Its current prevalence is estimated at 1:1,000,000,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> its occurrence is generally sporadic and it is described in association with PAX 6 gene mutations.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Occasionally it may occur in the context of Peters Plus syndrome, which is an autosomal recessive disorder that includes Peters’ anomaly plus other systemic disorders such as psychomotor developmental delay, hearing loss, dwarfism, heart disease and cleft lip and palate.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Being a rare entity, it is important to identify its main manifestations because there are various forms of treatment depending on the degree of involvement of the anterior and posterior pole.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Peters’ anomaly is currently classified as follows:</p><p id="par0015" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold"><span class="elsevierStyleUnderline">Peters’ anomaly Type I</span></span>: Corneal opacity with or without iridocorneal adhesions, without significant involvement of the crystalline lens or posterior pole.</p><p id="par0020" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold"><span class="elsevierStyleUnderline">Peters Anomaly Type II</span></span>: Corneal Opacity + Significant lens involvement with or without iris and posterior pole involvement.</p><p id="par0025" class="elsevierStylePara elsevierViewall">It was decided to evaluate the experience of this non-negligible case series and its clinicopathologic correlation with respect to its difference in visual prognosis and postoperative complications.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Target</span><p id="par0030" class="elsevierStylePara elsevierViewall">To report results in the management of a series of cases with Peters’ anomaly, propose management and therapeutic suggestions according to their clinicopathological findings.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Method</span><p id="par0035" class="elsevierStylePara elsevierViewall">The medical records of 27 patients (32 eyes) with a clinical diagnosis of Peters’ anomaly, who underwent different treatments: medical follow-up, peripheral iridectomy or penetrating keratoplasty (PKP), were analyzed. The different parameters and associated conditions, such as intraocular pressure (IOP), visual acuity (VA), surgical technique and postoperative complications were evaluated.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In patients diagnosed as Peters’ anomaly Type I, medical follow-up, peripheral iridectomy or PPK was performed, depending on the associated findings, mainly fundus visualization and size of the corneal opacity. In <span class="elsevierStyleBold">Peters Type II,</span> PPK + lensectomy + anterior vitrectomy was performed. All patients received amblyopia treatment independent of the type of Peters’ anomaly, including optical lenses and/or contact lenses, based on the results of their cycloplegia, in addition to the use of patching in the unaffected eye in unilateral cases or the less affected eye in bilateral cases. Patching was started for half of the patient's waking hours and then the hours were modified according to the Teller test or Snellen test result depending on the patient’s age.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Quantitative variables were described using mean and standard deviation and qualitative variables were described with absolute and relative frequencies. The Kruskal-Wallis test was used to analyze the association between qualitative and quantitative variables with a nonparametric distribution. Spearman’s correlation coefficient was used to correlate qualitative variables with each other. A p-value < 0.05 was considered statistically significant. The STATA version 10 program was used for statistical analysis.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">General data</span><p id="par0050" class="elsevierStylePara elsevierViewall">Thirty-two eyes were analyzed in 27 patients, 74% (20) male and 26% (7) female. Of the total number of patients 18.5% (five) were bilateral and 81.5% (22) unilateral. The average age of the first consultation was 57.62 days (median: 39) and the average age of the first surgery was 151 days (median: 103). The mean years of follow-up was 10.2 years and pre and postoperative IOP was 22 mmHg (SD 7.87) and 21 mmHg (SD 10.61), respectively. Patients were divided into Patients who did NOT undergo surgical procedures (No Qx), underwent Peripheral Iridectomy (PI), underwent Penetrating Keratoplasty (PPK), the latter subgroup was divided into those who had (C/pp) and those who did NOT have (S/pp) Posterior Pole involvement (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Regarding the preoperative classification, 15 patients (17 eyes) were classified as Peters’ anomaly Type I, of which medical follow-up was performed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A and B) in two eyes, peripheral iridectomy (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A and B) in 3 eyes and PPK (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>) in 12 eyes. Twelve patients (15 eyes) were classified as Peters <span class="elsevierStyleBold">Type II</span> anomaly, of which three patients (four eyes) had posterior pole involvement and nine patients (11 eyes) had no posterior pole involvement. In both cases the definitive diagnosis was made in the operating room.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">All patients classified as <span class="elsevierStyleBold">Type II</span> underwent PPK plus lensectomy and anterior vitrectomy (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Overall, the mean age at first checkup was 57 ± 55.63 days and the age at first surgery was 151 ± 119 days.