These IOLs are known as: multifocal lenses, accommodative, trifocal, bifocal and extended depth of focus (EDOF) multifocal IOLs.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Nonetheless, the Spanish public health system provides monofocal IOLs that correct far vision. Currently, most monofocal IOLs have an aspherical design to partially or completely compensate for the spherical aberration induced by the cornea, thereby improving the optical quality of the visual system.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6</span></a> On the other hand, a new monofocal IOL with a different design has become available, namely, the Eyhance IOL with EDOF. This new feature of monofocal IOLs allows them to provide a fuller range of clear vision without the need for multifocal IOLs. Numerous studies have been conducted on multifocal IOLs and diffractive EDOF IOLs,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,7,8</span></a> but this is not the case for EDOF monofocal IOLs.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In our setting, cataract surgery is the most common surgical intervention in older people. Having an EDOF monofocal IOL that could be used in the public health system would allow us to improve not only far vision of patients undergoing cataract surgery, but also their intermediate vision. This implies enhancing not only their visual acuity (VA), but also their quality of life.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The purpose of this study is to analyse whether an EDOF monofocal IOL (TECNIS Eyhance) provides better intermediate VA than another monofocal IOL (TECNIS 1-piece) from the same platform.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">We conducted a randomised prospective observational study. It was carried out in accordance with best practice guidelines and the principles of the Declaration of Helsinki and was approved by the Basque Ethics Committee on drug research (CEIm-E; PS2019021). Patients who were on the waiting lists of the Ophthalmology Department at Cruces University Hospital for bilateral cataract surgery were included. These patients were provided with information about the study and asked to give written informed consent to receive either TECNIS 1-piece or Eyhance monofocal IOLs. After, the patients were randomly assigned to each group using the SPSS vs 23.0 program. The specific characteristics of these IOLs are described in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0025" class="elsevierStylePara elsevierViewall">The exclusion criteria were: corneal astigmatism greater than 1.5 dioptres, previous corneal or retinal disease, previous ocular surgery and amblyopia. Both the examination prior to inclusion on the waiting list for surgery and postoperative treatment were carried out following the protocol established in the Ophthalmology Department at our hospital.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Corneal aberration parameters, root mean square high-order aberration (RMS-HOA) and spherical aberration (Z<span class="elsevierStyleInf">4</span><span class="elsevierStyleSup">0</span>) for a diameter of 6 mm, and pupil size were assessed in the examination before the cataract surgery. These parameters were measured with a Pentacam high-resolution camera system (Oculus; Optikgeräte, Wetzlar, Germany). All the tomographic images to be included in the study, we required that they were of adequate reliability, that is, the test had to obtain an “OK” rating by the Pentacam HR system.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The other measurements, namely, for the far and intermediate VA and defocus curve, were taken at 1 month after surgery.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The biometry measurements were made by the same evaluator (BLE) with the IOLMaster optical biometer (Carl Zeiss Meditec AG) using the SRK/T equation with constant A of 119.3 for both IOLs. The IOL power was selected with emmetropia as the target outcome.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Surgical technique</span><p id="par0045" class="elsevierStylePara elsevierViewall">All the surgical procedures were carried out by the same surgeon (NMA) with topical anaesthesia and sedation using tropicamide 1.0% and phenylephrine 10% to achieve mydriasis. A temporal surgical approach was used for both right and left eyes. A clear corneal incision of 2.4 mm was made in all patients.</p><p id="par0050" class="elsevierStylePara elsevierViewall">All patients received the same postoperative treatment with tapered doses of tobramycin 0.3% and dexamethasone 0.1% until 1-month after surgery.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Postoperative examinations</span><p id="par0055" class="elsevierStylePara elsevierViewall">At 1 month after surgery in the second eye, the tests for this study were carried out. The best far monocular and binocular VA was measured using the Early Treatment Diabetic Retinopathy Study (ETDRS) chart for testing at 4 m under photopic conditions (85 cd/m<span class="elsevierStyleSup">2</span>). In addition, the best monocular and binocular intermediate (66 cm) VA was measured using the ETDRS chart for testing at this distance.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The primary objective of the study was to assess the intermediate VA, using a binocular defocus curve ranging between −2.50 D and +0.50 D in 0.50 D steps. The defocus curve was obtained considering the best-corrected far VA under photopic conditions using the ETDRS chart as the reference refraction.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0065" class="elsevierStylePara elsevierViewall">Categorical variables were described using frequency tables and percentages and continuous variables using means and standard deviations.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Comparisons between groups were made with Mann-Whitney nonparametric tests for continuous variables and chi-squared tests for categorical variables.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The relationship between the continuous variables was assessed using the Spearman correlation, since the data were not normally distributed.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Statistical analysis was carried out with SPSS vs 23.0.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0085" class="elsevierStylePara elsevierViewall">We studied a total of 30 patients (60 eyes), 15 patients receiving TECNIS 1-piece and 15 patients Eyhance IOLs, with a total of 30 eyes in each group. