(F-H) Case 3: 10-year-old girl with a painless erythematous purple nodule in the cheek together with repeating palpebral nodules, mixed blepharitis and diffuse keratitis.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Idiopathic Facial Aseptic Granuloma (IFAG) was described in the French literature ("pyodermite froide du visage") as chronic, painless facial nodules that occur only in childhood.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In the last 10 years, 9 publications reported the association of palpebral and facial nodules (24 cases) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–9</span></a> Three cases of palpebral nodules in IFAG togeter with a review of published cases of palpebral nodules in IFAG in the last 10 years are described.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Cases</span><p id="par0015" class="elsevierStylePara elsevierViewall">Case 1. 10-year-old girl, with marfanoid habit and history of repeating palpebral nodules in the 4 eyelids, treated with 2 excisional biopsies due to absence of clinical improvement with palpebral hygiene and topical fusidic acid. In the examination, the patient exhibited retraction of the right eye lower eyelid (lower scleral exposure 3 mm), superficial dotted keratitis in both eyes, hyperemia and telangiectasias of the palpebral edge. Also papillae and conjunctival follicles were seen, as well as scars from palpebral surgeries. The malar zones exhibited two painless subcutaneous nodules with inflammatory component having a larger diameter of 1 cm with 4 months of evolution. The rest of the ophthalmological exploration was normal. Due to the lack of response to topical treatment with clindamycin and 5% benzoyl peroxide, lasting 3 months, the patient was seen in maxillofacial surgery where both malar lesions were removed. Cultures of the malar nodules were negative. Histology showed a chronic inflammatory granulomatous infiltrate in the deep dermis, with formation of large granulomas consisting of a purulent center with abundant neutrophil polymorphonuclei, without identifying pathogens. In the periphery, a chronic inflammatory component rich in histiocytes, lymphocytes and plasma cells was observed, with the presence of some multinucleated giant cells of foreign body type, without calcium deposits or phantom cells, apparently unrelated to hair-sebaceous follicles; typical acne scars were not identified. It was described as an aseptic, non-caseous granuloma consisting of an outer crown of lymphocytes that limits a thick layer of epithelioid histiocytes with vascular proliferation and a clear center of cellular detritus compatible with IFAG. No specific pathogens or foreign bodies were identified. The patient was referred to Dermatology, where the clinical diagnosis was confirmed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A-D).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">After 12 months the patient has not developed new facial granulomas or palpebral granulomas.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Case 2. 3-year-old boy with history of recurrent palpebral nodules in both eyes, resolved without treatment. Rest of ophthalmological examination was normal. Facial nodule on left cheek painless, not abscessed. Treatment with metronidazole gel 7,5 mg was recommended twice a day. After 8 months, the patient presented resolution of the facial and palpebral nodule (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>E).</p><p id="par0030" class="elsevierStylePara elsevierViewall">Case 3. 10-year-old girl with repeating palpebral nodules in both eyes, mixed blepharitis and diffuse keratitis, resolved without ophthalmological treatment. Rest of ophthalmological examination was normal. The cheek showed painless inflammatory nodule depressible on palpation and of large size. Metronidazole 0.75% topical was recommended twice a day for 4 months, with good response (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>F-H).</p><p id="par0035" class="elsevierStylePara elsevierViewall">The 3 cases were healthy, with no history of facial trauma or systemic infection.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0040" class="elsevierStylePara elsevierViewall">IFAG is a newly recognized entity. It is defined as painless facial nodules and usually present on the cheekbones.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Less frequent is the association with periocular nodules (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). In the present cases, both upper and lower eyelids were affected and facial granulomas were found in the malar area.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In this series, the age of presentation was 3 years in one patient, and 10 years in the other,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> a little later than reported in the literature (between 15 months and 4 years). We believe this difference may be due to the delay in diagnosis. Two girls and one boy were affected, a relationship similar to that found in the References (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The duration of the granulomas is described from 1 to 18 months (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), a similar period of time as our patients.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,7,8</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The etiopathogenesis of the IFAG is unknown, although different theories are suggested. The most recent is that IFAG is included in the spectrum of childhood rosacea,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,10</span></a> although other authors consider it to be an entity characteristic enough to be considered a different disease.