It is estimated that 50 % of patients with POAG are not aware of having said disease until reaching the advanced phases.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> In fact, the absence of symptoms hinders early detection, with patients seeking medical attention at a later phase and thus increasing the probability of optic nerve damage involving visual field impairment.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> These circumstances hinder therapeutic approaches which generally endeavor to control intraocular pressure and arrest the progression of the disease.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> In this context, patient involvement and commitment as well as treatment compliance are crucial factors.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">An important element in the comprehensive management of POAG is the knowledge of patients about their disease as this could be associated to the degree of compliance with treatment. Some studies point out that greater knowledge about the disease increases motivation and interest in complying with therapeutic prescriptions, translating into improved adherence.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10–12</span></a> However, in contrast with other countries,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> said level of knowledge has not been determined in Peru. In addition, it is also important to identify which aspects of the disease are less known by patients.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11,13</span></a> This would help health professionals to improve the comprehensive approach required by these patients,<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,14</span></a> even more so in a health system with limited resources such as that of Peru (SSP).<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Accordingly, valid and reliable instruments are required to assess the level of knowledge of POAG patients about their disease in the SSP.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Despite the availability of scales for assessing the level of knowledge of patients with glaucoma, the authors have not found validated instruments for the Spanish-speaking Peruvian population. Most instruments have been developed for English speaking populations,<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–18</span></a> those utilized in Spanish involve deficiencies in the validation process, have been developed for the Cuban population.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,9</span></a> For these reasons, the objective of this study was to describe the preparation, validation of a validation tool in the Spanish language to measure the level of knowledge of patients with POAG diagnostic receiving attention at the SSP. It is considered that said instruments will enable an objective assessment of the knowledge of said patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Identification of previous instruments</span><p id="par0025" class="elsevierStylePara elsevierViewall">A search was carried out in Medline (through PubMed), the Virtual Health Library (<span class="elsevierStyleItalic">Biblioteca Virtual de Salud</span> - BVS), Scielo and EMBASE in order to identify instruments for measuring the knowledge of POAG patients about their disease. The approach proposed by DiCenso et al.,<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> was followed and the present study took summary studies (systematic reviews) as the highest hierarchy level of previously assessed evidence.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Preparation of the instrument</span><p id="par0030" class="elsevierStylePara elsevierViewall">A panel of experts was established comprising 5 ophthalmologists with subspecialty in glaucoma of the <span class="elsevierStyleItalic">Dr. Francisco Contreras Campos National Ophthalmology Institute</span> of Peru (INO). Initially, the subspecialists selected questionnaires on the basis of the articles identified in the search (which were translated to Spanish), some of which were adapted in order to facilitate the understanding of patients. Subsequently, a database of questions was developed comprising those translated from the 4 questionnaires as well as additional questions added by the panel members on the basis of their experience. Once this was completed, the panel studied the domains of the questionnaire and selected a short list of questions based on relevance, potential adaptation to the context of the country and the possibility of inclusion in one of the predefined domains.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Pilot test</span><p id="par0035" class="elsevierStylePara elsevierViewall">A transversal pilot test was designed in order to evaluate the understanding by the population of the selected questions and select the definitive list of questions. This phase included the participation of 60 patients with POAG diagnostic that were receiving outpatient care in the INO glaucoma Dept.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Weighting</span><p id="par0040" class="elsevierStylePara elsevierViewall">The entire list of items was distributed in 3 domains as established by the subspecialists doctors. On the basis of the nature of each question, the expert panel established a weighted value of said 3 domains which added up should reach 100 %. Taking the mean value of the proposed weights, the score of each domain was established.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Administration of the questionnaire</span><p id="par0045" class="elsevierStylePara elsevierViewall">The instrument was applied between June and December 2017 at the INO, a highly complex national reference Center in Lima (Peru). This study included adult patients diagnosed with POAG by an ophthalmologist with subspecialization in glaucoma and were followed up at the INO glaucoma Dept. during at least one year since diagnostic, with best corrected visual acuity above 20/200 and with Spanish as mother tongue. The study excluded patients having any disease that could impair visual acuity such as diabetic macular edema, proliferative diabetic retinopathy, age-related macular degeneration, cognitive impairment or surgical intervention due to glaucoma. Sample size was estimated at 20 patients for each initial item and, as the questionnaire comprised 14 questions, at least 280 subjects were needed.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Two trained health professionals (the treating physician and nurses) administered the questionnaire. Mean application time was 5<span class="elsevierStyleHsp" style=""></span>min. The patients were selected from the daily schedule of the outpatient medical practice of the Glaucoma Dept.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Description and reference scale</span><p id="par0055" class="elsevierStylePara elsevierViewall">A database was generated in Microsoft Excel® (Microsoft Corporation, CA, USA) based on physical patient records. The proportion of subjects with adequate replies for each item was described. In addition, the overall scores for each patient were calculated on the basis of the weighted factor of each question. Normality was assessed by means of the Shapiro Wilk test and subsequently calculated using tertiles.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Ethical aspects</span><p id="par0060" class="elsevierStylePara elsevierViewall">The study protocol was approved by the INO Ethics Committee. The execution of the study observed the principles of the Helsinki declaration.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Patient signed an informed consent prior to the administration of the questionnaire.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Results</span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Searches and initial formulation of the questionnaire</span><p id="par0065" class="elsevierStylePara elsevierViewall">The search found a systematic review developed by Peralta et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> that reported instruments for measuring levels of knowledge in patients with glaucoma. In addition, an article by Rosdahl et al.