after the ophthalmology consultation 14 patients quit smoking (42.42%), 10 decreased their smoking habit (30.3%) and 9 patients smoked the same or more cigarettes than before (27.3%).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Patients’ characteristics among groups</span><p id="par0085" class="elsevierStylePara elsevierViewall">According to the evolution in the number of cigarettes smoked daily after the advice given by the ophthalmologist, patients were classified into three groups: patients who stopped smoking (if they were smokers already when they developed GO and ceased smoking at the time of the phone call), patients who smoked less cigarettes (if they still smoke but the number of cigarettes per day is lower than when they developed GO) and patients who smoked at least the same amount of cigarettes (if they smoke the same number or more cigarettes than when developed GO). The characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">Counselling in smoke cessation has been used widely as primary prevention in healthy people or as secondary prevention for multiple diseases. The counselling in smoking cessation for GO patients is included in the clinical practice guidelines of this disease. This advice must be provided to all patients in all circumstances because benefits outweigh risks. It is also recommended that patients should be referred to smoking cessation programmes. Furthermore, passive smokers should be identified and advised on the negative impact.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">23</span></a> However, there is no evidence about the effectiveness of counselling in smoking cessation in GO. The aim of this study was to assess the effectiveness of the counselling of smoking cessation in patients diagnosed with GO and attended by an ophthalmologist. We studied patients examined in the last 2 years in order to avoid memory bias, and 33 patients were included. In spite of the small sample size, the power of our study is 0.99 due to the paired structure and we found significant results.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The prevalence of current smokers in our study was 42.85% (33 smokers out of 77 patients) which is very similar to other cohort studies (40.2%).<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">24</span></a> The age and sex of our study were similar to other studies.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">24</span></a> However, the GO activity and severity of patients included in our study were not comparable to other studies,<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">24</span></a> probably due to the small sample size.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In our study, all the patients were examined first by an endocrinologist and then referred to the ophthalmologist. The endocrinologist counselled all the patients, but significant results in decreasing tobacco consumption were not achieved (from 15.4 cigarettes smoked before the consultation done by the endocrinologist to 13.6 after it, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.235). The ophthalmologist provided a firm and impressive counselling during the initial visit to the ophthalmologyst's office. The patients became aware of the importance of non-smoking after that consultation, and from there on we found a significant change in the number of cigarettes smoked daily (from 13.6 cigarettes smoked daily to 6.3, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0006). Of the 33 patients, 14 patients quit smoking (42.42%) and 10 decreased their smoking habit (30.3%). Our results are greater than those observed in other studies of verbal counselling which achieved a 2–3% of people who stopped smoking,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">18</span></a> and at least as high as some results found in patients with myocardial infarction.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">15</span></a> GO is a deforming disease which affects deeply the patients’ quality of life,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">3</span></a> therefore they are prone to change habits such as tobacco consumption. Probably, the endocrinologist examined GD patients without the development of ocular involvement and they did not insist in the relation between tobacco consumption and orbitopathy. All these reasons made the advice of smoke cessation less effective than the one given by the ophthalmologist in GO patients.</p><p id="par0105" class="elsevierStylePara elsevierViewall">There is some evidence that continuous counselling and nicotine replacement therapy are the most effective interventions for smoking cessation.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">25</span></a> We have proved that a strong counselling could persuade 42.4% of patients to quit smoking, but we had to reward and provide continuous counselling to obtain better results. In our study, we did not send patients to smoking cessation units nor were treatment or nicotine substitutive indicated directly. Furthermore, 28 patients from our sample did not use any drug or nicotine substitutive to avoid smoking relapse (28 out of 33, 84.84%), and only 12 achieved smoke cessation. Had those drugs been prescribed we could have obtained better results.</p><p id="par0110" class="elsevierStylePara elsevierViewall">It is important to highlight that there were similar number of attempts to stop smoking in the three groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.56). In other words, the advice to stop smoking was considered by all the patients in the same way, but only some of them achieved smoking cessation. In order to clarify the conditions that could have been related to achieve smoke cessation we analyzed the clinical and socioeconomic features of the patients.