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Other causative theories are the reaction to sting or mild trauma and the granulomatous response to unknown embryological remnants.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> We did not detect any of the above causes in our cases; only case 1 exhibited a marfan habit without a genetic Marfan diagnosis.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The diagnosis of IFAG is mainly clinical and exclusionary.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> The presence of one or more asymptomatic and aseptic nodules located in the cheeks, of at least one month of evolution and which resolve spontaneously in several monthsIt have been proposed as a clinical diagnostic criterion.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Biopsy is not necessary taking into account the clinical diagnosis, the anatomical location, the benign nature of the condition and the age of the patients.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> The performance of a biopsy in a young child often requires sedation and, since it is located in very visible areas of the face, unsightly scars may remain after surgery. This was the case in our case 1, in which the lesions were excised prematurely.</p><p id="par0065" class="elsevierStylePara elsevierViewall">For these reasons, histological examination was not performed in many of the published cases (Table 1).<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,6</span></a> When affected tissue is obtained, a nonspecific inflammatory granuloma similar to that of a foreign body or secondary to mycobacteria is seen, located in the superficial and deep dermis. It has a ring of lymphocytes, histiocytes, neutrophils and eosinophils. No characteristics of pilomatrixoma, nor findings of infectious disease.</p><p id="par0070" class="elsevierStylePara elsevierViewall">IFAG ultrasound could contribute to the diagnosis. A hypoecogenic oval lesion, well defined in the dermis, with subsequent hyperecogenicity without calcifications would be observed.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> Even so, the diagnosis continues to be eminently clinical and only in very doubtful cases the support of the techniques referred to becomes necessary, as in some cases in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><p id="par0075" class="elsevierStylePara elsevierViewall">IFAG palpebral nodules tend to persist for an average of one year, with spontaneous resolution, without leaving significant scars and without recurrence, as in cases 2 and 33. For this reason, expectant management can be considered as an appropriate option8. The most frequent is topical treatment with metronidazole, clindamycin or ivermectin,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> as in our patients 2 and 3. Clarithromycin (15 mg/kg, twice a day) or metronidazole (20 mg/kg/day) by mouth may shorten that recovery, as shown by the published cases in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Amoxicillin-clavulanic, azithromycin, erythromycin and cephalexin have proved ineffective9. Surgical treatment should be reserved for specific situations, and its convenience should be well evaluated due to the resulting unsightly scars in areas of great visual impact, as in our case 1.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Differential diagnosis<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> is necessary with chalazion, since we can find IFAG only as palpebral nodules, without facial involvement. IFAG palpebral granulomas are more superficial (dermis) and therefore tend to be reddish nodules, without acute inflammation and therefore without pain, differentiating them from sties/chalazions. The chalazion is seen in histology as a deep localized granuloma close to the tarsal plate and with optically empty spaces of lipid material.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> In addition, it can appear at the back of the tarsal plate, observed by conjunctival route. Palpebral tumor diseases that can simulate IFAG include pilomatrixoma, epidermal inclusion cyst, pyogenic granuloma and other vascular tumors, Spitz's nevus and xanthogranuloma.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">From the dermatological point of view, we must rule out infantile acne cystic nodule; the acneiform context with the presence of other elementary lesions, such as pimples or inflammatory papules, is usually definitive. The differential diagnosis with infectious disease should include bacterial abscesses, cutaneous leishmaniasis, mycobacterial infection and fungal infections (sporotrichosis, cryptococcosis, coccidioidomycosis).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">A careful history and clinical examination is usually sufficient to diagnose IFAG.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,8</span></a> When necessary, a skin biopsy, culture or other diagnostic techniques will rule out or confirm the tumor or infectious etiology.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Conclusions</span><p id="par0095" class="elsevierStylePara elsevierViewall">Asymptomatic, aseptic subepidermal palpebral or facial nodules, lasting at least one month and resolving spontaneously over the months, should alert us about IFAG3. Different medical treatments have been tried that seem to accelerate its healing, although its tendency is spontaneous resolution within several months. Painless recurrent eyelid nodules can be confused with chalazion.
