In children, visual field has a very limited application mainly due to the difficulty in obtaining valid values because this requires continued attention during a relatively long period of time, which is generally possible only in selected cases.</p><p id="par0010" class="elsevierStylePara elsevierViewall">However, since the development of optical coherence tomography (OCT), the diagnostic and therapeutic approach of many diseases has changed in our practices. To date, OCT is the only test that enables an objective study of ocular structures with a very short continued attention span required of patients. In addition, the fact that OCT does not require contact with eyes<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">1,2</span></a> is an added advantage for its application in the paediatric population.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Even though the first retina OCT was taken in 1989 by David Huang in the James Fujimoto lab, the first available OCT, named OCT 100, reached the market only in 1996.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a> At the time, this technology was criticized because it was very sensitive to small ocular movements and blinking. Accordingly, in order to minimize the image capture periods, spectral domain OCT (SD-OCT) was developed in 2001.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a> These new devices enabled extremely fast image captures.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a> Since then, image processing time has diminished while quality has increased.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The search for normal parameters for the paediatric population has always been a crucial factor to interpret various tests. It is well known that reference values for the adult population are not applicable in the paediatric domain. In fact, paediatric practices generally utilize antropometric graphs for following up healthy children and assessing their growth. The main objective is to determine the prevalence of abnormal antropometric measurements because the position of extreme percentiles is highly valuable.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">3</span></a> The most referenced graphs in Spain were drawn from the Orbegozo Foundation study<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">3</span></a> carried out with a sample of Spanish children. In addition, hemogram paediatric reference values are different to those of adults and vary according to age and sex of patients.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">4</span></a> Reference ranges are those which include 95% of the «normal» population, i.e., the mean of the population<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2 standard deviations.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">4</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Accordingly, since the paediatric specialty operates as described above, paediatric ophthalmology should also review the reference values utilized with OCT and the normative or reference databases should be preferably populated with the Spanish paediatric population.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Development</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Applications of optical coherence tomography in paediatric ophthalmology</span><p id="par0030" class="elsevierStylePara elsevierViewall">The literature comprises studies that utilized OCT for diagnosing and following up diseases such as paediatric glaucoma,<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">5,6</span></a> optic pathway gliomas,<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">7</span></a> non-glaucomatous optic neuropathy,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">8</span></a> amblyopia<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">9</span></a> and retinoblastoma.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">10</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Both in glaucomatous and non-glaucomatous optic neuropathies as well as in neurological diseases, OCT is becoming the objective test of choice for the paediatric population due to the fact that visual fields can provide reliable tests only in selected cases and in relatively older children. For instance, the peripapillary nerve fibre layer thickness (pRNFL) can be utilized for following up patients with papiledema. Studies involving an adult population have found a correlation between the loss of pRNFL and visual field loss.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">11</span></a> A correlation between increased pRNFL thickness with intracranial pressure increases in the case of idiopathic intracranial hypertension in adults has also been found.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">12</span></a> At present, ganglion cell complex (GCC) OCT is being utilized as early neuronal damage indicator in idiopathic intracranial hypertension.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">13</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">In addition, due to the quality of obtained images and the large amount of information it provides about the macular structure, OCT is an essential tool for diagnosing and managing retinal diseases such as those secondary to uveitis and retinoschisis linked to chromosome X among others because it allows a relatively fast assessment of the macula. In addition, this allows highly reliable follow-up and monitoring of response to treatment.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In what concerns papillary OCT images, these have become a genuinely useful and fast tool to differentiate papiledema from pseudo-papiledema in cases such as optic nerve drusen,<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">14–16</span></a> which are increasingly diagnosed in paediatric age. Said image quality has improved even more with spectral domain OCT that includes the enhanced depth imaging technology.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">17</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Definition and importance of paediatric normative databases</span><p id="par0050" class="elsevierStylePara elsevierViewall">Even though image capture and quality has substantially improved in recent years together with developments in devices, segmentation systems and software, very few changes have been introduced in reference databases.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> At this point in time there is no OCT device in the market that includes a normative database for the paediatric population. This involves a significant limitation in its usefulness and above all the interpretation of its images for this age group.