The lower sector defect in the nerve fiber layer of the retina has been marked with a yellow line in both images.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Fiber photography has traditionally been used to detect retinal nerve fiber layer (RNFL) defects in glaucoma patients.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This technique involves a technician with experience in acquiring this type of image and also requires a camera that incorporates an excitation filter at 495<span class="elsevierStyleHsp" style=""></span>nm to highlight the reflectance of the nerve fibers and minimize those coming from the deeper structures of the retina. These factors mean that this technique continues to be used in some centers, but is in some places difficult to implement in daily clinical practice due to the lack of adequate means.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Recently, the SPECTRALIS® Optical Coherence Tomography (OCT) (Heidelberg Engineering GmbH) has incorporated the MultiColor module that, using simultaneously 3 different wavelengths (infrared for the deepest structures, green and blue for the superficial ones) is capable of providing several images in which the retinal structures are identified according to depth. The information contained in these images is combined in a single image called "MultiColor", which includes structural information of the different retinal layers, visible due to their different reflectance.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Material and method</span><p id="par0015" class="elsevierStylePara elsevierViewall">The objective was to evaluate inter- and intra-observer matching for defect detection in RNFL using MultiColor <span class="elsevierStyleItalic">software</span> and traditional fiber photography. We randomly selected 20 eyes from 20 patients with primary mild open angle glaucoma (POAG) (as classified by Hodapp-Parrish-Anderson),<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> and 20 eyes from 20 control subjects (mostly from working staff at our center). After a complete ophthalmological examination, which included a visual field, MultiColor images were acquired from all participants using the wide-field non-contact lens (55°) provided by the SPECTRALIS® OCT; as well as a conventional RNFL photograph using a Canon CF-60D® Si retinogram (Canon Inc, Tokyo, Japan) connected to a Canon EOS 1Ds Mark III® camera. All purchases were made by the same expert technician (BC), after pupil dilatation (including MultiColor). Subjects were excluded if they exhibited any other ocular or systemic disease that could affect the fundus or a significant media opacity evaluated in the previous biomicroscopy. The descriptive characteristics of the sample for both groups are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">The images were evaluated by 2 ophthalmologists specialized in glaucoma (NG and BF), who did not know the diagnosis or any other data of the analyzed images. Their objective was to classify the images as "normal" or "pathological" based on the presence or absence of defects in the RNFL (either clefts or defects in the RNFL sector) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Images that could not be classified due to the presence of any artifact or poor image quality were labeled unclassifiable. Subsequently, inter- and intra-observer match in the evaluation of images was analyzed using the kappa coefficient (ĸ) with the Statistical Package for Social Sciences (SPSS® 20.0, SPSS Inc., Chicago, IL, USA). A match level was established based on the result of ĸ as: very good (ĸ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.81–1), good (ĸ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.61−0.80), or moderate (ĸ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.41−0.60).<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">No statistically significant differences were obtained between the two groups in any of the descriptive statistics analyzed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Interobserver match in the interpretation of the MultiColor images was good (ĸ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.746; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001); and intra-observer match between the MultiColor images and conventional RNFL photographs was moderate (Observer 1: ĸ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.433, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.014; Observer 2: ĸ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.573, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.011). Observer 1 identified 70% of the eyes with mild POAG, with 5% of unclassifiable images, and Observer 2 identified 65% of the eyes with mild POAG, with 13% of unclassifiable images, with MultiColor. The percentage of false positives (images classified as "pathological" but belonging to healthy subjects) with the MultiColor software was 25% for observer 1; and 20% for observer 2.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Discussion</span><p id="par0030" class="elsevierStylePara elsevierViewall">Classic fiber photography has traditionally been used to document incipient defects in the RNFL. However, this technique has some important drawbacks: it necessarily requires pupil dilatation, transparent optical media, close patient cooperation and an expert technician in the acquisition of this type of images. The color or redfree filter images obtained with some modern retinal scanners do not require pupil dilatation and are easy to perform; however, they have poor contrast and often little definition compared to the images obtained with the SPECTRALIS®3 MultiColor software.</p><p id="par0035" class="elsevierStylePara elsevierViewall">After evaluating our results and taking into account the previous experience we have in our service in interpreting fiber photographs, we consider the new OCT SPECTRALIS® MultiColor software to be a useful tool for identifying even incipient defects in RNFL. This application uses a confocal technology providing good quality images even without dilating the pupil; furthermore, it does not use a flash and does not require an expert technician. While it is true that some studies that claim that it is a good technique even in situations of small pupils or media opacity,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> in our experience it is very useful to improve the quality of the images, dilate the patient beforehand and even instill an artificial teardrop in case of unstable tear film.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The main limitation of our study is the small sample size. It should be borne in mind, as can be seen in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, that no statistically significant differences were found between the two groups in any of the descriptive statistics, including the IOP and visual field indices. This could be due on the one hand to the sample size and on the other to the fact that these glaucoma cases had incipient visual field defects (with little effect on visual field indices), as well as well-controlled IOP figures with treatment (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The MultiColor module is easy and fast to use, and it also allows us to obtain images at the same time as we perform a peripapillary RNFL OCT acquisition (it is the same device, so there is no need to move the patient). The visibility of defects in the RNFL, particularly in the green and blue reflectance spectrum, is good and we consider it better than that of conventional red-free retinography. However, from our experience, we do not consider MultiColor to be superior to classic fiber photography performed by a technician who is an expert in identifying these defects. Some studies have also reported the superiority of MultColor over conventional retinography in detecting myopic disc damage<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> and identifying epirretinal membranes, age-related macular degeneration, or macular edema.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Conclusion</span><p id="par0050" class="elsevierStylePara elsevierViewall">Although larger studies are needed to substantiate the present tudy, we find the new SPECTRALIS® MultiColor very useful in identifying defects in the RNFL, even in the early stages of glaucoma.