</p><p id="par0070" class="elsevierStylePara elsevierViewall">There was no significant correlation between age at surgery and type of Peters’ anomaly (p = 0.4852), nor a significant correlation between patient age at surgery and postoperative VA (p = 0.5786).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Visual acuity results</span><p id="par0075" class="elsevierStylePara elsevierViewall">To analyze the VA results, the conversion from Snellen to LogMAR was performed according to the method proposed by Holladay,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> assigning values to Non Light Projection (NPL), Projection of Light (PL), Hand Movement (HM) and Finger Counting (FC) according to the scheme proposed by Schulze-Bounsey<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>: to NPL: LogMAR 3.0, PL (LBP and LMP): LogMAR 2.7, HM: LogMAR 2.3 and FC: LogMAR 1.875 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The long-term VA result correlates directly to the type of Peters’ anomaly. Overall VA results were LogMAR 1.71 ± 1.04 and results by groups were as follows: <span class="elsevierStyleBold">Type I with medical follow-up</span> LogMAR 0.3 ± 0, <span class="elsevierStyleBold">Type I with peripheral iridectomy</span> LogMAR 0.97 ± 0.78, <span class="elsevierStyleBold">Type I with PPK</span> LogMAR 1.22 ± 0.97, <span class="elsevierStyleBold">Type II without posterior pole involvement</span> LogMAR 2.41 ± 0.8 and <span class="elsevierStyleBold">Type II with posterior pole involvement</span> LogMAR 2.56 ± 0.48.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">When analyzing the cases that underwent PPK there is a statistically significant difference in VA between the types of Peters disease, with a better result in cases of Peters <span class="elsevierStyleBold">type I</span> and a worse result in cases of Peters <span class="elsevierStyleBold">type II with posterior pole involvement</span> (p = 0.0087).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Prognosis of penetrating keratoplasty by Peters’ anomaly classification</span><p id="par0085" class="elsevierStylePara elsevierViewall">In patients in whom PPK was indicated (excluding patients in whom peripheral iridectomy and medical follow-up were performed), the final status of the cornea is directly related to the classification of Peters’ anomaly. Defined as graft failure any opaque cornea in which no pupillary red is visualized, in patients with Peters’ anomaly Type I the percentage of graft failure was 50%, Type II without posterior pole compromise 55% and Type II with posterior pole compromise 75% (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Postoperative complications</span><p id="par0090" class="elsevierStylePara elsevierViewall">Postoperative complications correlate directly with the classification of Peters’ anomaly, with Type II having the highest rate of medium and long-term complications, and patients in whom only peripheral iridectomy was indicated had no complications (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Non-classical preoperative and intraoperative findings of Peters’ anomaly</span><p id="par0095" class="elsevierStylePara elsevierViewall">The non-classical preoperative and intraoperative findings of Peters’ anomaly were more frequent in cases of Type II anomalies with posterior pole involvement (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Correlation between preoperative and intraoperative findings</span><p id="par0100" class="elsevierStylePara elsevierViewall">The correlation between preoperative and intraoperative classification is very different depending on the type of Peters’ anomaly, there is a low correlation between preoperative and intraoperative findings, mainly in Peters’ anomaly <span class="elsevierStyleBold">type II</span> with posterior pole involvement where the diagnostic correlation was only 50%, on the contrary the correlation of Peters anomalies <span class="elsevierStyleBold">type I</span> was 100% (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>).</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Below are four cases that exemplify each of the treatment modalities that were performed in our study.</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Case report 1: unilateral Peters’ anomaly Type I: observation and treatment only amblyopia</span><p id="par0110" class="elsevierStylePara elsevierViewall">Patient with unilateral central corneal opacity (LE) that did not interfere with fundus visualization or pupillary reflex. Medical follow-up and amblyopia treatment was decided (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A), and a check-up was performed at eight years of age, where VA RE 20/20 and VA LE 20/40 were assessed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B).</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Case report 2: bilateral Peters’ anomaly Type I with peripheral iridectomy</span><p id="par0115" class="elsevierStylePara elsevierViewall">Patient with bilateral sectorial corneal opacity, partially compromising the visual axis. Preoperative classification: Peters’ anomaly Type I, bilateral peripheral iridectomy was performed at 1.5 months (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A), control at 11 years of age, where she achieved VA RE 20/70 and LE 20/60 with correction (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>B).