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> All the normative databases included in OCT devices presently found in the market are based on patients aged 18 or more.<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">19,20</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The main objective of reference databases is to enable a comparison of obtained parameters within normality in order to produce a colour-coded report that facilitates diagnosis.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a> This also enables following up the changes that could appear in time.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a> Generally, OCT devices assign the green colour to the thickness values between 5 and 95%, utilizing these values as the reference for normality.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">21</span></a> This limitation means that, even though we can obtain a thickness map in OCT of children, the colour codes shown in the analysis reports will be allocated comparing the results obtained against the adult normative database.</p><p id="par0060" class="elsevierStylePara elsevierViewall">In this situation, the first question is whether the reference values of adults and children are similar. Bibliography includes studies indicating that this is not so and that in fact the opposite is true.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> In what concerns macular OCT, several authors have concluded that macular thickness in adults is greater in some areas.<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">19,22</span></a> In addition, pRNFL is larger in children according to some authors,<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">19,23</span></a> although others have found it very similar results.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">24</span></a> Bhoiwala et al. concluded that pRNFL and GCC thickness is larger in children than in adults in the 55–60 age bracket and that nerves exhibit smaller cups in children.<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">25</span></a> A study carried out by the authors’ research group with Spanish children between 5 and 18 years old with OCT Topcon 3D-2000 (Topcon Corporation, Tokyo, Japan) for analyzing GCC found that if said OCT included a reference database for children, more abnormally low GCC thickness cases would be identified.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> Totan et al. also found larger pRNFL and GCC in children than in adults.<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">26</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Accordingly, the thickness maps shown at present by OCT were not validated for ophthalmological examinations of patients under 18 years of age.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Normative databases for optical coherence tomography for the Spanish paediatric population</span><p id="par0070" class="elsevierStylePara elsevierViewall">Reference is made below to published studies on Spanish healthy paediatric populations and different OCT models. These are the databases that should be utilized in our daily clinic practice. In the absence of said data, it would be recommendable to apply reference databases based on the European paediatric population due to the fact that the literature has not yet arrived at a clear consensus about whether the origin of a patient could influence the thickness readings taken with OCT.<a class="elsevierStyleCrossRefs" href="#bib0320"><span class="elsevierStyleSup">23,27–32</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">On the other hand, it is important to emphasize that normative databases are not exchangeable between different OCT devices<a class="elsevierStyleCrossRefs" href="#bib0370"><span class="elsevierStyleSup">33,34</span></a> because each utilizes different segmentation algorithms in addition to the fact that the analyzed retina surface is not the same for all devices.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a><ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">1.</span><p id="par0080" class="elsevierStylePara elsevierViewall">Cirrus OCT (Carl Zeiss Meditec).</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">a.</span><p id="par0085" class="elsevierStylePara elsevierViewall">pRNFL thickness.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">•</span><p id="par0090" class="elsevierStylePara elsevierViewall">Study by Barrio-Barrio et al. on a sample of 301 Spanish children aged 4–17 (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> Said authors found a positive correlation between pRNFL thickness and spherical equivalent (SE).<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> The inferior quadrant was the thickest and the temporal quadrant was the thinnest.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> No thickness differences were found on the basis of age or sex.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">•</span><p id="par0095" class="elsevierStylePara elsevierViewall">Study by Elía et al. on a sample of 344 Spanish children aged 6–13 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">24</span></a> Said authors found that the thickest quadrant was the inferior, followed by the superior, nasal and temporal (ISNT), a pattern which had been reported in previous studies.<a class="elsevierStyleCrossRefs" href="#bib0380"><span class="elsevierStyleSup">35–37</span></a> No thickness differences were found on the basis of age or sex.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">24</span></a></p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">b.</span><p id="par0100" class="elsevierStylePara elsevierViewall">Macular thickness. Study by Barrio-Barrio et al. (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> Said authors found that the central area was the thinnest and the inner ring was thicker than the outer ring.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> The temporal areas were thinner than the nasal areas in both rings.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> Boys had a larger central thickness than girls.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> A positive correlation was found between age and average thickness and macular volume.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a> In addition, the above 2 parameters demonstrated a positive correlation with SE.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">19</span></a></p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">2.</span><p id="par0105" class="elsevierStylePara elsevierViewall">OCT Topcon 3D-2000. A study carried out by the authors’ research group on a sample of 126 children aged 5–18 for measuring the thickness of the ganglion cell complex with OCT (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> A positive correlation was found between the macular retinal nerve fibre layer thickness and age, as well as a positive correlation between the ganglion cell layer-inner plexiform layer thickness and SE.