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> summarises the preoperative characteristics of the study patients. The male-to-female ratio and Z<span class="elsevierStyleInf">4</span><span class="elsevierStyleSup">0</span> differed significantly between the groups.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">We did not find statistically difference in the postoperative subjective refraction between the 2 gropus. The SE was −030 ± 044 in the Eyhance group and -0.21 ± 0.47 in the TECNIS 1-piece group (p 0.205).</p><p id="par0095" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> summarises the VA results. Patients obtained a far VA of 0.00 logMAR both with the TECNIS 1-piece and the Eyhance lenses. On the other hand, for intermediate vision, the VA observed was better with the Eyhance EDOF monofocal IOL, the difference being statistically significant (p < 0.001).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> presents the binocular defocus curve obtained. As can be seen, for intermediate vision, the Eyhance IOL achieved better VA.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Regarding the correlation between the binocular intermediate VA and various parameters analysed (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), we only found a statistically significant correlation between RMS-HOA and Z<span class="elsevierStyleInf">4</span><span class="elsevierStyleSup">0</span> in the case of the Eyhance IOL.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">The Eyhance IOL is part of the same family of IOLs as the TECNIS 1-piece, that is, they have the same mechanical and behavioural properties when inserted into the capsular bag during cataract surgery. The main distinctive feature is that the power of the Eyhance TECNIS IOL gradually increases from the periphery towards the centre of the lens and this provides better intermediate VA.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In our public hospital setting, the IOLs used are always monofocal. The possibility of using an EDOF monofocal IOL would allow many patients to benefit from better intermediate vision.</p><p id="par0120" class="elsevierStylePara elsevierViewall">This is one of the first studies that analyses intermediate vision with an Eyhance IOL. Mencucci et al<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> obtein VA results similar to ours in intermediate vision (0.27 logMAR with Eyhance ICB00 and 0.37 logMAR with ZCB00). Our findings demonstrated that with an EDOF monofocal IOL, we were able to improve intermediate vision (VA of logMAR 0.37 with the TECNIS 1-piece monofocal IOL and 0.16 with the Eyhance IOL) without a negative impact on far vision (VA of logMAR −0.06 with the TECNIS 1-piece monofocal IOL and −0.05 with the Eyhance IOL).</p><p id="par0125" class="elsevierStylePara elsevierViewall">Other studies comparing diffractive EDOF IOLs have obtained similar findings to ours for intermediate and far vision. On the other hand, compared to multifocal IOLs, there are differences in intermediate and far vision, outcomes being worse than with EDOF IOLs.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,10</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Bellucci et al<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> compare variations on the same design of hydrophilic IOL: a monofocal IOL and a progressive aspheric IOL with EDOF. In that study, the EDOF IOL achieved better VA for intermediate but not for far distances, for which VA was better with the monofocal than with the EDOF IOL.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Various different studies have determined that depth of focus varies as a function of spherical aberration.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a> Therefore, we assessed whether the effectiveness of Eyhance and TECNIS 1-piece IOLs varies at intermediate distances as a function of corneal aberration. The correlation found between RMS-HOA and Z<span class="elsevierStyleInf">4</span><span class="elsevierStyleSup">0</span> in the case of the Eyhance IOL suggests that, with intermediate values of these aberrometric parameters, the Eyhance IOL provides better intermediate VA. This seems to indicate that, in order to optimise visual outcomes, corneal aberration should be a criterion for selection of the Eyhance IOL in cataract surgery. Nonetheless, there is a need for studies based on larger samples to be able to establish the values of the aberrometric parameters that would ensure the best outcomes in terms of vision. We should also note that in our study RMS-HOA and Z<span class="elsevierStyleInf">4</span><span class="elsevierStyleSup">0</span> values varied significantly between the two groups, again indicating the need for further studies with a larger number of eyes to confirm our findings. Additionally, the corneal RMS-HOA and Z<span class="elsevierStyleInf">4</span><span class="elsevierStyleSup">0</span> values in both groups are somewhat higher than normal mean values.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a> This underlines the need for further studies to confirm whether our findings are generalizable.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Neither age nor pupil size were correlated with better intermediate VA. That is, our findings indicate that the Eyhance IOL was not dependent on pupil size, as would be expected of a monofocal IOL.</p><p id="par0145" class="elsevierStylePara elsevierViewall">To conclude, the new monofocal Eyhance EDOF IOL offers sufficient depth of focus to provide patients with comfortable intermediate vision without impairing far vision. Patients who required cataract surgery in both eyes were included. Patients in one group were implanted with Tecnis 1-piece IOL and those in the other group with Eyhance EDOF monofocal IOL. One month after surgery in the second eye, we assessed the corrected intermediate VA (66 cm) and obtained defocus curves. We also explored the correlation of: high-order and spherical aberrations with intermediate VA, pupil size with intermediate VA and age with intermediate VA.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">We analysed 30 patients (60 eyes), 15 patients in the TECNIS 1-piece group and 15 in the Eyhance group.</p><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">Both groups achieved a far VA of 0.00 logMAR. In contrast, for intermediate vision, VA was better with the Eyhance EDOF monofocal IOLs (0.16 ± 0.12 LogMAR), the difference being statistically significant (p < 0.001).