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> reported searches for articles evidencing the validity and reliability of scales for measuring knowledge of glaucoma. On the basis of said articles, the expert panel selected 4 baseline questionnaires published by the National Eye Health Education Program<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> comprising 10 questions, the National Society for the Prevention of Blindness of the United States (Glaucoma Q test by <span class="elsevierStyleItalic">Prevent Blindness America</span>)<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> comprising 9 questions, by Hoovenars et al.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> with 37 questions and by Gray et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> with 11 questions. These surveys were translated from English to Spanish, and 5 additional items proposed by the panel of subspecialists physicians were added.</p><p id="par0070" class="elsevierStylePara elsevierViewall">At this stage, the questionnaire comprised 72 initial questions on the basis of which the expert panel developed an initial instrument in Spanish comprising 20 items. Inclusion was defined by simple majority for a binary response (include/exclude). A draw was impossible due to the fact that the panel comprised 5 experts. Subsequently, 6 questions were excluded on the basis of the pilot study, ending up with 14 items in the final version of the questionnaire (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Questionnaire structure, weighting and score</span><p id="par0075" class="elsevierStylePara elsevierViewall">The 14 items of the questionnaire were distributed in 3 domains: a) risk factor, 2 items; b) clinic aspects, 6 items and c) treatment, 6 items (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the weighted values allocated to each domain according to the criteria of the expert panel (definitive average percentages: risk factors 10 %, clinic aspects 36 % and treatment 54 %) and the weighting of factors for each domain.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Description and calculations</span><p id="par0080" class="elsevierStylePara elsevierViewall">The study included 314 patients with a mean age of 66.0 years (SD: 13.7 years), 56.7 % were females and 17.5 % had higher education (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The arithmetic mean of the overall scores produced by the questionnaire was 62.6 (SD: 14.1) with minimum and maximum values of 22 and 100 points, respectively. A normal distribution of said scores was observed with p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.612 in the Shapiro Wilk test.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">In what concerns the frequency of correct responses, it was observed that item Q10 on loss of vision due to glaucoma had the highest frequency (97.8 %), whereas item Q6, that explored ocular pressure, exhibited the lowest frequency of correct responses with only 15.3 % (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">A tertile calculation was developed to produce 3 levels of knowledge: a) low (from 0 to 55 points), b) middle (from 56 to 69 points) and c) high (from 70 to 100 points).</p></span></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">A new questionnaire in the Spanish language was developed evidencing validity to measure the level of knowledge about glaucoma in patients with said disease. Said questionnaire comprises 14 items distributed in 3 domains, i.e., risk factors, clinic aspects and treatment. Subsequently, on the basis of weight factors, an overall score is obtained which classifies the level of knowledge in 3 categories according to the tertile calculation. In the first application, said questionnaire evidenced the existence of critical and specific knowledge that must be strengthened due to the poor proportion of correct responses despite the fact that these results are preliminary. The authors consider that the instrument can be utilized successfully in outpatient medical practices for glaucoma patients.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The literature describes that the knowledge of a disease facilitates improved treatment compliance by patients and regular checkups related to the disease.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10,16,17</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> In the case of glaucoma, said knowledge is particularly important because the frequency of compliance problems is high, ranging between 10 and 80 %.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">25,26</span></a> In this regard, the present questionnaire is a tool that could indirectly contribute to improve compliance and become an additional element for the comprehensive management of patients with POAG in the framework of outpatient clinic practice. Awareness of patient knowledge about the disease will enable the identification of topics that should be reinforced by the healthcare team. This implies that, in what concerns improving treatment compliance and accordingly improving control of the disease, the results produced by the present instrument could be used as a basis for developing personalized management approaches in the framework of a follow-up and control program.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The present study exhibits several limitations. In the first place, the patients included in the study are cared by a specialized institute of the Health Ministry, which is not a representative sample of the entire SSP. This could limit the external validity, mainly due to the differences in the education level of Social Security patients in comparison to the private subsector. However, the INO is a national reference Center that cares for patients from different regions of Peru. Secondly, an initial qualitative approach was not considered even though it would have enabled the inclusion of the viewpoints of patients. However, as the questionnaire was aimed at assessing patient knowledge, the potential negative effect is smaller than if it explored attitudes and practices. Thirdly, statistical reliability with reproducibility strategies such as test-retest of sensitivity to change was not assessed. Even so, the average score in the pilot and definitive tests was similar for all 14 items.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Beyond the above limitations, the present questionnaire is novel and relevant. A bibliographic search produced instruments in the English language<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–18,23</span></a> but a validated instrument in the Spanish language could not be found. It is important to point out that, even though Triana et al. assessed the knowledge about glaucoma among patients in Cuba,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> it was based on a simple translation to Spanish of the NEHEP survey.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> In addition, the present instrument is the first validated tool by a health services institution of the SSP, a context in which medical care exhibits numerous limitations as well as a high degree of administrative complexity.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Accordingly, the authors consider that the present survey will contribute to a comprehensive assessment required by an outpatient with POAG, even more so in a system with scarce resources such as that of Peru, and will also support clinic decision-making with a significant repercussions in the area of public health and health services management.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,24,27,28</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">The authors recommend the development of new research to assess the levels of knowledge in patients diagnosed with POAG in different Spanish-speaking contexts and, specifically for the SSP, it is proposed to develop said research in representative samples including patients of the Social Security, of the armed and police forces as well as the private subsector. It is also proposed to explore factors associated to the level of knowledge which could be used as a basis for studying interventions. In addition, it would be relevant to carry out research validating scales related to attitudes and practices in patients with POAG in relation to their disease because these have a direct repercussion on the response to said disease and treatment compliance.</p><p id="par0120" class="elsevierStylePara elsevierViewall">In view of the above, the authors conclude that this instrument includes evidence of validity to assess the level of knowledge about POAG in Spanish-speaking Peruvian patients with said disease.</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span