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Although there are no great or significant differences between the patients who stop smoking or continue with their smoking habit, there are some important clues. 9 of the 14 patients (64.28%) who stopped smoking and 8 of 10 patients (80%) who decreased their cigarette consumption had active GO. On the other hand, just 5 of the 9 patients (55.5%) who increased cigarette consumption had active GO. There were only two patients with GO with sight threating severity, and both of them stopped smoking. It could be hypothesized that active and severe cases are more prone to quit smoking than mild and inactive cases. These results could be explained by the inverse relation between severity, and quality of life<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">26</span></a> and by the fear of illness.</p><p id="par0120" class="elsevierStylePara elsevierViewall">All our patients had become daily smokers at the same age (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.52). Most of them did not smoke before 60<span class="elsevierStyleHsp" style=""></span>min after waking up. In contrast to expectations, 64.28% of the patients who stopped smoking drank alcohol. We thought that the direct relationship between smoking and drinking alcohol would be present in our results.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">27</span></a> However, a relationship between smoking and the amount of alcoholic drinks per week was not made because of the variability and the difficulties to obtain accurate answers from the patients. Studies with bigger sample sizes should be made in order to clarify this fact. On the other hand, differences among the proportion of patients who drank caffeine or the number of caffeine drinks consumed in one day (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.73) and smoking habit were not found.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The social environment was important to achieve smoking cessation.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">28,29</span></a> 63% of the patients who stopped smoking did not have close friends and relatives who smoked, in contrast to 25% and 28.57% who decreased and increased cigarette smoking, respectively. The educational level was similar among the groups, but there were differences in the working status. 12 of 23 patients (52.17%) had a job while 2 of 9 (22.22%) unemployed patients stopped smoking. On the other hand, 11 of 23 (47.82%) of the working group while 7 of 9 (77.77%) from the unemployed group continued smoking. However, great differences in the annual income among groups were not found. The employment stability could be an important factor (even more determinant than annual income) which affect the response to the advice of smoke cessation.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Most of the patients in the three groups referred were married or had a stable couple. Their family support during the disease was excellent in most of them. Nevertheless, the majority of the patients (12 of 14, 85.71%) who achieved smoking cessation referred excellent family support. This fact confirmed, that social and personal stability could be important factors to achieve smoking cessation in patients with GO.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusion</span><p id="par0135" class="elsevierStylePara elsevierViewall">Smoke cessation counselling was effective in patients with GO. A strong advice and a detailed explanation emphasizing the relationship between GO and current smoking met our objective. We found that 42.42% of the patients ceased smoking (14 of 33) and 30.3% (10 of 33) decreased the amount of cigarettes smoked daily.</p><p id="par0140" class="elsevierStylePara elsevierViewall">There were no great social, economic or educational differences among patients who quit smoking or continue smoking. However, patients who achieved smoking cessation showed active GO (64.28%), were employed (85.71%) and had an excellent family support (85.71%).</p><p id="par0145" class="elsevierStylePara elsevierViewall">As far as we know, this is the first study to assess the effectiveness of advice to stop smoking in patients with GO. The purpose of this study is to assess the change in the smoking habit in patients affected with GO after an oral counselling for smoking cessation.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A retrospective cohort of GO patients was studied. The patients received a significant oral counsel during the first consultation with the ophthalmologist. 33 GO patients were explored in the ophthalmology clinic during 2013 and 2014 and the study was done throughout a telephone questionnaire in 2015. The main outcome was the number of cigarettes smoked daily before and after consultation with the endocrinologist and the ophthalmologist. Other medical and socioeconomic factors were recorded.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The mean number of cigarettes that were smoked was 13.6 (SD 9.66) and 6.3 (SD 7.73) before and after the consultation done at the ophthalmology office (<span class="elsevierStyleItalic">T</span>-test paired, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0006). 42.42% achieved smoking cessation and 30.3% decreased their smoking habit.