</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Case Report 3: Unilateral Peters’ anomaly Type I with PPK</span><p id="par0120" class="elsevierStylePara elsevierViewall">Patient with total corneal opacity LE. Preoperative diagnosis was Peters’ anomaly Type I and LE PPK was performed at 2.5 months of age. Control at five years of age AV RE: 20/20 LE: 20/200 (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Case report 4: bilateral Peters’ anomaly Type II with PPK + lensectomy + vitrectomy</span><p id="par0125" class="elsevierStylePara elsevierViewall">Patient with bilateral total corneal opacity. Peters’ anomaly Type II was diagnosed without posterior pole compromise, PPK + lensectomy + RE and LE vitrectomy was performed, RE at three months with posterior pole compromise and later, graft failure due to infectious corneal ulcer by contact lens. LE PPK was performed at 4.5 months with good long-term results. Control at four years where AV RE: NPL and AV LE: FC (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">Peters’ anomaly is an infrequent entity,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> in the United States “for example” it is diagnosed in 1.5 per 100,000 newborns.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> However, suspicion and diagnosis are mainly clinical and should always be supplemented with tests to rule out associated disorders and clarify the behavior and prognosis. It is essential to rule out glaucoma, extension of the corneal defect and other associated lesions of the anterior and posterior segment.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The possibilities of treatment and visual prognosis will depend, therefore, on the clinical findings and preoperative examinations such as IOP, ultrasound A and B, ultrasonic biomicroscopy, etc., often having to do a previous examination under general anesthesia.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">The best long-term VA outcome was directly correlated with the type of Peters’ anomaly, with better VA prognosis in <span class="elsevierStyleBold">Type I</span> Peters’ anomaly that only requires medical follow-up (0.3 LogMar) and then, those <span class="elsevierStyleBold">Type I</span> in which peripheral iridectomy was performed. Spierer et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> published in 2017 a series of 22 cases in which peripheral iridectomy was performed, with average VA results were 1.8 ± 0.6 LogMar. In the present study the average VA of these patients was 0.97 ± 0.78 LogMar, the number of patients (three) is too low to make any kind of conclusion, although the final VA results are quite encouraging when this technique is indicated.</p><p id="par0140" class="elsevierStylePara elsevierViewall">PPK in children has a high rate of failure and rejection which, according to different publications, ranges between 20 and 80%,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> in addition to multiple intraoperative and postoperative complications. In our study, the percentage of corneal failure is also directly related to the classification of the type of Peters’ anomaly, being 50% in patients with Peters’ anomaly type I, 55% in type II without posterior pole compromise and 75% in type II with posterior pole compromise, values in agreement with those published in the literature.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">In patients who underwent PPK with Peters’ anomaly type I and type II, without and with posterior pole involvement, their long-term final VA was 1.22 ± 0.97, 2.41 ± 0.8 and 2.56 ± 0.48 LogMar, respectively. The year 2012 Chang<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> published a case series, in which 24 eyes (23 patients) underwent PPK, their mean final VA was 2.344 LogMar, in our study the mean VA in patients undergoing PPK was 1.91 ± 0.9 LogMar.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Regarding the diagnostic correlation between preoperative and intraoperative findings in patients with Peters Type II anomaly, without and with posterior pole involvement, this was only 63 and 50%, respectively, as opposed to the correlation of Peters Type I anomalies which was 100% (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>). From the above it can be deduced that patients with Peters Type II disease have a high probability of encountering new findings in the intraoperative period that were not previously assessed.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Regardless of the above and its visual potential, due to its severity, poor natural prognosis and above all to give a better visual functionality, the authors believe that in these cases Type II should always be given the opportunity to surgically improve both eyes involved, since any of them could present some complication and failure in the short or long term. Each patient was subjected to different treatment modalities according to the type of Peters Anomaly, anywhere from medical follow-up clinics to a Penetrating Keratoplasty procedure (PKP).</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">Of the 27 patients (32 eyes), 74% were male and 26% female, with 18.5% (5) being bilateral and 81.5% (22) unilateral. The mean number of years of follow-up was 10.2 years (Range: 3.5 to 18 years). The results of long-term VA correlate directly with the type of Peters Anomaly. For the total number of patients, the VA results were LogMAR 1.71 ± 1.04. The results by groups were: Type I with only medical monitoring LogMAR 0.3 ± 0, Type I with only Optical Iridectomy (OI) LogMAR 0.97 ± 0.78, Type I with PKP LogMAR 1.22 ± 0.97, Type II without a compromised posterior pole with PKP LogMAR 2.41 ± 0.80, and Type II with a compromised posterior pole with PKP LogMAR 2.56 ± 0.48.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">The result of VA and long-term corneal failure is directly related to the type of Peters Anomaly. Patients with Type I who only required medical follow-ups had the most favourable prognosis.