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> The lower quadrant of macular retinal nerve fibres was thicker than the upper quadrant.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> This was the first study to research what would happen if OCT included a paediatric database and if differences could be found in thickness classifications (percentile of 5 or lower is taken as abnormal and figures above this threshold are taken as normal) in the patients included in the study. It was concluded that, despite a considerably high match between both databases, there is a risk of not detecting abnormal measurements when utilizing the adult database.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> The software included in the adult database classified only one of the 126 eyes with abnormal ganglion cell layer-inner plexiform layer (0.85%), while the paediatric database would have classified 7 eyes with abnormal values (5.5%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.031), which means that the sensitivity was of one out of 7 (14%, 0.4–57).<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a></p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">3.</span><p id="par0110" class="elsevierStylePara elsevierViewall">Spectralis (Heidelberg Engineering, Heidelberg, Germany). Study by Pérez-García et al. on a sample of 162 children age 3–14 measuring macular and pRNFL thicknesses (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a> Said authors found a positive correlation between SE and the nasal and inferior quadrants and the inferonasal sector of pRNFL.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a> The pattern found for pRNFL was again ISNT, with small differences between the nasal and temporal quadrants.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a> A positive correlation was found between SE and the nasal, inferior and inferonasal pRNFL thickness.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a> In what concerns macular thickness, a positive correlation was found between age and macular volume, foveolar thickness and central macular thickness.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a> In this study the temporal sectors were also found to be thinner than nasal sectors and the inner ring thicker than the outer ring.<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">39</span></a></p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></li></ul></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Normative databases for optical coherence tomography in the European paediatric population</span><p id="par0115" class="elsevierStylePara elsevierViewall">The literature comprises 3 studies on children of Sweden, 2 on macular thickness<a class="elsevierStyleCrossRefs" href="#bib0370"><span class="elsevierStyleSup">33,40</span></a> (the first with children 5–6 years of age with OCT 3 Stratus, Carl Zeiss, version 4.0.1; and the second with children aged 6–15 with OCT Cirrus version 6.0.2.81 and Stratus OCT 3 version 4.0.1, both by Carl Zeiss) and one pRNFL study with children aged 5–16 utilizing Stratus OCT 3 version 4.0.1 by Carl Zeiss and HRT-II/III by Heidelberg.<a class="elsevierStyleCrossRef" href="#bib0375"><span class="elsevierStyleSup">34</span></a> In addition, there is a study on Portuguese children aged 4–17 for macular thickness and pRNFL utilizing OCT Cirrus HD, model 4000 by Carl Zeiss.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Once again, the foveolar area of macular thickness was the thinnest while the inner ring was the thickest.<a class="elsevierStyleCrossRefs" href="#bib0370"><span class="elsevierStyleSup">33,40,41</span></a> The temporal areas were thinner than the nasal areas.<a class="elsevierStyleCrossRefs" href="#bib0370"><span class="elsevierStyleSup">33,40,41</span></a>. According to Queirós et al.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a> the fovea was significantly thicker in boys than in girls although Eriksson et al. found the same trend but without statistical significance.<a class="elsevierStyleCrossRef" href="#bib0405"><span class="elsevierStyleSup">40</span></a> Molnar et al. did not find a correlation between macular thicknesses and SE,<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">33</span></a> whereas Queirós et al. found a negative correlation between SE and foveolar thickness.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a> As regards age, Eriksson et al. did not find a correlation,<a class="elsevierStyleCrossRef" href="#bib0405"><span class="elsevierStyleSup">40</span></a> However, the Molnar et al.<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">33</span></a> group as well as that of Queirós et al.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a> found respectively a positive correlation between inner ring thickness and age<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">33</span></a> and between foveolar thickness and age.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">As regards pRNFL thickness, Queirós et al. and Larsson et al. also found the ISNT pattern although no statistically significant differences were found between the lower and upper quadrant.<a class="elsevierStyleCrossRefs" href="#bib0375"><span class="elsevierStyleSup">34,41</span></a> Both the Swedish and Portuguese studies did not find a correlation between pRNFL and age or sex.<a class="elsevierStyleCrossRefs" href="#bib0375"><span class="elsevierStyleSup">34,41</span></a> Queirós et al. described a positive correlation between SE and overall thickness of pRNFL and that of the upper and lower quadrant.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a></p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusion</span><p id="par0130" class="elsevierStylePara elsevierViewall">At present, for adequate utilization of OCT in paediatric ophthalmology it is essential to know and manage normative databases comprising paediatric populations because this can have a significant impact on the decisions taken in daily clinic practice. In addition, it must be borne in mind that an OCT on its own is not sufficient because it is crucial to take into account clinic records, anamnesis and patient examination (in all cases taking into account the SE, age and sex because these factors could alter thickness in different parameters). However, the use of the adult databases currently included in all devices could take some values as normal when actually they would not be so in a paediatric database. For this reason it would be of great interest to have said data in the OCT devices being utilized at present.
